| 9 | Rectal arterio-portal fistula: An unusual cause of persistent bleeding per rectum following a proximal spleno-renal shunt显示文摘Gastrointestinal arterio-venous malformations are a known cause of gastrointestinal bleeding. We present a rare case of persistent rectal bleeding due to a rectal arterio-portal venous fistula in the setting of portal hypertension secondary to portal vein thrombosis. The portal hypertension was initially surgically treated with splenectomy and a proximal splenorenal shunt. However, rectal bleeding persisted even after surgery, presenting us with a diagnostic dilemma. The patient was re-evaluated with a computed tomography mesenteric angiogram which revealed a rectal arterio-portal fistula. Arterio-portal fistulas are a known but rare cause of portal hypertension, and possibly the underlying cause of continued rectal bleeding in this case. This was successfully treated using angiographic localizationand super-selective embolization of the rectal arterioportal venous fistula via the right internal iliac artery. The patient subsequently went on to have a full term pregnancy. | 11 | Cap polyposis: A rare cause of rectal bleeding in children显示文摘AIM: To evaluate the clinicopathological features and treatment outcomes of cap polyposis in the pediatric population. METHODS: All pediatric patients with histologically proven diagnosis of cap polyposis were identified from our endoscopy and histology database over a 12 year period from 2000-2012 at our tertiary pediatric center, KK Women's and Children's Hospital in Singapore. The case records of these patients were retrospectively reviewed. The demographics, clinical course, laboratory results, endoscopic and histopathological features, treatments, and outcomes were analyzed. The study protocol was approved by the hospital institutional review board. The histological slides were reviewed by a pediatric histopathologist to confirm the diagnosis of cap polyposis. RESULTS: Eleven patients were diagnosed with cap polyposis. The median patient age was 13 years (range 5-17 years); the sample included 7 males and 4 females. All of the patients presented with bloody stools. Seven patients (63%) had constipation, while 4 patients (36%) had diarrhea. All of the patients underwent colonoscopy and polypectomies (excluding 1 patient who refused polypectomy). The macroscopic findings were of polypoid lesions covered by fibrinopurulent exudates with normal intervening mucosa. The rectum was the most common involvement site (n = 9, 82%), followed by the rectosigmoid colon (n = 3, 18%). Five (45%) patients had fewer than 5 polyps, and 6 patients (65%) had multiple polyps. Histological examination of these polyps showed surface ulcerations with a cap of fibrin inflammatory exudate. Four (80%) patients with fewer than 5 polyps had complete resolution of symptoms following the polypectomy. One patient who did not consent to the polypectomy had resolution of symptoms after being treated with sulphasalazine. All 6 patients with multiple polyps experienced recurrence of bloody stools on follow-up (mean = 28 mo). CONCLUSION: Cap polyposis is a rare and underrecognised cause of rectal bleeding in children. | 13 | Antiviral activity of traditional Chinese medicinal plants Dryopteris crassirhizoma and Morus alba against dengue virus显示文摘Dengue virus(DENV) has emerged as a major virus that is spread by mosquitoes. Recently, it has spread to more than a hundred nations but continues to lack specific treatable medication. Many traditional Chinese medicinal(TCM) plants are in practice for dengue fever in dengue endemic regions. These traditional medicines persevere with treatments, which modern medicines lack. The study aims to substantiate the anti-dengue potential of some traditional herbs and make them available for further studies to facilitate TCM users. Twelve TCM plants aqueous extracts were evaluated, which are described as cool herbs used for the diseases with high fever. Lead plants were established through detailed in vitro foci forming unit reduction analysis(FFURA) against all four serotypes and validated through quantitative real-time RT-PCR(q RT-PCR). Four plants potentially inhibited the virus in primary phenotypic in vitro evaluation. Two lead plants Dryopteris crassirhizoma(DC) and Morus alba(MA) were identified with half minimal inhibitory concentration(IC50) 130 and 221 μg m L^-1, respectively, while the selectivity indices(SI) were 4.21 and 4.62, respectively. Lead plants equally inhibited all four serotypes of DENV. Time-of-addition analysis suggested that, DC was active at later stages of viral replication, whereas MA was active during the early stages and even showed some prophylactic activity. Liquid chromatography-mass spectrometry(HPLC/MS) analysis revealed presence of flavonoids. DC and MA are identified as potential anti-dengue plants, active against varied stages of dengue virus replication cycle. | 18 | Effect of Microwave Power and Clamping Pressure on the Microwave Welding of Polypropylene Using Silicon Carbide Nanowhiskers as Microwave Susceptor显示文摘Due to their excellent dielectric properties and the rapid response to microwave irradiation,silicon carbide nanowhiskers(SiCNWs)were employed as microwave susceptor in this study to absorb microwave and locally melt the surrounding polypropylene(PP)substrates for the joining of PP substrates.Complete welded joint is formed after the melted PP was cooled and resolidified.Other than microwave susceptor,SiCNWs also acted as the nanofillers in strengthening the welded joint through the formation of SiCNWs reinforced PP nanocomposite at the interface of PP substrates.Besides,the effect of microwave power on the microwave welding of PP substrates using SiCNWs as susceptor was studied and reported.It was found that the tensile strength and modulus of elasticity of the welded joint improved as microwave power increased.However,it deteriorates the flexibility of the welded joint as high stiffness SiCNWs were incorporated deeper into the PP matrix which restricted the PP chain mobility.Aside from microwave power,clamping pressure is also critical in determining the mechanical properties of a welded joint.When compared to unclamped welded joint,the tensile strength,modulus of elasticity and flexibility of welded joint subjected to clamping pressure improved drastically.Moreover,the tensile strength of welded joint increased when the clamping pressure was increased from P1 to P3,but decreased when the clamping pressure was further increased to P4 due to the occurrence of flashing at welded joint.The formation mechanism of SiCNWs reinforced PP welded joint was also proposed in this study.Compared to conventional welding,this welding process is easy,straightforward and is able to produce welded joint with outstanding mechanical properties via precise controlling of the processing parameters.Thus,microwave welding is thought to offer an option for the joining of thermoplastics and other applications. | Phey Yee Foong Chun Hong Voon Bee Ying Lim Pei Leng Teh Mohd Afendi Bin Rojan Nor Azizah Parmin Subash C.B.Gopinath Mohd Khairuddin Md Arshad Foo Wah Low Savisha Mahalingam Abreeza Manap Ruslinda A.Rahim Uda Hashim | 2022 | Chinese Journal of Polymer Science2022,40,11: | 0 |
