1Diagnosis of Helicobacter pylori : What should be the gold standard?显示文摘Since the discovery of Helicobacter pylori(H. pylori) in 1983, numerous detection methods for the presence of the bacterium have been developed. Each one of them has been associated with advantages and disadvantages. Noninvasive tests such as serology, 13 C urea breath test(UBT) and stool antigen tests are usually preferred by the clinicians. Serology has its own limitation especially in endemic areas while 13 C UBT is technically very demanding. The stool antigen detection method, although specific, is usually associated with poor sensitivity. The 13 C UBT is believed to be specific, but with present revelation of the fact that stomach is colonized by many other urease producing bacteria makes it questionable. Histology, culture, rapid urease test and polymerase chain reaction(PCR) are the tests which are carried out on antral biopsies collected by invasive means. Histology has been proposed to be very sensitive and specific but the question is how by simply looking the morphology of the bacteria in the microscope, one can claim that the curved bacterium is exclusively H. pylori. Rapid urease test(RUT), the doctor's test, is also challenged because the presence of other urease producing bacteria in the stomach cannot be denied. Moreover, RUT has been reported with poor sensitivity specially, when density of the bacterium is low. Isolation of H. pylori is essential to investigate its growth requirements, antibiotic susceptibility testing, studying virulence factor to develop vaccine and many more explorations. It has also got several disadvantages i.e., special condition for transporting, media, incubation and few days waiting for the colonies to appear, apart from the speed essentially needed to process the specimens. Till date, majority of the microbiological laboratories in the world are not equipped and trained to isolate such fastidious bacterium. The option left is PCR methods to detect H. pylori 's DNA in gastric mucosa, gastric juice, saliva, dental plaques and environmental specimens. There are speculations for false positivity due to detection of non-pylori Helicobacters due to genetic sharing; and false negativity due to low bacterial counts and presence of PCR inhibitors. However, specimen collection, transportation and processing do not require speed and special conditions. PCR based diagnosis may be considered as gold standard by designing primers extremely specific to H. pylori and targeting at least more than one conserved genes. Similarly specificity of PCR may be improved by use of internal Primers. Further, nested PCR will take care of false negatives by countering the effect of PCR inhibitors and low bacterial counts. 2Pathogenic mechanisms of pancreatitis显示文摘Pancreatitis is inflammation of pancreas and caused by a number of factors including pancreatic duct obstruction, alcoholism, and mutation in the cationic trypsinogen gene. Pancreatitis is represented as acute pancreatitis with acute inflammatory responses and; chronic pan-creatitis characterized by marked stroma formation with a high number of infiltrating granulocytes(such as neutrophils, eosinophils), monocytes, macrophages and pancreatic stellate cells(PSCs). These inflammatory cells are known to play a central role in initiating and promoting inflammation including pancreatic fibrosis, i.e., a major risk factor for pancreatic cancer. A number of inflammatory cytokines are known to involve in pro-moting pancreatic pathogenesis that lead pancreatic fibrosis. Pancreatic fibrosis is a dynamic phenomenon that requires an intricate network of several autocrine and paracrine signaling pathways. In this review, we have provided the details of various cytokines and molecular mechanistic pathways(i.e., Transforming growth factor-β/SMAD, mitogen--activated protein kinases, Rho kinase, Janus kinase/signal transducers and activators, and phosphatidylinositol 3 kinase) that have a critical role in the activation of PSCs to promote chronic pancreatitis and trigger the phenomenon of pancreatic fibrogenesis. In this review of literature, we discuss the involvement of several pro-inflammatory and anti-inflammatory cytokines, such as in interleukin(IL)-1, IL-1β, IL-6, IL--8 IL-10, IL-18, IL--33 and tumor necrosis factor-α, in the pathogenesis of disease. Our review also highlights the significance of several experimental animal models that have an important role in dissecting the mechanistic pathways operating in the development of chronic pancreatitis, including pancreatic fibrosis. 