1Role of the renin angiotensin system in diabetic nephropathy显示文摘Diabetic nephropathy has been the cause of lot of mor bidity and mortality in the diabetic population.The renin angiotensin system(RAS) is considered to be involved in most of the pathological processes that result in diabetic nephropathy.This system has var ious subsystems which contribute to the disease pat ho logy.One of these involves angiotensin Ⅱ(Ang Ⅱ) which shows in c reased activity during diabetic nephropathy.This causes hypertrophy of various renal cells and has a pres sor effect on arteriolar smooth muscle resulting in increas ed vascular pressure.Ang Ⅱ also induces inflamm ation,apoptosis,cell growth,migration and differentiation.Monocyte chemoattractant protein-1 production responsible for renal f ibrosis is also regulated by RAS.Polymorphism of angiotensin converting enzym e(ACE) and Angiotensinogen has been shown to have effects on RAS.Available treatment modalities have proven effective in controlling the progression of nephropathy.Various drugs(based on antagonism of RAS) are currently in the market and others are still under trial.Amongst the approved drugs,ACE inhibitors and ang iot ensin receptor blockers(ARBs) are widely used in clinical practice.ARBs are shown to be superior to ACE inhibitors in terms of reducing proteinuria but the combined role of ARBs with ACE inhibitors in diabetic nephropathy is under debate.Tanuj Chawla Deepika Sharma Archana Singh 2010World Journal of Diabetes2010,1,5:34
2Scrub typhus:pathophysiology,clinical manifestations and prognosis显示文摘Scrub typhus is a zoonosis caused by the pathogen Orientia tsutsugamushi(O.tsvtsugamushi). 4Retinal structural-vascular-functional relationship using optical coherence tomography and optical coherence tomography -angiography in myopia显示文摘Background:To examine the retinal structure–vascular-function relationship using optical coherence tomography(OCT)and optical coherence tomography angiography(OCTA)in myopia.Methods:This was a prospective cross-sectional study comprising 86 eyes of 45 individuals with varying axial lengths and spherical equivalents and no posterior segment abnormalities.All eyes underwent optical coherence tomography with the Spectralis SD-OCT and OCTA with RTVue-XR Avanti;Optovue.Individual macular retinal layer thicknesses and flow areas and vessel densities were measured on OCT and OCTA,respectively.Linear correlations were made between the macular layer thicknesses,flow areas and vessel densities with axial length,spherical equivalent and visual acuity.Results:The participants’mean ages were 33.34±14.45 years,mean spherical equivalent refractions were−7.17±5.71 D and axial lengths were 25.95±2.41 mm.There were significant positive correlations of foveal angle(r=0.757,p=0.001),inner retinal(r=0.764,p=0.001)and outer plexiform layer(r=0.771,p=0.001)thickness on OCT and vessel densities in deep capillary plexus(r=0.313,p=0.003)on OCTA with axial length and negative correlations with spherical equivalents and visual acuity.Significant negative correlations of outer nuclear layer(r=−0.560,p=0.03)and photoreceptor outer segment layer thickness(r=−0.856,p<0.001)were noted on OCT with axial length and positive correlations with spherical equivalents and visual acuity.Conclusion:The lateral retinal stretching in myopia could possibly explain the correlation between retinal layer thickness,vascular density and visual acuity in these eyes.Further research is required to investigate this.Ramesh Venkatesh Shivani Sinha Deepika Gangadharaiah Santosh G.K.Gadde Ashwin Mohan Rohit Shetty Naresh Kumar Yadav 2019Eye and Vision2019,6,1:7

6Association of asymptomatic otitis media with effusion in patients with adenoid hypertrophy显示文摘Objective: Clinical symptoms of otitis media with effusion are rarely brought forward to the guardians of young children who the disease is most prevalent in. This often leads to poor scholastic performances and difficult social interactions. The objective of this study was to identify asymptomatic cases of otitis media with effusion present in individuals with adenoid hypertrophy. Material and Methods: In a cross sectional study advocated in Justice K.S.Hegde Hospital, Karnataka India we evaluated one hundred patients above the age of three from August 2016 to December 2017. Candidates who presented with an adenoid nasopharyngeal ratio of more than 0.5 were selected for the study. Individuals who complained of otological symptoms were not considered for the study. Patients cleared of other pathological otological conditions were underwent audiological evaluation with pure tone audiometry and tympanometry for evaluating the middle ear status and hearing loss. Results: The study showed a total of 36% of patients evaluated presented with