3Comparison of clinical outcomes between femtosecond laser-assisted versus conventional phacoemulsification显示文摘Background:To compare femtosecond laser-assisted versus conventional phacoemulsification in terms of visual and refractive outcomes,cumulative dissipated energy,anterior chamber inflammation and endothelial cell loss.Methods:In this retrospective cohort study,records of eyes that underwent femtosecond laser-assisted cataract surgery(FLACS)or conventional phacoemulsification(CP)were reviewed.The Victus femtosecond laser(Bausch and Lomb,Germany)was used to carry out corneal incisions,anterior capsulotomy,and lens fragmentation in FLACS procedures.Manifest refraction spherical equivalence(MRSE),uncorrected distance visual acuity(UDVA),corrected distance visual acuity(CDVA),cumulative dissipated energy(CDE),postoperative cells and flare and endothelial cell count data were collected.Subgroup analysis of the visual acuity tests was performed based on the type of intraocular lens implanted(monofocal,monofocal toric,multifocal,multifocal toric,accommodating).Results:A total of 735 eyes were included in the study(296 eyes for the FLACS group and 439 eyes for the CP group).At one year follow-up,120 eyes comprised the FLACS group and 265 eyes for the CP group.MRSE in the FLACS group was−0.16±0.58 D and−0.20±0.52 D in the CP group(P=0.50).UDVA in the FLACS group was 20/25(mean logMAR 0.12±0.13)and 20/25(mean logMAR 0.11±0.13)in the CP group(P=0.48).CDVA was 20/20(mean logMAR 0.03±0.07)in the FLACS group and 20/20(mean logMAR 0.02±0.06)in the CP group(P=0.15).No statistically significant trend was seen for FLACS versus CP by intraocular type for visual acuity.CDE for the different cataract grades ranged from 6.97±5.74 to 29.02±16.07 in the FLACS group and 7.59±6.42 to 35.69±18.30 in the CP group.The FLACS group was significantly lower for post-operative central corneal edema(P=0.05),cells and flare(P=0.01),and endothelial cell loss(P=0.04).Conclusions:Femtosecond laser-assisted cataract surgery and conventional phacoemulsification had similar refractive and visual outcomes.Phacoemulsification energy,anterior chamber inflammation and corneal endothelial cell loss were less in the femtosecond laser group.Robert Edward Ty Ang Michelle Marie Salcedo Quinto Emerson Marquez Cruz Mark Christian Reyes Rivera Gladness Henna Austria Martinez 2018Eye and Vision2018,5,1:10
4History and Progress of the North American Soil Geochemical Landscapes Project,2001-2010显示文摘In 2007,the U.S.Geological Survey,the Geological Survey of Canada,and the Mexican Geological Survey initiated a low-density(1 site per 1600 km2,13323 sites) geochemical and mineralogical survey of North American soils(North American Soil Geochemical Landscapes Project).Sampling and analytical protocols were developed at a series of workshops in 2003-2004 and pilot studies were conducted from 2004-2007.The ideal sampling protocol at each site includes a sample from 0-5 cm depth,a composite of the soil A horizon,and a sample from the soil C horizon.The <2-mm fraction of each sample is analyzed for Al,Ca,Fe,K,Mg,Na,S,Ti,Ag,Ba,Be,Bi,Cd,Ce,Co,Cr,Cs,Cu,Ga,In,La,Li,Mn,Mo,Nb,Ni,P,Pb,Rb,Sb,Sc,Sn,Sr,Te,Th,Tl,U,V,W,Y,and Zn by inductively coupled plasma-mass spectrometry and inductively coupled plasma-atomic emission spectrometry following a near-total digestion in a mixture of HCl,HNO3,HClO4,and HF.Separate methods are used for As,Hg,Se,and total C on this same size fraction.The major mineralogical components are determined by a quantitative X-ray diffraction method.Sampling in the conterminous U.S.was completed in 2010(c.4800 sites) with chemical and mineralogical analysis currently underway.In Mexico,approximately 66% of the sampling(871 sites) had been done by the end of 2010 with completion expected in 2012.After completing sampling in the Maritime provinces and portions of other provinces(472 sites,7.6% of the total),Canada withdrew from the project in 2010.Preliminary results for a swath from the central U.S.to Florida clearly show the effects of soil parent material and climate on the chemical and mineralogical composition of soils.A sample archive will be established and made available for future investigations.David B.Smith William F.Cannon Laurel G.Woodruff Francisco Moreira Rivera Andrew N.Rencz Robert G.Garrett 2012地学前缘2012,19,3:7
