1帕博利珠单抗单用或与放疗联用治疗转移性非小细胞肺癌:两个随机试验的汇总分析显示文摘背景放疗可以提高整个机体对免疫治疗的应答。在Ⅱ期PEMBRO-RT研究和Ⅰ/Ⅱ期MDACC研究中,患有转移性非小细胞肺癌(NSCLC)的患者被随机分配入组,接受免疫治疗(帕博利珠单抗)+放疗联合疗法,或免疫治疗单一疗法。当上述2个研究单独分析时,联合疗法组显示出潜在获益。由于每个研究的样本量较小,缓解率和结局并未显示出统计学意义,然而却有显著的临床获益。因此,本研究进行汇总分析,来判断放疗是否会改善转移性NSCLC患者的免疫治疗应答。方法PEMBRO-RT和MDACC研究纳入标准:患者年龄≥18岁,患有转移性NSCLC,且有≥1处未经放疗照射的病灶,以便进行射野外应答监测。PEMBRO-RT研究纳入曾接受过化疗患者,MDACC研究纳入曾接受过治疗或新诊断患者。2个研究中的患者均未接受过免疫治疗。在PEMBRO-RT研究中患者被等比例随机分配入组,并根据吸烟状态进行分层(分为<10年组和≥10年组)。MDACC研究的患者根据放疗计划可行性被等比例随机分配入2个受试组。由于联合治疗组的干预本质,每个研究中的放疗均不适用盲法。在2个研究中,不论是否进行放疗,均静脉滴入帕博利珠单抗(每3周200 mg)。在PEMBRO-RT研究中,在放疗(24 Gy 3次分割照射)结束后1周给予第1剂帕博利珠单抗。在MDACC研究中,在第1次放疗(50 Gy 4次分割照射或45 Gy 15次分割照射)同时给予帕博利珠单抗。仅检测未经照射病灶的应答。本研究的终点为最佳射野外(远隔)应答率(ARR)、最佳射野外疾病控制率(ACR)、12周时ARR、12周时ACR、无进展生存期(PFS)和总生存期(OS)。2个研究的意向治疗(ITT)人群均纳入分析。PEMBRO-RT研究(NCT02492568)和MDACC研究(NCT02444741)均在ClinicalTrials.gov上注册。发现纳入148例患者,76例接受帕博利珠单抗治疗,72例接受帕博利珠单抗+放疗治疗。所有患者随访时间中位数为33个月[四分位距(IQR):32.4~33.6]。148例患者中124例(84%)组织学特征为非鳞癌,111例(75%)患者曾经接受过化疗。组间没有基线特征差异,包括PD-L1表达状态和转移灶体积。最常见的照射部位为肺转移灶(39%,28/72)、胸腔内淋巴结(21%,15/72)和非原发灶(17%,12/72)。帕博利珠单抗组和联合治疗组的最佳ARR分别为19.7%(15/76)和41.7%(30/72),OR=2.96,95%CI:1.42~6.20,P=0.0039;最佳ACR分别为43.4%(33/76)和65.3%(47/72),OR=2.51,95%CI:1.28~4.91,P=0.0071;PFS中位数分别为4.4(IQR:2.9~5.9)和9.0个月(IQR:6.8~11.2),HR=0.67,95%CI:0.45~0.99,P=0.045;OS中位数分别为8.7(IQR:6.4~11.0)和19.2个月(IQR:14.6~23.8),OR=0.67,95%CI:0.54~0.84,P=0.0004。在汇总分析中没有发现新的安全问题。解读帕博利珠单抗免疫疗法+放疗显著提高转移性NSCLC患者的应答和改善治疗结局。这些结果需要在三期临床试验中进行验证。陈大卫(翻译) 于金明(校对) Willemijn S M E Theelen Vivek Verma Brian P Hobbs Heike M U Peulen Joachim G J V Aerts Idris Bahce Anna Larissa N Niemeijer Joe Y Chang Patricia M de Groot Quynh-Nhu Nguyen Nathan I Comeaux George R Simon Ferdinandos Skoulidis Steven H Lin Kewen He Roshal Patel John Heymach Paul Baas James W Welsh 2021中华肿瘤防治杂志2021,28,24:49
2骨质疏松性骨折人群跌倒风险、骨密度、肌力和体脂的相关性显示文摘目的探讨骨质疏松性骨折人群的骨密度与跌倒风险、体质量指数(body mass index,BMI)、肌力、体脂含量的相关性。方法通过对4 632例社区45岁以上绝经后女性的问卷调研及骨密度筛查,筛查出既往有脆性骨折史的骨质疏松性骨折人群。测定骨质疏松性骨折人群的骨密度、身高、体质量、跌倒风险、肌力和腹部脂肪含量。采用Spearman检验分析跌倒指数、体脂含量、年龄、身高、体质量、BMI与骨密度(bone mass density,BMD)之间的相关性。BMD校正年龄、身高、体质量、BMI、体脂含量,采用ANCOVA分析跌倒指数对BMD的影响;校正年龄、身高、体质量、BMI、跌倒指数,分析体脂含量对BMD的影响。结果对筛查出的839例骨质疏松性骨折患者的BMD、BMI、跌倒风险、腹部体脂含量、肌力等相关性分析结果显示,骨质疏松性骨折人群BMI、跌倒指数、肌力与BMD均显著相关(BMI r=0.161,跌倒指数r=0.238,肌力r=0.198,P均<0.001)。BMI、肌力、跌倒风险与BMD呈正相关;校正年龄、身高、体质量、BMI等因素,采用ANCOVA分析跌倒指数与BMD无显著相关(F=1.011,P>0.05)。结论跌倒风险和低BMD为骨折的主要危险因素。本研究显示体脂含量与BMD呈显著相关性。朱秀芬 彭志坚 Brian Lin 马大年 陈新 杨海明 林华 2017中华骨质疏松和骨矿盐疾病杂志2017,10,1:32

4DAPT Enhances the Apoptosis of Human Tongue Carcinoma Cells显示文摘Aim To investigate the effect of DAPT(γ-secretase inhibitor) on the growth of human tongue carcinoma cells and to determine the molecular mechanism to enable the potential application of DAPT to the treatment of tongue carcinoma. Methodology Human tongue carcinoma Tca8113 cells were cultured with DAPT.Cell growth was determined using Indigotic Reduction method.The cell cycle and apoptosis were analyzed by flow cytometry.Real-time PCR and Immuno-Fluorescence(IF) were