3Role of live microbial feed supplements with reference to anaerobic fungi in ruminant productivity: A review显示文摘To keep the concept of a safe food supply to the consumers, animal feed industries world over are showing an increasing interest in the direct-fed microbials(DFM) for improved animal performance in terms of growth or productivity. This becomes all the more essential in a situation, where a number of the residues of antibiotics and/or other growth stimulants reach in milk and meat with a number of associated potential risks for the consumers. Hence, in the absence of growth stimulants, a positive manipulation of the rumen microbial ecosystem to enhance the feedstuff utilization for improved production efficiency by ruminants has become of much interest to the researchers and entrepreneurs. A few genera of live microbes(i.e., bacteria, fungi and yeasts in different types of formulations from paste to powder) are infrequently used as DFM for the domestic ruminants. These DFM products are live microbial feed supplements containing naturally occurring microbes in the rumen. Among different DFM possibilities, anaerobic rumen fungi(ARF) based additives have been found to improve ruminant productivity consistently during feeding trials. Administration of ARF during the few trials conducted, led to the increased weight gain, milk production, and total tract digestibility of feed components in ruminants. Anaerobic fungi in the rumen display very strong cell-wall degrading cellulolytic and xylanolytic activities through rhizoid development, resulting in the physical disruption of feed structure paving the way for bacterial action. Significant improvements in the fiber digestibility were found to coincide with increases in ARF in the rumen indicating their role. Most of the researches based on DFM have indicated a positive response in nutrient digestion and methane reducing potential during in vivo and/or in vitro supplementation of ARF as DFM. 7Species diversity, regeneration and dominance as influenced by canopy gaps and their characteristics in tropical evergreen forests of Western Ghats, India显示文摘Canopy gaps play a significant role in maintaining structure and composition of tropical forests. This study was carried out in tropical evergreen forests of central Western Ghats in India to understand the influence of canopy gap size and the relationship of gap regime attributes to diversity measures and regeneration. The average gap size in the study area was found to be 396 m2 and around half of gaps were 4–8 years old. Gaps created by natural single tree fall were smaller in size but significantly higher in number. Diversity and regeneration of woody species were compared with canopy gaps and intact vegetation. Species richness and diversity was higher in gaps than in intact vegetation. Macaranga peltata, a shade intolerant species dominated gaps while intact vegetation was dominated by shade tolerant Kingiodendron pinnatum.Gap size significantly influenced species diversity and regeneration. Gap area and age were significantly and negatively correlated with diversity measures but positively correlated with regeneration. Among all the attributes of gaps, regeneration was significantly positively correlated with light intensity. Gaps maintained species diversity and favored regeneration of woody species. 8Prevalence and antibiogram study of Salmonella and Staphylococcus aureus in poultry meat显示文摘Objective:To evaluate the presence and antibiogram pattern of Salmonella and Staphylococcus aureus(S.aureus)in retail poultry meat products.Methods:Foodborne pathogens(Salmonella and S.aureus)were isolated from poultry meat and confirmed with the help of biochemical and immunological test.Antibiogram of the isolates were examined by following CLS1 methods.Results:A total number of 209 poultry meat samples were collected and studied in this study.Out of which,5.26%were found contaminated with Salmonella while 18.18%were found contaminated with S.aureus.All the Salmonella and S.aureus isolates were found resistant to at least one antibiotic.About 72.72%of the Salmonella isolates showed resistance to tetracycline,while S.aureus isolates were also found highly resistant to tetracycline equal to 44.73%.One of the Salmonella isolates showed multi-drug resistance to almost six antibiotics out of nine antibiotics used in the study.Multidrug resistant S.aureus isolates were also found in the study.Conclusions:The study confirmed the presence of Salmonella and S.aureus in retail poultry meat.It is a potential threat to consumer health.To reduce the risk of contamination,good hygiene practices are necessary from processing to storage.Ali Akbar Anil Kumar Anal 2013Asian Pacific Journal of Tropical Biomedicine2013,3,2:8
