1The Open Quantum Materials Database (OQMD): assessing the accuracy of DFT formation energies显示文摘The Open Quantum Materials Database(OQMD)is a high-throughput database currently consisting of nearly 300,000 density functional theory(DFT)total energy calculations of compounds from the Inorganic Crystal Structure Database(ICSD)and decorations of commonly occurring crystal structures.To maximise the impact of these data,the entire database is being made available,without restrictions,at www.oqmd.org/download.In this paper,we outline the structure and contents of the database,and then use it to evaluate the accuracy of the calculations therein by comparing DFT predictions with experimental measurements for the stability of all elemental ground-state structures and 1,670 experimental formation energies of compounds.This represents the largest comparison between DFT and experimental formation energies to date.The apparent mean absolute error between experimental measurements and our calculations is 0.096 eV/atom.In order to estimate how much error to attribute to the DFT calculations,we also examine deviation between different experimental measurements themselves where multiple sources are available,and find a surprisingly large mean absolute error of 0.082 eV/atom.Hence,we suggest that a significant fraction of the error between DFT and experimental formation energies may be attributed to experimental uncertainties.Finally,we evaluate the stability of compounds in the OQMD(including compounds obtained from the ICSD as well as hypothetical structures),which allows us to predict the existence of~3,200 new compounds that have not been experimentally characterised and uncover trends in material discovery,based on historical data available within the ICSD.Scott Kirklin James E Saal Bryce Meredig Alex Thompson Jeff W Doak Muratahan Aykol Stephan Rühl Chris Wolverton 2015npj Computational Materials2015,,1:62
2Non-alcoholic fatty liver disease and the metabolic syndrome:An update显示文摘Sedentary lifestyle and poor dietary choices are leading to a weight gain epidemic in westernized countries, subsequently increasing the risk for developing the metabolic syndrome and nonalcoholic fatty liver disease (NAFLD). NAFLD is estimated to affect approximate 30% of the general US population and is considered the hepatic manifestation of the metabolic syndrome. Recent findings linking the components of the metabolic syndrome with NAFLD and the progression to nonalcoholic steatohepatitis (NASH) will be reviewed; in particular, the role of visceral adipose tissue, insulin resistance, and adipocytokines in the exacerbation of these conditions. 3Portal vein thrombosis, mortality and hepatic decompensation in patients with cirrhosis: A meta-analysis显示文摘AIM: To determine the clinical impact of portal vein thrombosis in terms of both mortality and hepatic decompensations(variceal hemorrhage, ascites, portosystemic encephalopathy) in adult patients with cirrhosis.METHODS: We identified original articles reported through February 2015 in MEDLINE, Scopus, Science Citation Index, AMED, the Cochrane Library, and relevant examples available in the grey literature. Two independent reviewers screened all citations for inclusion criteria and extracted summary data. Random effects odds ratios were calculated to obtain aggregate estimates of effect size across included studies, with 95%CI.RESULTS: A total of 226 citations were identified and reviewed, and 3 studies with 2436 participants were included in the meta-analysis of summary effect. Patients with portal vein thrombosis had an increased risk of mortality(OR = 1.62, 95%CI: 1.11-2.36, P = 0.01). Portal vein thrombosis was associated with an increased risk of ascites(OR = 2.52, 95%CI: 1.63-3.89, P < 0.001). There was insufficient data available to determine the pooled effect on other markers of decompensation including gastroesophageal variceal bleeding or hepatic encephalopathy. CONCLUSION: Portal vein thrombosis appears to increase mortality and ascites, however, the relatively small number of included studies limits more generalizable conclusions. 