3Contemporary role of multi-tract percutaneous nephrolithotomy in the treatment of complex renal calculi显示文摘Complex renal calculi remain to be a challenge for the treating urologist due to sheer bulk and the technicalities involved.Percutaneous nephrolithotomy(PCNL)remains the treatment modality of choice in dealing with these large and complex stones.The limiting factor in their treatment continues to be the need for using additional tracts or the use of flexible nephroscopy for complete stone clearance.This systematic review focuses on the need for multi-tract PCNL for complex renal calculi.The literature review was performed using PubMed database using the keywords“multiple tract PCNL”or“multiperc”.We identified original articles published on the usage of multiple tracts for stone clearance in renal calculi between January 2000 to October 2018,and the search was restricted to available literature in English language only.Ten studies with n>20 were included for the final analysis.We analyzed the technical efficacy with respect to the number of tracts and stages that were required for stone clearance,outcomes and complications,especially,procedural bleeding and post-procedure infective complications of multiple-tract PCNL for large burden renal stones.Multiperc is found to be safe,feasible and effective for the management of large burden complex renal calculi with respect to stone clearance and morbidity associated with the procedure.It is cost effective and complete stone clearance as a single procedure is higher in comparison to flexible ureteroscopy and shockwave lithotripsy.Sudharsan Balaji Arvind Ganpule Thomas Herrmann Ravindra Sabnis Mahesh Desai 2020Asian Journal of Urology2020,7,2:11
4Online Adaptive Approximate Optimal Tracking Control with Simplified Dual Approximation Structure for Continuous-time Unknown Nonlinear Systems显示文摘This paper proposes an online adaptive approximate solution for the infinite-horizon optimal tracking control problem of continuous-time nonlinear systems with unknown dynamics.The requirement of the complete knowledge of system dynamics is avoided by employing an adaptive identifier in conjunction with a novel adaptive law, such that the estimated identifier weights converge to a small neighborhood of their ideal values.An adaptive steady-state controller is developed to maintain the desired tracking performance at the steady-state, and an adaptive optimal controller is designed to stabilize the tracking error dynamics in an optimal manner. For this purpose, a critic neural network(NN) is utilized to approximate the optimal value function of the Hamilton-Jacobi-Bellman(HJB) equation,which is used in the construction of the optimal controller. The learning of two NNs, i.e., the identifier NN and the critic NN, is continuous and simultaneous by means of a novel adaptive law design methodology based on the parameter estimation error.Stability of the whole system consisting of the identifier NN,the critic NN and the optimal tracking control is guaranteed using Lyapunov theory; convergence to a near-optimal control law is proved. 5Serum levels of soluble Fas, nitric oxide and cytokines in acute decompensated cirrhotic patients显示文摘AIM: To evaluate plasma levels of nitrite/nitrate (NOx), soluble Fas (sFas) antigen, tumor necrosis factor alpha (TNF-α) and interleukin-6 (IL-6) in patients with com- pensated and acute decompensated cirrhosis and to evaluate mediators causing acute decompensation in liver cirrhosis. METHODS: This prospective study was conducted in the medical intensive care unit of an academic tertiary center. Fifty-five patients with acute decompensation (gastrointestinal hemorrhage, encephalopathy, hydropic decompensation) and twenty-five patients with compen- sated liver cirrhosis were included. Blood samples were taken for analyses of sFas, Nox, IL-6, TNF-α. Liver en- zymes and kidney functions were also tested. RESULTS: In patients with acute decompensation, plas- ma sFas levels were higher than in non-decompensated patients (15 305 ±4646 vs 12 458 ±4322 pg/mL,, P < 0.05). This was also true for the subgroup of patients with alcoholic liver cirrhosis (P < 0.05). The other media- tors were not different and none of the parameters pre- dicted survival, except for ALT (alanine-aminotransfer- ase). In patients with portal-hypertension-induced acute hemorrhage, NOx levels were significantly lower than in patients with other forms of decompensation (70.8 ± 48.3 vs 112.9 ± 74.9 pg/mL, P < 0.05). When NOx lev- els were normalized to creatinine levels, the difference disappeared. IL-6, TNF-α and sFas were not different between bleeders and non-bleeders. In decompensated patients sFas, IL-6 and NOx levels correlated positively with creatinine levels, while IL-6 levels were dependent on Child class. CONCLUSION: In acute decompensated cirrhotic pa- tients sFas is increased, suggesting a role of apoptosis in this process and patients with acute bleeding have lower NOx levels. 