1Ampullary carcinoma:Effect of preoperative biliary drainage on surgical outcome显示文摘AIM: To evaluate the influence of preoperative biliary drainage on morbidity and mortality after surgical resection for ampullary carcinoma. METHODS: We analyzed retrospectively data for 82 patients who underwent potentially curative surgery for ampullary carcinoma between September 1993 and July 2007 at the Singapore General Hospital, a tertiary referral hospital. Diagnosis of ampullary carcinoma was confirmed histologically. Thirty-five patients underwent preoperative biliary drainage (PBD group), and 47 were not drained (non-PBD group). The mode of biliary drainage was endoscopic retrograde cholangiopancreatography (n = 33) or percutaneous biliary drainage (n = 2). The following parameters were analyzed: wound infection, intra-abdominal abscess, intra-abdominal or gastrointestinal bleeding, septicemia, biliary or pancreatic leakage, pancreatitis, gastroparesis, and re-operation rate. Mortality was assessed at 30 d (hospital mortality) and also longterm. The statistical endpoint of this study was patient survival after surgery. RESULTS: The groups were well matched for demographic criteria, clinical presentation and operative characteristics, except for lower hemoglobin in the non-PBD group (10.9 ± 1.6 vs 11.8 ± 1.6 in the PBD group).Of the parameters assessing postoperative morbidity, incidence of wound infection was signif icantly less in the PBD than the non-PBD group [1 (2.9%) vs 12 (25.5%)]. However, the rest of the parameters did not differ signif icantly between the groups, i.e. sepsis [10 (28.6%) vs 14 (29.8%)], intra-abdominal bleeding [1 (2.9%) vs 5 (10.6%)], intra-abdominal abscess [1 (2.9%) vs 8 (17%)], gastrointestinal bleeding [3 (8.6%) vs 5 (10.6%)], pancreatic leakage [2 (5.7%) vs 3 (6.4%)], biliary leakage [2 (5.7%) vs 3 (6.4%)], pancreatitis [2 (5.7%) vs 2 (4.3%)], gastroparesis [6 (17.1%) vs 10 (21.3%)], need for blood transfusion [10 (28.6%) vs 17 (36.2%)] and re-operation rate [1 (2.9%) vs 5 (10.6%)]. There was no early mortality in either group. Median survival was 44 mo (95% CI: 34.2-53.8) in the PBD group and 41 mo (95% CI: 27.7-54.3; P = 0.86) in the non-PBD group. CONCLUSION: Biliary drainage before surgery for ampullary cancer significantly reduced postoperative wound infection. 2Comparison of split-dosing vs non-split(morning) dosing regimen for assessment of quality of bowel preparation for colonoscopy显示文摘AIM: To compare(using the Ottawa Bowel Preparation Scale) the efficacy of split-dose vs morning administration of polyethylene glycol solution for colon cleansing in patients undergoing colonoscopy, and to assess the optimal preparation-to-colonoscopy interval.METHODS: Single-centre, prospective, randomized, investigator-blind stud in an academic tertiarycare centre. Two hundred patients requiring elective colonoscopy were assigned to receive one of the two preparation regimens(split vs morning) prior to colonoscopy. Main outcome measurements were bowel preparation quality and patient tolerability.RESULTS: Split-dose regimen resulted in better bowel preparation compared to morning regimen [Ottawascore mean 5.52(SD 1.23) vs 6.02(1.34); P = 0.017]. On subgroup analysis, for afternoon procedures, both the preparations were equally effective(P = 0.756). There was no difference in tolerability and compliance between the two regimens.CONCLUSION: Overall, previous evening- same morning split-dosing regimen results in better bowel cleansing for colonoscopy compared to morning preparation. 3Evaluation of Fujinon intelligent chromo endoscopy-assisted capsule endoscopy in patients with obscure gastroenterology bleeding显示文摘AIM:To investigate the potential benef it of Fujinon in-telligent chromo endoscopy(FICE)-assisted small bowel capsule endoscopy(SBCE)for detection and character-ization of small bowel lesions in patients with obscure gastroenterology bleeding(OGIB).METHODS:The SBCE examinations(Pillcam SB2,Giv-en Imaging Ltd)were retrospectively analyzed by two GI fellows(observers)with and without FICE enhance-ment.Randomization was such that a fellow did not assess the same examination with and without FICE enhancement.The senior consultant described f indings as P0,P1 and P2 lesions(non-pathological,intermedi-ate bleed potential,high bleed potential),which were considered as reference f indings.Main outcome mea-surements:Inter-observer correlation was calculated using kappa statistics.Sensitivity