1Nonalcoholic fatty liver disease and aging: Epidemiology to management显示文摘Nonalcoholic fatty liver disease(NAFLD) is common in the elderly, in whom it carries a more substantial burden of hepatic(nonalcoholic steatohepatitis, cirrhosis and hepatocellular carcinoma) and extra-hepatic manifestations and complications(cardiovascular disease, extrahepatic neoplasms) than in younger age groups. Therefore, proper identification and management of this condition is a major task for clinical geriatricians and geriatric hepatologists. In this paper, the epidemiology and pathophysiology of this condition are reviewed, and a full discussion of the link between NAFLD and the aspects that are peculiar to elderly individuals is provided; these aspects include frailty, multimorbidity, polypharmacy and dementia. The proper treatment strategy will have to consider the peculiarities of geriatric patients, so a multidisciplinary approach is mandatory. Non-pharmacological treatment(diet and physical exercise) has to be tailored individually considering the physical limitations of most elderly people and the need for an adequate caloric supply. Similarly, the choice of drug treatment must carefully balance the benefits and risks in terms of adverse events and pharmacological interactions in the common context of both multiple health conditions and polypharmacy. In conclusion, further epidemiological and pathophysiological insight is warranted. 2Intraocular inflammation after Ultrasound Cyclo Plasty for the treatment of glaucoma显示文摘This is a prospective interventional clinical study evaluating intraocular inflammation developed after Ultrasound Cyclo Plasty(UCP) for the treatment of glaucoma. Eighteen eyes of 18 patients were treated with UCP second-generation probes(Eye OP1). After treatment, the mean intraocular pressure(IOP) significantly decreased from 26.8±7.2 to 18.8±6.1 mm Hg at day 1 and to 14.7±3.4 mm Hg at month 6(all P<0.001). Mean laser flare-cell photometry value steeply increased after surgery from 12.1±7.5 to 64.1±53.9 ph/ms(P=0.001) at day 1, and then progressively decreased to respectively 60.6±49.7 at day 7, 43.5±38.5 at day 14 and 28.2±18.3 at month 1(all P<0.05), returning at levels similar to baseline ones at month 3 and month 6(respectively 16.7±6.2 and 12.8±10.2, both P>0.05). A significant negative correlation was found between postoperative increase of aqueous flare values and anterior chamber depth(R=-0.568, P=0.014). 3Intestinal epithelial barrier and neuromuscular compartment in health and disease显示文摘A number of digestive and extra-digestive disorders,including inflammatory bowel diseases,irritable bowel syndrome,intestinal infections,metabolic syndrome and neuropsychiatric disorders,share a set of clinical features at gastrointestinal level,such as infrequent bowel movements,abdominal distension,constipation and secretory dysfunctions.Several lines of evidence indicate that morphological and molecular changes in intestinal epithelial barrier and enteric neuromuscular compartment contribute to alterations of both bowel motor and secretory functions in digestive and extra-digestive diseases.The present review has been conceived to provide a comprehensive and critical overview of the available knowledge on the morphological and molecular changes occurring in intestinal epithelial barrier and enteric neuromuscular compartment in both digestive and extra-digestive diseases.In addition,our intent was to highlight whether these morphological and molecular alterations could represent a common path(or share some common features)driving the pathophysiology of bowel motor dysfunctions and related symptoms associated with digestive and extra-digestive disorders.This assessment might help to identify novel targets of potential usefulness to develop original pharmacological approaches for the therapeutic management of such disturbances.Vanessa D’Antongiovanni Carolina Pellegrini Matteo Fornai Rocchina Colucci Corrado Blandizzi Luca Antonioli Nunzia Bernardini 2020World Journal of Gastroenterology2020,26,14:6

5Pacing and predictors of performance during cross-country skiing races:A systematic review显示文摘Background: Cross-country skiing(XCS) racing, a popular international winter sport, is complex and challenging from physical, technical, and tactical perspectives. Despite the vast amount of research focusing on this sport, no review has yet addressed the pacing strategies of elite XCS racers or the factors that influence their performance. The aim was to review the scientific literature in an attempt to determine the effects of pacing strategy on the performance of elite XCS racers.Methods: Four electronic databases were searched using relevant subject headings and keywords. Only original research articles published in peerreviewed journals and the English language and addressing performance, biomechanics, physiology, and anthropometry of XCS racers were reviewed.Results: All 27 included articles