3美国临床肿瘤学会IV期非小细胞肺癌化疗的临床实践指南更新显示文摘本文旨在为IV期非小细胞肺癌患者的治疗提供更新版推荐。本文资料检索源自2002年以来公布的相关随机试验文献。此指南范围限于化疗与生物治疗。更新委员会对这些文献进行了总结并提供了推荐更新。162篇文献符合标准被纳入参考。本推荐基于可改善总生存期的治疗方法。仅改善无进展生存期的治疗方法推动了对毒性及生存质量的监测。对于体力状态评分为0分或1分患者的一线治疗,可推荐以铂类为基础的细胞毒性药物的两药联用。对铂类治疗有禁忌的患者,可采用非铂类细胞毒性两药联合。对于体力状态评分为2分的患者,单一细胞毒性药物即可。对于疾病进展或经过4个周期的治疗仍对治疗无反应的患者,应停止一线细胞毒性化疗。即使在6个周期后患者对治疗仍有反应,亦应停止两药细胞毒性化疗。对于伴有明确的表皮生长因子受体(epidermal growth factor receptor,EGFR)突变的患者,可推荐一线采用吉非替尼治疗;对于EGFR突变为阴性或不明确的患者,细胞毒性化疗更佳。除具有特定临床特征的患者外,可推荐贝伐单抗与卡铂-紫杉醇联用。对于通过免疫组化证实EGFR阳性的肿瘤患者,可推荐西妥昔单抗与顺铂-长春瑞滨联用。多西紫杉醇、厄洛替尼、吉非替尼或培美曲塞被推荐作为二线治疗。对于未曾接受过厄洛替尼或吉非替尼治疗的患者,可推荐厄洛替尼作为三线治疗。现有数据不足以推荐常规三线采用细胞毒性药物。已有的证据也不足以推荐常规应用分子标记物选择化疗。Christopher G. 4Crop Phenomics and High-Throughput Phenotyping:Past Decades,Current Challenges,and Future Perspectives显示文摘Since whole-genome sequencing of many crops has been achieved,crop functional genomics studies have stepped into the big-data and high-throughput era.However,acquisition of large-scale phenotypic data has become one of the major bottlenecks hindering crop breeding and functional genomics studies.Nevertheless,recent technological advances provide us potential solutions to relieve this bottleneck and to explore advanced methods for large-scale phenotyping data acquisition and processing in the coming years.In this article,we review the major progress on high-throughput phenotyping in controlled environments and field conditions as well as its use for post-harvest yield and quality assessment in the past decades.We then discuss the latest multi-omics research combining high-throughput phenotyping with genetic studies.Finally,we propose some conceptual challenges and provide our perspectives on how to bridge the phenotype-genotype gap.It is no doubt that accurate high-throughput phenotyping will accelerate plant genetic improvements and promote the next green revolution in crop breeding.Wanneng Yang Hui Feng Xuehai Zhang Jian Zhang John H.Doonan William David Batchelor Lizhong Xiong Jianbing Yan 2020Molecular Plant2020,13,2:38

8The Ozone Component of Global Change:Potential Effects on Agricultural and Horticultural Plant Yield,Product Quality and Interactions with Invasive Species显示文摘The productivity,product quality and competitive ability of important agricultural and horticultural plants in many regions of the world may be adversely affected by current and anticipated concentrations of ground-level ozone(O3).Exposure to elevated O3 typically results in suppressed photosynthesis,accelerated senescence,decreased growth and lower yields.Various approaches used to evaluate O3 effects generally concur that current yield losses range from 5% to 15% among sensitive plants.There is,how-ever,considerable genetic variability in plant responses to O3.To illustrate this,we show that ambient O3 concentrations in the eastern United States cause substantially different levels of damage to otherwise similar snap bean cultivars.Largely undesirable effects of O3 can also occur in seed and fruit chemistry as well as in forage nutritive value,with consequences for animal production.Ozone may alter herbicide efficacy and foster estab-lishment of some invasive species.We conclude that current and projected levels of O3 in many regions worldwide are toxic to sensitive plants of agricultural and horticultural significance.Plant breeding that incorporates O3 sensitivity into selection strategies will be increasingly necessary to achieve sustainable production with changing atmospheric composition,while reductions in O3 precursor emissions will likely benefit world food pro-duction and reduce atmospheric concentrations of an important greenhouse gas.Fitzgerald Booker Russell Muntifering Margaret McGrath Kent Burkey Dennis Decoteau Edwin Fiscus William Manning Sagar Krupa Arthur Chappelka David Grantz 2009Journal of Integrative Plant Biology2009,51,4:18

12Factors associated with time to laparoscopic cholecystectomy for acute cholecystitis显示文摘AIM: To determine patient and process of care factors associated with performance of timely laparoscopic cholecystectomy for acute cholecystitis. METHODS: A retrospective medical record review of 88 consecutive patients with acute cholecystitis was conducted. Data collected included demographic data, co-morbidities, symptoms and physical findings at presentation, laboratory and radiological investigations, length of stay, complications, and admission service (medical or surgical). Patients not undergoing cholecystectomy during this hospitalization were excluded from analysis. Hierarchical generalized linear models were constructed to assess the association of pre-operative diagnostic procedures, presenting signs, and admitting service with time to surgery.RESULTS: Seventy cases met inclusion and exclusion criteria, among which 12 were admitted to the medical service and 58 to the surgical service. Mean ± SD time to surgery was 39.3 ± 43 h, with 87% of operations performed within 72 h of hospital arrival. In the adjusted models, longer time to surgery was associated with number of diagnostic studies and endoscopic retrograde cholangio-pancreatography (ERCP, P = 0.01) as well with admission to medical service without adjustment for ERCP (P < 0.05). Patients undergoing both magnetic resonance cholangiopancreatography (MRCP) and computed tomography (CT) scans experienced the longest waits for surgery. Patients admitted to the surgical versus medical service underwent surgery earlier (30.4 ± 34.9 vs 82.7 ± 55.1 h, P < 0.01), had less post-operative complications (12% vs 58%, P < 0.01), and shorter length of stay (4.3 ± 3.4 vs 8.1 ± 5.2 d, P < 0.01).CONCLUSION: Admission to the medical service and performance of numerous diagnostic procedures, ERCP, or MRCP combined with CT scan were associated with longer time to surgery. 