5Nonalcoholic fatty liver disease and mitochondrial dysfunction显示文摘Nonalcoholic fatty liver disease (NAFLD) includes hepatic steatosis, nonalcoholic steatohepatitis (NASH), fibrosis, and cirrhosis. NAFLD is the most common liver disorder in the United States and worldwide. Due to the rapid rise of the metabolic syndrome, the prevalence of NAFLD has recently dramatically increased and will continue to increase. NAFLD has also the potential to progress to hepatocellular carcinoma (HCC) or liver failure. NAFLD is strongly linked to caloric overconsumption, physical inactivity, insulin resistance and genetic factors. Although significant progress in understanding the pathogenesis of NAFLD has been achieved in years, the primary metabolic abnormalities leading to lipid accumulation within hepatocytes has remained poorly understood. Mitochondria are critical metabolic organelles serving as 'cellular power plants'. Accumulating evidence indicate that hepatic mitochondrial dysfunction is crucial to the pathogenesis of NAFLD. 6Endoscopy and polyps-diagnostic and therapeutic advances in management显示文摘Despite multiple efforts aimed at early detection through screening, colon cancer remains the third leading cause of cancer-related deaths in the United States, with an estimated 51000 deaths during 2013 alone. The goal remains to identify and remove benign neoplastic polyps prior to becoming invasive cancers. Polypoid lesions of the colon vary widely from hyperplastic, hamartomatous and inflammatory to neoplastic adenomatous growths. Although these lesions are all benign, they are common, with up to one-quarter of patients over 60 years old will develop pre-malignant adenomatous polyps. Colonoscopy is the most effective screening tool to detect polyps and colon cancer, although several studies have demonstrated missed polyp rates from 6%-29%, largely due to variations in polyp size. This number can be as high as 40%, even with advanced (> 1 cm) adenomas. Other factors including sub-optimal bowel preparation, experience of the endoscopist, and patient anatomical variations all affect the detection rate. Additional challenges in decision-making exist when dealing with more advanced, and typically larger, polyps that have traditionally required formal resection. 7经肛全直肠系膜切除术治疗直肠癌的优点和局限显示文摘在美国,结直肠癌的发病率和死亡率分别位于恶性肿瘤的第4位和第2位,其中直肠癌约占结直肠癌的1/3。对于可切除直肠癌而言,全直肠系膜切除术(total mesorectal excision,TME)仍然是手术的金标准。但骨盆狭窄、新辅助治疗后肿瘤触摸不清、照明及术野显露困难、远切缘确定困难及吻合器无法放置等解剖、技术和患者方面的因素,会使开腹及腹腔镜TME手术困难。在此环境下,"自下而上"的TME手术——经肛全直肠系膜切除手术(taTME)应运而生。taTME手术结合了腹腔镜及机器人手术的微创化、高清内镜平台的可视化及功能化以及经肛经腹直肠癌TME手术的优势。2010—2013年期间,Lacy等研究者陆续系列报道了taTME手术的实施情况,将该术式推向了高潮。 taTME手术采取改良截石位。手术分为腹部手术和会阴手术两个部分。腹部手术具体步骤可由术者偏好调整,但总体包括游离左侧结肠和脾曲、高位结扎肠系膜下血管和标准的TME手术。会阴部手术先在肿瘤远端采用荷包缝合封闭肠腔,在进行直肠冲洗后,放入操作平台,环形切开肠壁,"自下而上"沿直肠系膜无血管层面进行游离,直至与腹部组会合。标本可以经肛或经造口部位取出。可采取手工缝合或吻合器吻合完成消化道重建。taTME手术的优点和局限如下。 1. taTME手术优点:taTME手术采用经肛进入盆腔的入路方式,即使在低位局部进展期直肠癌、肥胖、骨盆狭窄等操作难度大的手术中,依然可以获得满意的视野显露。同时,由于在肿瘤远端荷包缝合肠壁,故对肿瘤远端切缘的判断更加准确。taTME手术相对简便地完成远端直肠系膜的游离切除,可能更有利于确保手术标本切缘的安全性和淋巴清扫的彻底性,并能得到更高质量的TME手术切除标本,使直肠癌患者的生存获益。国际登记机构的结果显示,taTME术后系膜完整率为96%,环周切缘和远端切缘阳性率分别为2.4%和0.3%。与腹腔镜手术相比,taTME手术时间更短,术后并发症发生率更低。taTME术后吻合口漏发生率和病死率分别为6.7%和2.6%。 2. taTME手术局限:尽管taTME手术有上述优点,现有研究也提示,taTME术后短期肿瘤学安全性满意,但这毕竟是一种新术式,尚缺乏长期随访结果,需要更大样本的对照试验来探究其临床、肿瘤学和功能学结果。此外,对于一个新术式,在实践中会遇见一些新的并发症。特别是"自下而上"的路径,要求外科医生从不同的视角重新认识原来的解剖结构,如直肠后间隙的游离、血管神经束的保护等。此外,也需要重视和预防术中输尿管损伤、盆壁自主神经损伤、骶前静脉损伤和术后盆腔脓肿的发生。上述损伤尤其容易发生在系膜或前列腺层面上,因此,对于有肿瘤史、盆腔放疗史、前列腺切除史或前列腺肥大的患者尤其需要谨慎。必须指出的是,梗阻性直肠肿瘤、急诊手术和T4期肿瘤是taTME的禁忌证。 综上所述,taTME有利于骨盆狭窄的"高难度"低位直肠癌患者的手术切除和标本质量,但作为一种全新的术式,还需要进一步研究其临床、肿瘤学和功能学结果。Deborah S Keller Scott R Steele 楼征 张卫 2018中华胃肠外科杂志2018,21,3:16

