| 3 | Benefit of combination β-blocker and endoscopic treatment to prevent variceal rebleeding: A meta-analysis显示文摘AIM: To determine whether the association of β-blockers with endoscopic treatment is superior to endoscopic treatment alone for the secondary prophylaxis of oesophageal variceal bleeding. METHODS: Randomised controlled trials comparing sclerotherapy (SCL) with SCL plus β-blockers (BB) or banding ligation (BL) with BL plus BB were identif ied.Main outcomes were overall and 6, 12 and 24 mo rebleeding rates, as well as overall and 6, 12 and 24 mo mortality. Two statistical methods were used: Yusuf-Peto, and Der Simonian and Laird. Inter-trial heterogeneity was systematically taken into account. RESULTS: Seventeen randomised controlled trials were included, 14 with SCL and 3 with BL. Combination β-blocker and endoscopic treatment signif icantly reduced rebleeding rates at 6, 12 and 24 mo and overall [odds ratio (OR): 2.20, 95% conf idence interval (CI): 1.69-2.85, P<0.0001] compared to endoscopic treatment alone. Mortality at 24 mo was signif icantly lower for the combined treatment group (OR: 1.83, 95% CI:1.16-2.90, P= 0.009), as well as overall mortality (OR: 1.43, 95% CI:1.03-1.98, P= 0.03). | 4 | Upper-gastrointestinal bleeding secondary to peptic ulcer disease:Incidence and outcomes显示文摘AIM:To evaluate the incidence,surgery,mortality,and readmission of upper gastrointestinal bleeding(UGIB)secondary to peptic ulcer disease(PUD).METHODS:Administrative databases identified all hospitalizations for UGIB secondary to PUD in Alberta,Canada from 2004 to 2010(n=7079)using the International Classification of Diseases Codes(ICD-10).A subset of the data was validated using endoscopy reports.Positive predictive value and sensitivity with 95%confidence intervals(CI)were calculated.Incidence of UGIB secondary to PUD was calculated.Logistic regression was used to evaluate surgery,in-hospital mortality,and 30-d readmission to hospital with recurrent UGIB secondary to PUD.Co-variants accounted for in our logistic regression model included:age,sex,area of residence(i.e.,urban vs rural),number of Charlson comorbidities,presence of perforated PUD,undergoing upper endoscopy,year of admission,and interventional radiological attempt at controlling bleeding.A subgroup analysis(n=6356)compared outcomes of patients with gastric ulcers to those with duodenal ulcers.Adjusted estimates are presented as odds ratios(OR)with95%CI.RESULTS:The positive predictive value and sensitivity of ICD-10 coding for UGIB secondary to PUD were85.2%(95%CI:80.2%-90.2%)and 77.1%(95%CI:69.1%-85.2%),respectively.The annual incidence between 2004 and 2010 ranged from 35.4 to 41.2 per100000.Overall risk of surgery,in-hospital mortality,and 30-d readmission to hospital for UGIB secondary to PUD were 4.3%,8.5%,and 4.7%,respectively.Interventional radiology to control bleeding was performed in 0.6%of patients and 76%of these patients avoided surgical intervention.Thirty-day readmission significantly increased from 3.1%in 2004 to 5.2%in 2010(OR=1.07;95%CI:1.01-1.14).Rural residents(OR rural vs urban:2.35;95%CI:1.83-3.01)and older individuals(OR≥65 vs<65:1.57;95%CI:1.21-2.04)were at higher odds of being readmitted to hospital.Patients with duodenal ulcers had higher odds of dying(OR=1.27;95%CI:1.05-1.53),requiring surgery(OR=1.73;95%CI:1.34-2.23),and being readmitted to hospital(OR=1.54;95%CI:1.19-1.99)when compared to gastric ulcers.CONCLUSION:UGIB secondary to PUD,particularly duodenal ulcers,was associated with significant morbidity and mortality.Early readmissions increased over time and occurred more commonly in rural areas. | Samuel Quan Alexandra Frolkis Kaylee Milne Natalie Molodecky Hong Yang Elijah Dixon Chad G Ball Robert P Myers Subrata Ghosh Robert Hilsden Sander Veldhuyzen van Zanten Gilaad G Kaplan | 2014 | World Journal of Gastroenterology2014，20，46： | 21 |
| 5 | Haploinsufficiency of A20 (HA20): updates on the genetics, phenotype, pathogenesis and treatment显示文摘Background A20,a protein encoded by the tumor necrosis factor alpha-induced protein 3 gene(TNFAIP3),plays a vital role in the negative regulation of inflammation and immunity.Loss-of-function mutation in TNFAIP3 leads to a new described autoinflammatory disease-haploinsufficiency of A20(HA20).Since HA20 was first described in 2016,a number of new cases have been described in this literature,however,the disease and its pathogenesis are poorly understood.This review seeks to improve clinical recognition of this disorder,and promote both earlier diagnosis and initiation of targeted therapies to improve patients’outcomes.Methods We reviewed 26 papers about A20 and HA20,and we summarized genetic variants and clinical manifestations of a total of 61 reported patients from 26 families identified to have a genetic diagnosis of germline pathogenic variants in TNFAIP3/A20.Additionally,we discussed the pathogenesis and treatment of HA20.Results A total of 24 pathogenic variants of A20 had been reported.There was significant clinical heterogeneity,even among those with the same variants in TNFA1P3.Prior to receiving a molecular diagnosis of HA20,patients had been diagnosed with Behcet's disease,rheumatoid arthritis,rheumatic fever,juvenile idiopathic arthritis,systemic lupus erythematosus,and even adult-onset Stills'disease.The patients with HA20 that presented with inflammatory signatures in NF-kB signaling were mostly responsive to treatment.Conclusions HA20 is a monogenic autoinflammatory disease with highly variable clinical manifestations.This extensive heterogeneity makes it difficult to set a clinical diagnostic criteria,and genetic sequencing is necessary for a definitive diagnosis of HA20. | Mei-Ping Yu Xi-Sheng Xu Qing Zhou Natalie Deuitch Mei-Ping Lu | 2020 | World Journal of Pediatrics2020，16，6： | 17 |

