1骨密度测量的准确度和精密度评价显示文摘目的采用欧洲腰椎体模(ESP)评估不同CT机的定量CT(QCT)和双能X线吸收仪(DXA)设备测量骨密度的准确度和短期精确度。方法收集2016年1月至2020年4月全国多个中心(QCT和DXA分别来自31和32个中心)的40台不同品牌的CT设备(德国Siemens 12台、荷兰Philips 12台、美国GE 9台、日本Toshiba 5台和国产联影2台)和53台不同品牌DXA设备(美国GE Lunar 34台、美国Hologic 14台和法国Medlink 5台)。QCT扫描采用Mindways QCT系统,以常规腰椎扫描条件对ESP体模重复扫描10次,每次重新摆位,测量ESP中低、中、高密度椎体的骨密度值以及3个椎体的平均骨密度值。根据实测值与体模标定值的差异计算不同设备的准确度误差,并计算标准差均方根(RMS-SD)和变异系数的均方根(RMS-%CV)来评价短期精密度误差。采用重复测量的方差分析比较不同设备间测量的骨密度值的差异。结果不同CT和DXA设备测量的不同密度椎体和平均骨密度值差异均有统计学意义(P<0.001)。Siemens的准确度误差范围为1.20%~7.60%,Philips为-1.83%~0.20%,GE为1.18%~13.20%,Toshiba为-0.12%~3.55%,联影为-1.65%~6.32%,GE Lunar为6.59%~21.34%,Hologic为-6.65%~5.45%,Medlink为-6.97%~-0.68%。QCT和DXA测量的所有椎体骨密度值的RMS-%CV为0.38%~3.85%;QCT的RMS-SD为0.54~2.45 mg/cm3。DXA的RMS-SD为0.009~0.037g/cm^(2)。不同QCT和DXA设备测量的RMS-%CV值随着骨密度的升高而呈减低趋势,RMS-SD值则呈升高趋势。结论基于ESP,不同QCT和DXA设备测量的ESP骨密度值有显著差异。不同QCT和DXA设备测量骨密度的准确度误差和短期精密度误差在合理范围,可以用于临床随访观察。QCT的短期精密度误差和准确度误差波动范围较DXA略小。端木羊羊 王玲 张勇 邓克学 李凯 李娜 Blake Glen M 程晓光 2021中华放射学杂志2021,55,4:14

6不同双能X线骨密度仪交叉校准及其骨密度参考数据库标准化显示文摘目的采用欧洲脊柱体模(European spine phantom,ESP)和志愿者用常用不同品牌的双能X线吸收测量仪(dual energy X-ray absorptiometry,DXA)测量的脊柱和髋部骨密度(bone mineral density,BMD)值进行体模校准和体内交叉校准,为建立统一的中国人群骨密度参考数据库,实现全国不同品牌DXA T值的标准化提供基础数据。方法采用FUJIFILM、GE-Lunar、Hologic、Medilink和Osteosys 5个品牌的DXA。体模校准实验使用同一个ESP(编号SN 145)在每个扫描仪上扫描10次,每次重新定位。体内交叉校准研究在同一天使用GE-Lunar iDXA系统和其他4家品牌之一的DXA对100名20~80岁健康志愿者进行腰椎(L1-4)和髋部扫描。采用SPSS 22.0完成线性回归分析,将FUJIFILM、Hologic、Medilink和Osteosys的脊柱、股骨颈和全髋BMD与GE-Lunar相对应部位的BMD进行交叉校准。并根据一个采用GE-Lunar DXA的75321名20岁及以上中国成年人全国性骨密度调查数据库计算正常参考值,采用本次体内交叉校准换算方程推导出其他4种品牌DXA的等效青年人参考数据库。结果GE-Lunar、FUJIFILM、Hologic、Medilink和Osteosys DXA的ESP L2-4 BMD(单位:g/cm^(2))均值分别为1.170±0.005、0.930±0.005、0.980±0.005、1.000±0.009和1.100±0.003,各品牌间BMD差异有统计学意义(P<0.01)。10次重复扫描的变异系数(CV%)分别为0.31%、0.53%、0.37%、0.91%和0.25%。FUJIFILM、Hologic、Medilink和Osteosys DXA脊柱和髋部的BMD测量值与GE-Lunar测量值高度相关,Pearson相关系数r为0.911~0.982,并计算出每种DXA的BMD值与GE相应部位BMD值的换算公式。列出5家DXA等效中国青年人骨密度参考范围,实现这些DXA设备扫描同一患者时计算的T值一致。结论本研究提供了国内5种品牌DXA的ESP和体内交叉校准数据以及换算公式,获得了统一的中国人DXA骨密度正常参考值,为实现全国DXA脊柱和髋部骨密度T值的标准化提供基础数据。闫东 Glen M Blake 孙晓雷 崔珂 蔡舒 王文志 刘艳东 王文海 轩艳姣 程晓光 2022中华骨质疏松和骨矿盐疾病杂志2022,15,1:8

8Prevalence of vitreous floaters in a community sample of smartphone users显示文摘AIM: To estimate the prevalence and risk factors for vitreous floaters in the general population. ·METHODS: An electronic survey was administered through a smartphone app asking various demographic and health questions, including whether users experience floaters in their field of vision. Multivariate logistic regression analysis was used to determine risk factors. ·RESULTS: A total of 603 individuals completed the survey, with 76% reporting that they see floaters, and 33% reporting that floaters caused noticeable impairment in vision. Myopes were 3.5 times more likely (P =0.0004), and hyperopes 4.4 times more likely (P =0.0069) to report moderate to severe floaters compared to those with normal vision. Floater prevalence was not significantly affected by respondent age, race, gender, and eye color. ·CONCLUSION: Vitreous floaters were found to be a very common phenomenon in this non -clinical general population sample, and more likely to be impairing in myopes and hyperopes.Blake F. 13Critical analysis of feeding jejunostomy following resection of upper gastrointestinal malignancies显示文摘AIM To assess nutritional recovery,particularly regarding feeding jejunostomy tube(FJT)utilization,following