2A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. 3Animal experiment and clinical study of effect of gamma-interferon on hepatic fibrosis显示文摘AIM To evaluate the antifibrotic effect ofdifferent doses of recombinant human Gamma-Interferon (IFN-γ) intwo rat models of hepaticfibrosis, and to observe its effect on moderatechronic hepatitis B virus fibrosis.METNODS Hepatic fibrosis was successfullyinduced in 150 and 196 rats by subcutaneousinjection of carbon tetrachloride (CCl4) andintraperitoneal injection of dimethylnitrosamine(DMN), respectively. Each of the two modeldose IFN-γ group (15 MU/kg per day, i.m. for 8group (1.67 MU/kg daily, i.m. for 8 weeks).Another group of 10 rats without any treatmentwas used as normal controls. At the end of theexperiment, semi-quantitative histopathologicalscores of inflammation and fibrosis, liver (αsmooth muscle actin (α-SMA) expression level,liver hydroxyl proline content and serumhyaluronic acid levels were compared. And 47medium chronic hepatitis B viral fibrosispatients were studied. They were given IFN-γtreatment, 100MU/day i.m. for the first threemonths and 100MU qod i.m. for the next sixmonths. Semi-quantitative pathological scoresof inflammation and fibrosis and serum hepaticfibrosis indices were compared within the 9months.RESULTS In animal experiment, thepathological fibrosis scores and liver hydroxylproline content were found to be significantlylower in rats treated with different doses of IFN-γ as compared with rats in fibrotic model groupinduced by either CCI4 or DMN, in a dose-dependent manner. For CCI4-induced model,pathological fibrosis scores in high, medium andIow doses IFN-γ groups were 5.10 ± 2.88, 7.70 ±3.53 and 8.00 ± 3.30, respectively, but the scorewas 14.60 ± 7.82 in fibrotic model group.Hydroxyl proline contents were 2.83 ± 1.18, 3.59± 1.22 and 4.80 ± 1.62, in the three IFN-γgroups, and 10.01 ± 3.23 in fibrotic model group.The difference was statistically significant(P<0.01). Similar results were found in DMN-induced model. Pathological fibrosis scoreswere 6.30±0.48, 8.10 ±2.72 and 8.30 ±2.58, inhigh, medium and Iow doses IFN-γ groups, and12.60 ± 3.57 in fibrotic model group. Hydroxylproline contents were 2.72 ± 0.58, 3.14 ± 0.71and 3.62 ± 1.02, in the three IFN-γ groups, and12.79 ± 1.54 in fibrotic model group. Thedifference was statistically significant(P<0.01). Serum hepatic fibrosis indicesdecreased significantly in the 47 patients afterIFN-γ treatment (HA: 433.38 ± 373.00 vs 281.57± 220.48; LN: 161.22± 41.02 vs 146.35 ± 44.67;PCⅢ: 192.59 ± 89.95 vs 156.98 ± 49.22; C-Ⅳ:156.30 ± 44.01 vs 139.14 ± 34.47) and thedifferences between the four indices weresignificant (P<0.05). Thirty-three patientsCONCLUSION All the three doses of IFN-γ areeffective in treating rat liver fibrosis induced byeither CCl4 or DMN, the higher the dose, thebetter the effect. 5Mapping wetland changes in China between 1978 and 2008显示文摘Four wetland maps for all China have been produced,based on Landsat and CBERS-02B remote sensing data between 1978 and 2008 (1978,1990,2000 and 2008).These maps were mainly developed by manual interpretation and validated by substantial field investigation in 2009.Based on these maps,we analyzed the 2008 wetland distribution in China and discussed wetland changes and their drivers over the past 30 years.(i) There were about 324097 km 2 of wetlands in 2008,for which inland marshes or swamps were the most common wetland type (35%),with lakes (26%) second.Most of the wetlands were in Heilongjiang,Inner Mongolia,Qinghai and Tibet,occupying about 55% of the national wetland area.(ii) From 1978 to 2008,China's wetland area continually and significantly decreased,by about 33% based on changes in the wetland map.This was in sharp contrast to the increase in artificial wetlands,which increased by about 122%.Inland marshes accounted for the main loss of total wetlands from 1978 to 2000.From 2000 through 2008,riverine and lacustrine wetlands constituted the main wetland loss.Fortunately however,the rate of wetland loss decreased from 5523 to 831 km 2 /a.