and specif icity for P2 lesions was calculated for FICE and white light SBCE.RESULTS:In 60 patients,the intra-class kappa cor-relations between the observers and reference f indings were 0.88 and 0.92(P2),0.61 and 0.79(P1),for SBCE using FICE and white light,respectively.Overall 157 le-sions were diagnosed using FICE as compared to 114 with white light SBCE(P = 0.15).For P2 lesions,the sensitivity was 94% vs 97% and specif icity was 95% vs 96% for FICE and white light,respectively.Five(P2 le-sions)out of 55 arterio-venous malformations could be better characterized by FICE as compared to white light SBCE.Significantly more P0 lesions were diagnosed when FICE was used as compared to white light(39 vs 8,P < 0.001).CONCLUSION:FICE was not better than white light for diagnosing and characterizing signif icant lesions on SBCE for OGIB.FICE detected signif icantly more non-pathological lesions.Nevertheless,some vascular le-sions could be more accurately characterized with FICE as compared to white light SBCE.Tarun Gupta Mostafa Ibrahim Jacques Deviere André Van Gossum 2011World Journal of Gastroenterology2011,17,41:8
4Colorectal cancers in ulcerative colitis from a low-prevalence area for colon cancer显示文摘AIM: To determine the incidence and risk factors for colorectal cancer(CRC) in patients with ulcerative colitis from a low prevalence region for CRC.METHODS: Our prospective database yielded a cohort of 430 patients [age: 44 ± 14.6 years; 248 men(57.7%)] with ulcerative colitis(median disease duration 6, range: 1-39 years) for analysis. Of these, 131(30.5%) had left-sided colitis and 159(37%) extensive colitis. Patients with histologically confirmed CRC within the segment with colitis were compared with those without CRC, to determine the risk factors for the development of CRC.RESULTS: Twelve patients(2.8%) developed CRC. The overall incidence density was 3.56/1000 patient-years of disease- 3/1000 in the first 10 years, 3.3/1000 at 10 to 20 years, and 7/1000 at > 20 years. Three of our 12 patients developed CRC within 8 years of disease onset. On univariate analysis, extensive colitis, longer duration of disease, and poor control of disease were associated with development of CRC. 6An evaluation of the emission profile for two-wheelers at a traffic junction显示文摘Two-wheeler vehicles are an important mode of transportation in developing countries.However,the emissions from two-wheeler vehicles are significant.Urban two-wheeler vehicles with gasoline-fueled engines produce NO,and particulate matter emissions that affect urban air quality.During traffic light stops and programmed stops,for instance,pollutants are emitted and are dangerous to human health.In this experimental study,two-wheeler vehicles with different makes,technologies and engine capacities were tested for exhaust emissions including gravimetric and online measurements at different engine speeds and a no load condition at a si mutated traffic junction.Gravimetric measurements were performed by collecting the particulate mass(at two engine speeds:1500 and 2500 rpm) from a diluted engine-out exhaust on quartz filter paper.Next,these collected particulates were used to determine the presence of metals,as well as the benzene soluble organic fraction(BSOF).The total particulate mass,BSOF and trace elements were slightly higher at a higher engine speed(2500 rpm).Online measurements were performed by sampling the engine exhaust(at four engine speeds:1500.2000,2500,and 3000 rpm)and using online instruments to determine the particle number and size distribution,the particle-bound polyaromatic hydrocarbons(PAHs),the gaseous emissions and the smoke opacity.Engines with higher cubic capacity emitted a higher concentration of nano-particles.The particle-bound PAH concentration increased as the engine speed increased,but this concentration was notably low for the highest engine speed tested(3000 rpm).The regulated gaseous emissions increased as the engine speed increased for all vehicles.Avinash Kumar Agarwal Prakhar Bothra Tarun Gupta Pravesh Chandra Shukla 2015Particuology2015,13,1:4
