| 1 | One hundred and seventy-eight consecutive pancreatoduodenectomies without mortality:role of the multidisciplinary approach显示文摘BACKGROUND:Pancreatoduodenectomy offers the only chance of cure for patients with periampullary cancers.This,however,is a major undertaking in most patients and is associated with a significant morbidity and mortality.A multidisciplinary approach to the workup and follow-up of patients undergoing pancreatoduodenectomy was initiated at our institution to improve the diagnosis,resection rate,mortality and morbidity.We undertook the study to assess the effect of this approach on diagnosis,resection rates and short-term outcomes such as morbidity and mortality.METHODS:A prospective database of patients presenting with periampullary cancers to a single surgeon between April 2004 and April 2010 was reviewed.All cases were discussed at a multidisciplinary meeting comprising surgeons,gastroenterologists,radiologists,oncologists,radiation oncologists,pathologists and nursing staff.A standardized investigation and management algorithm was followed.Complications were graded according to the Clavien-Dindo classification.RESULTS:A total of 295 patients with a periampullary lesion were discussed and 178 underwent pancreatoduodenectomy (resection rate 60%).Sixty-one patients (34%) required either a vascular or an additional organ resection.Eighty-nine patients experienced complications,of which the commonest was blood transfusion (12%).Thirty-four patients (19%) had major complications,i.e.grade 3 or above.There was no in-hospital,30-day or 60-day mortality.CONCLUSIONS:Pancreatoduodenectomy can safely be performed in high-volume centers with very low mortality.The surgeon's role should be careful patient selection,intensive preoperative investigations,use of a team approach,and an unbiased discussion at a multidisciplinary meeting to optimize the outcome in these patients. | Jaswinder S Samra Raul Alvarado Bachmann Julian Choi Anthony Gill Michael Neale Vikram Puttaswamy Cameron Bell Ian Norton Sarah Cho Steven Blome Ritchie Maher Sivakumar Gananadha Thomas J Hugh | 2011 | Hepatobiliary & Pancreatic Diseases International2011,10,4: | 8 |
| 2 | Minimizing transmission loss using inspired ant colony optimization and Markov Chain Monte Carlo in underwater WSN environment显示文摘In Underwater Wireless Sensor Networks(UWSNs),the most important challenging issues are propagation delay,high error probability,high latency,high communication cost,limited bandwidth,limited memory,low packet delivery ratio,and transmission loss.In our proposed work,the various efforts are taken to minimize the propagation delay and transmission loss during data transmission in an underwater environment.A hybrid mechanism is implemented to improve energy efficiency for faster data transmission in underwater WSN.In the underwater environment of acoustic channel condition,propagation delay and transmission loss lead to high complexity in accessing the information and also it is difficult to obtain the Channel Status Information(CSI).To address this problem,Ant Colony Optimization(ACO)routing with Markov Chain Monte Carlo(MCMC)algorithm is used and to capture the transmission loss in the MCMC approach,CSI Forecast Prediction(FP)algorithm is used.The experimental simulations are evaluated by utilizing the performance evaluation metrics such as Transmission Loss,Probability Density Function,Average Delay and Throughput.From the simulation results it is evident that the proposed algorithm,ACO-MCMC has recorded the minimum transmission loss,increase in probability density function,minimum average delay and maximum throughput of the network when compared to the existing algorithms. | Raj Priyadarshini R Sivakumar N | 2019 | Journal of Ocean Engineering and Science2019,4,4: | 7 |

| 14 | Can the Grain-for-Green Program Really Ensure a Low Sediment Load on the Chinese Loess Plateau?显示文摘The Chinese Loess Plateau is the most seriously eroded area in the world and contributes the vast majority of the sediment that goes into the Yellow River.Since the 1950s,progressive soil and water conservation measures have been implemented—in particular,large-scale ecological restoration has been ongoing since 1999—resulting in a significant reduction of the sediment load.However,the mechanism of the sediment transport dynamics is not fully understood due to multiple and complicated influencing factors including climate change and human activities(e.g.,ecological restoration).A challenging question,then,arises:Is the current low sediment level a“new normal”in this era and in the future?To address this question,we selected a typical loess hilly region where considerable ecological restoration has been implemented,and which is regarded as the site of the first and most representative Grainfor-Green program in the Loess Plateau.We investigated the evolution of discharge–sediment relationships in the past decades(1960–2010)and their association with the soil and water conservation measures in this area.The results showed that there was a distinct change in the regression parameters of the commonly used annual discharge–sediment regression equation—a continuously increasing trend of parameter b and a decreasing trend of parameter a,accompanying the ecological restoration.The increase in exponent b(i.e.,a steeper slope)implies a potential lower sediment load resulting from low discharge and a potential higher sediment load resulting from large discharge.This finding may question the new normal of a low sediment level and implies the potential risk of a large sediment load during extremely wet years. | Pengcheng Sun Yiping Wu Zhifeng Yang Bellie Sivakumar Linjing Qiu Shuguang Liu Yanpeng Cai | 2019 | Engineering2019,5,5: | 3 |

No mortality was observed in controls.The chi-square values were significant at P<0.05 level. | 18 | Enabling Energy Efficient Sensory Data Collection Using Multiple Mobile Sink显示文摘Mobile sink is the challenging task for wireless sensor networks(WSNs).In this paper we propose to design an efficient routing protocol for single mobile sink and multiple mobile sink for data gathering in WSN.In this process,a biased random walk method is used to determine the next position of the sink.Then,a rendezvous point selection with splitting tree technique is used to find the optimal data transmission path.If the sink moves within the range of the rendezvous point,it receives the gathered data and if moved out,it selects a relay node from its neighbours to relay packets from rendezvous point to the sink.Proposed algorithm reduces the signal overhead and improves the triangular routing problem.Here the sink acts as a vehicle and collect the data from the sensor.The results show that the proposed model effectively supports sink mobility with low overhead and delay when compared with Intelligent Agent-based Routing protocol(IAR) and also increases the reliability and delivery ratio when the number of sources increases. | Madhumathy P Sivakumar D | 2014 | China Communications2014,11,10: | 3 |

| 20 | Effect of quarry dust addition on the performance of controlled low-strength material made from industrial waste incineration bottom ash显示文摘The performance of industrial waste incineration bottom ash in controlled low-strength material (CLSM) was investigated in this paper, as the quarry dust was added. CLSM mixtures were made from the industrial waste incineration bottom ash, quarry dust, and cement. Tests for fresh density, bleeding, compressive strength, shear strength, hydraulic conductivity, and excavatability were carried out. The com-pressive strength ranges from 60 kPa to 6790 kPa, the friction angle varies from 5° to 19°, and the cohesion is from 4 to 604 kPa. Most of the mixtures are found to be non-excavatable.
