1An update on transurethral surgery for benign prostatic obstruction显示文摘Clinical benign prostatic hyperplasia(BPH)is one of the most common cause of lower urinary tract symptoms and transurethral resection of prostate(TURP)has been the gold standard technique for surgical treatment of benign prostate obstruction(BPO)over the last 2 decades.Although monopolar TURP is considered a safe and effective option for surgical management of BPO,there are some disadvantages,namely bleeding,transurethral resection syndrome,incompleteness of treatment.This review aims to highlight these problems,and describe the advances in technology and techniques that have evolved to minimise such complications.With the advent of lasers and bipolar technology,as well as enucleative techniques to remove the prostatic adenoma/adenomata,the problems of bleeding,transurethral resection syndrome and incomplete treatment are significantly minimised.Monopolar TURPwill likely be replaced by such technology and techniques in the near future such that transurethral surgery of the prostate remain a safe and effective option in alleviating the harmful effects of BPO.Jonathan Shunming Teo Yee Mun Lee Henry Sun Sien Ho 2017Asian Journal of Urology2017,4,3:20
2Benign prostatic hyperplasia and male lower urinary symptoms:A guide for family physicians显示文摘Male patients with lower urinary tract symptoms(LUTS)and benign prostatic hyperplasia(BPH)are increasingly seen by family physicians worldwide due to ageing demographics.A systematic way to stratify patients who can be managed in the community and those who need to be referred to the urologist is thus very useful.Good history taking,physical examination,targeted blood or urine tests,and knowing the red flags for referral are the mainstay of stratifying these patients.Case selection is always key in clinical practice and in the setting of the family physician.The best patient to manage is one above 40 years of age,symptomatic with nocturia,slower stream and sensation of incomplete voiding,has a normal prostatespecific antigen level,no palpable bladder,and no haematuria or pyuria on the labstix.The roles of α blockers,5-α reductase inhibitors,and antibiotics in a primary care setting to manage this condition are also discussed.Farhad Fakhrudin Vasanwala Michael Yuet Chen Wong Henry Sun Sien Ho Keong Tatt Foo 2017Asian Journal of Urology2017,4,3:5

4MPTP Induces Systemic Parkinsonism in Middle-Aged Cynomolgus Monkeys:Clinical Evolution and Outcomes显示文摘In this study,we developed a systemic PD model in middle-aged cynomolgus monkeys using individualized low-dose MPTP,to explore effective indicators for the early prediction of clinical outcomes.MPTP was not stopped until the animals showed typical PD motor symptoms on days 10 to 13 after MPTP administration when the Kurlan score reached 10;this abrogated the differences in individual susceptibility to MPTP.The clinical symptoms persisted,peaking on days 3 to 12 after MPTP withdrawal(rapid progress stage),and then the Kurlan score plateaued.A Kurlan score at the end of the rapid progress stage >15 reflected stable or slowly-progressive PD,while a score <15 indicated spontaneous recovery.The entire clinical evolution and outcome of the systemic PD model was characterized in this study,thus providing options for therapeutic and translational research.Feng Yue Sien Zeng Rongping Tang Guoxian Tao Piu Chan 2017Neuroscience Bulletin2017,33,1:3

