1氯吡格雷的使用与药物洗脱支架植入后远期临床结果显示文摘背景:近来的冠状动脉内药物洗脱支架研究提示,现行的抗血小板治疗方案可能并不足以预防后期支架内血栓形成。目的:于接受药物洗脱支架(drug-eluting stents,DESs)和裸金属支架(bare-metal stents,BMSs)治疗的冠状动脉病患者中评估氯吡格雷使用与患者远期结果的关系。设计、地点及患者:于连续患者中进行观察性研究。患者于2000年1月1日至2005年7月31日在杜克心脏治疗中心(一家位于北卡罗来纳州达累姆市的三级治疗中心)接受冠状动脉内支架治疗。于6、12、24个月时进行随访,直至2006年9月7日。研究人群包括4666例最初接受BMS(n=3165)或DES(n=1501)经皮冠状动脉介入治疗的患者。对随访6和12个月没有事件(没有死亡、心肌梗死或血管重建手术)的患者进行界标分析(landmark analysis)。在这些时间点上根据支架类型以及自我报告之氯吡格雷的使用情况将患者分为4组:DES使用氯吡格雷组、DES不用氯吡格雷组、BMS使用氯吡格雷组以及BMS不用氯吡格雷组。主要观测指标:随访24个月时死亡、非致命性心肌梗死以及死亡或心肌梗死之复合终点。结果:在6个月时没有事件发生的DES组患者(637例使用、579例未使用氯吡格雷)中,氯吡格雷的使用是随访24个月校正死亡率较低(使用2.0%比未使用5.3%;差异,-3.3%;95%CI,-6.3%至-0.3%;P=0.03)以及死亡或心肌梗死发生率较低(3.1%比7.2%;差异,-4.1%;95%CI,-7.6%至-0.6%;P=0.02)的显著预测指标。然而,在BMS组患者中(417例使用、1976例未用氯吡格雷),死亡(3.7%比4.5%;差异,-0.7%;95%CI,-2.9%至1.4%;P=0.50)以及死亡或心肌梗死发生率(5.5%比6.0%;差异,-0.5%;95%CI,-3.2%至2.2%;P:0.70)没有差异。在12个月随访没有事件发生的DES组患者(252例使用、276例未用氯吡格雷)中,氯吡格雷的使用依然可以预测24个月死亡(0%比3.5%;差异,-3.5%;95%CI,-5.9%至-1.1%;P=0.004)以及死亡或心肌梗死发生率(0%比4.5%;差异,-4.5%;95%CI,-7.1%至-1.9%;P〈0.001)较低。然而,在BMS组患者中(346例使用、1644例未用氯吡格雷),在死亡(3.3%比2.7%;差异,0.6%;95%CI,1.5%至2.8%;P=0.57)以及死亡或心肌梗死(4.7%比3.6%;差异,1.0%;95%CI,-1.6%至3.6%;P=0.44)发生方面,依然没有差异。结论:在接受DES治疗的患者中,延长氯吡格雷的使用可以使死亡以及死亡或心肌梗死的发生率下降。然而,使用氯吡格雷的适宜期限只能通过大型临床随机试验确定。Eric L Eisenstein, DBA Kevin J. 2high-entropy silicide:(Mo_(0.2)Nb_(0.2)Ta_(0.2)Ti_(0.2)W_(0.2))Si_(2)显示文摘A high-entropy metal disilicide,(Mo_(0.2)Nb_(0.2)Ta_(0.2)Ti_(0.2)W_(0.2))Si_(2),has been successfully synthesized.X-ray diffraction(XRD),energy dispersive X-ray spectroscopy(EDX),and electron backscatter diffraction(EBSD)collectively show the formation of a single high-entropy silicide phase.This high-entropy(Mo_(0.2)Nb_(0.2)Ta_(0.2)Ti_(0.2)W_(0.2))Si_(2) possesses a hexagonal C40 crystal structure with ABC stacking sequence and a space group of P6222.This discovery expands the known families of high-entropy materials from metals,oxides,borides,carbides,and nitrides to a silicide,for the first time to our knowledge,as well as demonstrating that a new,non-cubic,crystal structure(with lower symmetry)can be made into highentropy phase.This(Mo_(0.2)Nb_(0.2)Ta_(0.2)Ti_(0.2)W_(0.2))Si_(2) exhibits high nanohardness of 16.7±1.9 GPa and Vickers hardness of 11.6±0.5 GPa.Moreover,it has a low thermal conductivity of 6.9±1.1Wm^(-1) K^(-1),which is approximately one order of magnitude lower than that of the widely-used tetragonal MoSi_(2) and ~1/3 of those reported values for the hexagonal NbSi_(2) and TaSi_(2) with the same crystal structure.Joshua Gild Jeffrey Braun Kevin Kaufmann Eduardo Marin Tyler Harrington Patrick Hopkins Kenneth Vecchio Jian Luo 2019Journal of Materiomics2019,5,3:27
3脐血间充质干细胞的生物学特征及其对造血干/祖细胞体外扩增的支持作用显示文摘目的探讨脐血间充质干细胞(MSC)的生物学特征及其对造血干/祖细胞体外扩增的支持作用。方法用液体培养法分离脐血贴壁细胞,采用ELISA方法检测贴壁细胞条件培养液中细胞因子的表达;用流式细胞术分析其免疫表型特征;在成软骨细胞诱导培养条件下诱导细胞分化,并用RTPCR方法检测分化后细胞原胶原Ⅱ型基因的表达。采用分阶段共培养方法观察脐血贴壁细胞对CD34+细胞体外扩增的支持作用。结果脐血单个核细胞纤维样细胞集落形成率为(3.5±0.7)/106。脐血MSC体外至少可以扩增15代。没有分化的脐血MSC表型为CD13、CD29、CD90、CD105、CD166、SH2、SH3和SH4阳性,CD45、CD34和CD14阴性;脐血MSC培养上清中干细胞因子、IL6和肿瘤坏死因子α检测阳性。在成软骨细胞诱导培养基培养条件下,脐血MSC原胶原Ⅱ型基因mRNA表达阳性。脐血MSC与CD34+细胞共掊养14d,CD34+细胞扩增率高于未共培养组4倍。结论脐血MSC具有类似于成体骨髓MSC的特征,对造血干细胞增殖有明显的支持作用。王丽娟 张叶萍 王金福 吴亦凡 项盈 解纯刚 贾冰冰 Jenny Harrington IanK. 5高光谱成像技术预测香葱贮藏品质显示文摘香葱是一种保质期很短的重要调味食品,水分与叶绿素是评估香葱采后品质的重要指标。本文旨在使用无损检测技术获取香葱在采后不同存储条件下的水分及叶绿素分布情况。实验采用高光谱成像技术获取431~962 nm波段的香葱反射光谱数据,通过卷积平滑(SG)、多元散射校正(MSC)、标准正态变异(SNV)三种预处理方法对原始光谱进行相应转换,并分别建立水分和叶绿素含量预测模型,比较模型预测精度后,选用降噪效果最好的MSC作为光谱预处理方法。随后使用竞争自适应加权采样算法分别选出11个和20个特征波段用于水分与叶绿素含量的预测。基于优选特征波段,利用偏最小二乘回归算法和支持向量机回归算法建立水分和叶绿素含量的预测模型。所建水分与叶绿素含量预测模型的最高预测决定系数分别达到0.9046和0.9143。最后根据所建模型取得不同存储条件下香葱水分及叶绿素含量分布图。综上,高光谱成像技术可用于快速无损检测香葱水分及叶绿素分布情况。本研究为后续便携式果蔬水分及叶绿素分布检测仪器的开发提供了理论依据。任怡 王成全 Bonah Ernest Joshua Harrington Aheto 王锋 黄星奕 2021食品工业科技2021,42,10:6