applied correlative designs to study the effectiveness of different pacing strategies. None of the articles involved the use of an experimental design. Furthermore, potential changes in external conditions(e.g., weather, ski properties) were not taken into consideration. A comparable number of studies focused on the skating or classical technique. In most cases, positive pacing was observed, with certain indications that higher-level athletes and those with more endurance and strength utilized a more even pacing strategy. The ability to achieve and maintain a long cycle length on all types of terrain was an important determinant of performance in all of the included studies, which was not the case for cycle rate. In general, uphill performance was closely related to overall race performance, with uphill performance being most closely correlated to the success of female skiers and performance on flat terrain being more important for male skiers. Moreover, pacing was coupled to the selection and distribution of technique during a race, with faster skiers employing more double poling and kick double poling,less diagonal stride, and more V2(double dance) than V1(single dance) skating across a race.Conclusion: We propose that skiers at all levels can improve their performance with more specific training in techniques(i.e., maintaining long cycles without compromising cycle rate and selecting appropriate techniques) in combination with training for endurance and more strength. 6Significant factors associated with fatal outcome in emergency open surgery for perforated peptic ulcer显示文摘AIM: To evaluate the main factors associated with mortality in patients undergoing surgery for perforated peptic ulcer referred to an academic department of general surgery in a large southern Italian city.METHODS: One hundred and forty-nine consecutive patients (M:F ratio=110:39, mean age 52 yrs, range 16-95) with peptic ulcer disease were investigated for clinical history (including age, sex, previous history of peptic ulcer, associated diseases, delayed abdominal surgery, ulcer site, operation type, shock on admission, postoperative general complications,and intra-abdominal and/or wound infections), serum analyses and radiological findings.RESULTS: The overall mortality rate was 4.0%. 7Constipation,deficit in colon contractions and alpha-synuclein inclusions within the colon precede motor abnormalities and neurodegeneration in the central nervous system in a mouse model of alphasynucleinopathy显示文摘Background:Gastrointestinal dysfunction can affect Parkinson’s disease(PD)patients long before the onset of motor symptoms.However,little is known about the relationship between gastrointestinal abnormalities and the development of PD.Contrary to other animal models,the human A53T alpha-synuclein(αS)transgenic mice,Line G2–3,developsαS-driven neurological and motor impairments after 9 months of age,displaying a long presymptomatic phase free of central nervous system(CNS)dysfunction.Methods:To determine whether this line can be suitable to study constipation as it occurs in prodromal PD,gastrointestinal functionality was assessed in young mice through a multidisciplinary approach,based on behavioral and biochemical analysis combined with electrophysiological recordings of mouse intestinal preparations.Results:We found that the A53TαS mice display remarkable signs of gastrointestinal dysfunction that precede motor abnormalities andαS pathology in the CNS by at least 6 months.YoungαS mice show a drastic delay in food transit along the gastrointestinal tract,of almost 2 h in 3 months old mice that increased to more than 3 h at 6 months.Such impairment was associated with abnormal formation of stools that resulted in less abundant but longer pellets excreted,suggesting a deficit in the intestinal peristalsis.In agreement with this,electrically evoked contractions of the colon,but not of the ileum,showed a reduced motor response in both longitudinal and circular muscle layers inαS mice already at 3 months of age,that was mainly due to an impaired cholinergic transmission of the underlying enteric nervous system.Interestingly,the presence of insoluble and aggregatedαS was found in enteric neurons in both myenteric and submucosal plexi only in the colon of 3 months oldαS mice,but not in the small intestine,and exacerbated with age,mimicking the increase in transit delay and the contraction deficit showed by behavioral and electrical recordings data.Conclusions:Gastrointestinal dysfunction in A53TαS mice represents an early sign ofαS-driven pathology without concomitant CNS involvement.We believe that this model can be very useful to study disease-modifying strategies that could extend the prodromal phase of PD and haltαS pathology from reaching the brain.Lucia Rota Carolina Pellegrini Laura Benvenuti Luca Antonioli Matteo Fornai Corrado Blandizzi Antonino Cattaneo Emanuela Colla 2019Translational Neurodegeneration2019,8,1:5
