Recent advances in chronic granulomatous disease显示文摘Chronic granulomatous disease(CGD)is an inherited defect of phagocyte function due to defective NADPH oxidase.Patients with CGD are not able to effectively clear the infections because of the defect in the phagocyte production of oxygen free radicals and are prone to recurrent bacterial and fungal infections.Inflammatory complications are also noted in CGD such as colitis,non-infective granulomas causing gastrointestinal or urinary tract obstruction, hemophagocytic lymphohistiocytosis, and arthritis.Studies on toll-like receptor pathways and neutrophil extracellular traps in CGD have shed light on the role of NADPH oxidase in the innate immunity and pathogenesis of infections in CGD.Some reports also indicate a reduction of memory B cells and defective production of functional antibodies in CGD.Though the exact mechanisms for non-infective inflammatory complications in CGD are not yet clear, studies on efferocytosis and defective autophagy with inflammasome activation have made a substantial contribution to our understanding of the pathogenesis of inflammation in CGD.We also discuss the clinical and molecular features of p40phox defects and a newer genetic defect,EROS.Clinical phenotypes of X-linked carriers of CYBB are also discussed.Gummadi Anjani Pandiarajan Vignesh Vibhu Joshi Jitendra Kumar Shandilya Dharmagat Bhattarai Jyoti Sharma Amit Rawat 2020Genes & Diseases2020,7,1:6
Effects of 42-year long-term fertilizer management on soil phosphorus availability, fractionation, adsorption–desorption isotherm and plant uptake in flooded tropical rice显示文摘Soil phosphorus(P) fractionation, adsorption, and desorption isotherm, and rice yield and P uptake were investigated in flooded tropical rice(Oryza sativa L.) following 42-year fertilizer and manure application. The treatments included low-input [unfertilized control without N, P, or K(C0N0)], farmyard manure(FYM)(C1N0), NP(C0NP), NPK(C0NPK), FYM + NP(C1NP), and high-input treatment, FYM + NPK(C1NPK). Grain yield was increased significantly by 74%over the control under the combined application of FYM + NPK. However, under low- and high-input treatments, yield as well as P uptake was maintained at constant levels for 35 years.During the same period, high yield levels and P uptake were maintained under the C0 NP, C0 NPK,and C1 NPK treatments. These are unique characteristics of a tropical flooded ecosystem, which is a self-sustaining system for rice production. The Fe–P fraction was highest compared to the Ca–P and Al–P fractions after 42 years of fertilizer application and was significantly higher under FYM + NPK treatment. The P adsorption capacity of soil was highest under the low-input treatment and lowest under long-term balanced fertilization(FYM + NPK). In contrast, P desorption capacity was highest under NPK and lowest in the control treatment. Long-term balanced fertilization in the form of FYM + NPK for 42 years lowered the bonding energy and adsorption capacity for P in soil but increased its desorption potential, increasing P availability to the plant and leading to higher P uptake and yield maintenance.Pratap Bhattacharyya Amaresh Kumar Nayak Mohammad Shahid Rahul Tripathi Sangita Mohanty Anjani Kumar Rajagounder Raja Bipin Bihari Panda Banwari Lal Priyanka Gautam Chinmaya Kumar Swain Koushik Singha Roy Pradeep Kumar Dash 2015The Crop Journal2015,3,5:5
An updated review on phenocopies of primary immunodeficiency diseases显示文摘Primary immunodeficiency diseases(PIDs)refer to a heterogenous group of disorders characterized clinically by increased susceptibility to infections, autoimmunity and increased risk of malignancies. These group of disorders present with clinical manifestations similar to PIDs with known genetic defects but have either no genetic defect or have a somatic mutation and thus have been labelled as“Phenocopies of PIDs”.These diseases have been further subdivided into those associated with somatic mutations and those associated with presence of auto-antibodies against various cytokines. In this review,we provide an update on clinical manifestations, diagnosis and management of these diseases.Ankita Singh Ankur K.Jindal Vibhu Joshi Gummadi Anjani Amit Rawat 2020Genes & Diseases2020,7,1:3
Genetics of severe combined immunodeficiency显示文摘Severe Combined Immunodeficiency(SCID)is an inherited group of rare, lifethreatening disorders due to the defect in T cell development and function.Clinical manifestations are characterised by recurrent and severe bacterial, viral, and fungal opportunistic infections that start from early infancy period.Haematopoietic stem cell transplantation(HSCT)is the treatment of choice. The pattern of inheritance of SCID may be X-linked or autosomal recessive.Though the diagnosis of SCID is usually established by flow cytometry-based tests, genetic diagnosis is often needed for genetic counselling, prognostication, and modification of pre-transplant chemotherapeutic agents. This review aims to highlight the genetic aspects of SCID.Rajni Kumrah Pandiarajan Vignesh Pratap Patra Ankita Singh Gummadi Anjani Poonam Saini Madhubala Sharma Anit Kaur Amit Rawat 2020Genes & Diseases2020,7,1:2