9Correlation of transient elastography with hepatic venous pressure gradient in patients with cirrhotic portal hypertension: A study of 326 patients from India显示文摘AIM To study the diagnostic accuracy of transient elastography(TE) for detecting clinically significant portal hypertension(CSPH) in Indian patients with cirrhotic portal hypertension.METHODS This retrospective study was conducted at the Institute of Liver, Gastroenterology, and Panceatico-Biliary Sciences, Sir Ganga Ram Hospital, New Delhi, on consecutive patients with cirrhosis greater than 15 years of age who underwent hepatic venous pressure gradient(HVPG) and TE from July 2011 to May 2016. Correlation between HVPG and TE was analyzed using the Spearman's correlation test. Receiver operating characteristic ( ROC) curves were prepared for determining the utility of TE in predicting various stages of portal hypertension. The best cut-off value of TE forthe diagnosis of CSPH was obtained using the Youden index.RESULTS The study included 326 patients [median age 52(range 16-90) years; 81% males]. The most common etiology of cirrhosis was cryptogenic(45%) followed by alcohol(34%). The median HVPG was 16.0(range 1.5 to 30.5) mm Hg. Eighty-five percent of patients had CSPH. A significant positive correlation was noted between TE and HVPG(rho 0.361, P < 0.001). The area under ROC curve for TE in predicting CSPH was 0.740(95%CI: 0.662-0.818)(P < 0.01). A cut-off value of TE of 21.6 k Pa best predicted CSPH with a positive predictive value(PPV) of 93%.CONCLUSION TE has a fair positive correlation with HVPG; thus, TE can be used as a non-invasive modality to assess the degree of portal hypertension. 11Mechanical and metallurgical properties of friction stir welded dissimilar joints of AZ91 magnesium alloy and AA 6082-T6 aluminium alloy显示文摘In the present research work,AZ91 magnesium alloy and AA6082-T6 aluminium alloy were joined by friction stir welding process.The comparison of microstructure and mechanical properties between different joints by varying the different materials on advance and retreating sides were mainly studied.Four different welds have been prepared to find the material mixing between the similar and dissimilar joints.The joint interfaces of the welds have been investigated by employing an optical microscope and scanning electron microscope.When Mg was placed on advancing side(AS),more aluminium content was soluble in nugget zone than the case where Mg was placed on the retreating side(RS).Thin intermetallic layer in the joint interface of Mg/Al and thick intermetallic layer with poor adhesion of the aluminium and magnesium have been observed in the dissimilar joints varying the sides.The highest UTS of 172.3 MPa was found for Mg-Al when Mg was placed on AS and lower UTS of 156.25 MPa was obtained when Mg was placed on RS.Hardness of 86 Hv and 89 Hv were observed in the Stir zone for the dissimilar AZ91 Mg alloy and AA6082-T6 Al alloy when AZ91Mg alloy was placed on the AS and on the RS respectively.Fractography was also carried out to find the mode of failure.Sameer MD Anil Kumar Birru 2019Journal of Magnesium and Alloys2019,7,2:7
12A Modified Plate Assay for Rapid Screening of Potassium-Solubilizing Bacteria显示文摘The utility of microorganisms for solubilizing the unavailable forms of potassium(K) from soil has led to renewed interest in fabrication of rapid and sensitive plate assays for their isolation and screening. The present study developed a modified plate assay and compared it with previously reported methods for the isolation and screening of K-solubilizing bacteria. The newly developed plate assay is based on improved visualization of halo zone formation around the colonies on agar plates, through inclusion of an acid-base indicator dye, bromothymol blue(BTB), to modify the previously reported Aleksandrov medium. The halo zone exhibited a significant correlation(R = 0.939) with K released in liquid medium. The visualization of potential K solubilizers was improved using this method, which would help in detection of weak/non-acid producers based on secretion of organic acids in the medium. Organic acids in plate diffuse radially and form halo zones in response to reaction with the acid-base indicator dye BTB. Furthermore, K solubilization on plates with this method can be observed within 48–72 h, against the incubation time of 4–5 d needed in the earlier method. METHODS: Eight of 70 patients presenting in