| 4 | Pooled genetic analysis in ultrasound measured non-alcoholic fatty liver disease in Indian subjects:A pilot study显示文摘AIM: To investigate genetic susceptibility in Indian subjects with non-alcoholic fatty liver disease(NAFLD) by performing a pooled genetic study.METHODS: Study subjects(n = 306) were recruited and categorized into NAFLD and control groups based on ultrasound findings of fatty infiltration. Of the 306 individuals, 156 individuals had fatty infiltration and thus comprised the NAFLD group. One hundred and fifty(n = 150) individuals were normal, without fatty infiltration of the liver, comprising the control group. Blood samples, demographic and anthropometric data from the individuals were collected after obtaining informed consent. Anthropometric data, blood glucose, lipids and liver function tests were estimated using standard methods. Genome wide association stud-ies done to date on NAFLD were identified, 19 single nucleotide polymorphisms(SNPs) were selected from these studies that were reported to be significantly associated with NAFLD and genotyping was performed on the Sequenom platform. Student's t test for continuous variables and χ2 test was applied to variant carriers from both groups. Required corrections were applied as multiple testing was done.RESULTS The mean age of the control group was 39.78 ± 10.83 and the NAFLD group was 36.63 ± 8.20 years. The waist circumference of males and females in the control and NAFLD groups were 80.13 ± 10.35; 81.77 ± 13.65 and 94.09 ± 10.53; 92.53 ± 8.27 respectively. The mean triglyceride and alanine transaminase(ALT) levels in the control and NAFLD groups were 135.18 ± 7.77; 25.39 ± 14.73 and 184.40 ± 84.31; 110.20 ± 67.05 respectively. When χ2 test was applied to the number of individuals carrying the variant risk alleles between the control and NAFLD group, a significant association was seen between rs738409 of the patatin-like phospholipase domain containing 3(PNPLA3) gene(P = 0.001), rs2073080 of the PARVB gene(P = 0.02), rs2143571 of SAMM50 gene(P = 0.05) and rs6487679 of the pregnancy zone protein(PZP) gene(P = 0.01) with the disease. Variant single nucleotide polymorphisms(SNPs) in NCAN and PNPLA3 gene were associated with higher levels of ALT, whereas variant SNPs in APOC3, PNPLA3, EFCAB4 B and COL13A1 were associated with high triglyceride levels. Apart from the above associations, rs2073080, rs343062 and rs6591182 were significantly associated with high BMI; rs2854117 and rs738409 with high triglyceride levels; and rs2073080, rs2143571, rs2228603, rs6487679 and rs738409 with high ALT levels.CONCLUSION: Pooled genetic analysis revealed an association of SNPs in PNPLA3, PARVB, SAMM50 and PZP genes with NAFLD. | 5 | Liver transplantation for metastatic neuroendocrine tumor: A case report and review of the literature显示文摘Neuroendocrine tumors are divided into gastrointestinal carcinoids and pancreatic neuroendocrine tumors. The WHO has updated the classification of these lesions and has abandoned the term 'carcinoid'. Both types of tumors are divided into functional and non-functional tumors. They are characterized by slow growth and frequent metastasis to the liver and may be limited to the liver for long periods. The therapeutic approach to hepatic