| 1 | 他汀、高密度脂蛋白胆固醇与冠状动脉粥样硬化消退显示文摘背景:他汀类药物可以降低低密度脂蛋白胆固醇(low—density lipoprotein cholesterol,LDL—C)水平并减缓冠状动脉粥样硬化的进展。然而,对于他汀类药物所致高密度脂蛋白胆固醇(high—density lipoprotein cholesterol,HDL—C)变化与疾病进展的关系,目前仍无相关数据描述。目的:了解LDL—C和HDL—C水平变化与动脉粥样硬化的相互关系。设计、地点及患者:对4项前瞻性随机试验(1999~2005年在美国、北美、欧洲和澳大利亚进行)的原始数据进行事后分析。其中1455例经血管造影证实的冠心病患者在接受他汀治疗(18个月或24个月)时进行了系列血管内超声扫描。所有超声分析均在相同的核心实验室中进行。主要观测指标:脂蛋白水平变化与冠状动脉粥样斑块体积的相互关系。结果:他汀治疗期间,LDL—C平均(SD)水平从124.0(38.3)mg,/dL(3.2[0.99]mmol/L)降至87.5(28.8)mg,/dL(2.3[0.75]mmol/L)(下降23.5%;P〈0.001),HDL—C水平从42.5(11.0)mg/dL(1.1[0.28]mmol/L)升至45.1(11.4)mg/dL(1.2[0.29]mmol/L)(上升7.5%;P〈0.001)。LDL—C与HDL—C比值从平均(SD)3.0(1.1)降至2.1(0.9)(下降26.7%;P〈0.001)。这些变化同时伴随平均(SD)百分粥样斑块体积的增加(从39.7%[9.8%]增至40.1%[9.7%])(增加0.5%[3.9%];P=0.001)以及平均(SD)总粥样斑块体积的减小(2.4[23.6]mm^3;P〈0.001)。在单变量分析中,LDL—C、总胆固醇、非HDL胆固醇、载脂蛋白B以及载脂蛋白B与载脂蛋白A-1比值平均水平和治疗引起的变化与动脉粥样硬化进展速率显著相关,而治疗所致HDL—C变化则与动脉粥样斑块体积负相关。在多变量分析中,平均LDL—C水平(β系数,0.11[95%可信区间,0.07~0.15])以及HDL—C升高(β系数,-0.26[95%可信区间,-0.41--0.10])依然为动脉粥样硬化消退的独立预测因子。动脉硬化显著消退(动脉粥样斑块体积减少≥5%)见于治疗期间LDL—C水平低于均值(87.5mg/dL)和HDL—C升高百分比大于均值(7.5%;P〈0.001)的患者。临床事件的发生未见显著差异。结论:在LDL—C显著降低以及HDL上升〉7.5%时,他汀治疗与冠状动脉粥样硬化消退相关。这些结果提示他汀治疗的获益源自致动脉粥样硬化脂蛋白的降低以及HDL.C的升高。尽管血脂水平的这些变化与动脉粥样硬化消退相关,但是后者是否可以转化为临床事件的显著减少,并改善临床预后仍有待进一步研究确定。 | Stephen J. Its high mortality is due to the poor prognosis of the disease caused by a late disease presentation, tumor heterogeneities within histological subtypes, and the relatively limited understanding of tumor biology. Importantly, lung cancer histological subgroups respond differently to some chemotherapeutic substances and side effects of some therapies appear to vary between subgroups. Biomarkers able to stratify for the subtype of lung cancer, prognosticate the course of disease, or predict the response to treatment are in high demand. In the last decade, microR NAs(miR NAs), measured in resected tumor samples or in fine needle aspirate samples have emerged as biomarkers for tumor diagnosis, prognosis and prediction of response to treatment, due to the ease of their detection and in their extreme specificity. Moreover, miR NAs present in sputum, in plasma, in serum or in whole blood have increasingly been explored in the last five years as less invasive biomarkers for the early detection of cancers. | 6 | Non-interventional management of resistant hypertension显示文摘Hypertension is one of the most popular fields of re-search in modern medicine due to its high prevalence and its major impact on cardiovascular risk and con-sequently on global health. Indeed, about one third of individuals worldwide has hypertension and is under increased long-term risk of myocardial infarction, stroke or cardiovascular death. On the other hand, resistant hypertension, the 'uncontrollable' part of arterial hy-pertension despite appropriate therapy, comprises a much greater menace since long-standing, high levels of blood pressure along with concomitant debilitating entities such as chronic kidney disease and diabetes mellitus create a prominent high cardiovascular risk milieu. However, despite the alarming consequences, resistant hypertension and its effective management still have not received proper scientific attention. As-pects like the exact prevalence and prognosis are yet tobe clarified. In an effort to manage patients with resis-tant hypertension