5Dietary advanced glycation end-products aggravate non-alcoholic fatty liver disease显示文摘AIM To determine if manipulation of dietary advanced glycation end product(AGE), intake affects nonalcoholic fatty liver disease(NAFLD) progression and whether these effects are mediated via RAGE. METHODS Male C57Bl6 mice were fed a high fat, high fructose, high cholesterol(HFHC) diet for 33 wk and compared with animals on normal chow. A third group were given a HFHC diet that was high in AGEs. Another group was given a HFHC diet that was marinated in vinegar to prevent the formation of AGEs. In a second experiment, RAGE KO animals were fed a HFHC diet or a high AGE HFHC diet and compared with wildtype controls. Hepatic biochemistry, histology, picrosirius red morphometry and hepatic mR NA were determined. RESULTS Long-term consumption of the HFHC diet generated significant steatohepatitis and fibrosis after 33 wk. In this model, hepatic 4-hydroxynonenal content(a marker of chronic oxidative stress), hepatocyte ballooning, picrosirius red staining, α-smooth muscle actin and collagen type 1A gene expression were all significantly increased. Increasing the AGE content of the HFHC diet by baking further increased these markers of liver damage, but this was abrogated by pre-marination in acetic acid. In response to the HFHC diet, RAGE-/-animals developed NASH of similar severity to RAGE+/+ animals but were protected from the additional harmful effects of the high AGE containing diet. Studies in isolated Kupffer cells showed that AGEs increase cell proliferation and oxidative stress, providing a likely mechanism through which these compounds contribute to liver injury. CONCLUSION In the HFHC model of NAFLD, manipulation of dietary AGEs modulates liver injury, inflammation, and liver fibrosis via a RAGE dependent pathway. 6Role of 3'-untranslated region translational control in cancer development, diagnostics and treatment显示文摘The messenger RNA 3'-untranslated region(3'UTR)plays an important role in regulation of gene expres-sion on the posttranscriptional level. The 3'UTR con-trols gene expression via orchestrated interactionbetween the structural components of mRNAs(cis-ele-ment) and the specific trans-acting factors(RNA bind-ing proteins and non-coding RNAs). The crosstalk ofthese factors is based on the binding sequences and/or direct protein-protein interaction, or just functionalinteraction. Much new evidence that has accumulatedsupports the idea that several RNA binding factors canbind to common mRNA targets: to the non-overlappingbinding sites or to common sites in a competitive fash-ion. Various factors capable of binding to the sameRNA can cooperate or be antagonistic in their actions.The outcome of the collective function of all factorsbound to the same mRNA 3'UTR depends on manycircumstances, such as their expression levels, affinity to the binding sites, and localization in the cell, which can be controlled by various physiological conditions. Moreover, the functional and/or physical interactions of the factors binding to 3'UTR can change the character of their actions. These interactions vary during the cell cycle and in response to changing physiological condi-tions. Abnormal functioning of the factors can lead to disease. In this review we will discuss how alterations of these factors or their interaction can affect cancer development and promote or enhance the malignant phenotype of cancer cells. Understanding these altera-tions and their impact on 3'UTR-directed posttran-scriptional gene regulation will uncover promising new targets for therapeutic intervention and diagnostics. 