1Etiopathogenesis of primary biliary cirrhosis显示文摘Primary biliary cirrhosis(PBC) is an autoimmune disease of the liver characterized by progressive bile duct destruction eventually leading to cirrhosis and liver failure.The serological hallmark of the disease is the presence of circulating antimitochondrial antibodies(AMA).These reflect the presence of autoreactive T and B cells to the culprit antigens,the E2 subunits of mitochondrial 2-oxo-acid dehydrogenase enzymes,chiefly pyruvate dehydrogenase(PDC-E2).The disease results from a combination of genetic and environmental risk factors.Genetic predisposition is indicated by the higher familial incidence of the disease particularly among siblings and the high concordance rate among monozygotic twins.Environmental triggering events appear crucial to disrupt a pre-existing unstable immune tolerance of genetic origin allowing,after a long latency,the emergence of clinical disease.Initiating mimetopes of the vulnerable epitope of the PDC-E2 autoantigen can be derived from microbes that utilize the PDC enzyme or,alternatively,environmental xenobiotics/chemical compounds that modify the structure of native proteins to make them immunogenic.A further alternative as a source of antigen is PDC-E2 derived from apoptotic cells.In the effector phase the biliary ductular cell,by reason of itsproclivity to express the antigen PDC-E2 in the course of apoptosis,undergoes a multilineage immune attack comprised of CD4+ and CD8+ T cells and antibody.In this article,we critically review the available evidence on etiopathogenesis of PBC and present interpretations of complex data,new developments and theories,and nominate directions for future research.Ana Lleo Pietro Invernizzi Ian R Mackay Harry Prince Ren-Qian Zhong M Eric Gershwin 2008World Journal of Gastroenterology2008,14,21:9

6两步法翻修术治疗全膝关节置换术后深部感染显示文摘目的探讨治疗全膝关节置换术(TKA)术后深部感染的有效措施。方法对自2002年2月-2006年9月收治的12例TKA术后感染患者采用两步法进行翻修,术后记录不同时期的关节功能恢复情况。结果术后随访平均11.2个月(6-36个月)。参照HSS膝关节功能评分.就诊时、第1次术后及第2次术后平均HSS评分分别为(47.50±17.62)分(28-77分)、(68.42±16.95)分(41-90分)及(81.75±12.97)分(56~97分),(P〈0.05)。结论两步法纯骨水泥压模假体技术可以增加关节内的抗生素浓度,有效控制感染;能保持膝关节活动度,是有效的治疗TKA术后感染的方法。张经纬 蒋垚 张先龙 Ian Harris 王琦 沈灏 邵俊杰 狄正林 曾炳芳 2009中国骨与关节损伤杂志2009,24,7:3

9A fast and accurate model for the creation of explosion fragments with improved fragment shape and dimensions显示文摘Explosion models based on Finite Element Analysis(FEA)can be used to simulate how a warhead fragments.However their execution times are extensive.Active protection systems need to make very fast predictions,before a fast attacking weapon hits the target.Fast execution times are also needed in real time simulations where the impact of many different computer models is being assessed.Hence,FEA explosion models are not appropriate for these real-time systems.The research presented in this paper delivers a fast simulation model based on Mott’s equation that calculates the number and weight of fragments created by an explosion.In addition,the size and shape of fragments,unavailable in Mott’s equation,are calculated using photographic evidence and a distribution of a fragment’s length to its width.The model also identifies the origin of fragments on the warhead’s casing.The results are verified against experimental data and a fast execution time is achieved using uncomplicated simulation steps.The developed model then can be made available for real-time simulation and fast computation.David Felix Ian Colwill Paul Harris 2022Defence Technology(防务技术)2022,18,2:2

11关节置换术治疗急性关节内骨折股骨头置换术与全髋关节置换术治疗股骨近端骨折的比较(对2007年研究的更新)显示文摘5项随机对照研究(RCT)或半RCT的证据提示,对于股骨近端骨折,与全髋关节置换术(THA)治疗相比,股骨头置换术(HA)治疗虽然会增加术后翻修的几率,但是术后关节脱位的发生率则明显降低。接受HA治疗的患者后期活动能力恢复可能较差,但是各研究间的统计学结果并不一致。同时基于目前报道的各种假体类型,两种治疗方法间死亡率比较差异无统计学意义。由于现有研究在方法学和假体类型选择上不一致,因此在解读本研究相关结论时仍需谨慎。Ian Harris 杨云峰 2012中华创伤骨科杂志2012,14,8:2