Gastric intestinal metaplasia development in African American predominant United States population显示文摘BACKGROUND Gastric cancer significantly contributes to cancer mortality globally.Gastric intestinal metaplasia(GIM)is a stage in the Correa cascade and a premalignant lesion of gastric cancer. The natural history of GIM formation and progression over time is not fully understood.Currently, there are no clear guidelines on GIM surveillance or management in the United States.AIM To investigate factors associated with GIM development over time in African American-predominant study population.METHODS This is a retrospective longitudinal study in a single tertiary hospital in Washington DC.We retrieved upper esophagogastroduodenoscopies(EGDs)with gastric biopsies from the pathology department database from January 2015 to December 2020.Patients included in the study had undergone two or more EGDswith gastric biopsy.Patients with no GIM at baseline were followed up until they developed GIM or until the last available EGD.Exclusion criteria consisted of patients age<18,pregnancy,previous diagnosis of gastric cancer, and missing data including pathology results or endoscopy reports. The study population was divided into two groups based on GIM status.Univariate and multivariate Cox regression was used to estimate the hazard induced by patient demographics,EGD findings, and Helicobacter pylori(H.pylori)status on the GIM status.RESULTS Of 2375 patients who had at least 1 EGD with gastric biopsy,579 patients were included in the study.138 patients developed GIM during the study follow-up period of 1087 d on average, compared to 857 d in patients without GIM(P=0.247).The average age of GIM group was 64 years compared to 56 years in the non-GIM group(P<0.001).In the GIM group, adding one year to the age increases the risk for GIM formation by 4%(P<0.001).Over time,African Americans,Hispanic, and other ethnicities/races had an increased risk of GIM compared to Caucasians with a hazard ratio(HR)of 2.12(1.16,3.87),2.79(1.09,7.13),and 3.19(1.5,6.76)respectively.No gender difference was observed between the study populations.Gastritis was associated with an increased risk for GIM development with an HR of 1.62(1.07,2.44).On the other hand,H.pylori infection did not increase the risk for GIM.CONCLUSION An increase in age and non-Caucasian race/ethnicity are associated with an increased risk of GIM formation. The effect of H.pylori on GIM is limited in low prevalence areas.Akram I Ahmad Arielle Lee Claire Caplan Colin Wikholm Ioannis Pothoulakis Zaynab Almothafer NishthaRaval Samantha Marshall Ankit Mishra Nicole Hodgins In Guk Kang Raymond K Chang Zachary Dailey Arvin Daneshmand Anjani Kapadia Jae Hak Oh Brittney Rodriguez Abhinav Sehgal Matthew Sweeney Christopher B Swisher Daniel F Childers Corinne O'Connor Lynette M Sequeira Won Cho 2022World Journal of Gastrointestinal Endoscopy2022,14,10:1

Utility of cardiac bioenzymes in predicting cardiovascular outcomes in SARS-CoV-2显示文摘BACKGROUND Cardiovascular complications have been increasingly recognized in the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)associated coronavirus disease 2019(COVID-19).Cardiac biomarkers are released because of this ongoing cardiovascular injury and can act as surrogate markers to assess the disease severity.AIM To review the variation and utility of these biomarkers in COVID-19 to ascertain their role in diagnosis, prognosis and clinical outcomes of the disease.METHODS We performed a literature search in PubMed,Medline and the Reference Citation Analysis(RCA),using the search terms“COVID-19”and“cardiac bioenzymes”or“cardiac biomarkers”.Additionally,we also used the latest reference citation analysis tool to identify more articles.RESULTS Cardiac troponin has been consistently elevated in patients with COVID-19 associated myocarditis, and strongly correlated with adverse prognosis.Natriuretic peptides including brain natriuretic peptide(BNP)and pro-BNP is elevated in patients with COVID-19 associated cardiac injury, irrespective of their prior heart failure status, and independently correlated with worst outcomes.Alongside these traditional biomarkers, novel cardiac bioenzymes including presepsin, soluble ST2 and copeptin, are also increasingly recognized as markers of cardiovascular injury in COVID-19 and can be associated with poor outcomes.CONCLUSION Assessment of cardiac bioenzymes at admission and their serial monitoring can help assess the severity of disease and predict mortality in patients with SARS-CoV-2 infection.Future studies are needed to elude the critical importance of novel biomarkers.Anjani Muthyala Sandeep Sasidharan Kevin John John Amos Lal Ajay K Mishra 2022World Journal of Virology2022,11,5:1
