1Inflammatory pathways of importance for management of inflammatory bowel disease显示文摘Inflammatory bowel disease(IBD)is a group of chronic disorders of the gastrointestinal tract comprising Crohn’s disease(CD)and ulcerative colitis(UC).Their etiologies are unknown,but they are characterised by an imbalanced production of pro-inflammatory mediators,e.g.,tumor necrosis factor(TNF)-α,as well as increased recruitment of leukocytes to the site of inflammation.Advantages in understanding the role of the inflammatory pathways in IBD and an inadequate response to conventional therapy in a large portion of patients,has over the last two decades lead to new therapies which includes the TNF inhibitors(TNFi),designed to target and neutralise the effect of TNF-α.TNFi have shown to be efficient in treating moderate to severe CD and UC.However,convenient alternative therapeutics targeting other immune pathways are needed for patients with IBD refractory to conventional therapy including TNFi.Indeed,several therapeutics are currently under development,and have shown success in clinical trials.These include antibodies targeting and neutralising interleukin-12/23,small pharmacologic Janus kinase inhibitors designed to block intracellular signaling of several pro-inflammatory cytokines,antibodies targeting integrins,and small anti-adhesion molecules that block adhesion between leukocytes and the intestinal vascular endothelium,reducing their infiltration into the inflamed mucosa.In this review we have elucidated the major signaling pathways of clinical importance for IBD therapy and highlighted the new promising therapies available.As stated in this paper several new treatment options are under development for the treatment of CD and UC,however,no drug fits all patients.Hence,optimisations of treatment regimens are warranted for the benefit of the patients either through biomarker establishment or other rationales to maximise the effect of the broad range of mode-of-actions of the present and future drugs in IBD.Jannie Pedersen Mehmet Coskun Christoffer Soendergaard Mohammad Salem Ole Haagen Nielsen 2014World Journal of Gastroenterology2014,20,1:28
2Bariatric surgery in patients with non-alcoholic fatty liver disease-from pathophysiology to clinical effects显示文摘Non-alcoholic fatty liver disease(NAFLD) is increasingly recognized as a significant liver disease,and it covers the disease spectrum from simple steatosis with a risk of development of non-alcoholic steatohepatitis(NASH) to fibrosis,subsequent cirrhosis,end-stage liver failure,and liver cancer with a potential need for liver transplantation.NAFLD and NASH are closely related to obesity,metabolic syndrome,and type 2 diabetes(T2 D).The role of gut hormones,especially glucagon-like peptide 1(GLP-1),is important in NAFLD.Bariatric surgery has the potential for inducing great weight loss and may improve the symptoms of metabolic syndrome and T2 D.Recent data demonstrated significant effects of bariatric surgery on GLP-1 and other gut hormones and important lipid metabolic and inflammatory abnormalities in the pathophysiology of NAFLD.Therefore,bariatric surgery may reverse the pathological liver changes in NAFLD and NASH patients.In the present review,we describe NAFLD and NASH pathophysiology and the primary effects of bariatric surgery on metabolic pathways.We performed a systematic review of the beneficial and harmful effects and focused on changes in liver disease severity in NAFLD and NASH patients.The specific focus was liver histopathology as assessed by the invasive liver biopsy.Additionally,we reviewed several non-invasive methods used for the assessment of liver disease severity following bariatric surgery.Tea L Laursen Christoffer A Hagemann Chunshan Wei Konstantin Kazankov Karen L Thomsen Filip K Knop Henning Grønbæk 2019World Journal of Hepatology2019,11,2:11
3A sensitivity study of the WRF model in offshore wind modeling over the Baltic Sea显示文摘Accurate wind modeling is important for wind resources assessment and wind power forecasting. To improve the WRF model configuration for the offshore wind modeling over the Baltic Sea, this study performed a sensitivity study of the WRF model to multiple model configurations, including domain setup,grid resolution, sea surface temperature, land surface data, and atmosphere-wave coupling. The simulated offshore wind was evaluated against LiDAR observations under different wind directions, atmospheric stabilities, and sea status. Generally, the simulated wind profiles matched observations, despite systematic underestimations. Strengthening the forcing from the reanalysis data through reducing the number of nested domains played the largest role in improving wind modeling. Atmosphere-wave coupling further improved the simulated wind, especially under the growing and mature sea conditions.Increasing the vertical resolution, and updating the sea surface temperature and the land surface information only had a slight impact, mainly visible during very stable conditions. Increasing the horizontal resolution also only had a slight impact, most visible during unstable conditions.
