1The effect of adjunct caudal block on postoperative analgesia in robot-assisted laparoscopic radical prostatectomy: A prospective randomized controlled, single blinded pilot study in a tertiary centre显示文摘Objective:Caudal block provides satisfactory postoperative pain relief in lower abdominal operations.This pilot study explores its safety and effect on postoperative pain control in patients who underwent robot-assisted laparoscopic radical prostatectomy(RARP).Methods:From 2013 to 2014,40 consecutive patients were randomized into two groups d one received caudal block using ropivacaine immediately after operation,the other received standard analgesia.Primary outcome measure was pain score based on 11-point Likert scale(0e10)recorded at recovery room,and at 6,12,24,48,and 72 h after operation.All analgesic requirements,opioid-related adverse events and time to passage of flatus were examined.Results:Mean age of the two groups was similar(60.4 vs.62.3 years,p Z 0.33),as was American Society of Anaesthesiologists(ASA)class,body mass index(BMI)and operation times.No significant difference in median pain scores was reported in recovery room(2 vs.3,p Z 0.34),and at 6 h(2 vs.2,p Z 0.94),12 h(0 vs.0,p Z 0.62),24 h(1 vs.0,p Z 0.58),48 h(1 vs.0,p Z 0.36)and 72 h(0 vs.0,p Z 0.78)postoperatively between control and caudal block groups,respectively.There was a higher mean opioid usage in the caudal block group which was not statistically significant.Although this was statistically insignificant while no significant difference in mean paracetamol usage was observed postoperatively.Median time to passage of flatus was similar(2.0 vs.2.0 days,p Z 0.97).There was one case of superficial wound infection and no opioid-related adverse events observed.Hospital stay was similar in both groups(2.5 vs.2.5 days,p Z 0.96).Kenneth Chen Allen Sim Alex Ford Kan 2018Asian Journal of Urology2018,5,2:3
2Enhancing IoT anomaly detection performance for federated learning显示文摘Federated Learning (FL) with mobile computing and the Internet of Things (IoT) is an effective cooperative learning approach. However, several technical challenges still need to be addressed. For instance, dividing the training process among several devices may impact the performance of Machine Learning (ML) algorithms, often significantly degrading prediction accuracy compared to centralized learning. One of the primary reasons for such performance degradation is that each device can access only a small fraction of data (that it generates), which limits the efficacy of the local ML model constructed on that device. The performance degradation could be exacerbated when the participating devices produce different classes of events, which is known as the class balance problem. Moreover, if the participating devices are of different types, each device may never observe the same types of events, which leads to the device heterogeneity problem. In this study, we investigate how data augmentation can be applied to address these challenges and improving detection performance in an anomaly detection task using IoT datasets. Our extensive experimental results with three publicly accessible IoT datasets show the performance improvement of up to 22.9% with the approach of data augmentation, compared to the baseline (without relying on data augmentation). 8A New Approach to Multivariate Network Traffic Analysis显示文摘Network traffic analysis is one of the core functions in network monitoring for effective network operations and management.While online traffic analysis has been widely studied,it is still intensively challenging due to several reasons.One of the primary challenges is the heavy volume of traffic to analyze within a finite amount of time due to the increasing network bandwidth.Another important challenge for effective traffic analysis is to support multivariate functions of traffic variables to help administrators identify unexpected network events intuitively.To this end,we propose a new approach with the multivariate analysis that offers a high-level summary of the online network traffic.With this approach,the current state of the network will display patterns compiled from a set of traffic variables,and the detection problems in network monitoring(e.g.,change detection and anomaly detection)can be reduced to a pattern identification and classification problem.In this paper,we introduce our preliminary work with clustered patterns for online,multivariate network traffic analysis with the challenges and limitations we observed.We then present a grid-based model that is designed to overcome the limitations of the clustered pattern-based technique.We will discuss the potential of the new model with respect to the technical challenges including streaming-based computation and robustness to outliers.Jinoh Kim Alex Sim 2019Journal of Computer Science & Technology2019,34,2:0
9Risk factors for hamstring injuries in Australian male professional cricket players显示文摘Background:Injuries to the hamstring are relatively common in professional cricketers(as they are in many team sports) and have increased in incidence in the 'T20 era'(introduction of 20-over matches) of cricket since 2006.Methods:This study analyzed incidence of hamstring injury in the various elite male match types over a 20-year period(1995–1996 to 2014–2015seasons).Risk factors for hamstring strain were assessed using a multivariate logistic regression analysis technique.Results:There were 276 match time-loss hamstring injuries recorded over a 20-year period at the Australian state or national player level,of which170 occurred in one of 40,145 player match sets.The overall rate of match onset rate was 22.5 hamstring injuries per 1000 team days.Fast bowling onset injuries were the highest subcategory at a rate of 10.9 injuries per 1000 team days,although batting onset injuries were particularly common in 50-over(one day) international matches.Significant risk factors in logistic regression analysis,in addition to hamstring injury history,were being a fast bowler relative risk(RR) 2.5(95% confidence interval(CI):1.3–4.5) and playing a match in Australia RR 2.3(95%CI:1.3–3.9).Conclusion:Fast bowlers suffer more hamstring injuries than other playing roles in cricket,particularly in First Class(multi-day) cricket.Batsmen are more likely to get injured in 50-over(one day) cricket.Playing in Australia(compared to overseas venues) leads to increased risk of hamstring injury.John W.Orchard Alex Kountouris Kevin Sims 2017Journal of Sport and Health Science2017,6,3:0
10Painful sesamoid of the great toe显示文摘The painful sesamoid can be a chronic and disabling problem and isolating the cause can be far from straightforward. There are a number of forefoot pathologies that can present similarly to sesmoid pathologies and likewise identifying the particular cause of sesamoid pain can be challenging. Modern imaging techniques can be helpful. This article reviews the anatomy,development and morphological variability present in the sesamoids of the great toe. We review evidence on approach to history, diagnosis and investigation of sesamoid pain. Differential diagnoses and management strategies, including conservative and operative are outlined. Our recommendations are that early consideration of magnetic resonance imaging and discussion with a specialist musculoskeletal radiologist may help to identify a cause of pain accurately and quickly. 11Larvicidal Evaluation of Aedes aegypti and Antioxidant,Cytotoxic and Antimicrobial Potential of the Aqueous Acid Extract of Pseudoxandra cuspidata(ANNOACEAE).显示文摘The Anonnaceae family has 2,500 species and 135 genera, which has about 14 representatives of species in Brazil,including Pseudoxandra cuspidata. The objective of this research was to study the aqueous-acid extract of P. cuspidata, evaluatingits antioxidant, larvicidal, antimicrobial and cytotoxic potential. In the preliminary phytochemical analysis, there were positivity toorganicacids and alkaloids. Inhibitory Activity of EAA was 65% against DPPH. The AAE presented to A. Salina nauplii at LC50%of 600.79 μg/mL for a periodof 24 hours. In relation to larvicidal activity to Aedes aegypti intermediates, AAE presented the LC50%in 24 Hours of 475.91 ppm and 290.73 ppm in 48 Hours. Regarding the antimicrobial action, the EAA presented 50 mg/mL MIC forP. Aeruginosa and MBC at 100 mg/mL, and for S. aureus it presented MIC of 50 mg/mL and did not present MBC. This researchcharacterized the presence of alkaloids and organicacids present in the aqueousacid extract of P. cuspidata. The extract presentedlowtoxicity in relation to the microcrustacean A. salina. It also had a relevant antioxidant potential (65% of Inhibition),larvicidalaction considered effective, antimicrobialaction for both P. Aeruginosa and S.
