1欧洲新生儿呼吸窘迫综合征防治共识指南:2016版显示文摘新生儿呼吸窘迫综合征(RDS)是早产儿的重要疾病,尽管治疗手段不断成熟,低龄早产儿存活率逐渐增高,但支气管肺发育不良(BPD)发病率也随之升高,其中部分原因是减少了出生后激素的使用.2006年以来,来自欧洲许多国家的新生儿专家每3年一次回顾最新文献,就RDS或有RDS风险早产儿的防治达成共识,立志于改善欧洲新生儿的预后.欧洲RDS防治指南2007年开始发布,2010年和2013年进行了更新,期间获得了欧洲围产医学学会的大力支持.RDS是由于肺表面活性物质(PS)缺乏导致生后早期出现呼吸窘迫,典型临床表现随着防治手段的改进发生了巨大变化.DavidG.Sweet Virgilio Carnielli Gorm Greisen Mikko Hallman Eren Ozek Richard Plavka Ola Didrik Saugstad Umberto Simeoni Christian P. 3Autologous CD34^+ and CD133^+ stem cells transplantation in patients with end stage liver disease显示文摘AIM:To assess the utility of an autologous CD34 + and CD133 + stem cells infusion as a possible therapeutic modality in patients with end-stage liver diseases.METHODS:One hundred and forty patients with endstage liver diseases were randomized into two groups.Group 1,comprising 90 patients,received granulocyte colony stimulating factor for five days followed by autologous CD34 + and CD133 + stem cell infusion in the portal vein.Group 2,comprising 50 patients,received regular liver treatment only and served as a control group.RESULTS:Near normalization of liver enzymes and improvement in synthetic function were observed in 54.5% of the group 1 patients;13.6% of the patients showed stable states in the infused group.None of the patients in the control group showed improvement.No adverse effects were noted.CONCLUSION:Our data showed that a CD34 + and CD133 + stem cells infusion can be used as supportive treatment for end-stage liver disease with satisfactory tolerability.Hosny Salama Abdel-Rahman N Zekri Abeer A Bahnassy Eman Medhat Hanan A Halim Ola S Ahmed Ghada Mohamed Sheren A Al Alim Ghada M Sherif 2010World Journal of Gastroenterology2010,16,42:16
42011年3月日本福岛核电站核泄漏在海洋中的传输显示文摘使用全球版本的迈阿密等密度海洋环流模式对2011年3月日本福岛核电站泄漏在海洋中的传输以及扩散进行了数值模拟。数值模式中核废料(示踪物)排放情景采取等通量连续排放,排放时间从3月25日开始,分别持续20d以及1a,两种情形分别积分20a。为了减少海洋环流年际变化带来的数值模拟的的不确定性,20a的模式积分分别用2010年、1991—2011年、1971—1991年以及1951—1971年4个不同时段的NCEP/NCAR逐日再分析资料作为大气强迫场,因此每种排放情形包含4个数值试验。模拟结果的分析表明,不同核废料排放情景及其在不同时段大气资料对海洋模式的驱动下,模拟的示踪物总体的传输扩散路径(包括表层以及次表层)、传输速率以及垂直扩展的范围没有显著的差异。集合平均数值模拟的结果显示:在两种排放情景下,日本福岛核泄漏在海洋的传输路径受北太平洋副热带涡旋洋流系统主导,其传输路径首先主要向东,到达东太平洋后,再向南向西扩散至西太平洋,可能在10~15a左右影响到我国东部沿海海域,且海洋次表层的传输信号比表层信号早5a左右。通过进一步分析模式积分过程中最大示踪物浓度随时间变化发现,在积分第20a(2031年3月),海洋表层和次表层浓度的最高值分别只有模式积分第一年浓度的0.1%和1%。在积分的20a里,排放的核废料主要滞留在北太平洋海域(超过86%±1.5%的核废料在积分结束时,滞留在北太平洋),而在积分的前10a(2021年之前),几乎所有的核废料滞留在北太平洋;在核废料的垂直分布上,主要集中在海洋表层至600m的深度,在积分的20a时间里,没有核废料信号扩散至1 000m以下的深度。数值模拟的结果也表明核废料浓度减弱的强度以及演变的时间特征主要受洋流系统的影响,与排放源的排放时间长短关系不大。值得指出的是,更加准确地评估一个真实的核泄漏事故对海洋环境所造成的可能影响,还需要考虑大气中的放射性物质的沉降以及海洋生态对核物质的响应。何晏春 郜永祺 王会军 JOHANNESSEN M Ola 于雷 2012海洋学报2012,34,4:11

6丙肝病毒相关的 hepatocellular 癌: 卓见进分子的机制和治疗学的策略显示文摘 Hepatitis C virus(HCV) infects more than 170 million people worldwide,and thereby becomes a series global health challenge.Chronic infection with HCV is considered one of the major causes of end-stage liver disease including cirrhosis and hepatocellular carcinoma.Although the multiple functions of the HCV proteins and their impacts on the modulation of the intracellular signaling transduction processes,the drive of carcinogenesis during the infection with HCV,is thought to result from the interactions of viral proteins with host cell proteins.Thus,the induction of mutator phenotype,in liver,by the expression of HCV proteins provides a key mechanism for the development of HCV-associated hepatocellular carcinoma(HCC).HCC is considered one of the most common malignancies worldwide with increasing incidence during the past decades.In many countries,the trend of HCC is attributed to several liver diseases including HCV infection.However,the development of HCC is very complicated and results mainly from the imbalance between tumor suppressor genes and oncogenes,as well as from the alteration of cellular factors leading to a genomic instability.Besides the poor prognosis of HCC patients,this type of tumor is quite resistance to the available therapies.Thus,understanding the molecular mechanisms,which are implicated in the development of HCC during the course of HCV infection,may help to design a general therapeutic protocol for the treatment and/or the prevention of this malignancy.This review summarizes the current knowledge of the molecular mechanisms,which are involved in the development of HCV-associated HCC and the possible therapeutic strategies.Denis Selimovic Abdelouahid El-Khattouti Hanan Ghozlan Youssef Haikel Ola Abdelkader Mohamed Hassan 2012World Journal of Hepatology2012,4,12:7
