1Effect of sinter layer porosity distribution on flow and temperature fields in a sinter cooler显示文摘When sinters are filled into the sinter cooler from the sintering machine, it is commonly seen that, due to segregation effects, sinters of larger size usually accumulate closer to the inner wall of the sinter cooler, whereas those of smaller size are to the outer wall. This nonuniform distribution of sinters has led to uneven cooling effect throughout the cooler. This causes the sinters leaving the cooler at a large temperature difference. This undesired temperature difference leads to the deformation and even the destruction of the conveyors. The computational fluid dynamics (CFD) technique was used in the present work to investigate the heat and fluid flow phenomena within the sinter cooler corresponding to the different distribution of sinter layer porosity, which was highly dependent on the arrangement and orientation of sinters within the sinter cooler. It is confirmed that a high mass flow rate within the sinter layer causes a low temperature region and vice versa. 2Adhesive small bowel obstruction: How long can patients tolerate conservative treatment?显示文摘AIM: To evaluate how long patients with small bowel obstruction caused by postoperative adhesions can tolerate conservative treatment.METHODS: The records of patients with small bowel obstruction due to postoperative adhesions were retrospectively reviewed. Data collected included the number of admissions, type of management for each admission,duration of conservative treatment, number of repeat laparotomies, and operative findings.RESULTS: One hundred fifty-five patients with this condition from January 1999 to December 2001, for a total of 293 admissions were enrolled in this study. Medical treatment alone was given in 220 admissions, and repeat laparotomy was performed in 73 admissions. The period of observation in patients managed medically ranged from 2 to 12 days (average: 6.9 days), while for those who underwent surgery,the range was 1 to 14 days (average 5.4 days). At surgery,adhesions were the only finding in 46 cases, while there were intestinal complications in 27, or 9.2 % of all 293admissions. 3MELD score can predict early mortality in patients with rebleeding after band ligation for variceal bleeding显示文摘AIM:To investigate the outcomes,as well as risk factors for 6-wk mortality,in patients with early rebleeding after endoscopic variceal band ligation (EVL) for esophageal variceal hemorrhage (EVH).METHODS:Among 817 EVL procedures performed for EVH between January 2007 and December 2008,128 patients with early rebleeding,defined as rebleeding within 6 wk after EVL,were enrolled for analysis.RESULT:The rate of early rebleeding after EVL for acute EVH was 15.6% (128/817).The 5-d,6-wk,3-mo,and 6-mo mortality rates were 7.8%,38.3%,55.5%,and 58.6%,respectively,in these early rebleeding patients.The use of beta-blockers,occurrence of hypovolemic shock,and higher model for end-stage liver disease (MELD) score at the time of rebleeding were independent predictors for 6-wk mortality.A cut-off value of 21.5 for the MELD score was found with an area under ROC curve of 0.862 (P < 0.001).The sensitivity,specificity,positive predictive value,and negative predictive value were 77.6%,81%,71.7%,and 85.3%,respectively.As for the 6-mo survival rate,patients with a MELD score ≥ 21.5 had a significantly lower survival rate than patients with a MELD score < 21.5 (P < 0.001).CONCLUSION:This study demonstrated that the MELD score is an easy and powerful predictor for 6-wk mortality and outcomes of patients with early rebleeding after EVL for EVH.Chun-Yen Lin I-Shyan Sheen Tsung-Nan Lin Chun-Jung Lin Wen-Juei Jeng Chien-Hao Huang Yu-Pin Ho Cheng-Tang Chiu 2011World Journal of Gastroenterology2011,17,16:18
4随机波浪作用下海床动力响应及液化的理论分析显示文摘运用随机波浪理论,基于广义Biot动力固结理论,建立了能够考虑波浪荷载随机特性影响的计算海床动力响应及液化深度的解析数值模型,并从时域上进行求解分析。通过与传统线性规则波浪荷载作用下海床动力响应及液化深度的数值结果的对比分析,详细讨论了考虑波浪荷载的随机特性对于求解海床动力响应及液化深度的影响程度。结果表明,根据随机波浪理论计算得到的海床中的超孔隙水压力、水平有效应力、竖向有效应力、剪应力表现出较强的不规则性,其幅值沿着海床深度方向的变化趋势与根据线性规则波浪理论计算得到结果的变化趋势基本相同,但在同数量级上更大,同时计算得到的海床最大液化深度明显大于根据线性规则波浪理论计算得到的结果。因此,在海洋地基设计和自由场地安全评估时应该合理地考虑波浪荷载的随机特性。王忠涛 栾茂田 Jeng Dong-Sheng 刘占阁 2008岩土力学2008,29,8:15
5Is p53 gene mutation an indicatior of the biological behaviors of recurrence of hepatocellular carcinoma?显示文摘AIM: To evaluate mutant p53 gene in primary hepatocellular carcinoma and to investigate the correlation between it and the recurrence of hepatocellular carcinoma.METHODS: Mutations of p53 gene were examined using antihuman p53 monoclonal antibody and immunohistochemical staining in 79 resected hepatocellular carcinomas. The correlations among variables of p53 positivity and invasiveness, disease free interval and survival were studied.In addition, in those who developed recurrence, the correlation among p53 positivity, clinical features and postrecurrence survival were also studied.RESULTS: Of these 79 cases, 64 (81%) had p53 mutation.Those patients with mutant p53 positivityhad significantly more tumor recurrence (76.6 % vs 40.0 %, P=0.0107).However, the COX proportional hazards model showed that p53 overexpression had only weak correlations with recurrence free interval and survival time (P=0.088 and 0.081), which was probably related to the short duration of follow-up. The invasiveness variables may be predictors of HCC recurrence. On univariate analysis, more patients with mutant p53 positivity had vascular permeation [78.1vs 40.0 %, P=0.0088, O.R. (odds ratio) =5.3], grade Ⅱ-ⅣV differentiation (98.4 vs 80.0 %, P=0.0203, O.R. =15.7), no complete capsule (82.8 vs 53.3 %, P=0.0346, O.R. =4.2)and daughter nodules (60.9 vs. 33.3 %, P=0.0527, O.R.