1北京市未婚人工流产女青年避孕知识、态度、行为的调查研究显示文摘目的 研究未婚人工流产 (人流 )女青年的避孕知识、态度、行为以及影响避孕行为的因素。方法 以Lawrence的Procede Proceed健康促进计划模式为基础的调查问卷 ,对 30 6例自愿要求人工流产 ,年龄在 1 8~ 2 4岁的未婚女青年进行横断面的调查。结果 近 1 2个月以来 ,仅有 1 3%的女青年坚持每次性行为都使用避孕方法 ,偶尔使用和从未使用者的比例分别为 2 6 %和 2 7%。在 2 2 4例曾经用过避孕方法的女青年中 ,最常用的方法分别是避孕套 ( 4 9% )、体外排精 ( 2 8% )、安全期( 1 6 % )。在从未使用任何避孕方法的女青年中 ,73%认为没想到会怀孕是最主要的不避孕的原因。logistic逐步回归分析显示 ,女青年对避孕知识的了解 ,对意外妊娠风险的认识 ,男友对避孕方法使用的态度 ,与男友讨论避孕方法 ,对避孕服务可及性的感受是影响女青年既往避孕行为的主要因素。结论 亟需对未婚青年开展有关避孕知识的性教育 ,提高对意外妊娠风险和人工流产并发症的认识 ,促进男性积极参与避孕 。吴久玲 Oratai Rauyajin Suvajee Good Vanawipha Pasandha-Natorn 王临虹 2001中华流行病学杂志2001,22,3:57

4Safety of endoscopic retrograde cholangiopancreatography in pregnancy: Fluoroscopy time and fetal exposure, does it matter?显示文摘AIM: To estimate the fetal radiation exposure using thermoluminescent dosimeters (TLD's) in pregnant patients undergoing endoscopic retrograde cholangiopancreatography (ERCP) and assess its relevance. METHODS: Data on thirty-five therapeutic ERCPs conducted in pregnant patients from 2001 to 2009 were retrieved from a prospective database. Techniques to minimize fluoroscopy time were implemented and the fluoroscopy times captured. TLD's were placed on the mother to estimate the fetal radiation exposure and the results were compared to the maximum allowed dose of radiation to the fetus [0.005 gray (Gy)]. Obstetrics consultations were obtained and the fetus was monitored before and after the ERCP. Fluoroscopy wasperformed at 75 kVp. ERCP was performed with the patients supine by dedicated biliary endoscopists performing more than 500 cases a year. RESULTS: A total of 35 pregnant patients underwent ERCP and biliary sphincterotomy (14 in first trimester, 11 in second trimester, and 10 in third trimester). Mean maternal age was 25 years (range 16-37 years) and mean gestational age was 18.9 wk (range 4-35 wk). Mean fluoroscopy time was 0.15 min (range 0-1 min). For 23 women, the estimated fetal radiation exposure was almost negligible (< 0.0001 Gy) while for 8 women, it was within the 0.0001-0.0002 Gy range. Three women had an estimated fetal radiation exposure between 0.0002 and 0.0005 Gy and 1 woman had an estimated fetal radiation exposure greater than 0.0005 Gy. Complications included 2 post-sphincterotomy bleeds, 2 post-ERCP pancreatitis, and 1 fatal acute respiratory distress syndrome. One patient developed cholecystitis 2 d after ERCP. 6Use of scoring systems for assessing and reporting the outcome results from shoulder surgery and arthroplasty显示文摘To investigate shoulder scoring systems used in Europe and North America and how outcomes might be classified after shoulder joint replacement. All research papers published in four major journals in 2012 and 2013 were reviewed for the shoulder scoring systems used in their published papers. A method of identifying how outcomes after shoulder arthroplasty might be used to categorize patients into fair, good, very good and excellent outcomes was explored using the outcome evaluations from patients treated in our own unit. A total of 174 research articles that were published in the four journals used some form of shoulder scoring system. The outcome from shoulder arthroplasty in our unit has been evaluated using the constant score(CS) and the oxford shoulder score and these scores have been used to evaluate individual patient outcomes. CSs of < 30 = unsatisfactory; 30-39 = fair; 40-59 = good; 60-69 = very good; and 70 and over = excellent. 