1Continuous regional arterial infusion therapy with gabexate mesilate for severe acute pancreatitis显示文摘AIM: To evaluate the effi cacy of continuous regional arterial infusion therapy (CRAI) with gabexate mesilate and antibiotics for severe acute pancreatitis (SAP). METHODS: We conducted a prospective study on patients who developed SAP with or without CRAI. Out of 18 patients fulf illed clinical diagnostic criteria for SAP in Japan, 9 patients underwent CRAI, while 9 patients underwent conventional systemic protease inhibitor and antibiotics therapy (non-CRAI). CRAI was initiated within 72 h of the onset of pancreatitis. Gabexate mesilate (2400 mg/d) was continuously administered for 3 to 5 d. The clinical outcome including serum infl ammation-related parameters were examined. RESULTS: The duration of abdominal pain in the CRAI group was 1.9 ± 0.26 d, whereas that in the non-CRAI group was 4.3 ± 0.50. The duration of SIRS in the CRAI group was 2.2 ± 0.22 d, whereas that in the non- CRAI group was 3.2 ± 0.28. Abdominal pain and SIRS disappeared signifi cantly in a short period of time after the initiation of CRAI using gabexate mesilate. The average length of hospitalization signifi cantly differed between the CRAI and non-CRAI groups, 53.3 ± 7.9 d and 87.4 ± 13.9 d, respectively. During the first two weeks, levels of serum CRP and the IL6/IL10 ratio in the CRAI group tended to have a rapid decrease compared to those in the non-CRAI group. 2Pancreatic neuroendocrine tumor and solid-pseudopapillary neoplasm: Key immunohistochemical profiles for differential diagnosis显示文摘AIM To reveal better diagnostic markers for differentiating neuroendocrine tumor(NET) from solid-pseudopapillary neoplasm(SPN), focusing primarily on immunohistochemical analysis.METHODS We reviewed 30 pancreatic surgical specimens of NET(24 cases) and SPN(6 cases). We carried out comprehensive immunohistochemical profiling using 9 markers: Synaptophysin, chromogranin A, pancytokeratin, E-cadherin, progesterone receptor,vimentin, α-1-antitrypsin, CD10, and β-catenin.RESULTS E-cadherin staining in NETs, and nuclear labeling of β-catenin in SPNs were the most sensitive and specific markers. Dot-like staining of chromogranin A might indicate the possibility of SPNs rather than NETs. The other six markers were not useful because their expression overlapped widely between NETs and SPNs. Moreover, two cases that had been initially diagnosed as NETs on the basis of their morphological features, demonstrated SPN-like immunohistochemical profiles. 3Advanced diffusion magnetic resonance imaging in patients with Alzheimer’s and Parkinson’s diseases显示文摘The prevalence of neurodegenerative diseases is increasing as human longevity increases. The objective biomarkers that enable the staging and early diagnosis of neurodegenerative diseases are eagerly anticipated. It has recently become possible to determine pathological changes in the brain without autopsy with the advancement of diffusion magnetic resonance imaging techniques. Diffusion magnetic resonance imaging is a robust tool used to evaluate brain microstructural complexity and integrity, axonal order, density, and myelination via the micron-scale displacement of water molecules diffusing in tissues. Diffusion tensor imaging, a type of diffusion magnetic resonance imaging technique is widely utilized in clinical and research settings;however, it has several limitations. To overcome these limitations, cutting-edge diffusion magnetic resonance imaging techniques, such as diffusional kurtosis imaging, neurite orientation dispersion and density imaging, and free water imaging, have been recently proposed and applied to evaluate the pathology of neurodegenerative diseases. 6Usefulness of urinary trypsinogen-2 and trypsinogen activation peptide in acute pancreatitis:A multicenter study in Japan显示文摘BACKGROUND Rapid urinary trypsinogen-2 dipstick test and levels of urinary trypsinogen-2 and trypsinogen activation peptide(TAP) concentration have been reported as prognostic markers for the diagnosis of acute pancreatitis.AIM To reconfirm the validity of all these markers in the diagnosis of acute pancreatitis by undertaking a multi-center study in Japan.METHODS Patients with acute abdominal pain were recruited from 17 medical institutions in Japan from April 2009 to December 2012. Urinary and serum samples were collected twice, at enrollment and on the following day for measuring target markers. The diagnosis and severity assessment of acute pancreatitis were assessed based on prognostic factors and computed tomography(CT) Grade of the Japanese Ministry of Health, Labour, and Welfare criteria.RESULTS A total of 94 patients were enrolled during the study period. The trypsinogen-2 dipstick test was positive in 57 of 78 patients with acute pancreatitis(sensitivity,73.1%) and in 6 of 16 patients with abdominal pain but without any evidence of acute pancreatitis(specificity, 62.5%). The area under the curve(AUC) score of urinary trypsinogen-2 according to prognostic factors was 0.704, which was highest in all parameter. The AUC scores of urinary trypsinogen-2 and TAP according to CT Grade were 0.701 and 0.692, respectively, which shows higher than other pancreatic enzymes. The levels of urinary trypsinogen-2 and TAP were significantly higher in patients with extended extra-pancreatic inflammation as evaluated by CT Grade.CONCLUSION We reconfirmed urinary trypsinogen-2 dipstick test is useful as a marker for the diagnosis of acute pancreatitis. 