7Greenland Ice Sheet Contribution to Future Global Sea Level Rise based on CMIP5 Models显示文摘Sea level rise(SLR) is one of the major socioeconomic risks associated with global warming. Mass losses from the Greenland ice sheet(GrIS) will be partially responsible for future SLR, although there are large uncertainties in modeled climate and ice sheet behavior. We used the ice sheet model SICOPOLIS(SImulation COde for POLythermal Ice Sheets) driven by climate projections from 20 models in the fifth phase of the Coupled Model Intercomparison Project(CMIP5) to estimate the GrIS contribution to global SLR. Based on the outputs of the 20 models, it is estimated that the GrIS will contribute 0–16(0–27) cm to global SLR by 2100 under the Representative Concentration Pathways(RCP) 4.5(RCP 8.5) scenarios. The projected SLR increases further to 7–22(7–33) cm with 2×basal sliding included. In response to the results of the multimodel ensemble mean, the ice sheet model projects a global SLR of 3 cm and 7 cm(10 cm and 13 cm with 2×basal sliding) under the RCP 4.5 and RCP 8.5 scenarios, respectively. 9Accumulation over the Greenland Ice Sheet as Represented in Reanalysis Data显示文摘Annual precipitation,evaporation,and calculated accumulation from reanalysis model outputs have been investigated for the Greenland Ice Sheet (GrIS),based on the common period of 1989-2001.The ERA-40 and ERA-interim reanalysis data showed better agreement with observations than do NCEP-1 and NCEP-2 reanalyses.Further,ERA-interim showed the closest spatial distribution of accumulation to the observation.Concerning temporal variations,ERA-interim showed the best correlation with precipitation observations at five synoptic stations,and the best correlation with in situ measurements of accumulation at nine ice core sites.The mean annual precipitation averaged over the whole GrIS from ERA-interim (363 mm yr 1) and mean annual accumulation (319 mm yr 1) are very close to the observations.The validation of accumulation calculated from reanalysis data against ice-core measurements suggests that further improvements to reanalysis models are needed.陈琳玲 Ola M.JOHANNESSEN 王会军 Atsumu OHMURA 2011Advances in Atmospheric Sciences2011,28,5:4

11Intravenous vs intraperitoneal mesenchymal stem cells administration:What is the best route for treating experimental colitis?显示文摘AIM:To investigate the therapeutic effects of mesenchymal stem cells(MSCs)transplanted intraperitoneally andintravenously in a murine model of colitis.METHODS:MSCs were isolated from C57BL/6 mouse adipose tissue.MSC cultures were analyzed according to morphology,cellular differentiation potential,and surface molecular markers.Experimental acute colitis was induced in C57BL/6 mice by oral administration of2%dextran sulfate sodium(DSS)in drinking water ad libitum from days 0 to 7.Colitis mice were treated with1×106 MSCs via intraperitoneal or intravenous injection on days 2 and 5.The disease activity index was determined daily based on the following parameters:weight loss,stool consistency and presence of blood in the feces and anus.To compare morphological and functional differences in tissue regeneration between different MSC injection modalities,mice were euthanized on day 8,and their colons were examined for length,weight,and histopathological changes.Inflammatory responses were determined by measuring the levels of different serum cytokines using a CBA Th1/Th2/Th17 kit.Apoptotic rates were evaluated by terminal deoxynucleotidyl transferase-mediated d UDPbiotin nick end labeling assay.RESULTS:Intravenous infusion of MSCs was more effective than intraperitoneal treatment(P<0.001)in reducing the clinical and histopathologic severity of colitis,which includes weight loss,diarrhea and inflammation.An histological evaluation demonstrated decreased colonic inflammation based on reduced crypt loss and reduced infiltration of inflammatory cells.This therapeutic effect was most likely mediated by the down-regulation of pro-inflammatory cytokines[interleukin(IL)-6 and tumor necrosis factor(TNF)];and by the up-regulation of anti-inflammatory cytokines(IL-10 and IL-4).Intravenous transplantation alsoinduced high levels of IFN that lead to activation of the immunosuppressive activity of the