1Prevalence and predictors of nonalcoholic fatty liver disease in South Asian women with polycystic ovary syndrome显示文摘BACKGROUND Polycystic ovary disease(PCOS)may be a risk factor for nonalcoholic fatty liver disease(NAFLD)due to common pathogenetic pathways,including insulin resistance and obesity.Both PCOS and NAFLD are more severe in South Asian women.Data on NAFLD in South Asian women with PCOS are lacking.AIM To investigate prevalence and predictors of NAFLD and liver fibrosis in PCOS patients from South Asia.METHODS We conducted an observational routine screening program by means of transient elastography(TE)with associated controlled attenuation parameter(CAP).NAFLD was defined as CAP≥288 decibels per meter.Significant liver fibrosis(stage 2 and higher out of 4)was defined as TE measurement≥8.0 kilopascals.Elevated alanine aminotransferase(ALT)was defined as ALT>24 IU/L,as per upper limit of normal reported in South Asian women.Biochemical hyperandrogenism was defined as free androgen index>5.Predictors of NAFLD were determined by logistic regression analysis.RESULTS 101 PCOS patients(mean age 36.3 years)with no significant alcohol intake or viral hepatitis were included.Prevalence of NAFLD and significant liver fibrosis was 39.6% and 6.9%,respectively.Elevated ALT was observed in 40%and 11.5%of patients with and without NAFLD,respectively.After adjusting for duration of PCOS and insulin resistance measured by homeostasis model for assessment of insulin resistance,independent predictors of NAFLD were higher body mass index[adjusted odds ratio(aOR)1.30,95% confidence interval(CI):1.13-1.52],hyperandrogenism(aOR:5.32,95%CI:1.56-18.17)and elevated ALT(aOR:3.54,95%CI:1.10-11.47).Lifetime cardiovascular risk was higher in patients with NAFLD compared to those without NAFLD(0.31±0.11 vs 0.26±0.13).CONCLUSION Despite their young age,NAFLD diagnosed by TE with CAP is a frequent comorbidity in South Asian women with PCOS and is strongly associated with higher body mass index and hyperandrogenism.Non-invasive screening strategies could help early diagnosis and initiation of interventions,including counselling on weight loss,cardiovascular risk stratification and linkage to hepatology care where appropriate.Mohamed Shengir Srinivasan Krishnamurthy Peter Ghali Marc Deschenes Philip Wong Tianyan Chen Giada Sebastiani 2020World Journal of Gastroenterology2020,26,44:11
2Operative complications and economic outcomes of cholecystectomy for acute cholecystitis显示文摘BACKGROUND Recent management of acute cholecystitis favors same admission(SA)or emergent cholecystectomy based on overall shorter hospital stay and therefore cost savings.We adopted the practice of SA cholecystectomy for the treatment of acute cholecystitis at our tertiary care center and wanted to evaluate the economic benefit of this practice.We hypothesized that the existence of complications,particularly among patients with a higher degree of disease severity,during SA cholecystectomy could negate the cost savings.AIM To compare complication rates and hospital costs between SA vs delayed cholecystectomy among patients admitted emergently for acute cholecystitis.METHODS Under an IRB-approved protocol,complications and charges for were obtained for SA,later after conservative management(Delayed),or elective cholecystectomies over an 8.5-year period.Patients were identified using the acute care surgery registry and billing database.Data was retrieved via EMR,operative logs,and Revenue Cycle Operations.The severity of acute cholecystitis was graded according to the Tokyo Guidelines.TG18 categorizes acute cholecystitis by Grades 1,2,and 3 representing mild,moderate,and severe,respectively.Comparisons were analyzed withχ2,Fisher’s exact test,ANOVA,ttests,and logistic regression;significance was set at P<0.05.RESULTS Four hundred eighty-six(87.7%)underwent a SA while 68 patients(12.3%)received Delayed cholecystectomy.Complication rates were increased after SA compared to Delayed cholecystectomy(18.5%vs 4.4%,P=0.004).The complication rates of patients undergoing delayed cholecystectomy was similar to the rate for elective cholecystectomy(7.4%,P=0.35).Mortality rates were 0.6%vs 0%for SA vs Delayed.Patients with moderate disease(Tokyo 2)suffered more complications among SA while none who were delayed experienced a complication(16.1%vs 0.0%,P<0.001).Total hospital charges for SA cholecystectomy were increased compared to a Delayed approach($44500±$59000 vs$35300±$16700,P=0.019).The relative risk of developing a complication was 4.2x[95%confidence interval(CI):1.4-12.9]in the SA vs Delayed groups.Among eight patients(95%CI:5.0-12.3)with acute cholecystitis undergoing SA cholecystectomy,one patient will suffer a complication.CONCLUSION Patients with Tokyo Grade 2 acute cholecystitis had more complications and increased hospital charges when undergoing SA cholecystectomy.This data supports a selective approach to SA cholecystectomy for acute cholecystitis.Christopher P Rice Krishnamurthy B Vaishnavi Celia Chao Daniel Jupiter August B Schaeffer Whitney R Jenson Lance W Griffin William J Mileski 2019World Journal of Gastroenterology2019,25,48:8