17Macrophage inhibitory cytokine-1/growth differentiation factor-15 in premalignant and neoplastic tumours in a high-risk pancreatic cancer cohort显示文摘BACKGROUND Pancreatic cancer(PC)is a leading cause of cancer related mortality worldwide,with poor survival due to late diagnosis.Currently,biomarkers have limited use in early diagnosis of PC.Macrophage inhibitory cytokine-1 or growth differentiation factor-15(MIC-1/GDF15)has been implicated as a potential serum biomarker in PC and other malignancies.AIM To determine the role of MIC-1/GDF15 in detecting pre-malignant pancreatic lesions and neoplastic tumours in an asymptomatic high-risk cohort part of Australian Pancreatic Cancer Screening Program.METHODS A feasibility prospective single centre cohort study was performed.Participants recruited for yearly surveillance with endoscopic ultrasound(EUS)had serial fasting blood samples collected before EUS for MIC-1/GDF15,C-reactive protein and carbohydrate antigen 19-9.Patients were stratified into five groups based on EUS findings:Normal;pancreatic cysts,branch-duct intraductal papillary mucinous neoplasm;diffuse non-specific abnormalities;and neoplastic tumours.MIC-1/GDF15 serum levels were quantified using ELISA.Participants in whom EUS demonstrated abnormalities but not malignancy were closely followed up with magnetic resonance imaging(MRI)or computed tomography.RESULTS One hundred twenty participants were prospectively recruited from 2011-2018.Forty-seven participants(39.2%)had an abnormal EUS and five participants(4.2%)were diagnosed with neoplastic tumours,three by EUS(two pancreatic and one liver)and two by MRI/computed tomography(breast cancer,bladder cancer),which were performed for follow up of abnormal EUS.Baseline serum MIC-1/GDF15 was a significant predictor of neoplastic tumours on receiver operator characteristic curve analysis[area under curve(AUC)=0.814,P=0.023].Baseline serum MIC-1/GDF15 had moderate predictive capacity for branch-duct intraductal papillary mucinous neoplasm(AUC=0.644)and neoplastic tumours noted on EUS(AUC=0.793),however this was not significant(P=0.188 and 0.081 respectively).Serial serum MIC-1/GDF15 did not demonstrate a significant percentage change between a normal and abnormal EUS(P=0.213).Median baseline MIC-1/GDF15 was greater in those with neoplastic tumours(Median=1039.6,interquartile range=727.0-1977.7)compared to those diagnosed with a benign lesion(Median=570.1,interquartile range=460.7-865.2)on EUS and MRI(P=0.012).CONCLUSION In this pilot study MIC-1/GDF15 has predictive capacity for neoplastic tumours in asymptomatic individuals with a genetic predisposition for PC.Further imagining may be warranted in patients with abnormal EUS and raised serum MIC-1/GDF15.Larger multicentric prospective studies are required to further define the role of MIC-1/GDF15 as a serological biomarker in pre-malignant pancreatic lesions and neoplastic tumours.Robert Sean O’Neill Sam Emmanuel David Williams Alina Stoita 2020World Journal of Gastroenterology2020,26,14:8
18History and Progress of the North American Soil Geochemical Landscapes Project,2001-2010显示文摘In 2007,the U.S.Geological Survey,the Geological Survey of Canada,and the Mexican Geological Survey initiated a low-density(1 site per 1600 km2,13323 sites) geochemical and mineralogical survey of North American soils(North American Soil Geochemical Landscapes Project).Sampling and analytical protocols were developed at a series of workshops in 2003-2004 and pilot studies were conducted from 2004-2007.The ideal sampling protocol at each site includes a sample from 0-5 cm depth,a composite of the soil A horizon,and a sample from the soil C horizon.The <2-mm fraction of each sample is analyzed for Al,Ca,Fe,K,Mg,Na,S,Ti,Ag,Ba,Be,Bi,Cd,Ce,Co,Cr,Cs,Cu,Ga,In,La,Li,Mn,Mo,Nb,Ni,P,Pb,Rb,Sb,Sc,Sn,Sr,Te,Th,Tl,U,V,W,Y,and Zn by inductively coupled plasma-mass spectrometry and inductively coupled plasma-atomic emission spectrometry following a near-total digestion in a mixture of HCl,HNO3,HClO4,and HF.Separate methods are used for As,Hg,Se,and total C on this same size fraction.The major mineralogical components are determined by a quantitative X-ray diffraction method.Sampling in the conterminous U.S.was completed in 2010(c.4800 sites) with chemical and mineralogical analysis currently underway.In Mexico,approximately 66% of the sampling(871 sites) had been done by the end of 2010 with completion expected in 2012.After completing sampling in the Maritime provinces and portions of other provinces(472 sites,7.6% of the total),Canada withdrew from the project in 2010.Preliminary results for a swath from the central U.S.to Florida clearly show the effects of soil parent material and climate on the chemical and mineralogical composition of soils.A sample archive will be established and made available for future investigations.David B.Smith William F.Cannon Laurel G.Woodruff Francisco Moreira Rivera Andrew N.Rencz Robert G.Garrett 2012地学前缘2012,19,3:7