6Responses of water productivity to irrigation and N supply for hybrid maize seed production in an arid region of Northwest China显示文摘Water and nitrogen(N) are generally two of the most important factors in determining the crop productivity. Proper water and N managements are prerequisites for agriculture sustainable development in arid areas. Field experiments were conducted to study the responses of water productivity for crop yield(WP_(Y-ET)) and final biomass(WP_(B-ET)) of film-mulched hybrid maize seed production to different irrigation and N treatments in the Hexi Corridor, Northwest China during April to September in 2013 and also during April to September in 2014. Three irrigation levels(70%–65%, 60%–55%, and 50%–45% of the field capacity) combined with three N rates(500, 400, and 300 kg N/hm^2) were tested in 2013. The N treatments were adjusted to 500, 300, and 100 kg N/hm^2 in 2014. Results showed that the responses of WP_(Y-ET) and WP_(B-ET) to different irrigation amounts were different. WP_(Y-ET) was significantly reduced by lowering irrigation amounts while WP_(B-ET) stayed relatively insensitive to irrigation amounts. However, WP_(Y-ET) and WP_(B-ET) behaved consistently when subjected to different N treatments. There was a slight effect of reducing N input from 500 to 300 kg/hm^2 on the WP_(Y-ET) and WP_(B-ET), however, when reducing N input to 100 kg/hm^2, the values of WP_(Y-ET) and WP_(B-ET) were significantly reduced. Water is the primary factor and N is the secondary factor in determining both yield(Y) and final biomass(B). Partial factor productivity from applied N(PFP_N) was the maximum under the higher irrigation level and in lower N rate(100–300 kg N/hm^2) in both years(2013 and 2014). Lowering the irrigation amount significantly reduced evapotranspiration(ET), but ET did not vary with different N rates(100–500 kg N/hm^2). Both Y and B had robust linear relationships with ET, but the correlation between B and ET(R^2=0.8588) was much better than that between Y and ET(R^2=0.6062). When ET increased, WP_(Y-ET) linearly increased and WP_(B-ET) decreased. Taking the indices of Y, B, WP_(Y-ET), WP_(B-ET) and PFP_N into account, a higher irrigation level(70%–65% of the field capacity) and a lower N rate(100–300 kg N/hm^2) are recommended to be a proper irrigation and N application strategy for plastic film-mulched hybrid maize seed production in arid Northwest China.RAN Hui KANG Shaozhong LI Fusheng DU Taisheng DING Risheng LI Sien TONG Ling 2017Journal of Arid Land2017,9,4:3
7Neck observation versus elective neck dissection in management of clinical T1/2N0 oral squamous cell carcinoma: a retrospective study of 232 patients显示文摘Objective: The management of early-stage(cT1/2N0) oral squamous cell carcinoma(OSCC) remains a controversial issue. The aim of this study was to compare the clinical outcomes of neck observation(OBS) and elective neck dissection(END) in treating patients with cT1/2N0 OSCC.Methods: A total of 232 patients with cT1/2N0 OSCC were included in this retrospective study. Of these patients, 181 were treated with END and 51 with OBS. The survival curves of 5-year overall survival(OS), diseasespecific survival(DSS), and recurrence-free survival(RFS) rates were plotted using the Kaplan-Meier method for each group, and compared using the Log-rank test.Results: There was no significant difference in 5-year OS and DSS rates between END and OBS groups(OS:89.0% vs. 88.2%, P=0.906; DSS: 92.3% vs. 92.2%, P=0.998). However, the END group had a higher 5-year RFS rate than the OBS group(90.1% vs. 76.5%, P=0.009). Patients with occult metastases in OBS group(7/51) had similar 5-year OS rate(57.1% vs. 64.1%, P=0.839) and DSS rate(71.4% vs. 74.4%, P=0.982) to those in END group(39/181). In the regional recurrence patients, the 5-year OS rate(57.1% vs. 11.1%, P=0.011) and DSS rate(71.4% vs. 22.2%, P=0.022) in OBS group(7/51) were higher than those in END group(9/181).Conclusions: The results indicated that OBS policy could obtain the same 5-year OS and DSS as END. 8Alternate partial root-zone irrigation with high irrigation frequency improves root growth and reduces unproductive water loss by apple trees in arid north-west China显示文摘Alternate partial root-zone irrigation(APRI)can improve water use efficiency in arid areas. However,the effectiveness and outcomes of different frequencies of APRI on water uptake capacity and physiological water use have not been reported. A two-year field experiment was conducted with two irrigation amounts(400 and500 mm) and three irrigation methods(conventional irrigation, APRI with high and low frequencies). Root length density, stomatal conductance, photosynthetic rate,transpiration rate, leaf water use efficiency, midday stem and leaf water potentials were measured. The results show that in comparison with conventional irrigation, APRI with high frequency significantly increased root length density and decreased water potentials and stomatal conductance.No differences in the above indicators between the two APRI frequencies were detected. A significantly positive relationship between stomatal conductance and root length density was found under APRI. 9A rare case of isolated castrate resistant bilateral testicular metastases in advanced prostate cancer显示文摘Testicular metastasis is rare with the prostate being the most common site of primary cancer.We report a case of a 72-year-old man with castration-resistant prostate cancer(CRPC)and known metastases to bone and lymph nodes,who developed bilateral painful swollen testes 3 years after the initial diagnosis of prostate cancer.He had first presented with lower urinary tract symptoms(LUTS)with suspicious findings on digital rectal examination of the prostate,and an elevated serum prostate specific antigen(PSA)level of 129 ng/mL.Transrectal prostate biopsy revealed Gleason 4þ5 adenocarcinoma.Radiological staging showed locally advanced prostate cancer with extensive metastases to bone and pelvic and retroperitoneal lymph nodes.He was given hormonal therapy for over 2 years until progression to CRPC.Six months later he developed painful bilateral testicular swellings,and serum markers for testicular germ cell cancer were normal.Bilateral orchiectomy was performed,showing metastatic prostate cancer(Gleason 4þ5)on histology.One month postoperatively his PSA level dropped to 0.1 ng/mL from a presurgery level of 6.24 ng/mL.Jiarui Su Edwin Jonathan Aslim Hakan Aydin Puay Hoon Tan Henry Sun Sien Ho 2018Asian Journal of Urology2018,5,2:2

20Effect of a new antitranspirant on the physiology and water use efficiency of soybean under different irrigation rates in an arid region显示文摘Antitranspirants are exogenous substances applied to leaves to reduce luxury transpiration by regulating stomatal conductance to increase water use efficiency(WUE). A cheap and environmentally-friendly antitranspirant, FZ, was newly developed, extracted mainly from Alhagi sparsifolia. Its effects on soybean water use were investigated in a field experiment using the locally-used irrigation rate and a low irrigation rate(The lower and upper limit of irrigation is 40%–70% of field capacity). Foliar application of FZ and measurement of leaf physiological characteristics, final biomass, seed yield and water use efficiency were carried out during the pod bearing and pod filling stages of drip-irrigated soybean with film-mulching. Under the low irrigation rate, leaf stomatal conductance(gs) and transpiration rate(Tr)decreased significantly by 7 d after spraying, but photosynthesis(Pn) and instantaneous water use efficiency(WUE_(in)) were not significantly affec ted. The stomatal frequency, stomatal aperture, gs, Tr and Pn decreased by 1 d after spraying, without significantly increasing WUE_(in).However, applying FZ during the pod bearing and pod filling stages did not significantly affect the final biomass,water consumption, seed yield and WUE of soybean.Under the locally-used irrigation rate, applying FZ increased the activities of superoxide dismutase and peroxidase in the leaves by 38% and 33%, respectively,but did not significantly affect gs, Tr, Pn, stomatal aperture and stomatal frequency. Applying FZ three times during pod bearing and pod filling stages enhanced seed yield and WUE by 24% and 21%, respectively, but did not significantly affect the final biomass and water consumption.