upper gastrointestinal resection for malignancy. METHODS A retrospective review was performed of a prospectively-maintained database of adult patients who underwent esophagectomy or gastrectomy(subtotal or total)for cancer with curative intent,from January 2001 to June 2014. Patient demographics,the approach to esophagectomy,the extent of gastrectomy,FJT placement and utilization at discharge,administration of parenteral nutrition(PN),and complications were evaluated. All patients were followed for at least ninety days or until death.RESULTS The 287 patients underwent upper GI resection,comprised of 182 esophagectomy(n=107 transhiatal,58.7%; n=56 Ivor-Lewis,30.7%)and 105 gastrectomy [n=63 subtotal(SG),60.0%; n=42 total(TG),40.0%]. 181 of 182 esophagectomy patients underwent FJT,compared with 47 of 105 gastrectomy patients(99.5% vs 44.8%,P < 0.0001),of whom most had undergone TG(n=39,92.9% vs n=8 SG,12.9%,P < 0.0001). Median length of stay was similar between esophagectomy and gastrectomy groups(14.7 d vs 17.1 d,P=0.076). Upon discharge,87 esophagectomy patients(48.1%)were taking enteral feeds,with 53(29.3%)fully and 34(18.8%)partially dependent. Meanwhile,20 of 39 TG patients(51.3%)were either fully(n=3,7.7%)or partially(n=17,43.6%)dependent on tube feeds,compared with 5 of 8 SG patients(10.6%),all of whom were partially dependent. Gastrectomy patients were significantly less likely to be fully dependent on tube feeds at discharge compared to esophagectomy patients(6.4% vs 29.3%,P=0.0006). PN was administered despite FJT placement more often following gastrectomy than esophagectomy(n=11,23.4% vs n=7,3.9%,P=0.0001). FJT-specific complications requiring reoperation within 30 d of resection occurred more commonly in the gastrectomy group(n=6),all after TG,compared to 1 esophagectomy patient(12.8% vs 0.6%,P=0.0003). Six of 7 patients(85.7%)who experienced tube-related complications required PN.CONCLUSION Nutritional recovery following esophagectomy and gastrectomy is distinct. Operations are associated with unique complication profiles. 14Enhancer variants reveal a conserved transcription factor network governed by PU.1 during osteoclast differentiation显示文摘Genome-wide association studies(GWASs) have been instrumental in understanding complex phenotypic traits. However, they have rarely been used to understand lineage-specific pathways and functions that contribute to the trait. In this study, by integrating lineage-specific enhancers from mesenchymal and myeloid compartments with bone mineral density loci, we were able to segregate osteoblast-and osteoclast(OC)-specific functions. Specifically, in OCs, a PU.1-dependent transcription factor(TF)network was revealed. Deletion of PU.1 in OCs in mice resulted in severe osteopetrosis. Functional genomic analysis indicated PU.1 and MITF orchestrated a TF network essential for OC differentiation. Several of these TFs were regulated by cooperative binding of PU.1 with BRD4 to form superenhancers. Further, PU.1 is essential for conformational changes in the superenhancer region of Nfatc1. 