=3.1) than patients with negative p53 staining. Onmultivariate analysis, only vascular permeation and grade of differentiation remained significant (P=0.042 and 0.012).There was no statistically significant correlation betweenthe status of p53 in the primary lesion and the clinical features of recurrent hepatocellular carcinomas examined,including extrahepatic metastasis (P=0.1103) and the number of recurrent tumors (P= 1.000) except for diseaseover more than one segment in the extent of recurrent tumors (P=0.0043). The post-recurrence median survival was lower in patients in whom p53 mutation had been detected in the primary lesion with a weak significance (3.42 months vs 11.0 months, P=0.051). CONCLUSION: Our findings suggest that p53 mutation correlates significantly with invasiveness including vascular permeation, grade of cellular differentiation, incomplete capsule and multinodular lesions. Hepatocellular carcinomas with p53 mutations had more tumor recurrence and p53mutation may also influence disease recurrence interval and survival time. Hepatocellular carcinomas with p53 mutations recur more extensively with a shorter survival. 6Is the expression of γ-glutamyl transpeptidase messenger RNA an indicator of biological behavior in recurrent hepatocellular carcinoma?显示文摘AIM: To investigate the correlation between gammaglutamyl transpeptidase (γ-GTP) expression in the primary HCC and post-resection recurrence and its biological behaviors.METHODS: Forty consecutive patients having curative resection for HCC were included in this study. The primers for reverse-transcription polymerase chain reaction (RTPCR) were corresponding to the 5'-noncoding human γ-GTP mRNA of fetal liver (type A), HepG2 cells (type B),and placenta (type C). Both the cancer and non-cancerous tissues of the resected liver were analyzed. The correlations between the expression of γ-GTP and the clinicopathological variables and outcomes (recurrence and survival) were studied.RESULTS: Those with type B γ-GTP mRNA in cancer had significant higher recurrence rate than those without it (63.6 % vs 14.3 %). Both those with type B in cancer and in non-cancer died significantly more than those without it (45.5 % vs 0 % and 53.6 % vs 0 %, respectively). By multivariate analysis, the significant predictors of recurrence included high serum AFP (P=0.0108), vascular permeation (P=0.0084), and type B γ-GTP mRNA in non-cancerous liver (P=0.0107). The significant predictors of postrecurrence death included high serum AFP (P=0.0141),vascular permeation (P=0.0130), and daughter nodules (P=0.0053). As to the manifestations (recurrent number ≥2, recurrent extent≥2 segments, extra-hepatic metastasis, and death) in recurrent patients, there were no statistical significant differences between those with type B in the primary tumor and those without it. 7NEURAL NETWORK MODELING FOR ESTIMATION OF SCOUR DEPTH AROUND BRIDGE PIERS显示文摘It is essential to predict the scour depth around bridge piers for hydraulic engineers involved in the economical design of bridge pier foundation. Conventional investigations have long been of the opinion that empirical scour prediction equations based on laboratory data over predict scour depths. In this article, the Back-Propagation Neural Network (BPN) was applied to predict the scour depth in order to overcome the problem of exclusive and the nonlinear relationships. The observations obtained from thirteen states in USA was verified by the present model. 8Adjuvant heparanase inhibitor PI-88 therapy for hepatocellular carcinoma recurrence显示文摘AIM: To demonstrate that administering heparanase inhibitor PI-88 at 160 mg/d is safe and promising in reducing hepatocellular carcinoma(HCC) recurrence for up to 3 year following curative resection. METHODS: A total of 143 patients(83.1% of the 172 participants in the phase Ⅱ study) participated in the follow-up study. Of these patients, 50 had received no treatment, 48 had received 160 mg/d PI-88, and 45 had received 250 mg/d PI-88 during the phase Ⅱ trial. Safety parameters and the following efficacy endpoints were investigated:(1) time to recurrence;(2) diseasefree survival; and(3) overall survival. RESULTS: PI-88 at 160 mg/d delayed the onset and frequency of HCC recurrence, and provided a clinically significant survival advantage for up to 3 years after treatment compared with those of the control group:(1) the recurrence-free rate increased from 50% to 63%, and(2) time to recurrence at the 36 th percentile was postponed by 78%. The efficacy of administering PI-88 at 250 mg/d was confounded by a high dropout rate(11 out of 54 patients). Additionally, subgroup analyses of patients with(1) multiple tumors or a single tumor ≥ 2 cm; and(2) hepatitis B or C revealed that administering PI-88 at 160 mg/d conferred the most significant survival advantage(56.8% improvement in disease-free survival, P = 0.045) for patients with both risk factors for recurrence. 