7早产儿视网膜病变早期治疗(ETROP)过程中发生视网膜脱离眼的预后显示文摘研究对象:在早产儿视网膜病变早期治疗(ETROP)研究中因早产儿视网膜病变(ROP)而发生视网膜脱离眼的结构以及功能改变。 研究方法:加入ETROP研究中的新生儿患有双眼高危阈前ROP,随即对其一眼进行早期治疗,另一眼进行传统治疗。只有单眼患病的早产儿。随机对高危阈前ROP眼进行早期治疗或传统治疗。当视网膜脱离被发现时。研究者会继续观察患者病变或对其行视网膜玻璃体手术(如巩膜加压术以肜或者玻璃体切除术)。在矫正9月龄时,对患者视网膜状况进行评估并使用条栅视敏度(grating acuity)对患儿视力进行检测。 研究结果:401例高危阈前ROP患者加入了ETROP研究。63例患者(89只眼)发生了视网膜脱离。对其中56例患者(78只眼)进行了随访。21例患者(38%)的视网膜脱离发生于双眼。其中30只眼分级为4A,14只眼为4B,16只眼为5级。余下的18只视网膜脱离眼未进行分级。对11例患者(12只眼)监控病情变化,52例患者(66只眼)行玻璃体视网膜手术。行玻璃体切除伴或不伴有巩膜加压术的56只眼中有17只(30%)在患者9月龄时保持或达到了黄斑复位;仅行巩膜加压术的10只眼中,6只(60%)达到;未行手术的12只眼中,2只(17%)达到。78只眼中有13只(17%)达到了较好的视力水平(I〉1.85cycles/degree)。达到正常视力水平(≥3.70cycles/degree)的6只眼的视网膜脱离分级均为4A(其分别为6只未行手术眼中的1眼,6只行巩膜加压术眼中的3眼。18只行玻璃体切除术眼中的2眼)。对11只视网膜脱离达5级的患眼行玻璃体视网膜手术。 治疗结果为6只眼无光感,3只眼仅有光感,2只眼只能看清低视力表。结论:在ETROP的研究中,ROP导致视网膜脱离的治疗结果普遍较差。因视网膜脱离行玻璃体视网膜手术的48只眼中,有16只眼黄斑未发生脱离。手术修复4A级视网膜脱离的24只眼中,有5只(21%)在术后达到了正常视力水平。对视网膜脱离5级眼行玻璃体视网膜手术可以使其达到结构复位,但功能恢复不佳。Michael X. 10Development and Application of High Performance X80 Line Pipe for the 2^(nd) West-East Gas Pipeline显示文摘It is the development tendency of gas pipeline to improve strength of line pipe and transmission pressure, which is an important symbol of the progress of gas pipeline technology. The 2nd West-East Gas Pipeline (the 2 nd WEGP) is the largest-scale X80 high pressure gas pipeline project in the world. Through the joint development project by Pipeline Company, research institute, and steel company, a series of achievements of technology have been obtained, which makes it possible to apply pressure of 12 MPa and X80 LSAW and HSAW pipe for the 2 nd WEGP. Specifications of X80 steel and pipe have been issued based on API 5L. High performance X80 steel and pipe have been developed and meets the Specification. General design conception of applying HSAW pipe and LSAW pipe on gas transmission trunk line with large diameter and high pressure has been established. Failure control of the 2 nd WEGP has been carried out to avoid failure and reduce the lost which based on analysis of pipeline failure mode. 18In silico Proteome-wide Amino aCid and Elemental Composition (PACE) Analysis of Expression Proteomics Data Provides A Fingerprint of Dominant Metabolic Processes显示文摘Proteome-wide Amino aCid and Elemental composition(PACE) analysis is a novel and informative way of interrogating the proteome.The PACE approach consists of in silico decomposition of proteins detected and quantifed in a proteomics experiment into 20 amino acids and fve elements(C,H,N,O and S),with protein abundances converted to relative abundances of amino acids and elements.The method is robust and very sensitive;it provides statistically reliable differentiation between very similar proteomes.In addition,PACE provides novel insights into proteome-wide metabolic processes,occurring,e.g.,during cell starvation.For instance,both Escherichia coli and Synechocystis down-regulate sulfur-rich proteins upon sulfur deprivation,but E.coli preferentially down-regulates cysteine-rich proteins while Synechocystis mainly downregulates methionine-rich proteins.Due to its relative simplicity,flexibility,generality and wide applicability,PACE analysis has the potential of becoming a standard analytical tool in proteomics.David M.