17Data-driven synthesis of multiple recommendation patterns to create situational Web mashups显示文摘As a typical situational application,Web mashup reflects and accommodates some key features of Internetware paradigm.Mashup provides a development fashion that integrates data,computation and UI elements from multiple resources into a single Web application,and promises the quick rollout of creating potential new functionalities opportunistically.This paper focuses on the problem of recommending useful suggestions for developing data-driven mashups by synthesis of multiple patterns.We present a rapid and intuitive system called iMashupAdvisor,for aiding mashup development based on a novel automated suggestion mechanism.The key observation guiding the development of iMashupAdvisor is that mashups developed by different users might share some common patterns,for instance,selecting similar mashup components for similar goals,and gluing them in a similar manner.Such patterns could reside in multiple sources,e.g.,the data dependency between mashup components,the interaction between users and mashup components,or the collective intelligence from existing applications created and maintained by programmers,etc.iMashupAdvisor leverages the synthesis of these patterns to recommend useful suggestions for a partial mashup,such as the missing components,connections between them,or potentially relevant options,to assist mashup completion.This paper presents the data model and ranking metrics of the synthesis process,and introduces efficient algorithms for the retrieval of recommendations.We also experimentally demonstrate the efficiency of our approach for benefiting the proposed rapid mashup development.MA Yun LU Xuan LIU XuanZhe WANG XuDong BLAKE M.Brian 2013Science China(Information Sciences)2013,56,8:3

19有为腹的 CT 的非线性的适应过滤器的申请的放射剂量减小显示文摘 AIM:To evaluate the effect of non-linear adaptive filters (NLAF) on abdominal computed tomography (CT) images acquired at different radiation dose levels.METHODS:Nineteen patients (mean age 61.6 ± 7.9 years,M:F=8:11) gave informed consent for an Institutional Review Board approved prospective study involving acquisition of 4 additional image series (200,150,100,50 mAs and 120 kVp) on a 64 slice multidetector row CT scanner over an identical 10 cm length in the abdomen.The CT images acquired at 150,100 and 50 mAs were processed with the NLAF.Two radiologists reviewed unprocessed and processed images for image quality in a blinded randomized manner.CT dose index volume,dose length product,patient weight,transverse diameters,objective noise and CT numbers wererecorded.Data were analyzed using Analysis of Variance and Wilcoxon signed rank test.RESULTS:Of the 31 lesions detected in abdominal CT images,28 lesions were less than 1 cm in size.Subjective image noise was graded as unacceptable in unprocessed images at 50 and 100 mAs,and in NLAF processed images at 50 mAs only.In NLAF processed images,objective image noise was decreased by 21% (14.4 ± 4/18.2 ± 4.9) at 150 mAs,28.3% (15.7 ± 5.6/21.9 ± 4) at 100 mAs and by 39.4% (18.8 ± 9/30.4 ± 9.2) at 50 mAs compared to unprocessed images acquired at respective radiation dose levels.At 100 mAs the visibility of smaller structures improved from suboptimal in unprocessed images to excellent in NLAF processed images,whereas diagnostic confidence was respectively improved from probably confident to fully confident.CONCLUSION:NLAF lowers image noise,improves the visibility of small structures and maintains lesion conspicuity at down to 100 mAs for abdominal CT.Sarabjeet Singh Mannudeep K Kalra Mi Kim Sung Anni Back Michael A Blake 2012World Journal of Radiology2012,4,1:3