metastases should consider the number and distribution of the liver metastases as well as the severity of symptoms related to hormone production and tumor bulk. Surgery is generally considered as the first line therapy. In patients with unresectable liver metastases,alternative treatments are dependent on the type and the growth rate. Initial treatments consist of long acting somatostatin analogs and/or interferon. Streptozocinbased chemotherapy is usually reserved for symptomatic patients with rapidly advancing disease, but generally the therapy is poorly tolerated and its effects are short-lived.Locoregional therapy directed such as hepatic-artery embolization and chemoembolization, radiofrequency thermal ablation and cryosurgery, is often used instead of systemic therapy, if the disease is limited to the liver.However, liver transplantation should be considered in patients with neuroendocrine metastases to the liver that are not accessible to curative or cytoreductive surgery and if medical or locoregional treatment has failed and if there are life threatening hormonal symptoms. | 10 | Budd-Chiari syndrome:A single-center experience显示文摘AIM:To investigate challenges,risk factors,prognostic indicators,and treatment outcomes associated with Budd-Chiari syndrome(BCS)at a tertiary care center.METHODS:A retrospective cohort study was conducted at the University of Pennsylvania in patients with a diagnosis of BCS or hepatic vein thrombosis.All patients receiving care at the University of Pennsylvania,and who had at least 2 clinical encounters in the University of Pennsylvania Health system from January1,2008 to September 10,2013 were eligible for study inclusion.Data were extracted from the electronic medical record of each patient,and recorded in a secure Research Electronic Data Capture database.Logistic regression analyses were applied to identify predictors of outcome of liver transplant(LT)or death.RESULTS:Between January 1,2008 and September10,2013,forty-seven patients were identified.Median age was 42.4 years.Thirty-one(66.0%)were women.A majority were Caucasian(68.1%).At diagnosis,43(91.5%)patients had ascites,27(57.4%)patients had a hematologic disorder associated with a hypercoagulable state and 26(55.3%)had cirrhosis.Forty(85.1%)patients were on anticoagulation(AC),30(63.8%)of whom were maintained on warfarin.Two patients(4.3%)underwent thrombolytic therapy.A transjugular intrahepatic portosystemic shunt(TIPS)was placed in21(44.7%)patients,19(90.5%)of whom were also on AC.Twenty-one(44.7%)received AC alone.Over a median of 974 d,8(17.0%)patients received LT,and10(21.3%)died.The median time from listing to death was 26 mo[interquartile range(IQR)=16,65)].TIPS with AC was utilized more frequently in younger patients(P=0.02).Age,cirrhosis and chronic kidney disease(CKD)were significant predictors of LT or death.CONCLUSION:AC alone was employed as frequently as TIPS with AC,though the latter was used more frequently in younger patients with polycythemia vera.There were no significant differences in treatment outcome regardless of the therapeutic intervention employed.Significant predictors of poor prognosis included age,cirrhosis and CKD. | Tanya M Pavri Alan Herbst Rajender Reddy Kimberly A Forde | 2014 | World Journal of Gastroenterology2014,20,43: | 2 |

