| 1 | Complications of hip fractures: A review显示文摘Nowadays, fracture surgery represents a big part of the orthopedic surgeon workload, and usually has associated major clinical and social cost implications. These fractures have several complications. Some of these are medical, and other related to the surgical treatment itself. Medical complications may affect around 20% of patients with hip fracture. Cognitive and neurological alterations, cardiopulmonary affections(alone or combined), venous thromboembolism, gastrointestinal tract bleeding, urinary tract complications, perioperative anemia, electrolytic and metabolic disorders, and pressure scars are the most important medical complications after hip surgery in terms of frequency, increase of length of stay and perioperative mortality. Complications arising from hip fracture surgery are fairly common, and vary depending on whether the fracture is intracapsular or extracapsular. The main problems in intracapsular fractures are biological: vascularization of the femoral head, and lack of periosteum-a major contributor to fracture healing- in the femoral neck. In extracapsularfractures, by contrast, the problem is mechanical, and relates to load-bearing. Early surgical fixation, the role of anti-thromboembolic and anti-infective prophylaxis, good pain control at the perioperative, detection and management of delirium, correct urinary tract management, avoidance of malnutrition, vitamin D supplementation, osteoporosis treatment and advancement of early mobilization to improve functional recovery and falls prevention are basic recommendations for an optimal maintenance of hip fractured patients. | Pedro Carpintero Jose Ramón Caeiro Rocío Carpintero Angela Morales Samuel Silva Manuel Mesa | 2014 | World Journal of Orthopedics2014，5，4： | 31 |

| 7 | 在经历 transcatheter 的病人的 atrial 纤维性颤动的临床、预示的含意大动脉的阀门培植显示文摘 AIM: To study a cohort of consecutive patients under-going transcatheter aortic valve implantation (TAVI) and compare the outcomes of atrial fibrillation (AF) patients vs patients in sinus rhythm (SR). METHODS: All consecutive patients undergoing TAVI in our hospital were included. The AF group comprised patients in AF at the time of TAVI or with history of AF, and were compared with the SR group. Procedural, echocardiographic and follow-up variables were compared. Likewise, the CHA 2 DS 2-VASC stroke risk score and HAS-BLED bleeding risk score and antithrombotic treatment at discharge in AF patients were compared with that in SR patients. RESULTS: From a total of 34 patients undergoing TAVI, 17 (50%) were allocated to the AF group, of whom 15 (88%) were under chronic oral anticoagulation. Patients in the AF group were similar to those in the SR group except for a trend (P = 0.07) for a higher logistic EuroSCORE (28% vs 19%), and a higher prevalence of hypertension (82% vs 53%) and chronic renal failure (17% vs 0%). Risk of both stroke and bleeding was high in the AF group (mean CHA 2 DS 2-VASC 4.3, mean HAS-BLED 2.9). In the AF group, treatment at discharge included chronic oral anticoagulation in all except one case, and in association with an antiplatelet drug in 57% of patients. During a mean follow-up of 11 mo (maximum 32), there were only two strokes, none of them during the peri-procedural period: one in the AF group at 30 mo and one in the SR group at 3 mo. There were no statistical differences in procedural success, and clinical outcome (survival at 1 year 81% vs 74% in AF and SR groups, respectively, P = NS). CONCLUSION: Patients in AF undergoing TAVI show a trend to a higher surgical risk. | 8 | Immediate effect of high-intensity exercise on brain-derived neurotrophic factor in healthy young adults:A systematic review and meta-analysis显示文摘Background Although brain-derived neurotrophic factor(BDNF)has been identified as a molecular biomarker of the neurophysiological effects induced by exercise,the acute effects of high-intensity exercise(HIE)on BDNF levels are inconclusive.This study aims to estimate the immediate effects of HIE on BDNF levels in healthy young adults.Methods A systematic search was conducted in the MEDLINE,Scopus,Cochrane CENTRAL,and SPORTDiscuss databases up to December 2020.Randomized controlled trials(RCTs)and non-RCTs reporting pre-post changes in serum or plasma BDNF after an acute intervention of HIE compared to a control condition were included.Pooled effect sizes(p-ESs)and 95%confidence intervals(95%CIs)were calculated for RCTs using a random effects model with Stata/SE(Version 15.0;StataCorp.,College Station,TX,USA).The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed.PROPERO registration number:CRD42020221047.Results A total of 22 studies with 552 individuals(age range:20-31 years;59.1%male)were included.The meta-analysis included 10 RCTs that reported valid outcome data.Higher BDNF levels were observed when HIE interventions were compared with non-exercise(p-ES=0.55,95%CI:0.12-0.98;I^(2)=25.7%;n=4 studies)and light-intensity exercise(p-ES=0.78,95%CI:0.15-1.40;I^(2)=52.4%;n=3 studies)but not moderate-intensity exercise(p-ES=0.93,95%CI:-0.16 to 2.02;I^(2)=88.5%;n=4 studies)conditions.Conclusion In comparison to non-exercise or light-intensity exercises,an immediate increase in BDNF levels may occur when young adults perform HIE.Given the benefits obtained maximizing circulating BDNF when performing HIE and its potential effects on brain health,our findings suggest that HIE could be recommended by clinicians as a useful exercise strategy to healthy adults. | Rubén Fernández-Rodríguez CeliaÁlvarez-Bueno Isabel A.Martínez-Ortega Vicente Martínez-Vizcaíno Arthur Eumann Mesas Blanca Notario-Pacheco | 2022 | Journal of Sport and Health Science2022，11，3： | 2 |