4Effect of process parameters on microstructural and mechanical properties of Ti.SS 304L explosive cladding显示文摘The present work pertains to the study of microstructure and mechanical properties of explosively cladded commercially pure titanium(cpTi) with austenitic stainless steel(SS 304L) subjected to varied process parameters, viz., loading ratios(mass of explosive or mass of flyer plate) and preset angles. The microstructural study reveals the transformation from straight to wavy interface while increasing loading ratios. Vickers hardness increases with loading ratios, and the maximum hardness is witnessed in the closer proximity of collision interface. Ram tensile and shear strength of explosive claddings are higher than that of parent plates. While the base plate fails in impact test, the flyer plate is deformed, indicating good impact strength. Increased mass of claddings, due to oxide formation, is witnessed in corrosion tests, which confirms the superiority of explosive claddings in corrosive environment. 5Inhibitory effect of two Indian medicinal plants on aldose reductase of rat lens in vitro显示文摘Objective:To assesse the inhibitor) effect of alcoholic extract of two Indian medicinal plants namely Ceasalpinia digyna Rottler and.Alangium lamarckii Thwaits on aldose reductase(AR) of rat lens.Methods:Rats lens were enucleated through posterior approach and their homogenate was prepared and centrifuged to obtain a clear supernatant for the determination of AR activity and protein content.Results:The alcoholic extract of Ceasalpirda digyna and Alangium lamarckii had a potent inhibitory effect on the lens AR enzyme.The IC50 values of alcoholic extract of the selected plants were calculated and were(46.29±11.17) and(106.00±5.11)μg/mL,respectively. 7Salivary gland disorders:A comprehensive review显示文摘Salivary glands are complex in nature. They could be either tubulo acinar, merocrine or exocrine glands secreting mainly saliva. Salivary gland is one of the main soft tissue structures in the maxillofacial area. Saliva is a clear, slightly acidic muco serous fluid that coats the teeth, mucosa and thereby helps to create and maintain a healthy environment in the oral cavity. Salivary glands may be affected by a number of diseases: local and systemic and the prevalence of salivary gland diseases depend on various etiological factors. The glands may be infected byviral, bacterial, rarely fungal or its ductal obstruction which may cause painful swelling or obstruction, affecting their functions. The salivary gland may also be affected by a various benign and malignant tumours. 8Omentum facilitates liver regeneration显示文摘AIM:To investigate the mechanism of liver regeneration induced by fusing the omentum to a small traumatic injury created in the liver. We studied three groups of rats. In one group the rats were omentectomized; in another group the omentum was left in situ and was not activated,and in the third group the omentum was activated by polydextran particles. METHODS:We pre-activated the omentum by injecting polydextran particles and then made a small wedge wound in the rat liver to allow the omentum to fuse to the wound. We monitored the regeneration of the liver by determining the ratio of liver weight/body weight,by histological evaluation (including immune staining for cytokeratin-19,an oval cell marker),and by testing for developmental gene activation using reverse transcription polymerase chain reaction (RT-PCR). RESULTS:There was no liver regeneration in the omentectomized rats,nor was there significant regeneration when the omentum was not activated,even though in this instance the omentum had fusedwith the liver. In contrast,the liver in the rats with the activated omentum expanded to a size 50% greater than the original,and there was histologically an interlying tissue between the wounded liver and the activated omentum in which bile ducts,containing cytokeratin-19 positive oval cells,extended from the wound edge. In this interlying tissue,oval cells were abundant and appeared to proliferate to form new liver tissue. In rats pre-treated with drugs that inhibited hepatocyte growth,liver proliferation was ongoing,indicating that regeneration of the liver was the result of oval cell expansion. 9Gas solid techniques for preparation of pure lanthanum hexaboride显示文摘The processes reported for the preparation of lanthanum hexaboride (LaB6) from lanthanum oxide involve the use of carbon either elemental or in the form of boron carbide or elemental boron itself as reducing agents, fused salts at high temperatures or reactions which require the product boride to be leached out. Each of these techniques either increases the process costs and/or increases chances of contamination in fi-nal product. Pure LaB6 can best be prepared by a reaction which produces a gaseous byproduct. In the present study, such a reaction was successfully used to yield pure lanthanum hexaboride. The process involved mixing of anhydrous lanthanum chloride with aluminium and boron and heating the charges under dynamic argon flow. Lanthanum chloride is known to be highly hygroscopic; hence the process using improperly dehydrated LaCl3 led to the formation of lanthanum oxychloride which does not convert to LaB6 under conditions wherein LaCl3 converts. 13Expedited extraction of xylan from corncob by power ultrasound显示文摘Some lignocellulosic biomass like corncob contains a large amount of xylan,a valuable material for applications in food,chemical and pharmaceutical industries.Conventional extraction of xylan is a time consuming process,requiring 12 to 24 hours to complete.In this study,the efficacy of power ultrasound for reducing the extraction time and increasing the xylan yield was investigated.Two acoustic energy densities(0.18 W/mL and 0.45 W/mL)of the power ultrasound were applied to corncob for 10,20,30,and 60 min.The treated samples were steamed at 110,118 and 121℃for 30 min.At 0.45 W/mL acoustic energy density and 121℃steaming temperature,ultrasonic treatment was able to extract 39%of xylan in 43 min.In contrast,the conventional treatment was able to extract only 34%of xylan in 24 h.Power ultrasound assisted extraction reduced the processing time by 97%in addition to yield increase by 14.7%.Power ultrasound can be a potential technology for enhanced xylan extraction from corncob to result in significant processing time savings.Wade Yang Vishnu K.Ajapur Kathiravan Krishnamurthy Hao Feng Ruijin Yang Taha M.Rababah 2009International Journal of Agricultural and Biological Engineering2009,2,4:2