asymptomatic otitis media with effusion. In candidates who presented with a bilateral B tympanogram, 40% had significant conductive hearing loss of more than 25dB. Conclusion: An objective test such as impedance audiometry in all patients with adenoid hypertrophy would aid in the diagnosis of fluid in the middle ear, so that timely intervention can be done and possible complications be averted.Vadisha Bhat Ivan Paraekulam Mani Rajeshwary Aroor Marina Saldanha M.K. 7Early stage colon cancer: Current treatment standards, evolving paradigms, and future directions显示文摘Colon cancer continues to be one of the leading causes of mortality and morbidity throughout the world despite the availability of reliable screening tools and effective therapies.The majority of patients with colon cancer are diagnosed at an early stage(stages I to III),which provides an opportunity for cure.The current treatment paradigm of early stage colon cancer consists of surgery followed by adjuvant chemotherapy in a select group of patients,which is directed at the eradication of minimal residual disease to achieve a cure.Surgery alone is curative for the vast majority of colon cancer patients.Currently,surgery and adjuvant chemotherapy can achieve long term survival in about two-thirds of colon cancer patients with nodal involvement.Adjuvant chemotherapy is recommended for all patients with stage III colon cancer,while the benefit in stage II patients is not unequivocally established despite several large clinical trials.Contemporary research in early stage colon cancer is focused on minimally invasive surgical techniques,strategies to limit treatment-related toxicities,precise patient selection for adjuvant therapy,utilization of molecular and clinicopathologic information to personalize therapy and exploration of new therapies exploiting the evolving knowledge of tumor biology.In this review,we will discuss the current standard treatment,evolving treatment paradigms,and the emerging biomarkers,that will likely help improve patient selection and personalization of therapy leading to superior outcomes.Sakti Chakrabarti Carrie Y Peterson Deepika Sriram Amit Mahipal 2020World Journal of Gastrointestinal Oncology2020,12,8:4

10Splenic injuries secondary to colonoscopy:Rare but serious complication显示文摘BACKGROUND Colonoscopy is a safe and routine diagnostic and therapeutic procedure for evaluation of large bowel diseases.Most common procedure related complications include bleeding and perforation but rarely a splenic Injury.AIM To investigate the reason for colonoscopy,presentation of patient with spleen injury,types of injury,diagnosis,management and outcomes of patients METHODS A structured search on four databases was done and 45 articles with 68 patients were selected.The reason for colonoscopy,presentation of patient with spleen injury,types of injury,diagnosis,management and outcomes of patients were identified and analyzed using SPSS.RESULTS The mean age of the patients was 62.7 years with 64%females.Twenty two percent had a complete splenic rupture with colonoscopy while 63%had subcapsular hematoma,spleen laceration and spleen avulsion.The most common reason for colonoscopy was screening(46%)followed by diagnostic colonoscopy(28%).Eighty seven percent of patients presented with abdominal pain.Patients with spleen rupture mostly required splenectomy(47%),while minor spleen hematomas and lacerations were managed conservatively(38%).Six percent of the patients were managed with proximal splenic artery splenic embolization and 4%were managed with laparoscopic repair.The overall mortality was 10%while 77%had complete recovery.The reason of colonoscopy against presentation specifically,abdominal pain showed no statistical significance P=0.69.The indication of colonoscopy had no significant impact on incidence of splenic injury(P=0.89).Majority of the patients(47%)were managed with splenectomy while the rest were managed conservatively(P=0.04).This association was moderately strong at a cramer’s V test(0.34).The Fisher exact test showed a higher mortality with spleen rupture(P=0.028).CONCLUSION Spleen rupture due to colonoscopy is a significant concern and is associated with high mortality.The management of the patients can be individualized based on clinical presentation.Waqas Ullah Mamoon Ur Rashid Asif Mehmood Yousaf Zafar Ishtiaq Hussain Deepika Sarvepalli Muhammad Khalid Hasan 2020World Journal of Gastrointestinal Surgery2020,12,2:4