employed to determine the intracellular expression levels. Results DAPT inhibited the growth of human tongue carcinoma Tca8113 cells by inducing G_0-G_1 cell cycle arrest and apoptosis.The mRNA levels of Hairy/Enhancer of Split-1(Hes-1),a target of Notch activation,were reduced by DAPT in a dose-dependent manner.Coincident with this observation,DAPT induced a dose-dependent promotion of constitutive Caspase-3 in Tca8113 cells. Conclusion DAPT may have a therapeutic value for human tongue carcinoma.Moreover,the effects of DAPT in tumor inhibition may arise partly via the modulation of Notch-1 and Caspase-3.Brian E. 5Osteogenesis of Adipose-Derived Stem Cells显示文摘Current treatment options for skeletal repair,including immobilization,rigid fixation,alloplastic materials and bone grafts,have significant limitations.Bone tissue engineering offers a promising method for the repair of bone deficieny caused by fractures,bone loss and tumors.The use of adipose derived stem cells(ASCs) has received attention because of the self-renewal ability,high proliferative capacity and potential of osteogenic differentiation in vitro and in vivo studies of bone regeneration.Although cell therapies using ASCs are widely promising in various clinical fields,no large human clinical trials exist for bone tissue engineering.The aim of this review is to introduce how they are harvested,examine the characterization of ASCs,to review the mechanisms of osteogenic differentiation,to analyze the effect of mechanical and chemical stimuli on ASC osteodifferentiation,to summarize the current knowledge about usage of ASC in vivo studies and clinical trials,and finally to conclude with a general summary of the field and comments on its future direction.Brian E. 7Significance of Vibrio species in the marine organic carbon cycle-A review显示文摘The genus Vibrio,belonging to Gammaproteobacteria of the phylum Proteobacteria,is a genetically and ecologically diverse group of heterotrophic bacteria,that are ubiquitous in marine environments,especially in coastal areas.In particular,vibrios dominate,i.e.up to 10%of the readily culturable marine bacteria in these habitats.The distribution of Vibrio spp.is shaped by various environmental parameters,notably temperature,salinity and dissolved organic carbon.Vibriospp.may utilize a wide range of organic carbon compounds,including chitin(this may be metabolized by most Vibrio spp.),alginic acid and agar.Many Vibrio spp.have very short replication times(as short as~10 min),which could facilitate them developing into high biomass content albeit for relatively short durations.Although Vibriospp.usually comprise a minor portion(typically^1%of the total bacterioplankton in coastal waters)of the total microbial population,they have been shown to proliferate explosively in response to various nutrient pulses,e.g.,organic nutrients from algae blooms and iron from Saharan dust.Thus,Vibrio spp.may exert large impacts on marine organic carbon cycling especially in marginal seas.Genomics and related areas of investigation will reveal more about the molecular components and mechanisms involved in Vibrio-mediated biotransformation and remineralization processes.Xiaohua ZHANG Heyu LIN Xiaolei WANG Brian AUSTIN 2018Science China Earth Sciences2018,61,10:5