7Preparation and Characterization of Polymeric Microspheres Having Isotactic Poly(methacrylic acid) on Their Surfaces显示文摘A poly( tert -butyl methacrylate)(PBMA) macromonomer was synthesized by anionic polymerization with bis(2,6-di- t -butyl phenoxy) methyl aluminum [MeAl(ODBP)2] as the initiator and trimethylsilylmethacrylate(TMSMA) as the end capping agent in dry THF. Then, a poly(methacrylic acid)(PMAA) macromonomer was obtained by means of hydrolysis reaction from the PBMA macromonomer in the presence of hydrochloric acid. The structures of the PBMA macromonomer were characterized by using 1H NMR. It was found that the resulted PBMA macromonomer has highly isotactic properties and the PMAA macromonomer has an end vinyl group on per polymer chain. The monodisperse polymeric microspheres, which consisted of polystyrene cores and PMAA branches on their surfaces, were prepared by the dispersion copolymerization of styrene with the PMAA macromonomer in an ethanol/water mixed solvent. 9Thermosensitive Particles with Unusual Morphology Prepared by Dispersion Copolymerization显示文摘The monodispersed polymeric particles with an unusual structure were prepared by the dispersion copolymerization of acrylonitrile/styrene(AN/St) in mixed solvents of ethanol/water by using the poly(N-isopropylacrylamide) (PNIPAAm) macromonomer as a reaction stabilizer. It was found that the AN monomer plays a key role in the formation of the particles with special morphology analyzed via scanning electron microscopy(SEM). The reaction parameters have remarkable influences on the particle size and morphology. 10Increase in medium-size rainfall events will enhance the C-sequestration capacity of biological soil crusts显示文摘Biological soil crusts(BSCs) play important roles in the carbon(C) balance in arid regions. Net C balance of BSCs is strongly dependent on rainfall and consequent activation of microbes in the BSCs. The compensation-rainfall size for BSCs(the minimum rainfall amount for a positive net C balance) is assumed to be different with BSCs of different developmental stages. A field experiment with simulated rainfall amount(SRA) of 0, 1, 5, 10, 20, and 40 mm was conducted to examine the C fluxes and compensation-rainfall size of BSCs in different parts of fixed dunes in the ecotone between the Badain Jaran Desert and the Minqin Oasis. We found algae-lichen crust on the interdunes and crest, algae crust on the leeward side, and lichen-moss crust on the windward. Even a small rainfall(1 mm) can activate both photosynthesis and respiration of all types of BSCs. The gross ecosystem production, ecosystem respiration, and net ecosystem exchange were significantly affected by SRA, hours after the simulated rainfall, position on a dune, and their interactions. The rapid activation of photosynthesis provides a C source and therefore could be responsible for the increase of C efflux after each rewetting. C-uptake and-emission capacity of all the BSCs positively correlated with rainfall size, with the lowest C fluxes on the leeward side. The compensation rainfall for a net C uptake was 3.80, 15.54, 8.62, and 1.88 mm for BSCs on the interdunes, the leeward side, the crest, and the windward side, respectively. The whole dune started to show a net C uptake with an SRA of 5 mm and maximized with an SRA of about 30 mm. The compensation-rainfall size is negatively correlated with chlorophyll content. 15WRF Environment Assessment in Guangzhou City with an Extracted Land-use Map from the Remote Sensing Data in 2000 as an Example显示文摘Mesoscale urban environment forecast combined with WRF is a current frontier in international academic. Taking Guangzhou as an example,the new land-use model presented by the present authors,the existing USGS and MODIS models in WRF were firstly compared to the remote sensing model in this article.The comparison result shows that the land-use model presented by the authors has the highest similarity with the remote sensing data. Secondly,the new land-use model was used to replace the defaulted land-use data in WRF for simulation. By comparing this simulation results with the WRF results using the defaulted USGS and MODIS model,it was showed that the geographic models have a great impact on the mesoscale environment forecast result. Also,the geographic information model presented in this article shows the best accuracy when comparing with the observation data.
