4Seasonal variations of acute appendicitis and nonspecific abdominal pain in Finland显示文摘AIM:To investigate whether seasonal changes had an effect on the incidence of acute appendicitis(AA)or nonspecific abdominal pain(NSAP).METHODS:We carried out a national register study of all patients with a hospital discharge diagnosis of AA and acute NSAP in Finland.Data were analyzed for the whole country and correlated to seasonal and weather parameters(temperature,humidity).Moreover,additional sub-analyses were performed for five geographically different area of Finland.RESULTS:The observation period spanned 21 years,with 186558 appendectomies,of which 137528(74%)cases were reported as AA.The incidence of AA declined for 32%over the study period.The average incidence of the NSAP was 34/10000 per year.The mean annual temperature,but not relative humidity,showed clear geographical variations.The incidence of AA decreased significantly during the cold months of the year.No correlation was detected between temperature and incidence of NSAP.Humidity had a statistically significant impact on NSAP.CONCLUSION:The incidence of acute appendicitis is declining in Finland.We detected a clear seasonality in the incidence of AA and NSAP.Imre Ilves Anne Fagerstrm Karl-Heinz Herzig Petri Juvonen Pekka Miettinen Hannu Paajanen 2014World Journal of Gastroenterology2014,20,14:6

6Cellulose structure and lignin distribution in normal and compression wood of the Maidenhair tree(Ginkgo biloba L.)显示文摘We studied in detail the mean micro fibril angle and the width of cellulose crystals from the pith to the bark of a 15-year-old Maidenhair tree(Ginkgo biloba L.). The orientation of cellulose micro fibrils with respect to the cell axis and the width and length of cellulose crystallites were determined using Xray diffraction. Raman microscopy was used to compare the lignin distribution in the cell wall of normal/opposite and compression wood, which was found near the pith. Ginkgo biloba showed a relatively large mean micro fibril angle,varying between 19° and 39° in the S2 layer, and the average width of cellulose crystallites was 3.1–3.2 nm. Mild compression wood without any intercellular spaces or helical cavities was observed near the pith. Slit-like bordered pit openings and a heavily lignified S2 L layer con firmed the presence of compression wood. Ginkgo biloba showed typical features present in the juvenile wood of conifers. The micro fibril angle remained large over the 14 annual rings. The entire stem disc,with a diameter of 18 cm, was considered to consist of juvenile wood. 11基于人工智能构建急腹症快速分诊系统显示文摘急腹症是一组以急性腹痛为起病表现的急诊疾病,既包含急性单纯性阑尾炎等简单疾病,也包括消化道穿孔、重症急性胰腺炎等可能危及生命的复杂疾病。在有限的时间内,依据患者对症状的描述快速对急腹症进行准确分诊,识别和筛选危重患者,并进行优先处理,是一待解决的重要问题。计算机技术的发展使得利用人工智能(artificial intelligence,AI)技术建立急腹症快速分诊系统成为了可能。急腹症智能分诊系统的研究主要包括以下几个方面:(1)根据医学教材以及病例诊断的文本信息建立知识图谱;(2)根据患者主诉,实现医学术语到知识图谱中概念的归一化;(3)利用深度学习的方法训练模型,使计算机判别不同的急腹症;(4)在医院以及急救中心等场所部署训练后的AI模型进行测试并基于临床使用效果和反馈进行优化,使得系统对急腹症诊断的准确率达到可接受的水平(根据一般标准为:F≥94%)。在开发过程中,自然语言处理是急腹症人工智能分诊系统建立所必需的核心技术。本文综述了近年来自然语言处理领域中的最新技术进展,并讨论了将其应用于构建急腹症快速智能辅助诊断系统的前景。此系统的开发可以作为范式推广到其他疾病,优化患者的就诊流程,提高急诊效率。张薇 孙明伟 曾俊 陈伟 彭谨 Pekka Kuosmanen 祝迪 江华 2019实用医院临床杂志2019,16,1:3
12结构性植骨修复膝关节翻修术中严重骨缺损的临床效果及相关分析(英文)显示文摘目的探讨同种异体结构性植骨在膝关节翻修术中大块骨缺损中应用的临床效果和意义。方法1994~2001年芬兰坦佩雷大学医院应用单一翻修假体及同种异体骨结构性植骨治疗膝关节置换术大块骨缺损患者10例(膝),男1例,女9例,平均年龄70岁(61—77岁),平均随访5年(1~8年),所有手术均由两名高年资专科医师执行,采取KSS评分评估术前术后膝关节功能。结果最后随访时,患者膝关节KSS评分由术前的平均39分(4~51分)提高至81分(28—102分;P〈0.05);疼痛评分由术前的18分(0—30分)提高至42分(10—50分;P〈0.05)。2例出现假体周围透亮线(〈1mm),但没有任何松动症状;所有结构性植骨均获得满意的愈合,最后随访时没有出现吸收征象,1例患者术后出现膝前疼痛,经髌骨表面置换后症状消失。结论同种异体骨结构性植骨应用在膝关节翻修术中大块骨缺损中可取得满意的临床效果,重建下肢力线、第三代骨水泥技术的应用及有由专科医师实施手术是获得良好临床效果的保证。盛璞义 Esa Jamsen Matti Lehto Yrj T Konttinen Jorma Pajamki Pekka Halonen 2009中华关节外科杂志(电子版)2009,3,4:3

20Effect of colouring green stage zirconia on the adhesion of veneering ceramics with different thermal expansion coefficients显示文摘This study evaluated the adhesion of zirconia core ceramics with their corresponding veneering ceramics, having different thermal expansion coefficients(TECs), when zirconia ceramics were coloured at green stage. Zirconia blocks(N5240; 6 mm37 mm37 mm) were manufactured from two materials namely, ICE Zirconia(Group 1) and Prettau Zirconia(Group 2). In their green stage, they were randomly divided into two groups. Half of the specimens were coloured with colouring liquid(shade A2). Three different veneering ceramics with different TEC(ICE Ceramic, GC Initial Zr and IPS e.max Ceram) were fired on both coloured and non-coloured zirconia cores. Specimens of high noble alloys(Esteticor Plus) veneered with ceramic(VM 13)(n516) acted as the control group. Core–veneer interface of the specimens were subjected to shear force in the Universal Testing Machine(0.5 mm?min21). Neither the zirconia core material(P50.318) nor colouring(P50.188) significantly affected the results(three-way analysis of variance, Tukey's test). But the results were significantly affected by the veneering ceramic(P50.000). Control group exhibited significantly higher mean bond strength values(45.768) MPa than all other tested groups((27.164.1)2(39.764.7) and(27.465.6)2(35.964.7) MPa with and without colouring, respectively)(P,0.001). While in zirconia–veneer test groups, predominantly mixed type of failures were observed with the veneering ceramic covering,1/3 of the substrate surface, in the metal–ceramic group, veneering ceramic was left adhered.1/3 of the metal surface. Colouring zirconia did not impair adhesion of veneering ceramic, but veneering ceramic had a significant influence on the core–veneer adhesion.
