| 3 | Efficacy of early treatment with infliximab in pediatric Crohn’s disease显示文摘AIM: To investigate the effectiveness of early infliximab use for induction and maintenance therapy in pediatric Crohn’s disease. METHODS: We performed a retrospective chart review of 36 patients with Crohn’s disease. Ten patients (group A) were treated with mesalamine after induction therapy with oral prednisolone, and 13 patients (group B) were treated with azathioprine after induction therapy with oral prednisolone. Thirteen patients (group C) received infliximab and azathioprine for induction and maintenance therapy for the first year, and were treated with azathioprine after 1 year. All patients were followed for at least 24 mo. Efficacy was determined by the relapse rate using the pediatric Crohn’s disease activity index score in each group at 12 and 24 mo. RESULTS: At the 1 year follow-up, the relapse ratee, Mi Jin Kim, Hae Jeong Lee, Yon Ho Choe (23.1%, 3 of 13 patients) in group C was lower than that (61.5%, 8 of 13 patients) in group B (P = 0.047). At the 2 years follow-up, the relapse rate (38.5%, 5 of 13 patients) in group C was lower than that (76.9%, 10 of 13 patients) in group B (P = 0.047). Adverse events in group C were fewer than in groups A and B. | 4 | Is cholecystectomy a reasonable treatment option for simple gallbladder polyps larger than 10 mm?显示文摘AIM: To determine the relevance of the 10-mm size criterion of the generally accepted surgical indication for gallbladder polyps(GBPs).METHODS: We collected data of patients who were confirmed to have GBPs through cholecystectomy at Samsung Medical Center between January 1997 andDecember 2012.Among the patients who underwent cholecystectomy for GBP, those with a definite evidence for malignancy such as adjacent organ invasion, metastasis on preoperative imaging studies, polyp larger than 20 mm, absence of preoperative imaging study results, and patients having gallstones were excluded.We retrospectively collected and analyzed information on patient's clinical characteristics, symptoms, ultrasonographic findings, and blood laboratory tests.RESULTS: A total of 836 patients who had undergone cholecystectomy were retrospectively analyzed.Seven hundred eighty patients(93%) had benign polyps, whereas 56 patients(7%) had malignant polyps.Of the 56 patients with malignancy, 4 patients(7%) had borderline GBP(10-12 mm) and a patient had small GBP(< 10 mm) with T2 stage.We conducted an ROC curve analysis to verify the 10-mm size criteria(AUC = 0.887, SD = 0.21, P < 0.001).In the ROC curve for polyp size and malignancy, sensitivity and specificity of the 10-mm size criterion was 98.2% and 19.6%, respectively.The specificity of the 11-mm and 12-mm size criteria was 44.6% and 56%, respectively, whereas the sensitivity of these two size criteria was similar.We defined the GBPs of 10 to 12 mm as a borderline-sized GBP, which were found in 411 patients(49%).In this group, there was a significant difference in age between patients with benign and malignant GBPs(47 years vs 60 years, P < 0.05).CONCLUSION: GBPs larger than 13 mm need immediate excision whereas for borderline-sized GBPs detected in young patients, careful medical observation can be a rational decision. | Hye Yon Park Se Hoon Oh Kwang Hyuck Lee Jong Kyun Lee Kyu Taek Lee | 2015 | World Journal of Gastroenterology2015，21，14： | 9 |
| 5 | Effect of Elevated CO_2 and Drought on Soil Microbial Communities Associated with Andropogon gerardii显示文摘Our understanding of the effects of elevated atmospheric CO2,singly and in combination with other environmental changes,on plant-soil interactions is incomplete.Elevated CO2 effects on C4 plants,though smaller than on C3 species,are mediated mostly via decreased stomatal conductance and thus water loss.Therefore,we characterized the interactive effect of elevated CO2 and drought on soil microbial communities associated with a dominant C4 prairie grass,Andropogon gerardii Vitman.Elevated CO2 and drought both affected resources available to the soil microbial community.For example,elevated CO2 increased the soil C:N ratio and water content during drought,whereas drought alone decreased both.Drought significantly decreased soil microbial biomass.In contrast,elevated CO2 increased biomass while ameliorating biomass decreases that were induced under drought.Total and active direct bacterial counts and carbon substrate use (overall use and number of used sources) increased significantly under elevated CO2.Denaturing gradient gel electrophoresis analysis revealed that drought and elevated CO2,singly and combined,did not affect the soil bacteria community structure.We conclude that elevated CO2 alone increased bacterial abundance and microbial activity and carbon use,probably in response to increased root exudation.Elevated CO2 also limited drought-related impacts on microbial activity and biomass,which likely resulted from decreased plant water use under elevated CO2.These are among the first results showing that elevated CO2 and drought work in opposition to modulate plant-associated soil-bacteria responses,which should then influence soil resources and plant and ecosystem function. | Issmat I. | 6 | Combined silicosis and mixed dust pneumoconiosis with rapid progression: A case report and literature review显示文摘BACKGROUND Rapidly progressive pneumoconiosis(RPP) occasionally occurs in coal workers, particularly those with high exposure to silica. Here, we report the case of a 64-year-old male miller with RPP. CASE SUMMARY The patient had a persistent cough for one month and had been clinically diagnosed with pulmonary tuberculosis in 2011. He worked in a stone processing factory from the ages of 20 through 37 and has owned his own mill for the past 25 years. His chest radiograph showed significant increases in the size and number of lung nodules since his last follow-up in 2013. By percutaneous needle lung biopsy, the nodular lesions showed diffuse infiltration of phagocytic macrophages and birefringent crystals by polarizing microscopy. He was finally diagnosed with RPP of mixed dust pneumoconiosis combined with silicosis. We transformed the native tobacco,Nicotiana attenuata to silence two core clock components,NaLHY(irLHY) and NaTOC_1(irTOC_1). We characterized growth and light-and dark-adapted photosynthetic rates(A_c) throughout a 24 h day in empty vector-transformed(EV),irLHY,and irTOC_1 plants in the field,and in NaPhy Aand NaPhy B1-silenced plants in the glasshouse. The growth rates of irLHY plants were lower than those of EV plants in the field. While irLHY plants reduced A_c earlier at dusk,no differences between irLHY and EV plants were observed at dawn in the field. irLHY,but not EV plants,responded tolight in the night by rapidly increasing A_c. Under controlled conditions,EV plants rapidly increased A_c in the day compared to dark-adapted plants at night;irLHY plants lost these time-dependent responses. The role of NaLHY in gating photosynthesis is independent of the light-dependent reactions and red light perceived by NaPhy A,but not NaPhyB_1.
