| 1 | Prognostic impact of atrial fibrillation on clinical outcomes of acute coronary syndromes,heart failure and chronic kidney disease显示文摘Atrial fibrillation(AF) is the most common type of sustained arrhythmia,which is now on course to reach epidemic proportions in the elderly population. AF is a commonly encountered comorbidity in patients with cardiac and major non-cardiac diseases. Morbidity and mortality associated with AF makes it a major healthcare burden. The objective of our article is to determine the prognostic impact of AF on acute coronary syndromes,heart failure and chronic kidney disease. Multiple studies have been conducted to determine if AF has an independent role in the overall mortality of such patients. | 7 | Electrospun Aligned PCL/Gelatin Scaffolds Mimicking the Skin ECM for Effective Antimicrobial Wound Dressings显示文摘Bacterial infections and multidrug-resistant bacteria are major health burdens in wound care.Biocompatible antimicrobial agents,e.g.,ε-polylysine(ε-PL),provide a broad spectrum of antibacterial properties and support dermal cell growth.Here,ε-PL was incorporated into polycaprolactone(PCL)/gelatin electrospun scaffolds collected at varying rotation speeds.Then,the samples were crosslinked using dopamine hydrochloride to provide highly proliferative dressings with broad antimicrobial activity.The morphological study showed that the electrospun wound dressings were smooth,continuous,and bead-free,with a mean diameter ranging from 267±7 to 331±8 nm for all random and aligned nanofibers.The fiber alignment of the electrospun PCL/gelatin scaffolds improved their tensile strength and modulus.Moreover,nanofiber mats are highly hydro-philic,which is crucial for an efficient wound dressing.The samples also demonstrated high antimicrobial properties against common wound bacterial strains,including methicillin-resistant Staphylococcus aureus(MRSA),Staphylococcus aureus(SA),Escherichia coli(EC),Acinetobacter baumannii(AB),and Pseudomonas aeruginosa(PA).Mammalian cell prolifera-tion and morphology assays involving primary human dermal fibroblasts(hDFs)and immortalized keratinocytes(HaCaT)showed excellent biocompatibility of the electrospun mats and remarkably aligned mats.Furthermore,aligned mats showed more cell migration than randomly oriented mats,which is desirable for more efficient wound healing.Therefore,it can be concluded that aligned PCL/gelatin mats containingε-PL are promising for potential use in wound dressings. | Erfan Rezvani Ghomi Rajamani Lakshminarayanan Vijila Chellappan Navin Kumar Verma Amutha Chinnappan Rasoul Esmaeely Neisiany Kottaiswamy Amuthavalli Zhi Sheng Poh Brandon Han Siang Wong Nileshkumar Dubey Roger Narayan Seeram Ramakrishna | 2023 | Advanced Fiber Materials2023,5,1: | 0 |

| 9 | Influence of hospital volume and outcomes of adult structural heart procedures显示文摘Hospital volume is regarded amongst many in the medical community as an important quality metric. This is especially true in more complicated and less commonly performed procedures such as structural heart disease interventions. Seminal work on hospital volume relationships was done by Luft et al more than 4 decades ago, when they demonstrated that hospitals performing > 200 surgical procedures a year had 25%-41% lower mortality than those performing fewer procedures. Numerous volume-outcome studies have since been done for varied surgical procedures. An old adage 'practice makes perfect' indicating superior operator and institutional experience at higher volume hospitals is believed to primarily contribute to the volume outcome relationship. Compelling evidence from a slew of recent publications has also highlighted the role of hospital volume in predicting superior post-procedural outcomes following structural heart disease interventions. These included transcatheter aortic valve repair, transcatheter mitral valve repair, septal ablation and septal myectomy for hypertrophic obstructive cardiomyopathy, left atrial appendage closure and atrial septal defect/patent foramen ovale closure. This is especially important since these structural heart interventions are relatively complex with evolving technology and a steep learning curve. The benefit was demonstrated both in lower mortality and complications as well as better economics in terms of lower length of stay and hospitalization costs seen at high volume centers.