61999年海城-岫岩地震序列的精准定位显示文摘1999年11月29日,北京时间12点10分39秒(04:10:39UTC)在我国辽宁海城—岫岩地区发生了一次MS5.4地震.这次地震是继1975年2月4日海城MS7.3地震之后该地区发生的震级最大的一次地震.区域性的辽宁数字地震台网记录了这一序列的前震、主震与余震.本文运用该台网的记录资料,用双差法对海城—岫岩地震序列重新定位,得到了该序列事件之间精确的相对位置;然后以中国地震局地球物理研究所(IGCEA)与美国南卫理大学(SMU)合作的辽宁海城—岫岩地区地震试验场宽频带地震台网(BSN)记录的、经过精准定位的地震作为近震源观测的'地面实况'(groundtruth,GT)事件进行校正,得到了这一地震序列的既精确又准确的震源参数.由此得出1999年海城—岫岩地震的发震构造是与1975年海城地震的发震构造海城河—大洋河断裂邻近的康家岭断裂.康家岭断裂的走向与海城河—大洋河断裂走向一致,均为WNW--ESE方向.它们都是在中国东北—华北地区近ENE--WSW向、几乎水平的压应力场作用下产生的断裂.1999年海城—岫岩地震是在海城河—大洋河断裂的影响带动下,邻近的、与其走向一致的康家岭断裂活动的结果.杨智娴 Brian W Stump 陈运泰 Robert B Herrmann Rongmao Zhou Chris T Hayward 2011地震学报2011,33,3:9

8Metasurface interferometry toward quantum sensors显示文摘Optical metasurfaces open new avenues for the precise wavefront control of light for integrated quantum technology.Here,we demonstrate a hybrid integrated quantum photonic system that is capable of entangling and disentangling two-photon spin states at a dielectric metasurface.Via the interference of single-photon pairs at a nanostructured dielectric metasurface,a path-entangled two-photon NOON state with circular polarization that exhibits a quantum HOM interference visibility of 86±4% is generated.Furthermore,we demonstrate nonclassicality andphase sensitivity in a metasurface-based interferometer with a fringe visibility of 86.8±1.1%in the coincidence counts.This high visibility proves the metasurface-induced path entanglement inside the interferometer.Our findings provide a promising way to develop hybrid-integrated quantum technology operating in the high-dimensional mode space in various applications,such as imaging,sensing,and computing.Philip Georgi Marcello Massaro Kai-Hong Luo Basudeb Sain Nicola Montaut Harald Herrmann Thomas Weiss Guixin Li Christine Silberhorn Thomas Zentgraf 2019Light(Science & Applications)2019,8,1:7
9Circulating metastasis associated in colon cancer 1 transcripts in gastric cancer patient plasma as diagnostic and prognostic biomarker显示文摘AIM: To evaluate the diagnostic and prognostic value of circulating Metastasis Associated in Colon Cancer 1(MACC1) transcripts in plasma of gastric cancer patients.METHODS: We provide for the first time a blood-based assay for transcript quantification of the metastasis inducer MACC1 in a prospective study of gastric cancer patient plasma.MACC1 is a strong prognostic biomarker for tumor progression and metastasis in a variety of solid cancers.We conducted a study to define the diagnostic and prognostic power of MACC1 transcripts using 76 plasma samples from gastric cancer patients,either newly diagnosed with gastric cancer,newly diagnosed with metachronous metastasis of gastric cancer,as well as follow-up patients.Findings were controlled by using plasma samples from 54 tumor-free volunteers.Plasma was separated,RNA was isolated,and levels of MACC1 as well as S100A4 transcripts were determined by quantitative RT-PCR.RESULTS: Based on the levels of circulating MACC1 transcripts in plasma we significantly discriminated tumorfree volunteers and gastric cancer patients(P < 0.001).Levels of circulating MACC1 transcripts were increased in gastric cancer patients of each disease stage,compared to tumor-free volunteers: patients with tumors without metastasis(P = 0.005),with synchronous metastasis(P = 0.002),with metachronous metastasis(P = 0.005),and patients during follow-up(P = 0.021).Sensitivity was 0.68(95%CI: 0.45-0.85) and specificity was 0.89(95%CI: 0.77-0.95),respectively.Importantly,gastric cancer patients with high circulating MACC1 transcript levels in plasma demonstrated significantly shorter survival whencompared with patients demonstrating low MACC1 levels(P = 0.0015).Furthermore,gastric cancer patients with high circulating transcript levels of MACC1 as well as of S100A4 in plasma demonstrated significantly shorter survival when compared with patients demonstrating low levels of both biomarkers or with only one biomarker elevated(P = 0.001).CONCLUSION: Levels of circulating MACC1 transcripts in plasma of gastric cancer patients are of diagnostic value and are prognostic for patient survival in a prospective study.Susen Burock Pia Herrmann Ina Wendler Markus Niederstrasser Klaus-Dieter Wernecke Ulrike Stein 2015World Journal of Gastroenterology2015,21,1:7