| 12 | Hepatitis C virus therapy with peg-interferon and ribavirin in Myanmar: A resource-constrained country显示文摘AIM To investigate peg-interferon(peg-IFN) and ribavirin(RBV) therapy in Myanmar and to predict sustained virologic response(SVR).METHODS This single-center, open-label, study was conducted in Myanmar between 2009 and 2014. A total of 288 patients infected with HCV genotypes 1, 2, 3 and 6 were treated with peg-IFN alpha-2a(180 μg/wk) or alpha-2b(50 to 100 μg as a weight-based dose) and RBV as a weight-based dose(15 mg/kg/d). Treatment duration was 48 wk for genotypes 1 and 6, 24 wk for genotype 2, and 24 or 48 wk for genotype 3 based on rapid virologic response(RVR). Those co-infected with hepatitis B received 48 wk of therapy.RESULTS Overall, SVR was achieved for 82% of patients and the therapy was well tolerated. All patients achieved SVR at equivalent rates regardless of HCV genotype(P = 0.314). Low fibrosis scores(P < 0.001), high baseline albumin levels(P = 0.028) and low baseline viral loads(P = 0.029) all independently predicted SVR. On the other hand, IL-28 B TT and CC genotypes were not found to significantly predict SVR(P = 0.634; P = 0.618). Among those who completed treatment, the occurrence of RVR showed a > 96% positive predictive value for achieving SVR. Treatment duration did not significantly impact the likelihood of achieving SVR for patients infected with genotype 3 HCV(P = 0.371). The most common adverse events were fatigue(71%) and poor appetite(60%). Among patients with genotype 3 HCV, more patients in the 48-wk treatment group required erythropoietin than in the 24-wk treatment group(61.1% vs 49.2%).CONCLUSION SVR rates were high with peg-IFN and RBV therapy in Myanmar. | 18 | 过渡金属电催化剂在硅基光阳极分解水产氧中的作用显示文摘光电化学分解水可将太阳能转换为绿色的氢能,为目前的能源危机和环境问题提供了一种理想的解决方案.在分解水反应中,涉及四空穴过程的产氧半反应是制约性能的关键步骤,往往需要在半导体表面沉积电催化剂以加速产氧反应动力学.因此,全面理解电催化剂在光电化学分解水体系中的作用至关重要.在目前的产氧电催化剂中,过渡金属羟基氧化物电催化剂(MOOH,M=Fe,Co,Ni)因其环保、廉价、高效以及稳定的特性,已被广泛用于半导体光阳极分解水器件中.而且,MOOH可用简单的电沉积方法沉积在光电极表面,易于大面积制备.然而,电沉积法制备的MOOH具有复杂的结构,对其作用机制的全面理解更加困难.因此,本文以电沉积MOOH修饰的硅基光阳极(n^(+)p-Si/SiO_(x)/Fe/FeOx/MOOH)作为模型,研究了不同电催化剂对硅光阳极光电化学产氧性能的影响.实验发现电催化剂的界面优化在电催化剂修饰的光电极中发挥着重要作用,这是因为优化的界面可以提升界面电荷传输,提供更多的催化反应活性位点以及更高的本征催化活性,从而更有利于光解水性能的提升.该项研究揭示了电催化剂在光解水器件中的作用,并为今后高效光解水器件的设计提供了一定指导.首先在多晶n^(+)p-Si基底上热蒸镀了一层30 nm的金属Fe膜,并通过电化学活化将Fe膜表面转换为FeOx得到Fe/FeOx(记作aFe)界面层,然后利用电沉积方法制备MOOH表面修饰层,最终得到n^(+)p-Si/SiO_(x)/aFe:MOOH光阳极.X射线光电子能谱、拉曼光谱以及扫描电子显微镜表面元素成像的表征结果均证实电极表面由于界面层金属Fe元素的掺杂而形成了Fe1-xNixOOH.在模拟太阳光下用于光解水产氧时,n^(+)p-Si/SiO_(x)/aFe:NiOOH电极的起始电位为~1.01 VRHE(相对于可逆氢电极的电势),在1.23 VRHE下的光电流为38.82 mA cm^(-2),显著优于n^(+)p-Si/SiO_(x)/aFe、n^(+)p-Si/SiO_(x)/aFe:FeOOH以及n^(+)p-Si/SiO_(x)/aFe:CoOOH三个对比样品,且其稳定性达到75 h.另外,我们发现n^(+)p-Si/SiO_(x)/aFe:MOOH电极的光电化学产氧性能均显著高于n^(+)p-Si/SiO_(x)/aFe电极,且p++-Si/SiO_(x)/aFe:MOOH的电催化产氧性能也高于p++-Si/SiO_(x)/MOOH,不仅证明了aFe界面层对Si与MOOH层之间的界面接触作用的有效调控,而且表明双电催化剂体系(aFe:MOOH)的电催化产氧活性高于单电催化剂(MOOH).热力学分析表明,n^(+)p-Si/SiO_(x)/aFe:MOOH光阳极的光电压大小与其光解水产氧性能并不一致,从而排除了热力学因素对性能的关键影响.进一步从塔菲尔斜率、电化学活性表面积和电化学阻抗谱对各电极的动力学进行了分析,证明了动力学因素在上述光阳极产氧性能中的主导作用.同时发现,由于aFe:NiOOH双电催化剂具有更高的本征电催化产氧性能,提供了更多的表面活性位点以及更有效地促进了光生载流子的传输,对动力学的提升效果更显著,从而使n^(+)p-Si/SiO_(x)/aFe:NiOOH光阳极表现出最高的光解水产氧性能. | Rajender Boddula 谢关才 郭北斗 宫建茹 | 2021 | Chinese Journal of Catalysis2021,42,8: | 2 |
