| 1 | Fever as the only manifestation of hypersensitivity reactions associated with oxaliplatin in a patient with colorectal cancer Oxaliplatin-induced hypersensitivity reaction显示文摘Hypersensitivity reactions (HSR) to oxaliplatin in patients with colorectal cancer include facial ushing, erythema, pruritis, fever, tachycardia, dyspnea, tongue swelling, rash/hives, headache, chills, weakness, vomiting, burning sensations, dizziness, and edema. We report a patient with fever as the sole manifestation of initial HSR, review the literature and discuss the management of HSR. A 57-year-old female with T3N2M0 rectal adenocarcinoma received modified FOLFOX-6. She tolerated the first 8 cycles without any toxicities except grade 1 peripheral neuropathy and nausea. During 9th and 10th infusions, she developed fever to a maximum of 38.3℃ with stable hemodynamic status despite medications. During 11th infusion, she developed grade 3 HSR consisting of symptomatic bronchospasm, hypotension, nausea, vomiting, cough, and fever. On examination, she was pale, cyanotic, with a temperature of 38.8℃, BP dropped to 95/43 mm Hg, pulse of 116/min and O2 saturation of 88%-91%. She was hospitalized for management and recovered in 24 h. Fever alone is not a usual symptom of oxaliplatin HSR. It may be indicative that the patient may develop serious reactions subsequently, as did our patient who developed hypotension with the third challenge. Treatment and prevention consists of slowing the infusion rate, use of steroids and antagonists of Type 1 and 2 histamine receptor antagonists, whereas desensitization could help to provide the small number of patients who experience severe HSR with the ability to further receive an effective therapy for their colorectal cancer. | M Wasif Saif Shailja Roy Leslie Ledbetter Jennifer Madison Kostas Syrigos | 2007 | World Journal of Gastroenterology2007,13,39: | 4 |
| 2 | Factors affecting residual exotropia after two muscle surgery for intermittent exotropia显示文摘AIM: To study the factors affecting residual exotropia(>10 PD) at 4-6wk postoperative visit following two rectus muscle surgery for intermittent exotropia [bilateral lateral rectus(LR) recession or unilateral recess resect procedure].METHODS: A retrospective chart review of patients with intermittent exotropia ≤50 PD who underwent two rectus muscle surgery in between Jan. 2011 to Dec. 2013 was performed. Possible factors were compared between patients with residual exotropia(>10 PD) and successful outcome(within 10 PD of orthotropia) at the 4-6wk postoperative visit. Effect/dose ratio was calculated by dividing the effect of surgery by the total amount(mm) of muscle surgery done.RESULTS: One hundred and fifty-seven patients with mean age of 14y(range 3-53y) were included. Twentyseven patients(17.2%) had residual exotropia at 4-6wk postoperative follow up. Age at surgery(P=0.009) and preoperative deviation for distance(P≤0.001) and near(P=0.001) were identified as important predictors of unsuccessful outcome. The occurrence of residual exotropia was not affected by amblyopia, anisometropia, lateral incomitance, pattern deviation, vertical deviation, type of exotropia or type of surgery done(recess-resect or bilateral LR recession). The effect/dose ratio was more in deviations >40 PD in the both recess-resect and bilateral LR recession type of surgery. The effect/dose ratio was less in patients with residual exotropia as compared to the successful outcome group(1.36 PD/mm vs 2.05 PD/mm in the bilateral LR recession surgery and 1.93 PD/mm vs 2.63 PD/mm in the unilateral recess-resect surgery).CONCLUSION: Residual exotropia is seen in 17% of patients after two muscle surgery for intermittent exotropia. | 11 | Tailoring of structural,optical and electrical properties of anatase TiO_(2)via doping of cobalt and nitrogen ions显示文摘Pure(pristine)anatase and mono–doped and co–doped derivatives of TiO_(2)having nitrogen(N)and cobalt(Co)as dopants with respective fixed doping concentrations of 0.7 mol.%and 1.0 mol.%were synthesized using auto–combustion sol–gel technique.The doping effects at corresponding non–metal and transition metal sites of TiO_(2)on the basis of the structural,optical and electrical properties have been investigated.X–ray diffraction(XRD)measurement confirms the formation of pure anatase phase of TiO_(2)for all samples having I41/amd space group of tetragonal structure which has been also verified by the Raman spectroscopy measurement.Various crystallographic parameters have been calculated by performing Rietveld refinement of XRD data including average crystallite size that has been observed in the range of10–15 nm.Pure anatase phase indicates the incorporation of Co^(2+)into TiO_(2)lattice which assists the substitution of N in place of oxygen in co–doped TiO_(2).The band gap tuning towards the visible region from3.2 to 2.1 e V has been achieved with mono–doping and co–doping of the N and Co in TiO_(2)lattice.This can be described in terms of the formation of localized levels of N–2p and Co–3d states in mono–doping and an isolated intermediate band formation in co–doping case.Electrical properties have been investigated in details and explained as the synergetic effects of structural and inherent ionic characters of various dopants.The observed band gaps of all doped samples lie within the visible region which makes them pertinent as the solar energy harnessing materials for photocatalytic and photovoltaic applications. | Anchal Sharma Puneet Negi Ruhit Jyoti Konwar Hemaunt Kumar Yogita Verma Shailja Prakash Chandra Sati Bhargav Rajyaguru Himanshu Dadhich N.A.Shah P.S.Solanki | 2022 | Journal of Materials Science & Technology2022,,16: | 1 |

| 20 | In vitro gene silencing of independent phosphoglycerate mutase (iPGM) in the filarial parasite Brugia malayi显示文摘Background:The phosphoglycerate mutase(PGM)enzyme catalyzes the interconversion of 2-and 3-phosphoglycerate in the glycolytic/gluconeogenic pathways that are present in the majority of cellular organisms.They can be classified as cofactor-dependent PGM(dPGM)or cofactor-independent PGM(iPGM).Vertebrates,yeasts,and many bacteria have only dPGM,while higher plants,nematodes,archaea,and many other bacteria have only iPGM.A small number of bacteria,including Escherichia coli and certain archaea and protozoa,contain both forms.The silencing of ipgm in Caenorhabditis elegans(C.elegans)has demonstrated the importance of this enzyme in parasite viability and,therefore,its potential as an anthelmintic drug target.In this study,the role of the Brugia malayi(B.malayi)ipgm in parasite viability,microfilaria release,embryogenesis,and in vivo development of infective larvae post-gene silencing was explored by applying ribonucleic acid(RNA)interference studies.Results:The in vitro ipgm gene silencing by small interfering RNA(siRNA)leads to severe phenotypic deformities in the intrauterine developmental stages of female worms with a drastic reduction(~90%)in the motility of adult parasites and a significantly reduced(80%)release of microfilariae(mf)by female worms in vitro.Almost half of the in vitro-treated infective L3 displayed sluggish movement.The in vivo survival and development of siRNA-treated infective larvae(L3)was investigated in the peritoneal cavity of jirds where a~45%reduction in adult worm establishment was observed.Conclusion:The findings clearly suggest that iPGM is essential for both larval and adult stages of B.malayi parasite and that it plays a pivotal role in female worm embryogenesis.The results thus validate the Bm-iPGM as a putative anti-filarial drug target. | Prashant Kumar Singh Susheela Kushwaha Shahab Mohd Manisha Pathak Shailja Misra-Bhattacharya | 2013 | Infectious Diseases of Poverty2013,2,1: | 0 |