11Evaluation of immunogenicity and reactogenicity of recombinant DNA hepatitis B vaccine produced in India显示文摘AIM: (1) To gain information on immune responses to an accelerated schedule of 0, 1, and 2 mo in paramedical staff and BDS students who are at an increased risk of getting hepatitis B infection and come under high risk groups. (2) To assess the efficacy and safety of EnivacHB in different age groups, using genetically modified yeast strain Pichia pastoris, a new recombinant hepatitis B vaccine developed and manufactured in India.METHODS: A prospective, comparative, and single blinded trial of rapid (0, 1, and 2 mo) hepatitis B immunization schedulewas reported. A total of three hundred and seven (212 females and 95 males) healthy volunteers divided into three age groups (18-29, 30-39,and 40-49) were enrolled after screening for markers of hepatitis B. All the volunteers received 20 mg of the vaccine intramuscularly at 0, 1, and 2 mo.RESULTS: Geometric mean titers were calculated pre and post vaccination. Before immunization the GMT was 0.0124 mIU/mL. One month after the administration of the third dose of recombinant vaccine 296/307 (96.5%)subjects achieved seroprotective levels of anti-HBs. The geometric mean anti-HBs titers achieved after one month of the third dose was 2 560.0 mIU/mL. The geometric mean anti-HBs titer of males was 2 029.0 mIU/mL, while that of the females was 2 759.0 mIU/mL. In the age group of 18-29 years, anti-HBs titer was 3 025.0 mIU/mL, while that in the age group of 30-39 years was 2096.0 mIU/mL. In third age group of 40-49 years, antiHBs titer was 1 592.0 mIU/mL. Hyper-responses (antiHBs≥100 mIU/mL) were shown in 88.0% (271/307) of subjects. Eleven (3.5%) subjects responded poorly to the vaccine in the age group of 40-49 years. 12Choice of dialysis modality prior to kidney transplantation:Does it matter?显示文摘The population of patients with end stage renal disease(ESRD) is increasing,lengthening waiting lists for kidney transplantation.Majority of the patients are not able to receive a kidney transplant in timely manner even though it is well established that patient survival and quality of life after kidney transplantation is far better when compared to being on dialysis.A large number of patients who desire a kidney transplant ultimately end up needing some form of dialysis therapy.Most of incident ESRD patients choose hemodialysis(HD) over peritoneal dialysis(PD) as the modality of choice in the United States,even though studies have favored PD as a better choice of pre-transplant dialysis modality than HD.PD is largely underutilized in the United States due to variety of reasons.As a part of the decision making process,patients are often educated how the choice regarding modality of dialysis would fit into their life but it is not clear and not usually discussed,how it can affect eventual kidney transplantation in the future.In this article we would like to discuss ESRD demographics and outcomes,modality of dialysis and kidney transplant related events.We have summarized the data comparing PD and HD as the modality of dialysis and its impact on allograft and recipient outcomes after kidney transplantation.Deepika Jain Danny B Haddad Narender Goel 2019World Journal of Nephrology2019,8,1:3

14A rare cause of drug-induced hepatitis in an immunocompromised patient and the role of glutathione显示文摘The Food and Drug Administration (FDA) has issued a warning on numerous herbal drugs, including many popular products at General Nutrition Centers (GNC), regarding unstudied hepatotoxicity. There have been recent reports of GNC products such as hydroxycut and herbalife, causing drug-induced hepatitis. Herbal medications are over-the-counter products and are not investigated thoroughly by the FDA. Given that the mostcommon outpatient laboratory abnormality is elevated liver transaminases, a sign of hepatocellular toxicity; it is not surprising that some of these products end up causing hepatic dysfunction, especially when taken in large volume. There are numerous herbal supplements that are hepatotoxic, however, these medications have a much more significant effect in human immunodeficiency virus (HIV)/ acquired immune deficiency syndrome patients, which is secondary to depleted glutathione. 15Discovery and preclinical development of ⅢM-160, a Bergenia ciliatabased anti-inflammatory and anti-arthritic botanical drug candidate显示文摘Objective: Bergenia ciliata(Haw.) Sternb. is used in the Indian traditional system of medicine to treat various ailments including rheumatism and to heal wounds. The objective of the present study was to perform a preclinical characterization of the B. ciliata-based botanical extract IIIM-160.Methods: ⅢM-160 was chemically standardized and analyzed for heavy metal content, aflatoxins,pesticides and microbial load. The in vitro and in vivo efficacies were determined in suitable models of inflammation, arthritis and nociception. An acute oral toxicity study was performed in Swiss albino mice.A suitable oral formulation was developed and characterized.Results: Bergenin was found to be the major