7Hepatic intra-arterial infusion of yttrium-90 microspheres in the treatment of recurrent hepatocellular carcinoma after liver transplantation: A case report显示文摘Hepatocellular carcinoma (HCC) recurs with a reported frequency of 12%-18% after liver transplantation. Recurrence is associated with a mortality rate exceeding 75%. Approximately one-third of recurrences develop in the transplanted liver and are therefore amenable to local therapy. A variety of treatment modalities have been reported including resection, transarterial chemo- embolization (TACE), radiofrequency ablation (RFA), ethanol ablation, cryoablation, and external beam irradiation. Goals of treatment are tumor control and the minimization of toxic effect to functional parenchyma. Efficacy of treatment is mitigated by the need for ongoing immunosuppression. Yttrium-90 microspheres have been used as a treatment modality both for primary HCC and for pre-transplant management of HCC with promising results. Twenty-two months after liver transplantation for hepatitis C cirrhosis complicated by HCC, a 42-year old man developed recurrence of HCC in his transplant allograft. Treatment of multiple right lobe lesions with anatomic resection and adjuvant chemotherapy was unsuccessful. Multifocal recurrence in the remaining liver allograft was treated with hepatic intra-arterial infusion of yttrium-90 microspheres (SIR-Spheres, Sirtex Medica Inc., Lake Forest, IL, USA). Efficacy was demonstrated by tumor necrosis on imaging and a decrease in alpha-fetoprotein (AFP) level. 8Antimicrobial activity,cytotoxicity,and phytochemical screening of Voacanga globosa(Blanco) Merr.leaf extract(Apocynaceae)显示文摘Objective:To determine the antibacterial,antifungal,antiprotozoal,cytotoxic,and phytochemical properties of ethanol extracts of leaves of Voacanga globosa(Blanco) Merr.(V.globosa). Methods:The extracts were tested against bacteria and fungus through disc diffusion assay; against protozoa through growth curve determination,antiprotozoal and cytotoxicity assays. Results:The extract revealed antibacterial activities,inhibiting the growth of Staphylococcus aureus,Bacillus cereus,Pseudomonas aeruginosa,Micrococcus luteus,and Salmonella typhimurium.Antifungal assay showed that it inhibited Candida albicans.The antiprotozoal assay against Trichomonas vaginalis and Entamoeba histolytica showed that V.globosa can inhibit the parasites,wherein the action can be comparable to metronidazole.With the in situ cell death detection kit.Trichomonas vaginalis and Entamoeba histolytica exposed to V.globosa leaf extract was observed to fluoresce simultaneously in red and yellow signals signifying apoptotic-like changes.Preliminary phytochemical screening revealed the chemical composition of plant extract containing alkaloids,saponins,2-deoxysugars,and hydrolysabie tannins.Conclusions: Thus,thus study provides scientific evidence on the traditional use of V.globosa leaf extract in treating microbial diseases.Further,the leaf extract can possibly be used to produce alternative forms of antimicrobials.Pierangeli G Vital Windell L Rivera 2011Asian Pacific Journal of Tropical Medicine2011,4,10:5

12Aminoguanidine impedes human pancreatic tumor growth and metastasis development in nude mice显示文摘AIM:To study the action of aminoguanidine on pancreatic cancer xenografts in relation to cell proliferation,apoptosis,redox status and vascularization.METHODS:Xenografts of PANC-1 cells were developed in nude mice. The animals were separated into two groups:control and aminoguanidine treated. Tumor growth,survival and appearance of metastases were determined in vivo in both groups. Tumors were excised and ex vivo histochemical studies were performed. Cell growth was assessed by Ki-67 expression. Apoptosis was studied by intratumoral expression of B cell lymphoma-2 protein (Bcl-2) family proteins and Terminal deoxynucleotidyl transferase biotin-dUTP Nick End Labeling (Tunel). Redox status was evaluated by the expression of endothelial nitric oxide synthase (eNOS),catalase,copper-zinc superoxide dismutase (CuZnSOD),manganese superoxide dismutase (MnSOD) and glutathione peroxidase (GPx). Finally,vascularization was determined by Massons trichromic staining,and by VEGF and CD34 expression.RESULTS:Tumor volumes after 32 d of treatment by aminoguanidine (AG) were significantly lower than in control mice (P < 0.01). Median survival of AG mice was significantly greater than control animals (P < 0.01). The appearance of both homolateral and contralateral palpable metastases was significantly delayed in AG group. 