| 1 | COX-2 polymorphisms-765G→C and-1195A→G and colorectal cancer risk显示文摘AIM:To determine the possible modulating effect of the COX-2 polymorphisms,-765G→C and-1195A→G, on the risk of colorectal cancer(CRC)in a Dutch population. METHODS:This case-control study includes 326 patients with CRC and 369 age-and gender-matched controls.Genotypes of the COX-2 polymorphisms -765G→C and-1195A→G were determined by polymerase chain reaction-based restriction fragment length polymorphism.COX-2 genotypes and haplotypes were analyzed and odds ratios with 95%confi- dence intervals were estimated by logistic regression. RESULTS:The-765GG genotype was associated with an increased risk of developing CRC(OR,1.45; 95%CI,1.03-2.04).No significant difference was observed in the genotype distribution of the-1195A→ G polymorphism between patients and controls.The GG/AC haplotype was present significantly less often in patients than in controls(OR 0.44;95%CI,0.22-0.85). When the AC,AG and GG haplotypes were investigated separately,the AC haplotype showed a tendency to be less frequent in patients than in controls(OR(AG/AC)0.78; 95%CI,0.57-1.06). | 2 | Liver resections can be performed safely without Pringle maneuver: A prospective study显示文摘AIM To evaluate liver resections without Pringle maneuver, i.e., clamping of the portal triad. METHODS Between 9/2002 and 7/2013, 175 consecutive liver resections(n = 101 major anatomical and n = 74 large atypical > 5 cm) without Pringle maneuver were performed in 127 patients(143 surgeries). Accompanying, 37 wedge resections(specimens < 5 cm) and 43 radiofrequency ablations were performed. Preoperative volumetric calculation of the liver remnant preceeded all anatomical resections. The liver parenchyma was dissected by waterjet. The median central venous pressure was 4 mmH g(range: 5-14). Data was collected prospectively. RESULTS The median age of patients was 60 years(range: 16-85). Preoperative chemotherapy was used in 70 cases(49.0%). Liver cirrhosis was present in 6.3%, and liver steatosis of ≥ 10% in 28.0%. Blood loss was median 400 mL(range 50-5000 mL). Perioperative blood transfusions were given in 22/143 procedures(15%). The median weight of anatomically resected liver specimens was 525 g(range: 51-1850 g). One patient died postoperatively. Biliary leakages(n = 5) were treated conservatively. Temporary liver failure occurred in two patients.CONCLUSION Major liver resections without Pringle maneuver are feasible and safe. | 3 | Colonoscopic yield of colorectal neoplasia in daily clinical practice显示文摘AIM:To assess the prevalence and location of ad-vanced neoplasia in patients undergoing colonoscopy,and to compare the yield per indication.METHODS:In a multicenter colonoscopy survey (n = 18 hospitals) in the Amsterdam area (Northern Holland),data of all colonoscopies performed during a three month period in 2005 were analyzed. The location and the histological features of all colonic neoplasia were recorded. The prevalence and the distribution ofadvanced colorectal neoplasia and differences in yield between indication clusters were evaluated. Advanced neoplasm was defi ned as adenoma > 10 mm in size,with > 25% villous features or with high-grade dyspla-sia or cancer.RESULTS:A total of 4623 eligible patients underwent a total colonoscopy. The prevalence of advanced neo-plasia was 13%,with 281 (6%) adenocarcinomas and 342 (7%) advanced adenomas. Sixty-seven percent and 33% of advanced neoplasia were located in the distal and proximal colon,respectively. Of all patients with right-sided advanced neoplasia (n = 228),51% had a normal distal colon,whereas 27% had a syn-chronous distal adenoma. Ten percent of all colono-scopies were performed in asymptomatic patients,7% of whom had advanced neoplasia. In the respective procedure indication clusters,the prevalence of right-sided advanced neoplasia ranged from 11%-57%. CONCLUSION:One out of every 7-8 colonoscopies yielded an advanced colorectal neoplasm.