(iii) The change ratio of lost natural wetlands (including inland and coastal wetlands) to non-wetlands has decreased slightly over the past 30 years.From 1978 to 1990,nearly all natural wetlands (98%) lost were transformed into non-wetlands.However,the ratio declined to 86% from 1990 to 2000,and to 77% from 2000 to 2008.(iv) All Chinese provinces were divided into three groups according to patterns of wetland changes,which could relate to the driving forces of such changes.Tibet was completely different from other provinces,as it was one representative example in which there was a net wetland increase,because of global warming and decreased human activity since 1990.Increased economic development caused considerable wetland loss in most eastern provinces,and artificial wetlands increased.NIU ZhenGuo ZHANG HaiYing WANG XianWei YAO WenBo ZHOU DeMin ZHAO KuiYi ZHAO Hui LI NaNa HUANG HuaBing LI CongCong YANG Jun LIU CaiXia LIU Shuang WANG Lin LI Zhan YANG ZhenZhong QIAO Fei ZHENG YaoMin CHEN YanLei SHENG YongWei GAO XiaoHong ZHU WeiHong WANG WenQing WANG Hong WENG YongLing ZHUANG DaFang LIU JiYuan LUO ZhiCai CHENG Xiao GUO ZiQi GONG Peng 2012Chinese Science Bulletin2012,57,22:51

8PREVALENCE AND RISK FACTORS OF PERIPHERAL ARTERIAL DISEASE IN DIABETIC PATIENTS OVER 50 YEARS OLD IN CHINA显示文摘Objective To investigate the prevalence of peripheral arterial disease (PAD) in China type 2 diabetic patients and to demonstrate the relationships between putative risk factors and PAD. Methods In total 1 397 type 2 diabetic patients aged 50 years and older were enrolled and determined ankle-brachial index (ABI) and brachial-ankle pulse wave velocity (baPWV) in 15 Class III Grade A hospitals in 7 major cities of China. Results Mean patient age was 63.7±8.2 years and mean duration of diabetes mellitus was 9.39±7.4 years. Two hundreds and seventy-two (19.47%) patients were diagnosed as PAD by ABI <0.9, 122 (18.37%) in male and 150 (20.46%) in female. PAD patients had a significantly longer duration of diabetes mellitus, higher hemoglobin Alc, and a significantly lower mean body mass index than non-PAD ones. Aging, smoking, and systolic blood pressure were found to be positively related with the prevalence of PAD. In terms of lipid profiles, no variable was found to relate with PAD. Notably, baPWV showed as the same significant guiding index for PAD, almost matched with ABI. Conclusions PAD is a common complication in China type 2 diabetic patients. 9A Spectral Index for Estimating Soil Salinity in the Yellow River Delta Region of China Using EO-1 Hyperion Data显示文摘Soil salinization is one of the most common land degradation processes. In this study, spectral measurements of saline soil samples collected from the Yellow River Delta region of China were conducted in laboratory and hyperspectral data were acquired from an EO-1 Hyperion sensor to quantitatively map soil salinity in the region. A soil salinity spectral index (SSI) was constructed from continuum-removed reflectance (CR-reflectance) at 2052 and 2203 nm, to analyze the spectral absorption features of the salt-affected soils. There existed a strong correlation (r = 0.91) between the SSI and soil salt content (SSC). Then, a model for estimation of SSC with SSI was established using univariate regression and validation of the model yielded a root mean square error (RMSE) of 0.986 and an R2 of 0.873. The model was applied to a Hyperion reflectance image on a pixel-by-pixel basis and the resulting quantitative salinity map was validated successfully with RMSE = 1.921 and R2 = 0.627. 