6Mediastinal node staging by positron emission tomographycomputed tomography and selective endoscopic ultrasound with fine needle aspiration for patients with upper gastrointestinal cancer:Results from a regional centre显示文摘AIM To investigate the impact of endoscopic ultrasound-guided fine-needle aspiration(EUS-FNA) and positron emission tomography-computed tomography(PET-CT) in the nodal staging of upper gastrointestinal(GI) cancer in a tertiary referral centre.METHODS We performed a retrospective review of prospectively recorded data held on all patients with a diagnosis of upper GI cancer made between January 2009 and December 2015. Only those patients who had both a PET-CT and EUS with FNA sampling of a mediastinal node distant from the primary tumour were included. Using a positive EUS-FNA result as the gold standard for lymph node involvement, the sensitivity, specificity, positive and negative predictive values(PPV and NPV) and accuracy of PET-CT in the staging of mediastinal lymph nodes were calculated. The impact on therapeutic strategy of adding EUS-FNA to PET-CT was assessed.RESULTS One hundred and twenty one patients were included. Sixty nine patients had a diagnosis of oesophageal adenocarcinoma(Thirty one of whom were junctional), forty eight had oesophageal squamous cell carcinoma and four had gastric adenocarcinoma. The FNA results were inadequate in eleven cases and the PET-CT findings were indeterminate in two cases, therefore thirteen patients(10.7%) were excluded from further analysis. There was concordance between PET-CT and EUS-FNA findings in seventy one of the remaining one hundred and eight patients(65.7%). The sensitivity, specificity, PPV and NPV values of PET-CT were 92.5%, 50%, 52.1% and 91.9% respectively. There was discordance between PET-CT and EUS-FNA findings in thirty seven out of one hundred and eight patients(34.3%). MDT discussion led to a radical treatment pathway in twenty seven of these cases, after the final tumour stage was altered as a direct consequence of the EUS-FNA findings. Of these patients, fourteen(51.9%) experienced clinical remission of a median of nine months(range three to forty two months). 8AIDS and associated malignancies显示文摘AIDS associated malignancies (ARL) is a major complication associated with AIDS patients upon immunosuppression. Chronically immunocompromised patients have a markedly increased risk of developing lymphoproliferative disease. In the era of potent antiretrovirals therapy (ARV), the malignant complications due to HIV-1 infection have decreased in developed nations where ARV is administered, but still poses a major problem in developing countries where HIV-1 incidence is high and ARV is still not yet widely available. Even in ARV treated individuals there is a concern that the prolonged survival of many HIV-1 carriers is likely to eventually result in an increased number of malignancies diagnosed. Malignancies that were found to have high incidence in HIV-infected individuals are Kaposi’s sarcoma (KS), Hodgkin’s disease (HD) and non-Hodgkin’s lymphoma (NHL). The incidence of NHL has increased nearly 200 fold in HIV-positive patients, and accounts for a greater percentage of AIDS defining illness in the US and Europe since the advent of HAART therapy. These AIDS related lymphomas are distinct from their counterparts seen in HIV-1 seronegative patients. For example nearly half of all cases of ARL are associated with the presence of a gamma herpesvirus, Epstein Barr virus (EBV) or human herpesvirus-8 (HHV-8)/ Kaposi’s sarcoma associated herpesvirus (KSHV). The pathogenesis of ARLs is complex. 