| 1 | Upper gastrointestinal tract capsule endoscopy using a nurse-led protocol: First reported experience显示文摘AIM To test the feasibility and performance of a novel upper gastrointestinal(GI) capsule endoscope using a nurse-led protocol. METHODS We conducted a prospective cohort analysis of patients who declined gastroscopy(oesophagogastroduodenoscopy, OGD) but who consented to upper GI capsule endoscopy. Patients swallowed the upper GI capsule following ingestion of 1 liter of water(containing simethicone). A series of positional changes were used to exploit the effects of water flow and move the upper GI capsule from one gravity-dependent area to another using a nurse-led protocol. Capsule transit time, video reading time, mucosal visualisation, pathology detection and patient tolerance was evaluated.RESULTS Fifty patients were included in the study. The mean capsule transit times in the oesophagus and stomach were 28 s and 68 min respectively. Visualisation of the following major anatomical landmarks was achieved(graded 1-5: Poor to excellent): Oesophagus, 4.8(± 0.5); gastro-oesophageal junction(GOJ), 4.8(± 0.8); cardia, 4.8(± 0.8); fundus, 3.8(± 1.2); body, 4.5(± 1); antrum, 4.5(± 1); pylorus, 4.7(± 0.8); duodenal bulb, 4.7(± 0.7); second part of the duodenum(D2), 4.7(± 1). The upper GI capsule reached D2 in 64% of patients. The mean video reading time was 48 min with standard playback mode and 20 min using Quickview(P = 0.0001). No pathology was missed using Quickview. Procedural tolerance was excellent. No complications were seen with the upper GI capsule. CONCLUSION The upper GI capsule achieved excellent views of the upper GI tract. | 2 | Capsule endoscopy:Current practice and future directions显示文摘Capsule endoscopy(CE)has transformed investigation of the small bowel providing a non-invasive,well tolerated means of accurately visualising the distal duodenum,jejunum and ileum.Since the introduction of small bowel CE thirteen years ago a high volume of literature on indications,diagnostic yields and safety profile has been presented.Inclusion in national and international guidelines has placed small bowel capsule endoscopy at the forefront of investigation into suspected diseases of the small bowel.Most commonly,small bowel CE is used in patients with suspected bleeding or to identify evidence of active Crohn’s disease(CD)(in patients with or without a prior history of CD).Typically,CE is undertaken after upper and lower gastrointestinal flexible endoscopy has failed to identify a diagnosis.Small bowel radiology or a patency capsule test should be considered prior to CE in those at high risk of strictures(such as patients known to have CD or presenting with obstructive symptoms)to reduce the risk of capsule retention.CE also has a role in patients with coeliac disease,suspected small bowel tumours and other small bowel disorders.Since the advent of small bowel CE,dedicated oesophageal and colon capsule endoscopes have expanded the fields of application to include the investigation of upper and lower gastrointestinal disorders.Oesophageal CE may be used to diagnose oesophagitis,Barrett’s oesophagus and varices but reliability in identifying gastroduodenal pathology is unknown and it does not have biopsy capability.Colon CE provides an alternative to conventional colonoscopy for symptomatic patients,while a possible role in colorectal cancer screening is a fascinating prospect.Current research is already addressing the possibility of controlling capsule movement and developing capsules which allow tissue sampling and the administration of therapy. | Melissa F Hale Reena Sidhu Mark E McAlindon | 2014 | World Journal of Gastroenterology2014,20,24: | 9 |

| 4 | Neuro-regulation of lower esophageal sphincter function as treatment for gastroesophageal reflux disease显示文摘The junction between the esophagus and the stomach is a specialized region, composed of lower esophageal sphincter (LES) and its adjacent anatomical structures, the gastric sling and crural diaphragm. Together these structures work in a coordinated manner to allow ingested food into the stomach while preventing reflux of gastric contents across the esophago-gastric junction (EGJ) into the esophagus. The same zone also permits retrograde passage of air and gastric contents into esophagus during belching and vomiting. The precise coordination required to execute such a complicated task is achieved by a finely-regulated high-pressure zone. This zone keeps the junction between esophagus and stomach continuously closed, but is still able to relax briefly via input from inhibitory neurons that are responsible for its innervation. | 5 | Variation among poplar clones for growth and crown traits under field conditions at two sites of North-western India显示文摘We evaluated the growth and crown traits of 36 poplar clones at two distinct agro-climatic regions of Punjab (Ludhiana and Bathinda) in northwestern India, following randomized block design with three replications and plot size of four trees. Significant differences among clones (p<0.001) were observed for diameter at breast height (DBH), tree height, volume, crown width and number of branches under both the site conditions. Clones 'G-3', '25-N' and '41-N' at Ludhiana and 'G-3', 'RD-01' and 'S 7 C 8 ' at Bathinda were found to be superior for volume production. All growth and crown traits registered significantly higher values at Ludhiana in comparison to those at Bathinda. Clone × site interaction was also significant (p<0.001). For volume, clones 'L-62/84', '113520', '25-N' and 'S 4 C 2 ' witnessed huge fluctuations in ranking between sites. The correlations between growth traits were positive and highly significant (p<0.001) at both sites. The clonal mean heritability was moderate for DBH and volume both at Ludhiana (0.61 0.66) and Bathinda (0.61 0.62). | 9 | Correlation between the proportion of breast volume involved by locally advanced tumors and invasion of the skin and posterior structures显示文摘AIM: To evaluate any differences between the percentages of involved breast volume, pathologic attributes,and tumor marker expression of T3 and T4a-c tumors in locally advanced breast cancers(BC).METHODS: All patients with T3 N > 0 and T4a-c BC without evidence of distant metastasis(M0), presenting to the Breast Clinic from 1980 to 2010, were examinedto determine whether their BC's involved ≥ 50% of their breast volumes, defined by gross replacement of at least one hemisphere. Core needle biopsy or postmastectomy specimens from tumors involving a known percent of breast volume were evaluated for:(1)pathological grades and lympho-vascular invasion(LVI);(2) hormone receptor(ER/PR) expression > 0; and(3)epidermoid growth factor 2(her2) over-expression(3+)by immune-histochemical staining or fluorescent in situ hybridization.RESULTS: The data base included 98 patients with T3N> 0 M0 and 120 with T4a-c, any N disease, M0 disease. T3 tumor masses involved 50% or more of the breast in 23/98(24%), and T4a-c tumors 65/120(54%)(P < 0.001). Only 1% of T3 tumors and 23% of T4a-c tumors presented with total breast replacement.