10Decomposition dynamics of coarse woody debris of three important central European tree species显示文摘Background:Coarse woody debris(CWD)is an important element of forest structure that needs to be considered when managing forests for biodiversity,carbon storage or bioenergy.To manage it effectively,dynamics of CWD decomposition should be known.Methods:Using a chronosequence approach,we assessed the decomposition rates of downed CWD of Fagus sylvatica,Picea abies and Pinus sylvestris,which was sampled from three different years of tree fall and three different initial diameter classes(>10 –≤ 20 cm,>20 –≤40 cm,>40 cm).Samples originating from wind throws in 1999 were collected along a temperature and precipitation gradient.Based on the decay class and associated wood densities,log volumes were converted into CWD mass and C content.Log fragmentation was assessed over one year for log segments of intermediate diameters(>20 – 40 cm)after 8 and 18 years of decomposition.Results:Significantly higher decomposition constants(k)were found in logs of F.sylvatica(0.054 year^(-1))than in P.abies(0.033 year^(-1))and P.sylvestris(0.032 year^(-1)).However,mass loss of P.sylvestris occurred mainly in sapwood and hence k for the whole wood may be overestimated.Decomposition rates generally decreased with increasing log diameter class except for smaller dimensions in P.abies.About 74 % of the variation in mass remaining could be explained by decomposition time(27 %),tree species(11 %),diameter(17 %),the interactive effects between tree species and diameter(4 %)as well as between decomposition time and tree species(3 %)and a random factor(site and tree; 9.5 %),whereas temperature explained only 2 %.Wood fragmentation may play a more important role than previously thought.Here,between 14 % and 30 % of the decomposition rates(for the first 18 years)were attributable to this process.Carbon(C)density(mg C · cm^(-3)),which was initially highest for F.sylvatica,followed by P.sylvestris and P.abies,decreased with increasing decay stage to similar values for all species.Conclusions:The apparent lack of climate effects on decomposition of logs in the field indicates that regional decomposition models for CWD may be developed on the basis of information on decomposition time,tree species and dimension only.These can then be used to predict C dynamics in CWD as input for C accounting models and for habitat management.Steffen Herrmann Tiemo Kahl Jürgen Bauhus 2015Forest Ecosystems2015,2,4:7

12Influence of antibiotic-regimens on intensive-care unit-mortality and liver-cirrhosis as risk factor显示文摘AIM: To assess the rate of infection, appropriateness of antimicrobial-therapy and mortality on intensive care unit(ICU). Special focus was drawn on patients with liver cirrhosis.METHODS: The study was approved by the local ethical committee. All patients admitted to the Internal Medicine-ICU between April 1, 2007 and December 31, 2009 were included. Data were extracted retrospectively from all patients using patient charts and electronic documentations on infection, microbiological laboratory reports, diagnosis and therapy. Due to the large hepatology department and liver transplantation center, special interest was on the subgroup of patients with liver cirrhosis. The primary statistical-endpoint was the evaluation of the influence of appropriate versusinappropriate antimicrobial-therapy on in-hospitalmortality.RESULTS: Charts of 1979 patients were available. The overall infection-rate was 53%. Multiresistantbacteria were present in 23% of patients with infection and were associated with increased mortality(p < 0.000001). patients with infection had significantly increased in-hospital-mortality(34% vs 17%, p < 0.000001). Only 9% of patients with infection received inappropriate initial antimicrobial-therapy, no influence on mortality was