12Effects of odors on behaviors of captive Amur leopards Panthera pardus orientalis显示文摘Captive environments often fail to resemble the wild environment in respects of limited space,unchanging habitat,lack of stimulus and contingency. Common animal welfare problems which occur in captive animals include low behavioral diversity,abnormal behavior and excessive inactivity. Environmental enrichment,as an effective strategy to tackle these problems and promote mental health of captive animals,has been recognized as an important principal for captive animal management. Among all the enrichment techniques,olfactory enrichment is a simple and effective method for improving the well-being of the olfactory sensitive felids. Behavioral problems were observed in six Amur leopards Panthera pardus orientalis at Beijing Zoological Garden. These were held in the older type exhibits which have now been rebuilt. These behaviors include stereotypic behavior and excessive inactivity caused by the spatially limited enclosures with low levels of stimuli. To determine the effects of predator,prey,and herb odors as potential enrichment materials for captive leopards,we conducted olfactory enrichment experiments for the leopards and tested the effects of nutmeg Myristica fragrans,feces of roe deer Capreolus capreolus and urine of Amur tiger Panthera tigris altaica to test for an increase in behavioral repertoire and activity. Odors provided in this study were also believed to improve the psychological and physiological health of individuals. To standardize the method of presentation the odors were introduced to the enclosures by rubbing or spraying onto a clean towel. Our results show that the selected three odors effectively increased the behavioral diversity. Ten new behavior types were observed in the nutmeg experiment,eight in the feces of roe deer experiment and six in the tiger urine experiment. Among the three odors,cats responded to nutmeg for the longest duration,followed by tiger urine and feces of roe deer. Leopards showed more play behavior in presence of nutmeg while more investigatory behavior in presences of feces of roe deer and tiger urine. Providing novel odors increased the spatial use of the exhibit and the animal's increased use of the logs,sleeping platforms and bars in the cages. 16温度限制串联相关网络-近红外光谱法用于药物甲硝唑的质量控制显示文摘Temperature-constrained cascade correlation networks(TCCCNs) were applied to the identification of the powder pharmaceutical samples of metronidazole based on near infrared(NIR) diffuse reflectance spectra. This work focused on the comparison of performances of the uni-output TCCCN(Uni-TCCCN) to multi-output TCCCN(Multi-TCCCN) by using near infrared diffuse reflectance spectra of metronidazole. The TCCCN models were verified with independent prediction samples by using the 'cross-validation' method. The networks were used to discriminate qualified, un-qualified and counterfeit metronidazole pharmaceutical powders. The results showed that multiple outputs network generally worked better than the single output networks. With proper network parameters the pharmaceutical powders can be classified at a rate of 100% in this work.
