| 3 | Spectrum of biliary complications following live donor liver transplantation显示文摘Liver transplantation is the optimal treatment for many patients with advanced liver disease, including decompensated cirrhosis, hepatocellular carcinoma and acute liver failure. Organ shortage is the maindeterminant of death on the waiting list and hence living donor liver transplantation(LDLT) assumes importance. Biliary complications are the most common post operative morbidity after LDLT and occur due to anatomical and technical reasons. They include biliary leaks, strictures and cast formation and occur in the recipient as well as the donor. | 4 | Colorectal cancers in ulcerative colitis from a low-prevalence area for colon cancer显示文摘AIM: To determine the incidence and risk factors for colorectal cancer(CRC) in patients with ulcerative colitis from a low prevalence region for CRC.METHODS: Our prospective database yielded a cohort of 430 patients [age: 44 ± 14.6 years; 248 men(57.7%)] with ulcerative colitis(median disease duration 6, range: 1-39 years) for analysis. Of these, 131(30.5%) had left-sided colitis and 159(37%) extensive colitis. Patients with histologically confirmed CRC within the segment with colitis were compared with those without CRC, to determine the risk factors for the development of CRC.RESULTS: Twelve patients(2.8%) developed CRC. The overall incidence density was 3.56/1000 patient-years of disease- 3/1000 in the first 10 years, 3.3/1000 at 10 to 20 years, and 7/1000 at > 20 years. Three of our 12 patients developed CRC within 8 years of disease onset. On univariate analysis, extensive colitis, longer duration of disease, and poor control of disease were associated with development of CRC. | 5 | Techniques and accuracy of thoracolumbar pedicle screw placement显示文摘Pedicle screw instrumentation has been used to stabilize the thoracolumbar spine for several decades. Although pedicle screws were originally placed via a freehand technique, there has been a movement in favor of pedicle screw placement with the aid of imaging. Such assistive techniques include fluoroscopy guidance and stereotactic navigation. Imaging has the benefit of increased visualization of a pedicle's trajectory, but can result in increased morbidity associated with radiation exposure, increased time expenditure, and possible workflow interruption. Many institutions have reported high accuracies with each of these three core techniques. However, due to differing definitions of accuracy and varying radiographic analyses, it is extremely difficult to compare studies side-by-side to determine which techniques are superior. From the literature, it can be concluded that pedicles of vertebrae within the mid-thoracic spine and vertebrae that have altered morphology due to scoliosis or other deformities are the most difficult to cannulate. Thus, spine surgeons would benefit the most from using assistive technologies in these circumstances. All other pedicles in thethoracolumbar spine should theoretically be cannulated with ease via a free-hand technique, given appropriate training and experience. Despite these global recommendations, appropriate techniques must be chosen at the surgeon's discretion. | 6 | Current state of renal transplant immunosuppression:Present and future显示文摘For kidney transplant recipients, immunosuppression commonly consists of combination treatment with a calcineurin inhibitor, an antiproliferative agent and a corticosteroid. Many medical centers use a sequential immunosuppression regimen where an induction agent, either an anti-thymocyte globulin or interleukin-2 receptor antibody, is given at the time of transplantation to prevent early acute rejection which is then followed by a triple immunosuppressive maintenance regimen. Very low rejection rates have been achieved at many transplant centers using combinations of these agents in a variety of protocols. Yet, a large number of recipients suffer chronic allograft injury and adverse events associated with drug therapy. Regimens designed to limit or eliminate calcineurin inhibitors and/or corticosteroid use are actively being pursued. An ideal immunosuppressive regimen limits toxicity and prolongs the functional life of the graft. | 10 | Extreme Rainfall Events and Associated Natural Hazards in Alaknanda Valley, Indian Himalayan Region显示文摘Entire Himalayan region is vulnerable to rain-induced (torrential rainfall) hazards in the form of flash flood, cloudburst or glacial lake outburst flood.Flash floods and cloudburst are generally caused by high intensity rainfall followed by debris flow or landslide often resulting into blockade of river channels. The examples of some major disasters caused by torrential rainfall events in last fifty years are the flash floods of 1968 in Teesta valley, in 1993 and 2000 in Sutlej valley, in 1978 in Bhagirathi and in 1970 in Alaknanda river valleys. The formation of landslide dams and subsequent breaching is also associated with such rainfall events. These dams may persist for years or may burst within a short span of its formation. Due to sudden surge of water level in the river valleys, havoc and panic are created in the down stream. In Alaknanda valley, frequencies of such extreme rainfall events are found to be increasing in last two decades. However, the monthly trend of extreme rainfall events has partly indicated this increase.