11Diagnostic value of platelet derived growth factor-BB,transforming growth factor-131,matrix metalloproteinase-1,and tissue inhibitor of matrix metalloproteinase-1 in serum and peripheral blood mononuclear cells for hepatic fibrosis显示文摘AIM: Noninvasive diagnosis of hepatic fibrosis has become the focus because of the limited biopsy, especially in the surveillance of treatment and in screening hepatic fibrosis. Recently, regulatory elements involved in liver fibrosis, such as platelet derived growth factor-BB (PDGF-BB), transforming growth factor-β1(TGF-β1), matrix metalloproteinase-1 (MMP-1), and tissue inhibitor of matrix metalloproteinase-1 (TIMP-1), have been studied extensively. To determine whether these factors or enzymes could be used as the indices for the diagnosis of hepatic fibrosis, we investigated them by means of receiver operating characteristic (ROC) curve. METHODS: Serum samples from sixty patients with chronic viral hepatitis B and twenty healthy blood donors were assayed to determine the level of PDGF-BB, TGF-β1, MMP-1, and TIMP-1 with ELISA, and HA, PCIII, C-IV, and LNlevel with RIA. The message RNA (mRNA) expression of TIMP-1 and MMP-1 in peripheral blood mononuclear cells (PBMCs) was detected by RT-PCR and Northern blot hybridization. Liver biopsy was performed in all patients. The biopsy samples were histopatholocjically examined. The trial was double-blind controlled. RESULTS: The serum level of PDGF-BB, TIMP-1, the ratio of TIMP-1 and MMP-1 (TIMP-1/MMP-1), mRNA expression of TIMP-1 (TIMP-lmRNA), and the ratio of TIMP-lmRNA and MMP-lmRNA (TIMP-lmRNA/MMP-lmRNA) in patients was significantly higher than those in the healthy blood donors (t=2.514-11.435, P=0.000-0.016). The serum level of PDGF-BB, TIMP-1, TIMP-1/MMP-1, and TIMP-lmRNA was positively correlated with fibrosis stage and inflammation grade (r=0.239-0.565, P=0.000-0.033), while the serum level of MMP-1 was negatively correlated with fibrosis stage and inflammation grade, and TIMP-lmRNA/MMP-lmRNA was positively correlated with inflammation grade. Through the analysis by ROC curve, serum PDGF-BB was the most valuable marker, and its sensitivity was the highest among the nine indices. The markers with the highest specificity were TIMP-lmRNA and TIMP-lmRNA/MMP-lmRNA in PBMCs. The area under the curve (AUC) of PDGF-BB, TIMP-lmRNA, TIMP-lmRNA/MMP-lmRNA, TIMP-1/MMP-1, HA, PCIII, TIMP-1, C-IV, and LN was 0.985, 0.876, 0.792, 0.748, 0.728, 0.727, 0.726, 0.583, and 0.463, respectively. The sensitivity and the specificity in the parallel test was 99.0% and 95.0 % when serum PDGF-BB, TIMP-lmRNA and TIMP-lmRNA/MMP-lmRNA was detected simultaneously. CONCLUSION: Serum level of PDGF-BB, TIIVIP-lmRNA, TIMP-lmRNA/MMP-lmRNA in PBMCs, and serum level of TIMP-1 and TIMP-1/MMP-1 can be used as the indices for the diagnosis of hepatic fibrosis, but the former three are more useful. 12Changes in intestinal mucosal immune barrier in rats with endotoxemia显示文摘AIM:To investigate the dysfunction of the immunological barrier of the intestinal mucosa during endotox-emia and to elucidate the potential mechanism of this dysfunction.METHODS:Male Wistar rats were randomly distributed into two groups:control group and lipopolysaccharide(LPS)group.Endotoxemia was induced by a single caudal venous injection of LPS.Animals were sacrifi ced in batches 2,6,12 and 24 h after LPS infusion.The number of microfold(M)-cells,dendritic cells(DCs),CD4+ T cells,CD8+ T cells,regulatory T(Tr)cells and IgA+ B cells in the intestinal mucosa were counted after immunohistochemical staining.Apoptotic lympho-cytes were counted after TUNEL staining.The levels of interleukin(IL)-4,interferon(IFN)-γ and forkhead box P3(Foxp3)in mucosal homogenates were measured by ELISA.The secretory IgA(sIgA)content in the total protein of one milligram of small intestinal mucus was detected using a radioimmunological assay.RESULTS:This research demonstrated that LPS-induced endotoxemia results in small intestinal mucosa injury.The number of M-cells,DCs,CD8+ T cells,and IgA+ B cells were decreased while Tr cell and apoptotic lymphocyte numbers were increased signifi cantly.The number of CD4+ T cells increased in the early stages and then slightly decreased by 24 h.The level of IL-4 significantly increased in the early stages and then reversed by the end of the study period.The level of IFN-γ increased slightly in the early stages and then decreased markedly by the 24 h time point.Level of Foxp3 increased whereas sIgA level decreased.CONCLUSION:Mucosal immune dysfunction forms part of the intestinal barrier injury during endotoxemia.The increased number and function of Tr cells as well as lymphocyte apoptosis result in mucosal immunodefi ciency.Chong Liu Ang Li Yi-Bing Weng Mei-Li Duan Bao-En Wang Shu-Wen Zhang 2009World Journal of Gastroenterology2009,15,46:35