10Pathophysiology of cerebral oedema in acute liver failure显示文摘Cerebral oedema is a devastating consequence of acute liver failure(ALF)and may be associated with the development of intracranial hypertension and death.In ALF,some patients may develop cerebral oedema and increased intracranial pressure but progression to lifethreatening intracranial hypertension is less frequent than previously described,complicating less than one third of cases who have proceeded to coma since the advent of improved clinical care.The rapid onset of encephalopathy may be dramatic with the development of asterixis,delirium,seizures and coma.Cytotoxic and vasogenic oedema mechanisms have been implicated with a preponderance of experimental data favouring a cytotoxic mechanism.Astrocyte swelling is the most consistent neuropathological finding in humans with ALF and ammonia plays a definitive role in the development of cytotoxic brain oedema.The mechanism(s)by which ammonia induces astrocyte swelling remains unclear but glutamine accumulation within astrocytes has led to the osmolyte hypothesis.Current evidence also supports an alternate‘Trojan horse’hypothesis,with glutamine as a carrier of ammonia into mitochondria,where its accumulation results in oxidative stress,energy failure and ultimately astrocyte swelling.Although a complete breakdown of the blood-brain barrier is not evident in human ALF,increased permeation to water and other small molecules such as ammonia has been demonstrated resulting from subtle alterations in the protein composition of paracellular tight junctions.At present,there is no fully efficacious therapy for cerebral oedema other than liver transplantation and this reflects our incomplete knowledge of the precise mechanisms underlying this process which remain largely unknown.Teresa R Scott Victoria T Kronsten Robin D Hughes Debbie L Shawcross 2013World Journal of Gastroenterology2013,19,48:8
11Operative considerations for rectovaginal fistulas显示文摘To describe the etiology, anatomy and pathophysiology of rectovaginal fistulas(RVFs); and to describe a systematic surgical approach to help achieve optimal outcomes. A current review of the literature was performed to identify the most up-to-date techniques and outcomes for repair of RVFs. RVFs present a difficult problem that is frustrating for patients and surgeons alike. Multiple trips to the operating room are generally needed to resolve the fistula, and the recurrence rate approaches40% when considering all of the surgical options. At present, surgical options range from collagen plugs and endorectal advancement flaps to sphincter repairs or resection with colo-anal reconstruction. There are general principles that will allow the best chance for resolution of the fistula with the least morbidity to the patient. 12Efficacy of peroral endoscopic myotomy vs other achalasia treatments in improving esophageal function显示文摘AIM: To assess and compare the esophageal function after peroral endoscopic myotomy(POEM) vs other conventional treatments in achalasia.METHODS: Chart review of all achalasia patients who underwent POEM, laparoscopic Heller myotomy(LHM) or pneumatic dilation(PD) at our institution between January 2012 and March 2015 was performed. Patient demographics, type of achalasia, prior treatments, pre- and post-treatment timed barium swallow(TBE) and high-resolution esophageal manometry(HREM) findings were compared between the three treatment groups. Patients who had both pre- and 2 mo posttreatment TBE or HREM were included in the final analysis. TBE parameters compared were barium column height, width and volume of barium remaining at 1 and 5 min. HREM parameters compared were basal lower esophageal sphincter(LES) pressures and LES-integrated relaxation pressures(IRP). Data are presented as mean ± SD, median [25th, 75 th percentiles] or frequency(percent). Analysis of variance, KruskalWallis test, Pearsons χ~2 test and Fishers Exact tests were used for analysis.RESULTS: A total of 200 achalasia patients were included of which 36 underwent POEM, 22 underwent PD and 142 underwent LHM. POEM patients were older(55.4 ± 16.8 