observed. Independent risk-factors for in-hospital-mortality were the presence of septicshock, prior chemotherapy for malignoma and infection with pseudomonas spp. Infection and mortality-rate among 175 patients with liver-cirrhosis was significantly higher than in patients without liver-cirrhosis. Infection increased mortality 2.24-fold in patients with cirrhosis. patients with liver cirrhosis were at an increased risk to receive inappropriate initial antimicrobial therapy.CONCLUSION: The results of the present study report the successful implementation of early-goal-directed therapy. Liver cirrhosis patients are at increased risk of infection, mortality and to receive inappropriate therapy. 13Assessment of hemostatic profile in patients with mild to advanced liver cirrhosis显示文摘BACKGROUND Hemostasis of patients suffering from liver cirrhosis is challenging due to both,pro-and anticoagulatory disorders leading to hemostatic alterations with distinct abnormalities of coagulation.Pathological changes in conventional coagulation analysis and platelet count are common manifestations of decreased liver synthesis of coagulation factors and reduced platelet count in these patients.However,conventional coagulation analysis and platelet count do not reflect invivo coagulation status or platelet function.The purpose of this present observational study was therefore to assess the haemostatic profile including plasmatic coagulation using thrombelastometry and impedance aggregometry for platelet function in patients suffering from liver cirrhosis.AIM To assess the hemostatic profile of cirrhotic patients according to model for endstage liver disease(MELD)score.METHODS Our study included both in-and outpatients suffering from liver cirrhosis attending the out-and inpatient care of the department of hepatology.Demographic and biochemical data as well as medical history including cause of liver cirrhosis,end stage kidney failure and medication with anticoagulants were recorded.To assess the hemostatic profile,platelet function was analyzed by multiple electrode aggregometry(MEA)using Multiplate^■(ADP-,ASPI-and TRAP-test)and thrombelastometry using ROTEM^■(EXTEM,INTEM,FIBTEM).Data were compared using Mann-Whitney U-or χ^2-test.Spearman correlation was performed to analyze the association between MELD Score and results of thrombelastometry and MEA.RESULTS A total of 68 patients attending the out-and inpatient care suffering from liver cirrhosis were screened.Of these,50 patients were included and assigned to groups according to MELD score 6 to 11(n=25)or≥17(n=25).Baseline patient characteristics revealed significant differences for MELD score(8 vs 22,P<0.0001)and underlying laboratory parameters(international normalized ratio,bilirubine,creatinine)as well as fibrinogen level(275 mg/dL vs 209 mg/dL,P=0.006)and aPTT(30 s vs 35 s,P=0.047).MEA showed a moderately impaired platelet function(medians:AUCADP=43U,AUCASPI=71U,AUCTRAP=92U)but no significant differences between both groups.Thrombelastometry using ROTEM?(EXTEM,INTEM,FIBTEM)revealed values within normal range in both groups.No significant correlation was observed between MELD score and results of MEA/thrombelastometry.CONCLUSION Our data demonstrate a partially impaired hemostatic profile in liver cirrhosis patients unrelated to MELD score.An individual assessment of a potential coagulopathy should therefore be considered.Elisabeth Hannah Adam Madara Mohlmann Eva Herrmann Sonia Schneider Kai Zacharowski Stefan Zeuzem Christian Friedrich Weber Nina Weiler 2020World Journal of Gastroenterology2020,26,17:5

15Environmental photocatalysis:Perspectives for China显示文摘Photocatalysis is based on the double aptitude of the photocatalyst(essentially titania) to simultaneously adsorb reactants and absorb efficient photons.Heterogeneous photocatalysis is able to be efficient in Fine,'Green' and Environmental Chemistry.Photocatalysis induces mild oxidations in the absence of water by generating active neutral atomic O* species.For instance,the oxidation of 4-tert-butyl-toluene is 100% selective in 4-tert-butyl-benzaldehyde.In water treatment,many toxic inorganic ions are oxidized in their harmless upper oxidized state.The elimination of organic pollutants is the main field of water photocatalytic decontamination.Most of aliphatic and aromatic pollutants are totally mineralized