| 4 | Preservation solutions used during abdominal transplantation:Current status and outcomes显示文摘Organ preservation remains an important contributing factor to graft and patient outcomes. During donor organ procurement and transportation, cellular injury is mitigated through the use of preservation solutions in conjunction with hypothermia. Various preservation solutions and protocols exist with widespread variability among transplant centers. In this review of abdominal organ preservation solutions, evolution of transplantation and graft preservation are discussed followed by classification of preservation solutions according to the composition of electrolytes, impermeants, buffers, antioxidants, and energy precursors. Lastly, pertinent clinical studies in the setting of hepatic, renal, pancreas, and intestinal transplantation are reviewed for patient and graft survival as well as financial considerations. In liver transplants there may be some benefit with the use of histidine-tryptophan-ketoglutarate(HTK) over University of Wisconsin solution in terms of biliary complications and potential cost savings. Renal grafts may experience increased initial graft dysfunction with the use of Euro-Collins thereby dissuading its use in support of HTK which can lead to substantial cost savings. University of Wisconsin solution and Celsior are favored in pancreas transplants given the concern for pancreatitis and graft thrombosis associated with HTK. No difference was observed with preservation solutions with respect to graft and patient survival in liver, renal, and pancreas transplants. Studies involving intestinal transplants are sparse but University of Wisconsin solution infused intraluminally in combination with an intra-vascular washout is a reasonable option until further evidence can be generated. | 8 | Colonoscopy quality with Entonox? vs intravenous conscious sedation:18608 colonoscopy retrospective study显示文摘AIM To compare colonoscopy quality with nitrous oxide gas(Entonox?) against intravenous conscious sedation using midazolam plus opioid.METHODS A retrospective analysis was performed on a prospectively held database of 18608 colonoscopies carried out in Lothian health board hospitals between July 2013 and January 2016.The quality of colonoscopies performed with Entonox was compared to intravenous conscious sedation(abbreviated in this article as IVM).Furthermore,the quality of colonoscopies performed with an unmedicated group was compared to IVM.The study used the following key markers of colonoscopy quality:(1) patient comfort scores;(2) caecal intubation rates(CIRs); and (3) polyp detection rates (PDRs).We used binary logistic regression to model the data.RESULTS There was no difference in the rate of moderate-toextreme discomfort between the Entonox and IVM groups (17.9% vs 18.8%; OR = 1.06,95%CI: 0.95-1.18,P = 0.27).Patients in the unmedicated group were less likely to experience moderate-to-extreme discomfort than those in the IVM group(11.4% vs 18.8%; OR = 0.71,95%CI: 0.60-0.83,P < 0.001).There was no difference in caecal intubation between the Entonox and IVM groups(94.4% vs 93.7%; OR = 1.08,95%CI: 0.92-1.28,P = 0.34).There was no difference in caecal intubation between the unmedicated and IVM groups (94.2% vs 93.7%; OR = 0.98,95%CI: 0.79-1.22,P = 0.87).Polyp detection in the Entonox group was not different from IVM group (35.0% vs 33.1%; OR = 1.01,95%CI: 0.93-1.10,P = 0.79).Polyp detection in the unmedicated group was not significantly different from the IVM group (37.4% vs 33.1%; OR = 0.97,95%CI: 0.87-1.08,P = 0.60).CONCLUSION The use of Entonox was not associated with lower colonoscopy quality when compared to intravenous conscious sedation using midazolam plus opioid. | Alexander R Robertson Nicholas A Kennedy James A Robertson Nicholas I Church Colin L Noble | 2017 | World Journal of Gastrointestinal Endoscopy2017，9，9： | 3 |

| 10 | Acid and non-acid reflux in patients refractory to proton pump inhibitor therapy:Is gastroparesis a factor?显示文摘AIM:To determine whether an increased number and duration of non-acid reflux events as measured using the multichannel intraluminal impedance pH(MII-pH)is linked to gastroparesis(GP).METHODS:A case control study was conducted in which 42 patients undergoing clinical evaluation for continued symptoms of gastroesophageal reflux disease(both typical and atypical symptoms)despite acid suppression therapy.MII-pH technology was used over 24 h to detect reflux episodes and record patients’symptoms.Parameters evaluated in patients with documented GP and controls without GP by scintigraphy included total,upright,and supine number of acid and non-acid reflux episodes(pH<4 and pH>4,respectively),the duration of acid and non-acid reflux in a 24-h period,and the number of reflux episodes lasting longer than 5 min.RESULTS:No statistical difference was seen between the patients with GP and controls with respect to the total number or duration of acid reflux events,total number and duration of non-acid reflux events or the duration of longest reflux episodes.The number of nonacid reflux episodes with a pH>7 was higher in subjects with GP than in controls.In addition,acid reflux episodes were more prolonged(lasting longer than 5min)in the GP patients than in controls;however,these values did not reach statistical significance.Thirty-five patients had recorded symptoms during the 24 h study and of the 35 subjects,only 9%(n=3)had a positive symptom association probability(SAP)for acid/non-acid reflux and 91%had a negative SAP.CONCLUSION:The evaluation of patients with a documented history of GP did not show an association between GP and more frequent episodes of non-acid reflux based on MII-pH testing. | Anna Tavakkoli Bisma A Sayed Nicholas J Talley Baharak Moshiree | 2013 | World Journal of Gastroenterology2013，19，37： | 2 |
