| 2 | Therapeutic targets and interventional strategies in COVID-19:mechanisms and clinical studies显示文摘Owing to the limitations of the present efforts on drug discovery against severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and the lack of the understanding of the biological regulation mechanisms underlying COVID-19,alternative or novel therapeutic targets for COVID-19 treatment are still urgently required.SARS-CoV-2 infection and immunity dysfunction are the two main courses driving the pathogenesis of COVID-19.Both the virus and host factors are potential targets for antiviral therapy.Hence,in this study,the current therapeutic strategies of COVID-19 have been classified into'target virus'and'target host' categories.Repurposing drugs,emerging approaches,and promising potential targets are the implementations of the above two strategies.First,a comprehensive review of the highly acclaimed old drugs was performed according to evidence-based medicine to provide recommendations for clinicians.Additionally,their unavailability in the fight against COVID-19 was analyzed.Next,a profound analysis of the emerging approaches was conducted,particularly all licensed vaccines and monoclonal antibodies(mAbs)enrolled in clinical trials against primary SARS-CoV-2 and mutant strains.Furthermore,the pros and cons of the present licensed vaccines were compared from different perspectives.Finally,the most promising potential targets were reviewed,and the update of the progress of treatments has been summarized based on these reviews. | Yu-Wen Zhou Yao Xie Lian-Sha Tang Dan Pu Ya-Juan Zhu Ji-Yan Liu Xue-Lei Ma | 2021 | Signal Transduction and Targeted Therapy2021,6,9: | 1 |

| 5 | The effect of renin–angiotensin–aldosterone system inhibitors in patients with hypertension and COVID-19:A meta-analysis of randomized controlled trials and propensity score-matched studies显示文摘Background:High-quality evidence for whether the use of renin–angiotensin–aldosterone system(RAAS)in-hibitors worsens clinical outcomes for patients with coronavirus disease 2019(COVID-19)is lacking.The present study aimed to evaluate the effect of RAAS inhibitors on disease severity and mortality in patients with hyper-tension and COVID-19 using randomized controlled trials(RCTs)and propensity score-matched(PSM)studies.Methods:A literature search was conducted with PubMed,Embase,and Scopus databases from 31 December 2019 to 10 January 2022.We included RCTs and PSM studies comparing the risk of severe illness or mortality in patients with hypertension and COVID-19 treated or not treated with RAAS inhibitors.Individual trial data were combined to estimate the pooled odds ratio(OR)with a random-effects model.Results:A total of 17 studies(4 RCTs and 13 PSM studies)were included in the meta-analysis.The use of RAAS inhibitors was not associated with an increased risk of severe illness(OR=1.00,95%confidence interval[CI]:0.88–1.14,I^(2)=28%)or mortality(OR=0.96,95%CI:0.83–1.11,I^(2)=16%)for patients with hypertension and COVID-19.Furthermore,there was no significant difference in the severity of COVID-19 when patients continued or discontinued treatment with RAAS inhibitors(OR=1.01,95%CI:0.78–1.29,I^(2)=0%).Conclusions:This study suggests that there was no association between treatment with RAAS inhibitors and worsened COVID-19 disease outcomes.Our findings support the current guidelines that RAAS inhibitors should be continued in the setting of the COVID-19 pandemic.However,the benefit of RAAS inhibitor medications for COVID-19 patients should be further validated with more RCTs. | Kai Zhang Lanxin Cao Nanxia Xuan Tiancha Huang Baoping Tian Wei Cui Gensheng Zhang Shufang Zhang | 2022 | Journal of Intensive Medicine2022,2,4: | 0 |

| 7 | COVID-19,the Pandemic of the Century and Its Impact on Cardiovascular Diseases显示文摘COVID-19 caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection likely ranks among the deadliest diseases in human history.As with other coronaviruses,SARS-CoV-2 infection damages not only the lungs but also the heart and many other organs that express angiotensin-converting enzyme 2(ACE2),a receptor for SARS-CoV-2.COVID-19 has upended lives worldwide.Dietary behaviors have been altered such that they favor metabolic and cardiovascular complications,while patients have avoided hospital visits because of limited resources and the fear of infection,thereby increasing out-hospital mortality due to delayed diagnosis and treatment.Clinical observations show that sex,age,and race all influence the risk for SARS-CoV-2 infection,as do hypertension,obesity,and pre-existing cardiovascular conditions.Many hospitalized COVID-19 patients suffer cardiac injury,acute coronary syndromes,or cardiac arrhythmia.SARS-CoV-2 infection may lead to cardiomyocyte apoptosis and necrosis,endothelial cell damage and dysfunction,oxidative stress and reactive oxygen species production,vasoconstriction,fibrotic and thrombotic protein expression,vascular permeability and microvascular dysfunction,heart inflammatory cell accumulation and activation,and a cytokine storm.Current data indicate that COVID-19 patients with cardiovascular diseases should not discontinue many existing cardiovascular therapies such as ACE inhibitors,angiotensin receptor blockers,steroids,aspirin,statins,and PCSK9 inhibitors.This review aims to furnish a framework relating to COVID-19 and cardiovascular pathophysiology. | Yuanyuan Zhang Mingjie Wang Xian Zhang Tianxiao Liu Peter Libby Guo-Ping Shi | 2021 | Cardiology Discovery2021,1,4: | 0 |
| 8 | Effects of angiotensin Ⅱ receptor blocker usage on viral load,antibody dynamics, and transcriptional characteristics among COVID-19 patients with hypertension显示文摘Epidemiological evidence suggests that patients with hypertension infected with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)are at increased risk of acute lung injury.However,it is still not clear whether this increased risk is related to the usage of renin-angiotensin system(RAS)blockers.We collected medical records of coronavirus disease 2019(COVID-19)patients from the First Affiliated Hospital,Zhejiang University School of Medicine(Hangzhou,China),and evaluated the potential impact of an angiotensin II receptor blocker(ARB)on the clinical outcomes of COVID-19 patients with hypertension.A total of 30 hypertensive COVID-19 patients were enrolled,of which 17 were classified as non-ARB group and the remaining 13 as ARB group based on the antihypertensive therapies they received.Compared with the non-ARB group,patients in the ARB group had a lower proportion of severe cases and intensive care unit(ICU)admission as well as shortened length of hospital stay,and manifested favorable results in most of the laboratory testing.Viral loads in the ARB group were lower than those in the non-ARB group throughout the disease course.No significant difference in the time of seroconversion or antibody levels was observed between the two groups.The median levels of soluble angiotensin-converting enzyme 2(sACE2)in serum and urine samples were similar in both groups,and there were no significant correlations between serum sACE2 and biomarkers of disease severity.Transcriptional analysis showed 125 differentially expressed genes which mainly were enriched in oxygen transport,bicarbonate transport,and blood coagulation.Our results suggest that ARB usage is not associated with aggravation of COVID-19.These findings support the maintenance of ARB treatment in hypertensive patients diagnosed with COVID-19. | Baihuan FENG Dan ZHANG Qi WANG Fei YU Qianda ZOU Guoliang XIE Ruonan WANG Xianzhi YANG Weizhen CHEN Bin LOU Shufa ZHENG Yu CHEN | 2021 | Journal of Zhejiang University-Science B(Biomedicine & Biotechnology)2021,22,4: | 0 |