component(9.1% w/w) of ⅢM-160. The botanical lead displayed inhibition of lipopolysaccharide-induced production of proinflammatory cytokines in THP-1 cells, with selectivity toward interleukin-6(IL-6) and had an excellent safety-window. It showed anti-inflammatory, anti-arthritic and antinociceptive activity in animal models and was not toxic at oral doses up to 2 g/kg in Swiss-albino mice. The gastroretentive, sustained-release capsule formulation showed sustained-release of the bergenin over the period of 24 h, resulting in improved plasma-exposure of bergenin in Sprague–Dawley rats.Conclusion: The dual-activity of IL-6 inhibition and antinociception marks the suitability of ⅢM-160 for treating rheumatoid arthritis. 16Epo/EpoR signaling in osteoprogenitor cells is essential for bone homeostasis and Epo-induced bone loss显示文摘High erythropoietin(Epo)levels are detrimental to bone health in adult organisms.Adult mice receiving high doses of Epo lose bone mass due to suppressed bone formation and increased bone resorption.In humans,high serum Epo levels are linked to fractures in elderly men.Our earlier studies indicated that Epo modulates osteoblast activity;however,direct evidence that Epo acts via its receptor(EpoR)on osteoblasts in vivo is still missing.Here,we created mice lacking EpoR in osteoprogenitor cells to specifically address this gap.Deletion of EpoR in osteoprogenitors(EpoR:Osx-cre,cKO)starting at 5 weeks of age did not alter red blood cell parameters but increased vertebral bone volume by 25%in 12-week-old female mice.This was associated with low bone turnover.Histological(osteoblast number,bone formation rate)and serum(P1NP,osteocalcin)bone formation parameters were all reduced,as were the number of osteoclasts and TRAP serum level.Differentiation of osteoblast precursors isolated from cKO versus control mice resulted in lower expression of osteoblast marker genes including Runx2,Alp,and Col1a1 on day 21,whereas the mineralization capacity was similar.Moreover,the RANKL70PG ratio,which determines the osteoclast-supporting potential of osteoblasts,was substantially decreased by 50%.Similarly,coculturing cKO osteoblasts with control or cKO osteoclast precursors produced significantly fewer osteoclasts than coculture with control osteoblasts.Finally,exposing female mice to Epo pumps(10 U-d_1)for 4 weeks resulted in trabecular bone loss(-25%)and increased osteoclast numbers(1.7-fold)in control mice only,not in cKO mice.Our data show that EpoR in osteoprogenitors is essential in regulating osteoblast function and osteoblast-mediated osteoclastogenesis via the RANKI70PG axis.Thus,osteogenic Epo/EpoR signaling controls bone mass maintenance and contributes to Epo-induced bone loss.Martina Rauner Marta Murray Sylvia Thiele Deepika Watts Drorit Neumann Yankel Gabet Lorenz C.Hofbauer Ben Wielockx 2021Bone Research2021,9,4:3

20CEACAM-1 Induced CSF3-receptor Downregulation in Bone Marrow Associated With Refractory Neutropenia in Advanced Cirrhosis显示文摘Background and Aims:Cirrhosis patients exhibit cyto-penia,and,at times refractory neutropenia to granulocyte colony-stimulating factor(G-CSF),which acts through the CSF3-receptor(CSF3R),and changes in CSF3R can affect the response.We conducted this study to assess the CSF3R status and its relevance in cirrhotic patients.Methods:Cirrhotic patients(n=127)and controls(n=26)with clini-cally indicated bone marrow(BM)examination were stud-ied.BM assessment was done by qRT-PCR and immunohis-tochemistry(IHC)for CSF3R.Circulating G-CSF,CSF3R,and carcinoembryonic antigen cell adhesion molecule-1(CEACAM1)were measured.BM hematopoietic precursor cells and their alterations were examined by flow cytom-etry.The findings were validated in liver cirrhosis patients who received G-CSF for severe neutropenia.Results:The mean age was 48.6±13.4 years,and 80.3%were men.Circulatory CSF3R reduction was noted with the advance-ment of cirrhosis,and confirmed by qRT-PCR and IHC in BM.CSF3R decline was related to decreased hematopoietic stem cells(HSCs)and downregulation of CSF3R in the re-maining HSCs.Cocultures confirmed that CEACAM1 led to CSF3R downregulation in BM cells by possible lysosomal degradation.Baseline low peripheral blood-(PB)-CSF3R also predisposed development of infections on follow-up.Decreased CSF3R was also associated with nonresponse to exogenous G-CSF treatment of neutropenia.Conclu-sions:Advanced liver cirrhosis was associated with low CSF3R and high CEACAM1 levels in the BM and circula-tion,making patients prone to infection and inadequate response to exogenous G-CSF.Chhagan Bihari Sukriti Baweja Seggere Murlaikrishna Shasthry Deepika Lal Preeti Negi Swati Thangariyal Dinesh Mani Tripathi Shiv Kumar Sarin 2022Journal of Clinical and Translational Hepatology2022,10,1:2
