| 1 | Incidence of hepatocellular carcinoma in outpatients with cirrhosis in Brazil: A 10-year retrospective cohort study显示文摘AIM To determine the incidence of hepatocellular carcinoma(HCC) and the impact of HCC surveillance on early diagnosis and survival of cirrhotic outpatients. METHODS In this retrospective cohort study, cirrhotic outpatients undergoing HCC surveillance between March 2005 and March 2014 were analyzed. Exclusion criteria were HIV coinfection; previous organ transplantation; diagnosis of HCC at first consultation; missing data in the medical chart; and less than 1 year of follow-up. Surveillance was carried out every six months using ultrasound and serum alpha-fetoprotein determination. Ten-year cumulative incidence and survival were estimated through Kaplan-Meier analysis. RESULTS Four hundred and fifty-three patients were enrolled, of which 57.6% were male. Mean age was 55 years. Hepatitis C virus and heavy use of alcohol were the main etiologic agents of cirrhosis. HCC was diagnosed in 75 patients(16.6%), with an estimated cumulative incidence of 2.6% in the 1st year, 15.4% in the 5th year, and 28.8% in the 10 th year. Median survival was estimated at 17.6 mo in HCC patients compared to 234 mo in non-HCC patients(P < 0.001). Early-stage HCC was more often detected in patients who underwent surveillance every 6 mo or less(P = 0.05). | 4 | Polymorphisms and resistance mutations of hepatitis C virus on sequences in the European hepatitis C virus database显示文摘AIM To evaluate the occurrence of resistant mutations in treatment-na?ve hepatitis C virus(HCV) sequences deposited in the European hepatitis C virus database(euH CVdb). METHODS The sequences were downloaded from the eu HCVdb(http://gffzzc7adb7d73a2545c0s5xn95cvp6f5f6fqx.ffgz.tsg.suse.edu.cn/eu HCVdb/). The search was performed for full-length NS3 protease, NS5 A and NS5 B polymerase sequences of HCV, separated by genotypes 1a, 1b, 2a, 2b and 3a, and resulted in 798 NS3, 708 NS5 A and 535 NS5 B sequences from HCV genotypes1a, 1b, 2a, 2b and 3a, after the exclusion of sequences containing errors and/or gaps or incomplete sequences, and sequences from patients previously treated with direct antiviral agents(DAA). The sequence alignment was performed with MEGA 6.06 MAC and the resulting protein sequences were then analyzed using the BioE dit 7.2.5. for mutations associated with resistance. Only positions that have been described as being associated with failure in treatment in in vivo studies, and/or as conferring a more than 2-fold change in replication in comparison to the wildtype reference strain in in vitro phenotypic assays were included in the analysis.RESULTS The Q80 K variant in the NS3 gene was the most prevalent mutation, being found in 44.66% of subtype 1a and 0.25% of subtype 1b. Other frequent mutations observed in more than 2% of the NS3 sequences were: I170V(3.21%) in genotype 1a, and Y56F(15.93%), V132I(23.28%) and I170V(65.20%) in genotype 1b. For the NS5 A, 2.21% of the genotype 1a sequences have the P58 S mutation, 5.95% of genotype 1b sequences have the R30 Q mutation, 15.79% of subtypes 2a sequences have the Q30 R mutation, 23.08% of subtype 2b sequences have a L31 M mutation, and in subtype 3a sequences, 23.08% have the M31 L resistant variants. For the NS5 B, the V321 L RAV was identified in 0.60% of genotype 1a and in 0.32% of genotype 1b sequences, and the N142 T variant was observed in 0.32% of subtype 1b sequences. | 17 | Serological evidence of Toxoplasma gondii infection as potential risk for the development of lepromatous leprosy in an endemic area for both neglected tropical diseases in Brazil显示文摘Background:Mycobacterium leprae and Toxoplasma gondii infections are both neglected tropical diseases highly prevalent in Brazil.Infection with certain parasite species can significantly alter susceptibility to other important pathogens,and/or influence the development of pathology.Here we investigated the possible influence of M.leprae/T.gondii co-parasitism on the manifestation of leprosy and its clinical forms.Methods:Participants(n=291)were recruited in Campos dos Goytacazes city,Rio de Janeiro state,southeast Brazil,from August 2015 to December 2019 and clinically diagnosed for leprosy.Participants were selected based on the presence(patients)or absence(healthy controls)of the leprosy disease.Contacts of patients were also recruited for this study.Serum samples from patients(n=199)with leprosy,contacts(n=40)and healthy controls(n=52)were investigated for levels of IgM and IgG anti-phenolic glycolipid-1(PGL-1)by ELISA.Additionally,IgG antibody against soluble Toxoplasma antigen(STAg)was measured in sera samples from leprosy patients,contacts and healthy controls for Toxoplasma gondii serology by ELISA.Anti-PGL-1 IgG and IgM levels were compared using one-way ANOVA Kruskal-Wallis or Mann-Whitney,while Spearman test was used to correlate levels of IgG anti-STAg and IgM/IgG anti-PGL-1 from seropositive and seronegative individuals for T.gondii infection.The risk of T.gondii infection for leprosy disease was assessed using Fisher’s test.Results:Levels of IgM anti-PGL-1 antibodies were significantly higher in multibacillary(MB)patients compared to paucibacillary(PB)patients(P=0.0068).Higher IgM and IgG levels anti-PGL-1 were detected in patients with the lepromatous forms.The serologic prevalence for T.gondii infection was 74.9%.We detected increased anti-STAg antibody levels in leprosy patients(79.4%),reaching 88.8%within those with lepromatous form of this disease.The leprosy risk increase in T.gondii seropositive individuals was two-fold(odds ratio[OR]=2.055;95% confidence intervals[95%CI]:1.18–3.51)higher than those seronegative,and considering the lepromatous leprosy risk this increase was even dramatic(OR=4.33;95%CI:1.76–9.69)in T.gondii seropositive individuals.Moreover the leprosy risk in T.gondii seropositive individuals was weakly correlated to the levels of IgG anti-STAg and IgM/IgG anti-PGL-1.Conclusions:Altogether,our results suggest that T.gondii infection may exert immunomodulatory properties that influence to the susceptibility of leprosy,mainly on its more severe clinical form.A better understanding of parasite immunomodulation can ultimately contribute to the development of medical applications. | Luciana Regina Pereira Oliveira Lívia Mattos Martins Rebeka da Conceição Souza Yuri Scheidegger de Castro Letícia Silva Nascimento Juliana Azevedo da Silva Edilbert Pellegrini Nahn Junior Wilmar Dias da Silva Alba Lucínia Peixoto-Rangel | 2020 | Infectious Diseases of Poverty2020,9,1: | 0 |
