| 1 | Diagnosing, planning and evaluating osteochondral ankle defects with imaging modalities显示文摘This current concepts review outlines the role of diffe-rent imaging modalities in the diagnosis, preoperative planning, and follow-up of osteochondral ankle defects. An osteochondral ankle defect involves the articular cartilage and subchondral bone(usually of the talus) and is mostly caused by an ankle supination trauma. Conventional radiographs are useful as an initial imaging tool in the diagnostic process, but have only moderate sensitivity for the detection of osteochondral defects. Computed tomography(CT) and magnetic resonance imaging(MRI) are more accurate imaging modalities. Recently, ultrasonography and single photon emission CT have been described for the evaluation of osteochondral talar defects. CT is the most valuable modality for assessing the exact location and size of bony lesions. Cartilage and subchondral bone damage can be visualized using MRI, but the defect size tends to be overestimated due to bone edema. CT with the ankle in full plantar flexion has been shown a reliable tool for preoperative planning of the surgical approach. Postoperative imaging is useful for objective assessment of repair tissue or degenerative changes of the ankle joint. | 3 | Chemoattractants and cytokines in primary ciliary dyskinesia and cystic fibrosis: key players in chronic respiratory diseases显示文摘Patients with primary ciliary dyskinesia(PCD)and cystic fibrosis(CF),two inherited disorders,suffer from recurrent airway infections characterized by persistent bacterial colonization and uncontrollable inflammation.Although present in high counts,neutrophils fail to clear infection in the airways.High levels of C-X-C motif chemokine ligand 8/interleukin-8(CXCL8/IL-8),the most potent chemokine to attract neutrophils to sites of infection,are detected in the sputum of both patient groups and might cause the high neutrophil influx in the airways.Furthermore,in CF,airway neutrophils are highly activated because of the genetic defect and the high levels of proinflammatory chemoattractants and cytokines(e.g.,CXCL8/IL-8,tumor necrosis factor-αand IL-17).The overactive state of neutrophils leads to lung damage and fuels the vicious circle of infection,excessive inflammation and tissue damage.The inflammatory process in CF airways is well characterized,whereas the lung pathology in PCD is far less studied.The knowledge of CF lung pathology could be useful to guide molecular investigations of the inflammatory processes in PCD lungs.Current available therapies can not completely remedy the chronic airway infections in these diseases.This review gives an overview of the role that chemoattractants and cytokines play in these neutrophil-dominated lung pathologies.Finally,the most frequently applied treatments in CF and PCD and new experimental therapies to reduce neutrophil-dominated airway inflammation are described. | Maaike Cockx Mieke Gouwy Jo Van Damme Sofie Struyf | 2018 | Cellular & Molecular Immunology2018，15，4： | 3 |

| 20 | Calcineurin Inhibitors in the Treatment of Adult Autoimmune Hepatitis:A Systematic Review显示文摘Background and Aims:A considerable number of au-toimmune hepatitis(AIH)patients completely or partially fail on first-line treatment.Several studies on the use of calcineurin inhibitors(CNIs)in the treatment of AIH have been published without focusing on indication.The aim was to assess the efficacy of CNIs in the treatment of adult AIH patients,specifically focusing on indication:first-line intolerant and with first-line insufficient response(failure to achieve or maintain remission),and with second versus third-line treatment.Methods:A literature search included studies on the use of CNIs in adult AIH.Patients with past or present use of CNIs from the Dutch AIH group cohort were added.The primary endpoint was biochemical remission while using CNIs.Secondary endpoints were biochemical response,treatment failure,and adverse effects.Results:Twenty studies from the literature and nine Dutch patients were included describing the use of cyclosporine in 59 and tacrolimus in 219 adult AIH patients.The CNI remission rate was 53%in patients with insufficient response to first-line treatment and 67%in patients intolerant to first-line treat-ment.CNIs were used as second-line treatment in 73%with a remission rate of 52%and as third-line treatment in 22%with a remission rate of 26%.Cyclosporine was discontin-ued in 13%and tacrolimus in 11%of patients because of adverse events.Conclusions:CNIs as rescue treatment in adult AIH patients are reasonably effective and safe both with insufficient response or intolerance to previous treat-ment.Prospective studies are needed. | Martine AMC Baven-Pronk Joffre M.Hew,Jr Maaike Biewenga Maarten E.Tushuizen Aad P.van den Berg Gerd Bouma Johannes T.Brouwer Bart van Hoek 无 | 2022 | Journal of Clinical and Translational Hepatology2022，10，6： | 1 |