tertiary care centre of North India with massive pulmonary embolism within 4 years had subacute presentation (symptom onset more than 2 wk). These patients were subjected to pulmonary angiography with intention to treat basis via mechanical breakdown and intra lesional thrombolysis. Mechanical breakdown of embolus was accomplished with 5-F multipurpose catheter to reestablish flow, followed by intralesional infusion of urokinase (4400 IU/kg over 10 min followed by 4400 IU/kg per hour over 24 h). RESULTS: Eight patients, mean age 47.77±12.20 years presented with subacute pulmonary embolism (mean duration of symptoms 2.4 wk). At presentation, mean heart rate, shock index, miller score and mean pulmonary pressures were 101.5±15.2/min, 0.995±0.156, 23.87±3.76 and 37.62±6.67 mmHg which reduced to 91.5±12.2/min (P=0.0325), 0.789±0.139 (P=0.0019), 5.87±1.73 (P=0.0000004) and 27.75±8.66 mmHg (P=0.0003) post procedurally. Mean BP improved from 80.00±3.09 mmHg to 90.58±9.13 mmHg (P=0.0100) post procedurally. Minor complications in the form of local hematoma-minor hematoma in 1 (12.5%), and pseudoaneurysm (due to femoral artery puncture) in 1 (12.5 %) patient were seen. At 30 d and 6 mo follow up survival rate was 100% and all the patients were asymptomatic and in New York Heart Association class 1. 15All ileo-cecal ulcers are not Crohn's:Changing perspectives of symptomatic ileocecal ulcers显示文摘AIM To investigated clinical,endoscopic and histopathological parameters of the patients presenting with ileocecal ulcers on colonoscopy.METHODS Consecutive symptomatic patients undergoing colonoscopy,and diagnosed to have ulcerations in the ileocecal(I/C) region,were enrolled.Biopsy was obtained and theirclinical presentation and outcome were recorded.RESULTS Out of 1632 colonoscopies,104 patients had ulcerations in the I/C region and were included in the study.Their median age was 44.5 years and 59% were males.The predominant presentation was lower GI bleed(55,53%),pain abdomen ± diarrhea(36,35%),fever(32,31%),and diarrhea alone(9,9%).On colonoscopy,terminal ileum was entered in 96(92%) cases.The distribution of ulcers was as follows:Ileum alone 40%(38/96),cecum alone 33%(32/96),and both ileum plus cecum 27%(26/96).The ulcers were multiple in 98% and in 34% there were additional ulcers elsewhere in colon.Based on clinical presentation and investigations,the etiology of ulcers was classified into infective causes(43%) and noninfective causes(57%).Fourteen patients(13%) were diagnosed to have Crohn's disease(CD).CONCLUSION Non-specific ileocecal ulcers are most common ulcers seen in ileo-cecal region.And if all infections are clubbed together then infection is the most common(> 40%) cause of ulcerations of the I/C region.Cecal involvement and fever are important clues to infective cause.On the contrary CD account for only 13% cases as a cause of ileo-cecalulcers.So all symptomatic patients with I/C ulcers on colonoscopy are not Crohn's.Jay Toshniwal Romesh Chawlani Amit Thawrani Rajesh Sharma Anil Arora Hardik L Kotecha Mohan Goyal Vijendra Kirnake Pankaj Jain Pankaj Tyagi Naresh Bansal Munish Sachdeva Piyush Ranjan Mandhir Kumar Praveen Sharma Vikas Singla Rinkesh Bansal Vineet Shah Sunita Bhalla Ashish Kumar 2017World Journal of Gastrointestinal Endoscopy2017,9,7:5
16Unexpected alliance between syndecan-1 and innatelike T cells to protect host from autoimmune effects of interleukin-17显示文摘Innate-like T cells, namely natural killer T(NKT) and γδ T cells, play critical roles in linking innate and adaptive immune responses through rapid production of cytokines. Prominent among these cytokines is interleukin-17(IL-17), which is a potent proinflammatory cytokine that plays a critical role in host defense against fungi and extracellular bacteria. However, excessive IL-17-production promotes autoimmune diseases, including psoriasis, multiple sclerosis, rheumatoid arthritis, inflammatory bowel disease, and systemic lupus erythematosus. IL-17 has also been implicated in regulating body fat, which is highly relevant given rises in obesity and type 2 diabetes. NKT cells, γδ T cells and mucosal-associated invariant T cells(MAIT) are the major sources of IL-17 involved in protection of mucosal surfaces from opportunistic infections and causing autoimmunity when become dysregulated. Given the pathogenic effects of IL-17, efforts have been directed towards understanding mechanisms that guard against IL-17 overproduction. One novel potent mechanism is mediated by the heparan sulfate proteoglycan, syndecan-1(sdc1), which is selectively expressed by IL-17-producing subsets