| 1 | Pseudolymphoma (reactive lymphoid hyperplasia) of the liver:A clinical challenge显示文摘Reactive lymphoid hyperplasia(RLH), also known as pseudolymphoma or nodular lymphoid lesion of the liver is an extremely rare condition, and only 51 hepatic RLH cases have been described in the literature since the first case was described in 1981. The majority of these cases were asymptomatic and incidentally found through radiological imaging. The precise etiology of hepatic RLH is still unknown, but relative high prevalence of autoimmune disorder in these cases suggests an immune-based liver disorder. Imaging features of hepatic RLH often suggest malignant lesions such as hepatocellular carcinoma and cholangiocarcinoma. In this report, we discuss two cases of hepatic RLH in patients with autoimmune hepatitis. We also present pathologic and magnetic resonance imaging findings, including one case utilizing a hepatocellular contrast agent, Eovist. | 3 | Microstructural homogeneity and mechanical behavior of a selective laser melted Ti-35Nb alloy produced from an elemental powder mixture显示文摘Although using elemental powder mixtures may provide broad alloy selection at low cost for selective laser melting(SLM), there is still a concern on the resultant microstructural and chemical homogeneity of the produced parts. Hence, this work investigates the microstructure and mechanical properties of a SLM-produced Ti-35 Nb composite(in wt%) using elemental powder. The microstructural characteristics including ? phase, undissolved Nb particles and chemical homogeneity were detailed investigated.Nanoindentation revealed the presence of relatively soft undissolved Nb particles and weak interface bonding around Nb-rich regions in as-SLMed samples. Solid-solution treatment can not only improve chemical homogeneity but also enhance bonding through grain boundary strengthening, resulting in43 % increase in tensile elongation for the heat-treated Ti-35 Nb compared to the as-SLMed counterpart. The analyses of tensile fractures and shear bands further confirmed the correlation between the different phases and the ductility of Ti-35 Nb. In particular, the weak bonding between undissolved Nb and the matrix in the as-SLMed sample reduces its ductility while the ? grains in solid-solution treated Ti-Nb alloy can induce a relatively stable plastic flow therefore better ductility. | 4 | Experimental investigations of transient pressure variations in a high head model Francis turbine during start-up and shutdown显示文摘Penetration of the power generated using wind and solar energy to electrical grid network causing several incidents of the grid tripping, power outage, and frequency drooping. This has increased restart(star-stop) cycles of the hydroelectric turbines significantly since grid connected hydroelectric turbines are widely used to manage critical conditions of the grid. Each cycle induces significant stresses due to unsteady pressure loading on the runner blades. The presented work investigates the pressure loading to a high head( H P=377 m, DP =1.78 m) Francis turbine during start-stop. The measurements were carried out on a scaled model turbine( H M=12.5 m, DM =0.349 m). Total four operating points were considered. At each operating point, three schemes of guide vanes opening and three schemes of guide vanes closing were investigated. The results show that total head variation is up to 9% during start-stop of the turbine. On the runner blade, the maximum pressure amplitudes are about 14 kPa and 16 kPa from the instantaneous mean value of 121 kPa during rapid start-up and shutdown, respectively, which are about 1.5 times larger than that of the slow start-up and shutdown. | 6 | Perioperative glucose management and outcomes in liver transplant recipients: A qualitative systematic review显示文摘AIM To investigate the relationship between post-liver transplantation(LT) glycemic control and LT outcomes. METHODS A qualitative systematic review on relevant prospective interventions designed to control glucose levels including insulin protocols. Studies investigating an association between glycemic control and post-LT outcomes such as mortality, graft rejection, and infection rate were reviewed. Pub Med, EMBASE, and other databases were searched through October 2016. RESULTS Three thousands, six hundreds and ninety-two patients from 14 studies were included. Higher mortality rate was seen when blood glucose(BG) ≥ 150 mg/dL(P = 0.05). BG ≥ 150 mg/dL also led to higher rates of infection. Higher rates of graft rejection were seen at BG > 200 mg/dL(P < 0.001). Mean BG ≥ 200 mg/dL was associated with more infections(P = 0.002).Nurse-initiated protocols and early screening strategies have shown a reduction in negative post-LT outcomes.CONCLUSION Hyperglycemia in the perioperative period is associated with poor post-LT outcomes.