As a result, cassava is becoming an increasingly important ingredient in poultry diets, largely due to its high availability. Efficient use of cassava products has been shown to reduce feed costs of poultry production. The utilisation of cassava is,however, limited by a number of factors, including its high fibre and low energy content and the presence of anti-nutritional factors, primarily hydrocyanic acid(HCN). With correct processing the inclusion level of cassava in poultry diets could be increased. Extensive research has been conducted on cassava products for poultry, but there is still a lack of consistency amongst the measured nutritive values for cassava and its products, hence variation exists in results from poultry studies. | 15 | Effect of arabinoxylo-oligosaccharides and arabinoxylans on net energy and nutrient utilization in broilers显示文摘Arabinoxylo-oligosaccharides(AXOS) are hydrolytic degradation products of arabinoxylans(AX) that can be fermented by the gut microbiota, thus potentially displaying prebiotic properties. This study examined the effects of AX and AXOS on net energy(NE) and nutrient utilization in broilers. Ross 308 broilers(n = 90, 30 birds per treatment) were fed wheat-soybean diets supplemented with pure AX, AXOS produced by exposing the AX to xylanase in vitro(AXOS), or AX with xylanase(AX + E) from d 10 to 21.Performance parameters were measured from d 10 to 21. On d 15, 10 birds per treatment were allocated to closed-circuit net energy chambers to assess the impact of AX and AXOS on dietary energy utilization,through assessment of both metabolisable energy(ME) and NE. Ileal and caecal digesta samples were collected on d 21 to determine the effect of AX and AXOS on ileal and total tract dry matter digestibility,ileal digestible energy, digesta pH, short chain fatty acids(SCFA) and microbiota concentration. Feed conversion ratio was numerically the lowest in birds fed the diet supplemented with AXOS, which is 1.26 compared to 1.37 and 1.30 for AX and AX + E, respectively. Ileal dry matter digestibility was higher in birds fed AXOS than those fed AX(P = 0.047). Ileal digestible energy and total tract dry matter digestibility were higher in birds fed AXOS than those fed AX or AX + E(P = 0.004 and P = 0.001,respectively). Birds fed AXOS had higher ME intake(P = 0.049) and nitrogen retention(P = 0.001) and a strong trend of higher NE(P = 0.056), NE intake(P = 0.057) and retained energy(P = 0.054) compared to those fed AX. Ileal total SCFA, lactic and formic acid concentrations were higher in birds fed AXOS than those fed AX(P = 0.011, P = 0.012 and P = 0.023, respectively). Birds fed AXOS or AX + E had higher caecal total SCFA, acetic, butyric and isovaleric acid concentrations compared to those fed AX(P = 0.001,P = 0.004, P = 0.016 and P = 0.008, respectively), and caecal propionic acid concentration was higher in birds fed AX + E than those fed AX(P = 0.050). Ileal and caecal microbiota concentrations were numerically higher and pH was lower in birds fed AXOS and AX + E than those fed AX. | 19 | Galvanically induced potentials to enable minimal tribochemical wear of stainless steel lubricated with sodium chloride and ionic liquid aqueous solution显示文摘The effect of galvanically induced potentials on the friction and wear behavior of a 1 RK91 stainless steel regarding to tribocorrosion was investigated using an oscillating ball-on-disk tribometer equipped with an electrochemical cell. The aim of this investigation is to develop a water-based lubricant. Therefore 1 molar sodium chloride(NaCl) and 1% 1-ethyl-3-methylimidazolium chloride [C_2 mim][Cl] water solutions were used. Tribological performance at two galvanically induced potentials was compared with the non-polarized state: cathodic potential-coupling with pure aluminum- and anodic potential-coupling with pure copper. Frictional and electrochemical response was recorded during the tests. In addition, wear morphology and chemical composition of the steel were analyzed using scanning electron microscopy(SEM) and X-ray photoelectron spectroscopy(XPS), respectively. The galvanically induced cathodic polarization of the stainless steel surface results in electrochemical corrosion protection and the formation of a tribolayer. Cations from the electrolyte(sodium Na^+ and 1-ethyl- 3-methylimidazolium [C_2 mim]^+) interact and adhere on the surface. These chemical interactions lead to considerably reduced wear using 1 NaC l(86%) and 1% 1-ethyl-3-methylimidazolium chloride [C_2 mim][Cl](74%) compared to the nonpolarized system. In addition, mechanical and corrosive part of wear was identified using this electrochemical technique.