of NKT and γδ T cells. This unexpected role for sdc1 is uncovered by analysis of NKT and γδ T cells in sdc1-deficient mice. 18Antifungal Effect of Plant Extract and Essential Oil显示文摘The advancement of phytochemical and phytopharmacological sciences has enabled elucidation of the composition and biological activities of several medicinal plant products including plant extract and essential oils. These products have been widely used around the world since ancient times for the treatment of various disorders such as diabetes, hypertension, peptic ulcer disease, microbial infection, sexual disorder and many more. Its popularity in the modern system of medicine is mainly due to their availability and fewer adverse reactions compared to synthetic drugs. Various scientific investigations have been conducted to look for the potential of the extract from the plant or isolated compounds for the continued use of these products in the treatment and prevention of various kinds of human diseases. It is evident from the available literature and scientific investigations that many plant species possess potential for use as a beneficial therapeutic remedy with multiple pharmacological actions such as analgesic, anti-inflammatory, antipyretic, hypoglycemic, hypotensive, antimicrobial, antiulcer and anticonvulsant activities. The present review aims to provide relevant updated information about certain plant products, its composition, preparation and its fungicidal or fungistatic effects on different species of fungus as evaluated by studies done in the past. It introduces six medicinal plants that have been studied for their antifungal property and are found to be effective. 19Production,activation,and applications of biochar in recent times显示文摘Biomass is a green energy source and is available in abundance.Biochar is a carbon-rich material derived from a wide range of biomass or organic waste through the thermochemical route.Biochar has received increasing attention because of its distinctive properties such as high carbon content,greater specific surface area,cation exchange capacity,nutrient retention capacity,and stable structure.This review paper extensively studies and reports the different pyrolysis processes,reactor types,the effect of process parameters on biochar yield,and its physicochemical properties,biochar activation methods,and applications.It also details the status of the research and development(R&D)progress in biochar production through conventional and advanced technologies.The study found that unlike many products(at R&D stage)biochar has high potential to scale up and has a direct impact on crop yield,water purification(for domestic and industrial application),alternative fuels(clean solid fuel for cookstove),air purification,catalyst,biogas production,purification,and storage.In addition,the paper lists the merits and challenges in the novel biochar applications like hydrogen storage,electrochemical capacitor,and fuel cell technology.Anil Kumar Sakhiya Abhijeet Anand Priyanka Kaushal 2020Biochar2020,2,3:4
20Role of angiotensin converting enzyme and angiotensinogen gene polymorphisms in angiotensin converting enzyme inhibitor-mediated antiproteinuric action in type 2 diabetic nephropathy patients显示文摘AIM To investigate the role of genetic variants of angiotensin converting enzyme(ACE) and angiotensinogen(AGT) genes in the antiproteinuric efficacy of ACE inhibitor therapy in diabetic nephropathy(DN) patients.METHODS In the present study, 270 type 2 diabetes mellitus patients with nephropathy were enrolled and treated with ACE inhibitor(ramipril) and followed at 6 mo for renal function and albumin excretion by estimating serum creatinine, end stage renal disease, and albumin/creatinine ratio(ACR) in urine. Genotyping of ACE I/D and AGT M235 T polymorphisms were performed by using primer specific polymerase chain reaction(PCR) and PCR-RFLP techniques, respectively. RESULTS Forty-eight percent of DN patients(responders) benefited with respect to proteinuria from ACE inhibitor therapy at 6 mo follow-up. A significant reduction in ACR was observed after 6 mo treatment with ACE inhibitor irrespective of whether DN patients were micro-albuminuric(≥ 30 and < 300 mg/g creatinine) or macro-albuminuric(≥ 300 mg/g creatinine) at the time of enrollment. However, macro-albuminuric patients(55%) showed better response to therapy. A reduction in urinary ACR was found independent of genotypes of ACE I/D and AGT M235 T polymorphisms although macro-albuminuric patients having TT genotype showed statistically insignificant increased response(72%).
