| 1 | Computed tomography-based diagnostics might be insufficient in the determination of pancreatic cancer unresectability显示文摘AIM: To inquire into a question of an overestimation of arterial involvement in patients with pancreatic cancer (PC). METHODS: Radiology data were compared with the findings from 51 standard, 58 extended and 17 total pancreaticoduodenectomies; 9 distal resections with celiac artery (CA) excision; and 28 palliations for PC. The survival of 11 patients with controversial computed tomography (CT) and endoscopic ultrasound data with regard to arterial invasion, after R0/R1 procedures (false-positive CT results, Group A), was compared to survival after eight R2 resections (false-negative CT results, Group B) and after 12 bypass procedures for locally advanced cancer (true-positive CT results, Group C).RESULTS: In all of the cases in group A, operative exploration revealed no arterial invasion, which was predicted by CT. The one-year survival in Group A was 88.9%, and the two-year survival was 26.7%, with a median follow-up of 22 mo. One-year survival was not attained in groups B and C, with a significant difference in survival (P a-b = 0.0029, P b-c = 0.003).CONCLUSION: Arterial encasement on CT does not necessarily indicate arterial invasion. Whenever PC is considered unresectable, endoUS should be used. | 2 | The role of endoscopy in the management of hereditary diffuse gastric cancer syndrome显示文摘Hereditary diffuse gastric cancer(HDGC) syndrome is an inherited cancer risk syndrome associated with path-ogenic germline CDH1 variants. Given the high risk for developing diffuse gastric cancer, CDH1 carriers are recommended to undergo prophylactic total gastrectomy for cancer risk reduction. Current guidelines recommend upper endoscopy in CDH1 carriers prior to surgery and then annually for individuals deferring prophylactic total gastrectomy.Management of individuals from HDGC families without CDH1 pathogenic variants remains less clear, and management of families with CDH1 pathogenic variants in the absence of a family history of gastric cancer is particularly problematic at present. Despite adherence to surveillance protocols, endoscopic detection of cancer foci in HDGC is suboptimal and imperfect for facilitating decision-making. Alternative endoscopic modalities, such as chromoendoscopy,endoscopic ultrasound, and other non-white light methods have been utilized,but are of limited utility to further improve cancer detection and risk stratification in HDGC. Herein, we review what is known and what remains unclear about endoscopic surveillance for HDGC, among individuals with and without germline CDH1 pathogenic variants. | 5 | Thermal and microstructural characterization of a novel ductile cast iron modified by aluminum addition显示文摘In high-temperature applications,like exhaust manifolds,cast irons with a ferritic matrix are mostly used.However,the increasing demand for higher-temperature applications has led manufacturers to use additional expensive materials such as stainless steels and Ni-resist austenitic ductile cast irons.Thus,in order to meet the demand while using low-cost materials,new alloys with improved high-temperature strength and oxidation resistance must be developed.In this study,thermodynamic calculations with Thermo-Calc software were applied to study a novel ductile cast iron with a composition of 3.5wt%C,4wt%Si,1wt%Nb,0‒4wt%Al.The designed compositions were cast,and thermal analysis and microstructural characterization were performed to validate the calculations.The lowest critical temperature of austenite to pearlite eutectoid transformation,i.e.,A1,was calculated,and the solidification sequence was determined.Both calculations and experimental data revealed the importance of aluminum addition,as the A1 increased by increasing the aluminum content in the alloys,indicating the possibility of utilizing the alloys at higher temperature.The experimental data validated the transformation temperature during solidification and at the solid state and confirmed the equilibrium phases at room temperature as ferrite,graphite,and MC-type carbides. | Gülsah Aktas Celik Maria-Ioanna TTzini Seyda Polat SHakan Atapek Gregory NHaidemenopoulos | 2020 | International Journal of Minerals,Metallurgy and Materials2020，27，2： | 2 |
