1Comparison of double balloon enteroscopy in adults and children显示文摘AIM: To compare results of double balloon enteroscopy (DBE) procedures in pediatric and adult patients. METHODS: The medical files of patients who underwent DBE at Gazi University School of Medicine, Ankara, Turkey between 2009 and 2011 were examined retrospectively. Adult and pediatric patients were compared according to DBE indications, procedure duration, final diagnosis, and complications. DBE procedures were performed in an operating room under general anesthesia by two endoscopists. An oral or anal approach was preferred according to estimated lesion sites. Overnight fasting of at least 6 h prior to the start of the procedure was adequate for preprocedural preparation of oral DBE procedures. Bowel cleansing was performed by oral administration of sennosides A and B solution, 2 mL/kg, and anal saline laxative en-ema. The patients were followed up for 2 h after the procedure in terms of possible complications. RESULTS: DBE was performed in 35 patients (5 pediatric and 30 adult). DBE procedures were performed for abdominal pain, chronic diarrhea, bleeding, chronic vomiting, anemia, and postoperative evaluation of anastomosis. Final diagnosis was diffuse gastric angiodysplasia (n = 1); diffuse jejunal angiodysplasia (n = 1); ulceration in the bulbus (n = 1); celiac disease (n = 1); low differentiated metastatic carcinoma (n = 1); PeutzJeghers syndrome (n = 1); adenomatous polyp (n = 1) and stricture formation in anastomosis line (n = 1). During postprocedural follow-up, abdominal pain and elevated amylase levels were noted in three patients and one patient developed abdominal perforation. 2一名无精症和开咬合畸形男性体内的额外标记染色体15显示文摘额外标记染色体15(sSMC[15])是最常见的标记染色体,通常认为如果不包含Prader-Willi/Angelman综合症关键区(Prader-Willi/Angelman syndrome critical region,PWACR),这一标记染色体就无关紧要。较大标记的关键区域的临床意义在几乎所有与标记染色体15相关的报告中都被提及,较小的标记仅与轻微畸形、无精症、习惯性流产相关。然而,这些不包含PWACR的小额外标记染色体15也可能决定某一基因表现型。我们在一个遗传学门诊对一名无精症病人进行畸形检查,根据标准的细胞遗传学方法进行外周血淋巴染色体分析,还对Y染色体微缺失进行了核仁组织区(NOR)连接、C-连接、荧光原位杂交(Fluorescence in situ hybridization,FISH)以及分子学研究。临床评估确定为畸形特征伴有无精症及重度“安氏Ⅱ类1分类开咬合畸形”。分子细胞遗传结果显示有小额外标记染色体15存在,此外,Y染色体微缺失分析显示无精症不是Y染色体微缺失的结果。额外标记染色体15和下颌不规则的症状同时出现在这一病例中,这可能只是巧合,但也可能标明了一个不仅只是无精症的特定表现型,其特征包括不育、颧骨发育不全、下颌不规则、瘢痕瘤形成以及轻微畸形特征。Altug Koc Odul Onur Mehmet Ali Ergun E. 3Differential diagnosis in ulcerative colitis in an adolescent: Chronic granulomatous disease needs extra attention显示文摘Chronic granulomatous disease(CGD) is a primary immune deficiency that is commonly diagnosed under the age of 5 years(95%) and is rarely seen in adulthood. CGD may manifest as inflammatory bowel disease(IBD) in childhood. Without proper diagnosis, these patients may be monitored for years as IBD; some may even be regarded as steroid-resistant ulcerative colitis(UC) and end up having a colectomy. 18An Improved Text-Based and Image-Based CAPTCHA Based on Solving and Response Time显示文摘CAPTCHA is an acronym that stands for Completely Automated Public Turing Test to tell Computers and Humans Apart(CAPTCHA),it is a good example of an authentication system that can be used to determine the true identity of any user.It serves as a security measure to prevent an attack caused by web bots(automatic programs)during an online transaction.It can come as text-based or image-based depending on the project and the programmer.The usability and robustness,as well as level of security,provided each of the varies and call for the development of an improved system.Hence,this paper studied and improved two different CAPTCHA systems(the text-based CAPTCHA and image-based CAPTCHA).The textbased and image-based CAPTCHAwere designed using JavaScript.Response time and solving time are the two metrics used to determine the effectiveness and efficiency of the two CAPTCHA systems.The inclusion of response time and solving time improved the shortfall of the usability and robustness of the existing system.The developed system was tested using 200 students from the Federal College of Animal Health and Production Technology.The results of each of the participants,for the two CAPTCHAs,were extracted from the database and subjected to analysis using SPSS.The result shows that textbased CAPTCHAhas the lowest average solving time(21.3333 s)with a 47.8%success rate while image-based CAPTCHA has the highest average solving time was 23.5138 s with a 52.8%success rate.The average response time for the image-based CAPTCHA was 2.1855 s with a 37.9%success rate lower than the text-based CAPTCHA response time(3.5561 s)with a 62.1%success rate.This indicates that the text-based CAPTCHA is more effective in terms of usability tests while image-based CAPTCHA is more efficient in terms of system responsiveness and recommended for potential users.Ademola Olusola Adesina Patrick Seun Ayobioloja Ibidun Christiana Obagbuwa Tola John Odule Adenrele AAfolorunso Sunday Adeola Ajagbe 2023Computers, Materials & Continua2023,,2:0