13Accuracy of biometric formulae for intraocular lens power calculation in a teaching hospital显示文摘AIM: To evaluate the accuracy of three commonly used biometric formulae across different axial lengths(ALs) at one United States Veterans Affairs teaching hospital.METHODS: A retrospective chart review was conducted from November 2013 to May 2018. One eye of each patient who underwent cataract surgery with a monofocal intraocular lens(IOL) was included. The range of postoperative follow-up period was from 3 wk to 4 mo. The Holladay 2, Barrett Universal II, and Hill-Radial Basis Function(Hill-RBF) formulae were used to predict the postoperative refraction for all cataract surgeries. For each formula, we calculated the prediction errors [including mean absolute prediction error(MAE)] and the percentage of eyes within ±0.25 diopter(D) and ±0.5 D of predicted refraction. We performed subgroup analyses for short(AL<22.0 mm), medium(AL 22.0-25.0 mm), and long eyes(AL>25.0 mm).RESULTS: A total of 1131 patients were screened, and 909 met the inclusion criteria. Resident ophthalmologists were the primary surgeons in 710(78.1%) cases. We found no statistically significant difference in predictive accuracy among the three formulae over the entire AL range or in the short, medium, and long eye subgroups. Across the entire AL range, the Hill-RBF formula resulted in the lowest MAE(0.384 D) and the highest percentage of eyes with postoperative refraction within ±0.25 D(42.7%) and ±0.5 D(75.5%) of predicted. All three formulae had the highest MAEs(>0.5 D) and lowest percentage within ±0.5 D of predicted refraction(<55%) in short eyes.CONCLUSION: In cataract surgery patients at our teaching hospital, three commonly used biometric formulae demonstrate similar refractive accuracy across all ALs. 14Comparison of direct fecal smear microscopy,culture,and polymerase chain reaction for the detection of Blastocystis sp.in human stool samples显示文摘Objective:To compare the sensitivity and specificity of direct fecal smear microscopy,culture,and polymerase chain reaction in the detection of Blastocystis sp.in human stool.Methods:Human stool samples were collected from a community in San Isidro,Rodriguez,Rizal,Philippines.These samples were subjected to direct fecal smear microscopy,culture and polymerase chain reaction to detect the presence of Blastocystis sp.Results:Of the 110 stool samples collected,28(25%)were detected positive for the presence of Blastocystis sp.by two or more tests.Culture method detected the highest number of Blastocystis-positive stool samples(n=36),followed by PCR of DNA extracted from culture(n=26),PCR of DNA extracted from stool(n=10),and direct fecal smear(n=9).Compared to culture,the sensitivity of the other detection methods were 66.7%for PCR from culture and 19.4%for both PCR from stool and direct fecal smear.Specificity of the methods was high,with PCR from culture and direct fecal smear having97.3%,while PCR from stool at 95.9%.Conclusions:In this study,in vitro culture is the best method for detecting Blastocystis sp.in human stool samples.Herbert J Santos Windell L Rivera 2013Asian Pacific Journal of Tropical Medicine2013,6,10:3
15Approach to early-onset colorectal cancer:Clinicopathological,familial,molecular and immunohistochemical characteristics显示文摘AIM:To characterize clinicopathological and familial features of early-onset colorectal cancer(CRC) and compare features of tumors with and without microsatellite instability(MSI).METHODS:Forty-five patients with CRC aged 45 or younger were included in the study.Clinical information,a three-generation family history,and tumor samples were obtained.MSI status was analyzed and mismatch repair genes were examined in the MSI families.Tumors were included in a tissue microarray and an immunohistochemical study was carried out with a panel of selected antibodies.RESULTS:Early onset CRC is characterized by advanced stage at diagnosis,right colon location,low-grade of differentiation,mucin production,and presence of polyps.Hereditary forms represent at least 21% of cases.Eighty-one percent of patients who died during followup showed a lack of expression of cyclin E,which could be a marker of poor prognosis.β-catenin expression was normal in a high percentage of tumors.CONCLUSION:Early-onset CRC has an important familial component,with a high proportion of tumors showing microsatellite stable.Cyclin E might be a poor prognosis factor.Jose Perea Edurne Alvaro Yolanda Rodríguez Cristina Gravalos Eva Sánchez-Tomé Barbara Rivera Francisco Colina Pablo Carbonell Rogelio González-Sarmiento Manuel Hidalgo Miguel Urioste 2010World Journal of Gastroenterology2010,16,29:3