7Emission profiling of diesel and gasoline cars at a city traffic junction显示文摘In congested urban roads,cars must stop at intersections because of city traffic lights.As a result,pedestrians and traffic police personnel are exposed to pollutants emanating from the tailpipe of various vehicles at such city traffic junctions.In this study,various gasoline- and diesel-fueled cars complying with different emission standards were tested for their emissions in simulated city traffic junction conditions.The engine exhaust from these cars was subjected to physicochemical characterization at different engine speeds under no-load conditions.These engine conditions were chosen because the cars idle at different engine speeds at a city traffic junction.Gravimetric and real-time measurements were performed for the tailpipe exhaust sampled from these vehicles.Exhaust particles were collected on 47 mm diameter quartz filter papers and subjected to gravimetric analysis for determining the total particulate mass(TPM)and trace metals while the engines were operated at two different engine idling speeds,1500 rpm(representing low idling) and 2500 rpm(representing high idling).At similar engine operating conditions,TPM and trace metals were lower for the exhaust from gasoline engines compared to the exhaust from diesel engines.Real-time measurements were performed for particle-bound poly-aromatic hydrocarbons(PAHs),particle number and size distribution,regulated gaseous emissions and smoke opacity of the exhaust at four different engine speeds,1500,2000,2500,and 3000 rpm.Particle-bound PAHs showed a decreasing trend for the vehicles that complied with stricter vehicular emission standards.Higher particle peak number concentrations were observed for diesel exhausts compared to the results for gasoline exhaust.Regulated gaseous emissions were also compared.Avinash Kumar Agarwal Tarun Gupta Prakhar Bothra Pravesh Chandra Shukla 2015Particuology2015,13,1:4
8Photocatalytic reduction of organic pollutant under visible light by green route synthesized gold nanoparticles显示文摘We report a rapid method of green chemistry approach for synthesis of gold nanoparticles(AuNPs)using Lagerstroemia speciosa leaf extract(LSE). L. speciosa plant extract is known for its effective treatment of diabetes and kidney related problems. The green synthesis of Au NPs was complete within 30 min at 25°C. The same could also be achieved within 2 min at a higher reaction temperature(80°C). Both UV–visible spectroscopy and transmission electron microscopy results suggest that the morphology and size distribution of Au NPs are dependent on the pH of gold solution,gold concentration,volume of LSE,and reaction time and temperature. Comparison between Fourier transform infrared spectroscopy(FT-IR)spectra of LSE and the synthesized Au NPs indicate an active role of polyphenolic functional groups(from gallotannins,lagerstroemin,and corosolic acid)in the green synthesis and capping of Au NPs. The green route synthesized Au NPs show strong photocatalytic activity in the reduction of dyes viz.,methylene blue,methyl orange,bromophenol blue and bromocresol green,and 4-nitrophenol under visible light in the presence of Na BH_4. The non-toxic and cost effective LSE mediated Au NPs synthesis proposed in this study is extremely rapid compared to the other reported methods that require hours to days for complete synthesis of Au NPs using various plant extracts. 15Pharmacognostic evaluation of Cayratia trifolia (Linn.)leaf显示文摘Objective:To present a detailed pharmacognostic study of the leaf of Cayratia trifolia(C.trifolia)Linn.(Vitaceae),an important plant in the Indian system of medicine.Methods:The macroscopy,microscopy,physiochemical analysis,preliminary testing,fluorescence analysis of powder of the plant and other WHO recommended methods for standardization were investigated.Results:Leaves are trifoliolated with petioles(2-3 cm)long.Leaflets are ovate to oblong-ovate,(2-8 cm)long,(1.5-5 cm)wide,pointed at the tip.The leaf surface shows the anisocytic type stomata covered with guard cells followed by epidermis layer.Leaf surface contents including veins.vein islet and vein termination were also determined.Transverse section of leaf shows the epidermis layer followed by cuticle layer and vascular bandies(xylem and phloem).The mesophyll is differentiated into palisade and spongy parenchyma.Abundant covering trichomes emerge from the upper epidermis.Trichomes are uniseriate and multicellular.Strips of collenchyma are present below and upper layer of epidermis.Conclusions:It can be concluded that the pharmacognostic profile of the C.trifolia is helpful in developing standards for quality,purity and sample identification.Dinesh Kumar Jyoti Gupta Sunil Kumar Renu Arya Tarun Kumar Ankit Gupta 2012Asian Pacific Journal of Tropical Biomedicine2012,2,1:0