9Hepatic arterial infusion chemotherapy for hepatocellular carcinoma with portal vein tumor thrombosis显示文摘AIM: Hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT) is associated with poor prognosis. The aim of this prospective study was to evaluate the efficacy of hepatic arterial infusion chemotherapy (HAIC) for patients with this disease.METHODS: Eighteen HCC patients with PVTT were treated with HAIC via a subcutaneously implanted injection port. A course of chemotherapy consisted of daily cisplatin (10 mg for one hour) followed by 5-fiuorouracil (250 mg for five hours) zfor five continuous days within a given week. The patients were scheduled to receive four consecutive courses of HAIC.Responders were defined in whom either a complete or partial response was achieved, while non-responders were defined based on stable or progressive disease status. The prognostic factors associated with survival after treatment were analyzed.RESULTS: Six patients exhibited partial response to this form of HAIC (response rate = 33 %). The 3, 6, 9, 12 and 18-month cumulative survival rates for the 18 patients were 83 %, 72 %, 50 %, 28 %, and 7 %, respectively. Median survival times for the six responders and 12 non-responders were 15.0 (range, 11-18) and 7.5 (range, 1-13) months,respectively, It was demonstrated by both univariate and multivariate analyses that the therapeutic response and hepatic reserve function were significant prognostic factors.CONCLUSION: HAIC using low-dose cisplatin and 5-fluorouracil may be a useful alternative for the treatment of patients with advanced HCC complicated with PVTT. 16Malignant fibrous histiocytoma presenting as hemoperitoneum mimicking hepatocellular carcinoma rupture显示文摘Malignant fibrous histiocytoma (MFH) is a pleomorphic mesenchynal sarcoma. It is uncommonly arises primarily from the intra-peritoneal cavity. Primary peritoneal MFH with tumor bleeding and rupture is rare. We describe the imaging features of a 70-year-old patient presenting with ruptured hemorrhagic peritoneal MFH at subhepatic area,accompanied by massive hemoperitoneum,mimicking a ruptured pedunculated hepatocellular carcinoma. Computed tomography (CT) revealed a large heterogeneous enhanced subhepatic mass with adjacent liver,gallbladder and colon invasion. Tumor hemorrhage and rupture complicated with peritoneal seeding and massive bloody ascites were also detected. Angiography showed a hypervascular tumor fed by enlarged right hepatic arteries,cystic artery and omental branches of gastroepiploic artery. The patient underwent laparotomy for tumor resection,but the tumor recurred one month after operation. 18Failure mode and dynamic response of a double-sided slope with high water content of soil显示文摘A double-sided slope with high water content in sandy clay was considered under the action of seismic load. Its failure mode and dynamic response were investigated using a hydraulic servo shaking table test. The typical characteristic of failure mode and dynamic responses of the double-sided slope were analyzed. Experimental results show that slope failure undergoes a process of progressive deformation. The slope failure mode can be explained as creep sliding landslide. AFA(Amplification Factor of Acceleration) at the surface and inner parts of the slope shows an increasing trend with the increase of relative elevation. The relationship between AFA and EAA(Excitation Amplitude of Acceleration) is nonlinear. An empirical formula is proposed to describe preferably the relationship between AFA,relative elevation and dimensionless EAA. 19Consolidation and dynamics of 3D unsaturated porous seabed under rigid caisson breakwater loaded by hydrostatic pressure and wave显示文摘In this study,based on the dynamic Biot's theory 'u-p' approximation,a 3D finite element method(FEM) numerical soil model is developed,in which the Generalized Newmark-β method is adopted to determine the time integration.The developed 3D FEM soil model is a part of the coupled model PORO-WSSI 3D for 3D wave-seabed-marine structures interaction problem,and is validated by the analytical solution proposed by Wang(2000) for a laterally infinite seabed loaded by a uniform force.By adopting the developed 3D soil model,the consolidation of seabed under a caisson breakwater and hydrostatic pressure is investigated.The numerical results show that the caisson breakwater built on seabed has very significant effect on the stresses/displacements fields in the seabed foundation after the transient deformation and primary consolidation are completed.The parametric study indicates that the Young's modulus E of seabed is the most important parameter to affect the settlement of breakwater,and the displacement fields in seabed foundation.Taking the consolidation status as the initial condition,the interaction between ocean wave,caisson breakwater and seabed foundation is briefly investigated.The 3D ocean wave is determined by solving the Navier-Stokes equations with finite volume method(FVM).The numerical results indicate that there is intensive interaction betweenYE JianHong JENG DongSheng CHAN A H C 2012Science China(Technological Sciences)2012,55,8:4