13Epidemiological Status of Chronic Diabetic Complications in China显示文摘With the booming development of industrialization and urbanization, popularization of high calorie diet and sedentary lifestyle, and the rapid growth of aging, the prevalence of diabetes increased nearly 10 times during the past three decades in China.[1-3] China has become the country with the largest number of people with diabetes and thus will face considerable challenge of chronic complications in the next 10-20 years.Diabetes-associated chronic complications, such as vision disorder, kidney disease, neuropathy, peripheral vascular disease, cardiopathy, and stroke, are the major causes of physical disability and mortality, making it a nonnegligible problem of social healthcare system.Therefore, it is of importance to summarize and update current prevalence and risk factors of diabetic complications in China, not only to establish the strategy of prevention and treatment to delay the development of chronic complications, but also to improve the living quality of patients and alleviate the burden of medical cost.Jian-Ping Weng Yan Bi 2015Chinese Medical Journal2015,,24:37

15Targeting PI3K/AKT signaling for treatment of idiopathic pulmonary fibrosis显示文摘Idiopathic pulmonary fibrosis(IPF) is a chronic progressive fibrotic interstitial pneumonia with unknown causes. The incidence rate increases year by year and the prognosis is poor without cure.Recently, phosphatidylinositol 3-kinase(PI3 K)/protein kinase B(PKB/AKT) signaling pathway can be considered as a master regulator for IPF. The contribution of the PI3 K/AKT in fibrotic processes is increasingly prominent, with PI3 K/AKT inhibitors currently under clinical evaluation in IPF. Therefore,PI3 K/AKT represents a critical signaling node during fibrogenesis with potential implications for the development of novel anti-fibrotic strategies. This review epitomizes the progress that is being made in understanding the complex interpretation of the cause of IPF, and demonstrates that PI3 K/AKT can directly participate to the greatest extent in the formation of IPF or cooperate with other pathways to promote the development of fibrosis. 17Ankylosing spondylitis: etiology, pathogenesis, and treatments显示文摘Ankylosing spondylitis(AS), a common type of spondyloarthropathy, is a chronic inflammatory autoimmune disease that mainly affects spine joints, causing severe, chronic pain;additionally, in more advanced cases, it can cause spine fusion. Significant progress in its pathophysiology and treatment has been achieved in the last decade. Immune cells and innate cytokines have been suggested to be crucial in the pathogenesis of AS, especially human leukocyte antigen(HLA)?B27 and the interleukin?23/17 axis.However, the pathogenesis of AS remains unclear. The current study reviewed the etiology and pathogenesis of AS, including genome-wide association studies and cytokine pathways. 18ROC curves in evaluation of serum fibrosis indices for hepatic fibrosis显示文摘AIM: Use Receiver operating characteristic (ROC) curves to find out the relationship between serum level of hyaluronic acid (HA), type Ⅲ procollagen (PCⅢ), N-terminal procollagen Ⅲ peptide (PⅢNP), laminin (LN), type Ⅳ collagen (C-Ⅳ) and hepatic fibrosis, as well as to determine their value in clinical practice.METHODS: 114 serum samples from chronic hepatitis patients were assayed for fibrosis indices including HA, PCⅢ, PⅢNP, LN and Ⅳ-C with radioimmunoassay (RIA). Liver biopsy was also performed in all these patients and the biopsy material was examined histopathologically.RESULTS: ROC curves analysis showed that area under the curve (AUC) of PⅢNP, HA, PCⅢ, C-Ⅳ and LN was 0.800,0.728, 0.727, 0.583 and 0.463, respectively. The analysis also showed that PⅢNP (r=0.452), HA (r=0.497) and PCⅢ (r=0.404) have greater diagnosis performances than C-Ⅳ (r=0.238) and LN (r=0.128) according to fibrosis staging. The sensitivity of HA plus PⅢNP was 55.1%, it was the most sensitive combination. Combined three or more than three indices that based on HA, the specificity was 100 %.Using combination assays can improve the specificity, but its sensitivity was not high. Serum fibrosis indices increased as the grade of inflammation aggravated. But only PⅢNP and PCⅢ had significant difference between G1 and G2 (PⅢNP: 13.16±8.07 VS8.32±5.09; PCⅢ: 164.22±65.69 VS 138.23±77.63). The coefficient correlation of the results of inflammation grade and fibrosis staging to HA was 0.525 and 0.553 respectively, that to PCⅢ, 0.446 and 0.412, that to LN, 0.234 and 0.194, and that to Ⅳ-C, 0.363 and 0.351, respectively.CONCLUSION: Serum fibrosis indices can indicate tendency of hepatic fibrosis, but it cannot replace liver biopsy. 19Low-temperature SCR activity and SO_2 deactivation mechanism of Ce-modified V_2O_5–WO_3/TiO_2 catalyst显示文摘The promotion effect of ceria modi fi cation on the low-temperature activity of V2O5-WO3/TiO2 catalyst was evaluated for the selective catalytic reduction of NO with NH3(NH3-SCR). The catalytic activity of 1 wt% V2O5-WO3/TiO2 was signi fi cantly enhanced by the addition of 8 wt%ceria, which exhibited a NO x conversion above 80% in a broad temperature range 190–450 1C. This performance was comparable with 3 wt%V2O5-WO3/TiO2, indicating that the addition of ceria contributed to reducing the usage of toxic vanadia in developing low-temperature SCR catalysts. Moreover, V1 Ce WTi exhibited approximately 10% decrease in NOx conversion in the presence of 60 ppm SO2. The characterization results indicated that active components of V, W and Ce were well dispersed on TiO2 support. The synergetic interaction between Ce and V species by forming V–O–Ce bridges enhanced the reducibility of VCe WTi catalyst and thus improved the low-temperature activity. The sulfur poisoning mechanism was also presented on a basis of the designed TPDC(temperature-programmed decomposition) and TPSR(temperatureprogrammed surface reaction) experiments. The deposition of(NH4)2SO4on V1 Ce WTi catalyst was much smaller compared with that on V1 Ti.On the other hand, the oxidation of SO2 to SO3was signi fi cantly promoted on the CeO2-modi fi ed catalyst, accompanied by the formation of cerium sulfates. 20Taxus chinensis ameliorates diabetic nephropathy through down-regulating TGF-β1/Smad pathway显示文摘Diabetic nephropathy(DN) is one of the common microvascular complications of diabetes mellitus. Renal fibrosis is closely related to the deterioration of renal function. The present study aimed to investigate protective effect of Taxus chinensis on high-fat diet/streptozotocin-induced DN in rats and explore the underlying mechanism of action. The rat DN model was established via feeding high fat diet for 4 weeks and subsequently injecting streptozotocin(30 mg·kg^(-1) body weight) intraperitoneally. The rats with blood glucose levels higher than 16.8 mmol·L^(-1) were selected for experiments. The DN rats were treated with Taxus chinensis orally(0.32, 0.64, and 1.28 g·kg^(-1)) once a day for 8 weeks. Taxus chinensis significantly improved the renal damage, which was indicated by the decreases in 24-h urinary albumin excretion rate, blood serum creatinine, and blood urea nitrogen. Histopathological examination confirmed the protective effect of Taxus chinensis. The thickness of glomerular basement membrane was reduced, and proliferation of mesangial cells and podocytes cells and increase in mesangial matrix were attenuated. Further experiments showed that Taxus chinensis treatment down-regulated the expression of TGF-β1 and α-SMA, inhibited phosphorylation of Smad2 and Smad3.