8骨化三醇干预绝经后骨质疏松妇女静态平衡功能显示文摘目的探讨不同剂量的骨化三醇干预对绝经后骨质疏松妇女静态平衡功能(跌到风险)的影响。方法静态平衡功能测试提示跌倒中高风险161名绝经后骨质疏松妇女,采用数字表法随机分成A、B两组,给予不同剂量的骨化三醇治疗。A组81例,骨化三醇0.25μg/d,B组80例,骨化三醇0.5μg/d;所有患者在接受骨化三醇治疗的同时,每天补充元素钙600 mg和维生素D 125 IU。疗效评价包括:跌倒风险指数;血清钙磷、25羟维生素D(25 hydroxy vitamin D,25OHD)、甲状旁腺素、肾功能;尿钙;泌尿系统超声及相关不良反应。结果本研究两组患者最终随访人数分别为A组73例(90%),B组74例(93%)。经3个月骨化三醇治疗后,两组患者的静态平衡功能测试提示,跌倒风险指数均有所降低,较干预前自身比较,差异有统计学意义(A组t=2.37,P=0.050;B组t=2.06,P=0.030),组间比较差异无统计学意义(P>0.05);治疗6个月后,两组患者跌倒风险指数进一步下降,B组人群跌倒风险降低明显优于A组,差异有统计学意义(t=2.11,P=0.030)。所有患者研究期间肾功能正常,未见血清25OHD水平显著变化,无高钙血症、高尿钙症及泌尿系统新结石发生。结论骨化三醇干预可有效改善绝经后骨质疏松患者的静态平衡功能,降低跌倒风险;骨化三醇0.5μg/d较0.25μg/d对降低绝经后骨质疏松妇女跌到风险效果更明显。林华 朱秀芬 Brian Lin 陈新 刘畅畅 范璐 2018中华骨质疏松和骨矿盐疾病杂志2018,11,1:5
9On-chip photonic Fourier transform with surface plasmon polaritons显示文摘The Fourier transform(FT),a cornerstone of optical processing,enables rapid evaluation of fundamental mathematical operations,such as derivatives and integrals.Conventionally,a converging lens performs an optical FT in free space when light passes through it.The speed of the transformation is limited by the thickness and the focal length of the lens.By using the wave nature of surface plasmon polaritons(SPPs),here we demonstrate that the FT can be implemented in a planar configuration with a minimal propagation distance of around 10 mm,resulting in an increase of speed by four to five orders of magnitude.The photonic FT was tested by synthesizing intricate SPP waves with their Fourier components.The reduced dimensionality in the minuscule device allows the future development of an ultrafast on-chip photonic information processing platform for large-scale optical computing.Shan Shan Kou Guanghui Yuan Qian Wang Luping Du Eugeniu Balaur Daohua Zhang Dingyuan Tang Brian Abbey Xiao-Cong Yuan Jiao Lin 2016Light(Science & Applications)2016,5,1:4
10Comparison of Effects of Mechanical Stretching on Osteogenic Potential of ASCs and BMSCs显示文摘Mechanical forces play critical roles in the development and remodeling processes of bone.As an alternative cell source for bone engineering,adipose-derived stem cells(ASCs)should be fully investigated for their responses to mechanical stress.Similarly,the osteogenic potential,stimulated by mechanical stress,should be compared with bone marrow stromal cells(BMSCs),which have been clinically used for bone tissue engineering.In this study,ASCs and BMSCs were osteogenic-induced for 48 hours,and then subjected to uniaxial mechanical stretching for 2 or 6 hours.Cell orientation,osteogenic regulatory genes,osteogenic genes and ALP activities were measured and compared between