| 1 | Simulating hydrological responses to climate change using dynamic and statistical downscaling methods: a case study in the Kaidu River Basin, Xinjiang, China显示文摘Climate change may affect water resources by altering various processes in natural ecosystems. Dynamic and statistical downscaling methods are commonly used to assess the impacts of climate change on water resources. Objectively, both methods have their own advantages and disadvantages. In the present study, we assessed the impacts of climate change on water resources during the future periods (2020–2029 and 2040–2049) in the upper reaches of the Kaidu River Basin, Xinjiang, China, and discussed the uncertainties in the research processes by integrating dynamic and statistical downscaling methods (regional climate models (RCMs) and general circulation modes (GCMs)) and utilizing these outputs. The reference period for this study is 1990–1999. The climate change trend is represented by three bias-corrected RCMs (i.e., Hadley Centre Global Environmental Model version 3 regional climate model (HadGEM3-RA), Regional Climate Model version 4 (RegCM4), and Seoul National University Meso-scale Model version 5 (SUN-MM5)) and an ensemble of GCMs on the basis of delta change method under two future scenarios (RCP4.5 and RCP8.5). We applied the hydrological SWAT (Soil and Water Assessment Tool) model which uses the RCMs/GCMs outputs as input to analyze the impacts of climate change on the stream flow and peak flow of the upper reaches of the Kaidu River Basin. The simulation of climate factors under future scenarios indicates that both temperature and precipitation in the study area will increase in the future compared with the reference period, with the largest increase of annual mean temperature and largest percentage increase of mean annual precipitation being of 2.4°C and 38.4%, respectively. Based on the results from bias correction of climate model outputs, we conclude that the accuracy of RCM (regional climate model) simulation is much better for temperature than for precipitation. The percentage increase in precipitation simulated by the three RCMs is generally higher than that simulated by the ensemble of GCMs. As for the changes in seasonal precipitation, RCMs exhibit a large percentage increase in seasonal precipitation in the wet season, while the ensemble of GCMs shows a large percentage increase in the dry season. Most of the hydrological simulations indicate that the total stream flow will decrease in the future due to the increase of evaporation, and the maximum percentage decrease can reach up to 22.3%. The possibility of peak flow increasing in the future is expected to higher than 99%. | 5 | Rates and risk factors for suicidal ideation,suicide attempts and suicide deaths in persons with HIV:a systematic review and meta-analysis显示文摘Background People living with HIV/AIDS(PLWHA)must contend with a significant burden of disease.However,current studies of this demographic have yielded wide variations in the incidence of suicidality(defined as suicidal ideation,suicide attempt and suicide deaths).Aims This systematic review and meta-analysis aimed to assess the lifetime incidence and prevalence of suicidality in PLWHA.Methods Publications were identified from PubMed(MEDLINE),SCOPUS,OVID(MEDLINE),Joanna Briggs Institute EBP and Cochrane Library databases(from inception to before 1 February 2020).The search strategy included a combination of Medical Subject Headings associated with suicide and HIV.Researchers independently screened records,extracted outcome measures and assessed study quality.Data were pooled using a random-effects model.Subgroup and meta-regression analyses were conducted to explore the associated risk factors and to identify the sources of heterogeneity.Main outcomes were lifetime incidence of suicide completion and lifetime incidence and prevalence of suicidal ideation and suicide attempt.Results A total of 185199 PLWHA were identified from 40 studies(12 cohorts,27 cross-sectional and 1 nested case-control).The overall incidence of suicide completion in PLWHA was 10.2/1000 persons(95%CI:4.5 to 23.1),translating to 100-fold higher suicide deaths than the global general population rate of 0.11/1000 persons.The lifetime prevalence of suicide attempts was 158.3/1000 persons(95%CI:106.9 to 228.2)and of suicidal ideation was 228.3/1000 persons(95%CI:150.8 to 330.1).Meta-regression revealed that for every 10-percentage point increase in the proportion of people living with HIV with advanced disease(AIDS),the risk of suicide completion increased by 34 per 1000 persons.The quality of evidence by Grading of Recommendations,Assessment,Development and Evaluations for the suicide deaths was graded as‘moderate’quality.Conclusions The risk of suicide death is 100-fold higher in people living with HIV than in the general population.Lifetime incidence of suicidal ideation and attempts are substantially high.Suicide risk assessments should be a priority in PLWHA,especially for those with more advanced disease. | Matt Pelton Matt Ciarletta Holly Wisnousky Nicholas Lazzara Monica Manglani Djibril M Ba Vernon M Chinchillli Ping Du Anna E Ssentongo Paddy Ssentongo | 2021 | General Psychiatry2021,34,2: | 3 |
| 6 | Bilateral adrenocortical adenomas causing adrenocorticotropic hormone-independent Cushing's syndrome: A case report and review of the literature显示文摘BACKGROUND Adrenocorticotropic hormone(ACTH)-independent Cushing's syndrome(CS) is mostly due to unilateral tumors, with bilateral tumors rarely reported. Its common causes include primary pigmented nodular adrenocortical disease,ACTH-independent macronodular adrenal hyperplasia, and bilateral adrenocortical adenomas(BAAs) or carcinomas. BAAs causing ACTHindependent CS are rare; up to now, fewer than 40 BAA cases have been reported. The accurate diagnosis and evaluation of BAAs are critical for determining optimal treatment options. Adrenal vein sampling(AVS) is a good way to diagnose ACTH-independent CS.CASE SUMMARY A 31-year-old woman had a typical appearance of CS. The oral glucose tolerance test showed impaired glucose tolerance and obviously increased insulin and Cpeptide levels. Her baseline serum cortisol and urine free cortisol were elevated and did not show either a circadian rhythm or suppression with dexamethasone administration. The peripheral 1-deamino-8-D-arginine-vasopressin(DDVAP)stimulation test showed a delay of the peak level, which was 1.05 times as high as the baseline level. Bilateral AVS results suggested the possibility of BAAs.Abdominal computed tomography showed bilateral adrenal adenomas with atrophic adrenal glands(right: 3.1 cm × 2.0 cm × 1.9 cm; left: 2.2 cm × 1.9 cm × 2.1 cm). Magnetic resonance imaging of the pituitary gland demonstrated normal findings. A left adenomectomy by retroperitoneoscopy was performed first,followed by resection of the right-side adrenal mass 3 mo later. Biopsy results of both adenomas showed cortical tumors. Evaluations of ACTH and cortisol showed a significant decrease after left adenomectomy but could still not be suppressed, and the circadian rhythm was absent. Following bilateral adenomectomy, this patient has been administered with prednisone until now,all of her symptoms were alleviated, and she had normal blood pressure without edema in either of her lower extremities.CONCLUSION BAAs causing ACTH-independent CS are rare.
