| 1 | 应用中国商环包皮环切手术标准化方案对328例成年男性包皮环切的临床报告显示文摘目的:成年男性包皮环切手术标准化和培训可显著降低因手术引起的并发症发生率。由于目前国内外尚无可供参考的成年男性包皮环切手术标准化方案和培训制度,使得临床医生开展成年男性包皮环切手术的质量无法保证。为了建立成年男性包皮环切手术标准化和培训制度,便于临床医生和相关医护人员学习和掌握包皮环切器的临床应用技巧,以中国商环(简称'商环,ShangRing')包皮环切技术为例,根据商环的技术特点和临床实践经验,特别制订了商环包皮环切手术标准化方案,并将其标准化操作运用于临床。方法:采用商环包皮环切手术标准化方案对328例受术者(年龄18~58岁,平均27.8岁;已婚134例,未婚194例;包茎25例,包皮过长303例)进行包皮环切手术。对手术时间、疼痛评分(视觉模拟评分法,VAS)、术后并发症、包皮创口愈合时间、术后外观满意情况进行观察和随访。结果:手术时间为(4.70±1.31)min。手术时受术者疼痛(VAS)评分为(0.24±0.59)分,术后24h疼痛评分为(1.59±0.95)分,去环前24h内疼痛评分(1.72±1.11)分,去环时疼痛评分为(2.72±1.37)分。主要并发症发生率为术后感染0.6%(2/328)、出血0.6%(2/328)、伤口裂开0.6%(2/328)和水肿4.89%(16/328)。在发生出血和伤口裂开的受术者中无1例需要缝合。术后包皮创口完全愈合时间为(20.30±6.69)d。受术者对术后外观满意率99.7%(327/328)。结论:商环包皮环切术具有手术方式易于标准化、手术时间短、术中疼痛轻、术后无须特殊护理、容易完整保留系带,并完全暴露冠状沟、术后外观满意度高、受术者易于接受等优点。初步证明了中国商环包皮环切手术标准化方案的可接受性,严格按照此方案实施手术和随访护理,能最大化地实现商环包皮环切技术的诸多临床优势,将并发症发生率控制在可接受的范围内。 | 程跃 彭弋峰 刘毅东 田龙 吕年青 苏新军 严泽军 胡嘉盛 Richard LEE Howard KIM David C. | 4 | 泽泻提取物Alisol Monoacetate A和B对HepG2细胞株胆固醇代谢的影响显示文摘目的:考察泽泻有效成分Alisol Monoacetate A和B对人类肝癌细胞株(HepG2)胆固醇合成与代谢的影响。方法:以HepG2细胞为模型,Lipitor为阳性对照,加入不同浓度的Alisol Monoacetate A和B,培养24h后分别收集与测定培养液及细胞内的胆固醇含量。结果:Alisol Monoacetate A和B在10μM、20μM浓度时开始出现10%左右的细胞毒性,但线粒体代谢活性却增强约50%。而当浓度超过50μM时细胞毒性显著增强达70%以上。随着Alisol Monoacetate A和B浓度的增加(0、3、10、20μM),细胞内胆固醇含量也显著升高,分别为24.4、26.7、32.3、38.3μg/mg蛋白质,存在正量效关系;但培养液内的胆固醇含量无明显不同。结论:Alisol Monoac-etate A和B可能具有增强细胞的线粒体代谢活性而促进HepG2合成胆固醇的作用,其机制有待进一步研究。 | 吴水生 郭改革 施红 王宏 David Lee | 2007 | 中华中医药杂志2007,22,7: | 21 |

| 7 | Endoscopy and polyps-diagnostic and therapeutic advances in management显示文摘Despite multiple efforts aimed at early detection through screening, colon cancer remains the third leading cause of cancer-related deaths in the United States, with an estimated 51000 deaths during 2013 alone. The goal remains to identify and remove benign neoplastic polyps prior to becoming invasive cancers. Polypoid lesions of the colon vary widely from hyperplastic, hamartomatous and inflammatory to neoplastic adenomatous growths. Although these lesions are all benign, they are common, with up to one-quarter of patients over 60 years old will develop pre-malignant adenomatous polyps. Colonoscopy is the most effective screening tool to detect polyps and colon cancer, although several studies have demonstrated missed polyp rates from 6%-29%, largely due to variations in polyp size. This number can be as high as 40%, even with advanced (> 1 cm) adenomas. Other factors including sub-optimal bowel preparation, experience of the endoscopist, and patient anatomical variations all affect the detection rate. Additional challenges in decision-making exist when dealing with more advanced, and typically larger, polyps that have traditionally required formal resection. | 8 | 急性心肌梗死医疗保险患者介入治疗与药物治疗强度区域变化对远期预后的影响显示文摘背景:与药物治疗比较,急性心肌梗死(acute myocardial infarction,AMI)患者介入治疗强度存在显著的地区差异,这种差异在卫生及政策方面的意义还不清楚。目标:评价接受积极治疗患者的相关临床特征;评估居住于积极采取介入治疗策略地区的AMI患者与居住于积极采取药物治疗策略地区的患者相比,是否具有更好的远期生存率。设计、地点及患者:在158831例老年医疗保险患者中进行该项全国性队列研究。患者于1994~1995年因首次发生并确诊AMI而入院,随访7年(平均随访3.6年)。根据居住地区介入治疗(30天内接受心导管手术)与药物治疗(出院时对适宜的患者应用β-阻滞剂)强度进行分析。基线资料来自心血管协作项目,并与医疗保险管理资料联合。主要观察指标:7年随访期间的远期生存率。结果:各地区AMI基线严重程度相似。在所有地区,较年轻、较健康的患者与年老的高危患者相比,更可能接受介入治疗和药物治疗。更多实施介入治疗的地区具备更多的心脏导管室条件;这些地区的患者无论年龄、临床指征还是危险特点如何,均更多接受介入治疗。介入和药物治疗策略采用率最高的地区与介入和药物治疗采用率最低的地区,两者未经校正的7年绝对生存率差异为6.2%。对于ST抬高和非ST抬高AMI患者,其生存率随着地区介入治疗和药物治疗强度的增高而升高。在药物治疗比例较高的地区,随着介入治疗的增加,相应的生存率提高很少甚或没有提高。结论:老年医疗保险AMI患者接受较为积极的药物治疗对改善人群的生存有益。 | Therese A. | 9 | Portal hypertension: Imaging of portosystemic collateral pathways and associated image-guided therapy显示文摘Portal hypertension is a common clinical syndrome, defined by a pathologic increase in the portal venous pressure. Increased resistance to portal blood flow, the primary factor in the pathophysiology of portal hypertension, is in part due to morphological changes occurring in chronic liver diseases. This results in rerouting of blood flow away from the liver through collateral pathways to low-pressure systemic veins. Through a variety of computed tomographic, sonographic, magnetic resonance imaging and angiographic examples, this article discusses the appearances and prevalence of both common and less common portosystemic collateral channels in the thorax and abdomen. A brief overview of established interventional radiologic techniques for treatment of portal hypertension will also be provided. Modern studies in the 1970 s revealed that isoflavonoids extracted from P. lobata were the bioactive components of an herbal remedy namely Yufeng Ningxin Tablets for the treatment of patients after stroke. This article reviews recent application of P. lobota in the treatment of diabetics and in reducing alcohol drinking. | 17 | Impact of disease severity on gastric residual volume in critical patients显示文摘AIM: To investigate whether illness severity has an impact on gastric residual volume (GRV) in medical critically ill patients. METHODS: Medical intensive care unit (ICU) patients requiring nasogastric feeding were enrolled. Sequential Organ Failure Assessment (SOFA) score was assessed immediately preceding the start of the study. Acute Physiology and Chronic Health Evaluation (APACHE) Ⅱ scores were recorded on the first, fourth, seventh, and fourteenth day of the study period. GRV was measured every 4 h during enteral feeding. The relationship be-tween mean daily GRV and SOFA scores and the correlation between mean daily GRV and mean APACHE Ⅱ score of all patients were evaluated and compared. RESULTS: Of the 61 patients, 43 patients were survivors and 18 patients were non-survivors. The mean daily GRV increased as SOFA scores increased (P < 0.001, analysis of variance). Mean APACHE Ⅱ scores of all patients correlated with mean daily GRV (P = 0.011, Pearson correlation) during the study period. Patients with decreasing GRV in the first 2 d had better survival than patients without decreasing GRV (P = 0.017, log rank test). CONCLUSION: GRV is higher in more severely ill medical ICU patients. | 20 | A comparison of three methods of decellularization of pig corneas to reduce immunogenicity显示文摘·AIM: To investigate whether decellularization using different techniques can reduce immunogenicity of the cornea, and to explore the decellularized cornea as a scaffold for cultured corneal endothelial cells(CECs).Transplantation of decellularized porcine corneas increases graft transparency and survival for longer periods compared with fresh grafts.·METHODS: Six-month-old wild-type pig corneas were cut into 100-200 μm thickness, and then decellularized by three different methods: 1) 0.1% sodium dodecyl sulfate(SDS); 2) hypoxic nitrogen(N2); and 3) hypertonic NaCl. Thickness and transparency were assessed visually. Fresh and decellularized corneas were stained with hematoxylin/eosin(H&E), and for the presence of galactose-α1,3-galactose(Gal) and N-glycolylneuraminic acid(NeuGc, a nonGal antigen). Also, a human IgM/IgG binding assay was performed. Cultured porcine CECs were seeded on the surface of the decellularized cornea and examined after H&E staining.· RESULTS: All three methods of decellularization reduced the number of keratocytes in the stromal tissue by >80% while the collagen structure remained preserved. No remaining nuclei stained positive for Gal or NeuGc, and expression of these oligosaccharides on collagen was also greatly decreased compared to expression on fresh corneas. Human IgM/IgG binding to decellularized corneal tissue was considerably reduced compared to fresh corneal tissue.
