2Blumgart's technique of pancreaticojejunostomy: Analysis of safety and outcomes显示文摘Background: Blumgart's pancreaticojejunostomy(PJ) has been described with low pancreatic leak rates. This study aimed to evaluate our experience with this technique regarding the pancreatic leak and other perioperative outcomes. Methods: We performed a single-center retrospective analysis of a cohort of 81 patients who underwent pancreaticoduodenectomy in our department from January 2011 to February 2018. The primary endpoint was the occurrence of a clinically relevant postoperative pancreatic fistula(CR-POPF) and analysis of its risk factors. Results: The CR-POPF rate was 12.3%. Fistula risk score(FRS) was the only significant risk factor for the occurrence of overall POPF in multivariate analysis. However, none of the other factors including FRS was found to be significantly associated with CR-POPF risk. A strong positive correlation was found between the CR-POPF and the incidence of delayed gastric emptying, post-pancreatectomy hemorrhage and increased length of hospital stay. Conclusion: Blumgart's technique is a safe technique of pancreatico-enteric anastomosis with low rates of CR-POPF. CR-POPF with this technique is independent of most of the preoperative and intraoperative factors. 4Surrogate-based modeling and dimension reduction techniques for multi-scale mechanics problems显示文摘Successful modeling and/or design of engineering systems often requires one to address the impact of multiple 'design variables' on the prescribed outcome.There are often multiple,competing objectives based on which we assess the outcome of optimization.Since accurate,high fidelity models are typically time consuming and computationally expensive,comprehensive evaluations can be conducted only if an efficient framework is available.Furthermore,informed decisions of the model/hardware's overall performance rely on an adequate understanding of the global,not local,sensitivity of the individual design variables on the objectives.The surrogate-based approach,which involves approximating the objectives as continuous functions of design variables from limited data,offers a rational framework to reduce the number of important input variables,i.e.,the dimension of a design or modeling space.In this paper,we review the fundamental issues that arise in surrogate-based analysis and optimization,highlighting concepts,methods,techniques,as well as modeling implications for mechanics problems.To aid the discussions of the issues involved,we summarize recent efforts in investigating cryogenic cavitating flows,active flow control based on dielectric barrier discharge concepts,and lithium(Li)-ion batteries.It is also stressed that many multi-scale mechanics problems can naturally benefit from the surrogate approach for 'scale bridging.'Wei Shyy Young-Chang Cho Wenbo Du Amit Gupta Chien-Chou Tseng Ann Marie Sastry 2011Acta Mechanica Sinica2011,27,6:4

10Deep Learning Based Automated Detection of Diseases from Apple Leaf Images显示文摘In Agriculture Sciences, detection of diseases is one of the mostchallenging tasks. The mis-interpretations of plant diseases often lead towrong pesticide selection, resulting in damage of crops. Hence, the automaticrecognition of the diseases at earlier stages is important as well as economicalfor better quality and quantity of fruits. Computer aided detection (CAD)has proven as a supportive tool for disease detection and classification, thusallowing the identification of diseases and reducing the rate of degradationof fruit quality. In this research work, a model based on convolutional neuralnetwork with 19 convolutional layers has been proposed for effective andaccurate classification of Marsonina Coronaria and Apple Scab diseases fromapple leaves. For this, a database of 50,000 images has been acquired bycollecting images of leaves from apple farms of Himachal Pradesh (H.P)and Uttarakhand (India). An augmentation technique has been performedon the dataset to increase the number of images for increasing the accuracy.The performance analysis of the proposed model has been compared with thenew two Convolutional Neural Network (CNN) models having 8 and 9 layersrespectively. The proposed model has also been compared with the standardmachine learning classifiers like support vector machine, k-Nearest Neighbour, Random Forest and Logistic Regression models. 13Metallurgical Studies of Austenitic Stainless Steel 304 under Warm Deep Drawing显示文摘Austenitic stainless steel 304 was deep drawn with different blank diameters under warm conditions using20 thydraulic press.A number of deep drawing experiments both at room temperature and at 150℃ were conducted to study the metallography.Also,tensile test experiments were conducted on a universal testing machine up to 700 ℃and the broken specimens were used to study the fractography of the material using scanning electron microscopy in various regions.The microstructure changes were observed at limiting draw ratio(LDR)when the cup is drawn at different temperatures.In austenitic stainless steel,martensite formation takes place that is not only affected by temperature,but also influenced by the rate at which the material is deformed.In austenitic stainless steel 304,dynamic strain regime appears above 300℃ and it decreases the formability of material due to brittle fracture as studied in its fractography.From the metallographic studies,the maximum LDR of the material is observed at 150 ℃ before dynamic strain regime.It is also observed that at 150 ℃,grains are coarse in the drawn cups at LDR.Jayahari Lade Balu Naik Banoth Amit Kumar Gupta Swadesh Kumar Singh 2014Journal of Iron and Steel Research(International)2014,21,12:2
14Gallbladder perforation:A single-center experience in north India and a step-up approach for management显示文摘Background: Spontaneous gallbladder perforation(GBP) is an uncommon diagnosis. This study presented the experience of managing spontaneous GBP over nine years at a large, tertiary care university hospital in north India and investigated the outcomes and treatment strategies. Methods: A retrospective review of prospectively maintained digital database of consecutive patients was performed. All patients received medical and/or surgical treatment for spontaneous GBP in our department between January 2010 and June 2018. Results: We identifed 151 patients(81 females and 70 males) with mean age of 53 years. Most common presenting features were pain(96.7%), fever(54.3%) and jaundice(31.1%). Most common cause was gallbladder stones(84.8%) followed by common bile duct stones(30.5%), xanthogranulomatous cholecystitis(17.9%) and malignancy(11.9%). As per Niemeier classifcation, 8.6% had type 1 GBP(free perforation in peritoneal cavity), 76.2% had type 2 GBP(localized perforation) and 13.2% had type 3 GBP(cholecystoenteric fstula). About 60% of the perforations were diagnosed preoperatively. Type 1 was more common in patients with diabetes and also had the worst prognosis. Surgery was performed in 109 patients(72.2%). Seven patients(4.6%) had a postoperative morbidity of Clavien-Dindo Ⅲ or higher. There were three mortalities in patients who underwent surgery. Conclusions: High index of suspicion is required for preoperative diagnosis of GBP, especially in types 2 and 3. Laparoscopic cholecystectomy can be difficult in these patients and patients may require open or partial cholecystectomy.