ASCs and BMSCs.ASCs could align in a perpendicular way to the direction of stretching stress,while BMSCs did not present a specific alignment.Both 2 and 6 hours mechanical stretching could enhance the mRNA expression of Osx and Runx2 in BMSCs and ASCs,while OCN mRNA only increased in ASCs after 6 hours mechanical loading.Mechanical stretching enhanced the BMP-2 mRNA expression in ASCs,while only after 6 hours of mechanical loading significantly increased the BMP-2 gene expression in BMSCs.Significant differences only exist between ASCs and BMSCs loaded at 2 hours of mechanical stretching.It is concluded that ASCs are more rapid responders to mechanical stress,and have greater potential than BMSCs in osteogenesis when stimulated by mechanical stretching,indicating their usefulness for bone study in a rat model.Brian E. 11Restless legs syndrome following the use of ziprasidone: a case report显示文摘Restless legs syndrome(RLS)is a common sleep-related movement disorder characterised by an uncomfortable urge to move the legs that occurs during periods of inactivity.Although there have been many case reports on antipsychotic-induced RLS,ziprasidone has never been reported as a cause of RLS.We present a case of a female patient with schizophrenia who presented with symptoms of RLS following the administration of high doses of ziprasidone added to quetiapine and valproate.The patient’s symptoms of RLS occurred following the administration and titration of ziprasidone to 160 mg,and were relieved upon reducing the dose to 120 mg/day.Other potential causative medications and differential diagnoses that could have caused similar symptoms were excluded.Clinicians should be aware of the potential for ziprasidone-induced RLS.Dopamine and serotonin interaction could be the mechanism underlying ziprasidone-induced RLS.Cuizhen Zhu Ran Bi Yuliang Hu Hui Zhou Daomin Zhu Brian Isaacson Qingwei Li Yezhe Lin 2020General Psychiatry2020,33,2:3
12Liver resection versus liver transplantation for hepatocellular carcinoma within Milan criteria:a meta-analysis of 18,421 patients显示文摘Background:Outcomes after liver resection(LR)and liver transplantation(LT)for hepatocellular carcinoma(HCC)are heterogenous and may vary by region,over time periods and disease burden.We aimed to compare overall survival(OS)and disease-free survival(DFS)between LT versus LR for HCC within the Milan criteria.Methods:Two authors independently searched Medline and Embase databases for studies comparing survival after LT and LR for patients with HCC meeting the Milan criteria.Meta-analyses and meta-regression were conducted using random-effects models.Results:We screened 2,278 studies and included 35 studies with 18,421 patients.LR was associated with poorer OS[hazard ratio(HR)=1.44;95%confidence interval(CI):1.14-1.81;P<0.01]and DFS(HR=2.71;95%CI:2.23-3.28;P<0.01)compared to LT,with similar findings among intention-to-treat(ITT)studies.In uninodular