| 2 | Expression of the oxygen-sensitive transcription factor subunit HIF-1α in patients suffering from secondary Raynaud syndrome显示文摘Anti-ischemic therapy remains a challenge due to the complexity of hypoxia response pathways. Hypoxia-inducible factor (HIF)-1 is a heterodimer tran scription factor con sisti ng of 2 sub units, HIF-1α and HIF-1β. Hypoxia-depe ndent activatio n of HIF-1α regulates cellular 02 homeostasis. Raynaud syndrome (RS), as a comorbidity of the autoimmune disease systemic sclerosis (SS), is characterized by vasospasms that limit blood flow to the limbs, resulting in hypoxia. A single-center randomized study was con ducted to compare prostagla ndin E1 (PgEI) therapy with a treatme nt combi ning PgE1 and an en dotheli n-1 blocker, bosentan. A total of 30 patients suffering from SS with RS were enrolled. We examined the regulation of HIF-1α, its target heme oxygenase-1 (HMOX-1), and the serum levels of the HIF-1α protein in a subset of patients as well as in ten healthy individuals. The expression of HIF-1α and HMOX-1 in monocytes was measured using absolute plasmid-based quantitative real-time PCR, whereas serum HIF-1α levels were measured with ELISA. Samples were taken at the time of randomization and after 24 weeks. We found that HIF-1α and HMOX-1 mRNA expression in monocytes and serum HIF-1α protein levels were significantly higher in the SS/RS patients compared to the healthy control group. Single-drug therapy significantly increased HIF-1α and HMOX-1 mRNA expression in monocytes and serum HIF-1α protein levels in the SS/RS patients compared to those at the time of randomization, whereas combining PgE1 with an en dotheli n-1 blocker preve nted the further in creases in HIF-1α and HMOX-1 expressi on. | 3 | Assessing adequacy of emergency provider documentation among interhospital transferred patients with acute aortic dissection显示文摘BACKGROUND: Acute aortic dissection(AoD) is a hypertensive emergency often requiring the transfer of patients to higher care hospitals; thus, clinical care documentation and compliance with the Emergency Medical Treatment and Active Labor Act(EMTALA) is crucial. The study assessed emergency providers(EP) documentation of clinical care and EMTALA compliance among interhospital transferred AoD patients.METHODS: This retrospective study examined adult patients transferred directly from a referring emergency department(ED) to a quaternary academic center between January 1, 2011 and September 30, 2015. The primary outcome was the percentage of records with adequate documentation of clinical care(ADoCC). The secondary outcome was the percentage of records with adequate documentation of EMTALA compliance(ADoEMTALA). RESULTS: There were 563 electronically identified patients with 287 included in the final analysis. One hundred and five(36.6%) patients had ADoCC while 166(57.8%) patients had ADoEMTALA. Patients with inadequate documentation of EMTALA(IDoEMTALA) were associated with a higher likelihood of not meeting the American Heart Association(AHA) ED Departure SBP guideline(OR 1.8, 95% CI 1.03–3.2, P=0.04). Male gender, handwritten type of documentation, and transport by air were associated with an increased risk of inadequate documentation of clinical care(IDoCC), while receiving continuous infusion was associated with higher risk of IDoEMTALA.CONCLUSION: Documentation of clinical care and EMTALA compliance by Emergency Providers is poor. Inadequate EMTALA documentation was associated with a higher likelihood of patients not meeting the AHA ED Departure SBP guideline. | 5 | 基于时域叠加的声学埋地PE管道定位新方法显示文摘聚乙烯(PE)管道凭借耐腐蚀、使用年限长等优点在城镇中、低压埋地燃气管网建设中被广泛应用,但由于管道信息系统不完善,且已有的埋地PE管道定位技术还不成熟,导致管道位置不明确,易造成施工建设中对管道的损伤和破坏。为此,基于弹性波反射原理,建立了弹性波在土壤—PE管道介质中的传播理论模型,提出了基于时域叠加的埋地PE管道定位新方法,并通过有限元仿真分析和现场管道定位测试验证了该定位方法的可行性。研究结果表明:(1)土壤中传播的声波可分为纵波和横波,其在土壤—PE管道分界面将发生反射、透射以及波形转换,其中反射回地面的弹性波含有地下管道位置信息,因此可利用反射回波信号来定位埋地PE管道;(2)使用时域叠加法对仿真得到的6路检波器信号进行处理并对定位区域成像,由成像图可较准确地定位出埋地PE管道,图中颜色最亮的单元几何中心点表示的是埋地PE管道的顶点;(3)分别进行了埋深为0.5 m、1.0 m、1.5 m的PE管道定位测试,最大定位误差依次为0.090 m、0.243 m、0.304 m,即管道埋深越大,定位误差越大。结论认为,该方法计算得到的定位精度可以满足现场定位埋地PE管道的需要,研究成果可用于指导声学埋地PE管道探测定位装备的研发。 | 葛亮 王甜 肖小汀 吴佳晔 Mark Robinson 卓勇 杨国强 | 2022 | 天然气工业2022，42，9： | 2 |

| 7 | Quantifying cold front induced water transport of a bay with in situ observations using manned and unmanned boats显示文摘The generation of high-resolution data is increasingly important in understanding the complexities of coastal ocean and developing sound management strategies, especially in view of the long-term impact of severe weather systems. The impact of severe weather systems, when integrated over time, can be significant when compared with tidal oscillations. This paper presents a study of water transport out of Vermilion Bay in response to a short,intense event associated with a passing atmospheric cold front, and reports the application of an Acoustic Doppler Current Profiler(ADCP) mounted on an Automated Surface Craft(ASC), known as the auto-boat or unmanned boat, developed in our lab at the Louisiana State University, to generate high resolution data accurately at a fraction of the cost of a manned boat. In our study, we used a manned boat and an unmanned boat, each for over 24 h to cover an entire diurnal tidal cycle, to measure flow velocity profiles to calculate the total transport.