into CO2 and innocuous inorganic anions.More complex molecules,such as pesticides(herbicides,insecticides,fungicides,etc.) or dyes,are totally destroyed.Another example of green chemistry is the total degradation of dyes in water,in particular for the azo-dyes,with 100% selective degradation of -N=N-azo-groups into di-nitrogen.Photocatalysis is also active in the 'bio-world' by killing bacteria(E.Coli,streptococcus,etc.) in water without re-growth.Air pollutants can also be destroyed,especially all the VOC's(volatile organic compounds),providing certain air humidity enabling titania to produce cracking OH* radicals.For chemical engineering reasons,the photocatalyst has to be fixed on a photo-inert support.In these conditions,UV-irradiated titania-based photocatalysis could be applied to the elimination of air pollutants,VOC's,solvents,odors,chemicals,etc.Air treatment has to be associated with water and solid waste treatment because of odors.This is conducted by covering water treatment ponds or lagunas by rafts on which large sheets of Ahlstrom paper are deposited,supporting titania associated with activated carbon.Eventually,virus AH5N2,a model virus close to H5N1,responsible for the avian flu could be totally inactivated.Photocatalysis is directly connected with all 12 principles of Green Chemistry defined by Anastas(1998) and possesses open perspectives for China.HERRMANN Jean Marie 2010Science China Chemistry2010,53,9:4
16Clinical use of bone-targeting radiopharmaceuticals with focus on alpha-emitters显示文摘Various single or multi-modality therapeutic options are available to treat pain of bone metastasis in patients with prostate cancer.Different radionuclides that emitβ-rays such as 153Samarium and 89Strontium and achieve palliation are commercially available.In contrast toβ-emitters,223Radium as a a-emitter has a short path-length.The advantage of the a-emitter is thus a highly localized biological effect that is caused by radiation induced DNA double-strand breaks and subsequent cell killing and/or limited effectiveness of cellular repair mechanisms.Due to the limited range of the a-particles the bone surface to red bone marrow dose ratio is also lower for 223Radium which is expressed in a lower myelotoxicity.The a emitter 223Radium dichloride is the first radiopharmaceutical that significantly prolongslife in castrate resistant prostate cancer patients with wide-spread bone metastatic disease.In a phaseⅢ,randomized,double-blind,placebo-controlled study 921patients with castration-resistant prostate cancer and bone metastases were randomly assigned.The analysis confirmed the 223Radium survival benefit compared to the placebo(median,14.9 mo vs 11.3 mo;P<0.001).In addition,the treatment results in pain palliation and thus,improved quality of life and a delay of skeletal related events.At the same time the toxicity profile of223Radium was favourable.Since May 2013,223Radium dichloride(Xofigo?)is approved by the US Food and Drug Administration.Hinrich A Wieder Michael Lassmann Martin S Allen-Auerbach Johannes Czernin Ken Herrmann 2014World Journal of Radiology2014,6,7:4

20Alcohol binging causes peliosis hepatis during azathioprine therapy in Crohn's disease显示文摘Patients with inflammatory bowel disease have normal life expectancy and, due to modern immunosuppressive therapies, also a normal quality of life. Since mostly young people are affected, their social behaviour suits this environment. Alcohol binging is an increasingly disturbing factor among young people. We describe a patient with Crohn's disease, treated with azathioprine, who developed peliosis hepatis after three epsiodes of alcohol binging. Liver toxicity was not observed previously during the course of the treatment. Azathioprine-induced peliosis hepatis is thought to be idiosyncratic in humans. From animal studies, however, it is clear that hepatic depletion of glutathione leads to azathioprine toxicity to the sinusoidal endothelial cells. Damage of these cells causes peliosis hepatis. Since alcohol binging leads to hepatic glutathione depletion, we conclude that in our patient the episodes of binging have reduced liver gluathione content and therefore this has increased azathioprine toxicity causing peliosis hepatis. The problem of alcohol binging has not yet been addressed in IBD patients undertaking immunosuppressive therapy.