disease,OS in LR was comparable to LT(P=0.13)but DFS remained poorer(HR=2.95;95%CI:2.30-3.79;P<0.01).By region,LR had poorer OS versus LT in North America and Europe(P≤0.01),but not Asia(P=0.25).LR had inferior survival versus LT in studies completed before 2010(P=0.01),but not after 2010(P=0.12).Cohorts that underwent enhanced surveillance had comparable OS after LT and LR(P=0.33),but cohorts undergoing usual surveillance had worse OS after LR(HR=1.95;95%CI:1.24-3.07;P<0.01).Conclusions:Mortality after LR for HCC is nearly 50%higher compared to LT.Survival between LR and LT were similar in uninodular disease.The risk of recurrence after LR is threefold that of LT.Jin Hean Koh Darren Jun Hao Tan Yuki Ong Wen Hui Lim Cheng Han Ng Phoebe Wen Lin Tay Jie Ning Yong Mark D.Muthiah Eunice X.Tan Ning Qi Pang Beom Kyung Kim Nicholas Syn Alfred Kow Brian K.P.Goh Daniel Q.Huang 2022Hepatobiliary Surgery and Nutrition2022,11,1:3
13骨质疏松性椎体骨折可控风险因素分析显示文摘目的 分析骨质疏松性椎体骨折的相关可控风险因素,提示管理可控风险因素的临床价值.方法 绝经后女性626例,收集患者年龄、身高、体重、腰椎和髋部骨密度值及腹部体脂含量等信息,采用Tetrax平衡测试系统评估跌倒风险.通过胸腰段X线侧位片将患者分为椎体骨折组和无椎体骨折组,对两组临床资料进行分析比较,并采用Spearman检验,分析椎体骨折与年龄、身高、体重、体质指数(BMI)及骨密度、跌倒风险、体脂含量等可控因素的相关性.结果 椎体骨折组328例,无椎体骨折组298例,骨折涉及426个椎体.椎体骨折组与无椎体骨折组比较,椎体骨折组具有年龄大[(68.67±9.29)岁比(63.04±9.30)岁]、身高矮[(151.10±4.39)cm比(154.90±5.86)cm]、骨密度低[腰椎骨密度(0.85±0.16)g/cm2比(0.93±0.17)g/cm2,全髋骨密度(0.72±0.18)g/cm2比(0.81±0.13)g/cm2]及腹部脂肪含量高[(41.30±5.20)%比(36.30±5.90)%]、跌倒风险高(41.38±25.79比36.20±26.22)的特点,差异均有统计学意义(P均〈0.001);相关性分析显示,年龄、身高、BMI、腹部体脂含量、跌倒风险腰椎和全髋骨密度与椎体骨折显著相关,r值依次为0.358、-0.323、0.169、0.186、0.135、-0.398、-0.364(P均〈0.001).结论 骨量丢失、跌倒风险增高、腹部脂肪含量增加是绝经后女性骨质疏松性椎体压缩性骨折的重要可控风险因素.朱秀芬 何占德 Brian Lin 曹威 陈新 杨海明 林华 2017中华健康管理学杂志2017,11,4:3

20Opto-thermoelectric pulling of light-absorbing particles显示文摘Optomechanics arises from the photon momentum and its exchange with low-dimensional objects.It is well known that optical radiation exerts pressure on objects,pushing them along the light path.However,optical pulling of an object against the light path is still a counter-intuitive phenomenon.Herein,we present a general concept of optical pulling-opto-thermoelectric pulling(OTEP)—where the optical heating of a light-absorbing particle using a simple plane wave can pull the particle itself against the light path.This irradiation orientation-directed pulling force imparts self-restoring behaviour to the particles,and three-dimensional(3D)trapping of single particles is achieved at an extremely low optical intensity of 10^(−2)mWμm^(−2).Moreover,the OTEP force can overcome the short trapping range of conventional optical tweezers and optically drive the particle flow up to a macroscopic distance.The concept of selfinduced opto-thermomechanical coupling is paving the way towards freeform optofluidic technology and lab-on-achip devices.Linhan Lin Pavana Siddhartha Kollipara Abhay Kotnala Taizhi Jiang Yaoran Liu Xiaolei Peng Brian A.Korgel Yuebing Zheng 2020Light(Science & Applications)2020,9,1:1