years vs 46.5 ± 15.7 years, P = 0.013) and had higher BMI than LHM(29.1 ± 5.9 kg/m^2 vs 26 ± 5.1 kg/m^2, P = 0.012). More number of patients in POEM and PD groups had undergone prior treatments compared to LHM group(72.2% vs 68.2% vs 44.3% respectively, P = 0.003). At 2 mo post-treatment, all TBE parameters including barium column height, width and volume remaining at 1 and 5 min improved significantly in all three treatment groups(P = 0.01 to P < 0.001) except the column height at 1 min in PD group(P = 0.11). At 2 mo post-treatment, there was significant improvement in basal LES pressure and LES-IRP in both LHM(40.5 mm Hg vs 14.5 mm Hg and 24 mm Hg vs 7.1 mm Hg respectively, P < 0.001) and POEM groups(38.7 mm Hg vs 11.4 mm Hg and 23.6 mm Hg vs 6.6 mm Hg respectively, P < 0.001). However, when the efficacy of three treatments were compared to each other in terms of improvement in TBE or HREM parameters at 2 mo, there was no significant difference(P > 0.05).CONCLUSION: POEM, PD and LHM were all effective in improving esophageal function in achalasia at shortterm. 15Air quality monitoring using mobile microscopy and machine learning显示文摘Rapid,accurate and high-throughput sizing and quantification of particulate matter(PM)in air is crucial for monitoring and improving air quality.In fact,particles in air with a diameter of≤2.5μm have been classified as carcinogenic by the World Health Organization.Here we present a field-portable cost-effective platform for high-throughput quantification of particulate matter using computational lens-free microscopy and machine-learning.This platform,termed c-Air,is also integrated with a smartphone application for device control and display of results.This mobile device rapidly screens 6.5 L of air in 30 s and generates microscopic images of the aerosols in air.It provides statistics of the particle size and density distribution with a sizing accuracy of~93%.We tested this mobile platform by measuring the air quality at different indoor and outdoor environments and measurement times,and compared our results to those of an Environmental Protection Agency–approved device based on beta-attenuation monitoring,which showed strong correlation to c-Air measurements.Furthermore,we used c-Air to map the air quality around Los Angeles International Airport(LAX)over 24 h to confirm that the impact of LAX on increased PM concentration was present even at 47 km away from the airport,especially along the direction of landing flights.With its machinelearning-based computational microscopy interface,c-Air can be adaptively tailored to detect specific particles in air,for example,various types of pollen and mold and provide a cost-effective mobile solution for highly accurate and distributed sensing of air quality.Yi-Chen Wu Ashutosh Shiledar Yi-Cheng Li Jeffrey Wong Steve Feng Xuan Chen Christine Chen Kevin Jin Saba Janamian Zhe Yang Zachary Scott Ballard Zoltán Göröcs Alborz Feizi Aydogan Ozcan 2017Light(Science & Applications)2017,6,1:5

18A new Early Cretaceous dinosaur track assemblage and the first definite non-avian theropod swim trackway from China显示文摘The trackway of a swimming theropod (ichnogenus Characichnos) is reported from the Lower Cretaceous Feitianshan Formation of Sichuan, China. These swim tracks help confirm that non-avian theropods were capable of forging moderately deep bodies of water. The trackway occurs on the same surface as a typical walking trackway of a sauropod (ichnogenus Brontopodus). Both occurrences are the first reported from the Cretaceous of Sichuan, and the swim tracks are the first well-preserved example of a Characichnos trackway from China. Additionally, a theropod walking trackway and several ornithopod walking trackways (similar to the ichnogenus Caririchnium) occur in the same horizon. The ornithopod trackways show a parallel orientation, suggesting gregarious behavior of the trackmakers, which may have been iguanodontiforms and/or hadrosauriforms.