MSCs,which did not occur with intraperitoneal transplantation(P=0.006).An increase in apoptotic T cells was observed after intravenous,but not intraperitoneal,MSC infusion,suggesting that MSCs can induce apoptosis in resistant T cells in colonic inflammation(P=0.027).CONCLUSION:Our results demonstrate that intravenous treatment is a superior method for reducing colon inflammation compared with intraperitoneal therapy.Fabiany da Costa Goncalves Natália Schneider Fernanda Otesbelgue Pinto Fabíola Schons Meyer Fernanda Visioli Bianca Pfaffenseller Patrícia Luciana da Costa Lopez Eduardo Pandolfi Passos Elizabeth Obino Cirne-Lima Luíse Meurer Ana Helena Paz 2014World Journal of Gastroenterology2014,20,48:4
12Long term recurrence,pain and patient satisfaction after ventral hernia mesh repair显示文摘AIM: To compare long term outcomes of laparoscopic and open ventral hernia mesh repair with respect to recurrence, pain and satisfaction.METHODS: We conducted a single-centre follow-up study of 194 consecutive patients after laparoscopic and open ventral hernia mesh repair between March 2000 and June 2010. Of these, 27 patients(13.9%) died and 12(6.2%) failed to attend their follow-up appointment. One hundred and fifty-three(78.9%) patients attended for follow-up and two patients(1.0%) were interviewed by telephone. Of those who attended the follow-up appointment, 82(52.9%) patients had received laparoscopic ventral hernia mesh repair(LVHR) while 73(47.1%) patients had undergone open ventral hernia mesh repair(OVHR), including 11 conversions. The follow-up study included analyses of medical records, clinical interviews, examination of hernia recurrence and assessment of pain using a 100 mm visual analogue scale(VAS) ruler anchored by word descriptors. Overall patient satisfaction was also determined. Patients with signs of recurrence were examined by magnetic resonance imaging or computed tomography scan.RESULTS: Median time from hernia mesh repair to follow-up was 48 and 52 mo after LVHR and OVHR respectively. Overall recurrence rates were 17.1% after LVHR and 23.3% after OVHR. Recurrence after LVHR was associated with higher body mass index. Smoking was associated with recurrence after OVHR. Chronic pain(VAS > 30 mm) was reported by 23.5% in the laparoscopic cohort and by 27.8% in the open surgery cohort. Recurrence and late complications were predictors of chronic pain after LVHR. Smoking was associated with chronic pain after OVHR. Sixty point five percent were satisfied with the outcome after LVHR and 49.3% after OVHR. Predictors for satisfaction were absence of chronic pain and recurrence. Old age and short time to follow-up also predicted satisfaction after LVHR.CONCLUSION: LVHR and OVHR give similar long term results for recurrence, pain and overall satisfaction. 19Prospective evaluation of the cause of acute pancreatitis,with special attention to medicines显示文摘AIM: To investigate the cause of acute pancreatitis(AP) by conducting a thorough investigation of drugs and their possible etiological role.METHODS: We investigated the cause of AP in a large retrospective cohort of 613 adult patients admitted with AP at the Akershus University Hospital, Norway, from 2000 until 2009, who were evaluated with standard ward investigations. This group was compared with a prospectively evaluated group(n = 57) admitted from January 2010 until September 2010 who investigated more extensively using medical history and radiological assessment.RESULTS: The groups were comparable with regards to gender, age, comorbidity and severity. The most common etiology was bile stones and alcohol, occurring in 60% in both groups. The prospective group was examined more thoroughly with regards to the use of alcohol and medicines. An increased number of radiological investigations during hospital stay and at follow-up were also performed. A more extensive use of radiological evaluation did not increase the detection frequency of bile stones. In the prospective group, more than half of the patients had two or more possible causes of pancreatitis, being mostly a combination of bile stones and drugs. No possible cause was found in only 3.5% of these patients, compared with 29.7% in the retrospective group.CONCLUSION: A detailed medical history and extensive radiological evaluation may determine a possible etiology in almost all cases of AP.