appropriately, clinical doctors are still racking their brains in order to find the best therapeutic algorithm and surmount the substantial difficulties in controlling this clinical entity. | 9 | Synthesis,microstructure,and properties of high purity Mo_(2)TiAlC_(2) ceramics fabricated by spark plasma sintering显示文摘The synthesis,microstructure,and properties of high purity dense bulk Mo_(2)TiAlC_(2) ceramics were studied.High purity Mo_(2)TiAlC_(2) powder was synthesized at 1873 K starting from Mo,Ti,Al,and graphite powders with a molar ratio of 2:1:1.25:2.The synthesis mechanism of Mo_(2)TiAlC_(2) was explored by analyzing the compositions of samples sintered at different temperatures.It was found that the Mo_(2)TiAlC_(2) phase was formed from the reaction among Mo3Al2C,Mo2C,TiC,and C.Dense Mo_(2)TiAlC_(2) bulk sample was prepared by spark plasma sintering(SPS)at 1673 K under a pressure of 40 MPa.The relative density of the dense sample was 98.3%.The mean grain size was 3.5μm in length and 1.5μm in width.The typical layered structure could be clearly observed.The electrical conductivity of Mo_(2)TiAlC_(2) ceramic measured at the temperature range of 2-300 K decreased from 0.95×10^(6) to 0.77×10^(6)Ω^(-1)·m^(-1).Thermal conductivity measured at the temperature range of 300-1273 K decreased from 8.0 to 6.4 W·(m·K)^(-1).The thermal expansion coefficient(TEC)of Mo_(2)TiAlC_(2) measured at the temperature of 350-1100 K was calculated as 9.0×10^(-6) K^(-1).Additionally,the layered structure and fine grain size benefited for excellent mechanical properties of low intrinsic Vickers hardness of 5.2 GPa,high flexural strength of 407.9 MPa,high fracture toughness of 6.5 MPa·m^(1/2),and high compressive strength of 1079 MPa.Even at the indentation load of 300 N,the residual flexural strength could hold 84% of the value of undamaged one,indicating remarkable damage tolerance.Furthermore,it was confirmed that Mo_(2)TiAlC_(2) ceramic had a good oxidation resistance below 1200 K in the air. | Yunhui NIU Shuai FU Kuibao ZHANG Bo DAI Haibin ZHANG Salvatore GRASSO Chunfeng HU | 2020 | Journal of Advanced Ceramics2020,9,6: | 4 |

| 15 | Neither hereditary periodic fever nor periodic fever, aphthae, pharingitis, adenitis: Undifferentiated periodic fever in a tertiary pediatric center显示文摘AIM To describe the frequency and clinical characteristics of patients with undifferentiated periodic fever(UPF) and to investigate whether a clinical classification of UPF based on the PRINTO-Eurofever score can help predicting the response to treatment and the outcome at follow-up.METHODS Clinical and therapeutic information of patients with recurrent fever who presented at a single pediatric rheumatology center from January 2006 through April 2016 were retrospectively collected. Patients with a clinical suspicion of hereditary periodic fever(HPF) syndrome and patients with clinical picture of periodic fever, aphthae, pharingitis, adenitis(PFAPA) who were refractory to tonsillectomy underwent molecular analysis of five HPF-related genes: MEFV(NM_000243.2), MVK(NM_000431.3), TNFRSF1 A(NM_001065.3), NLRP3(NM_001079821.2), NLRP12(NM_001277126.1). All patients who had a negative genetic result were defined as UPF and further investigated. PRINTO-Eurofever score for clinical diagnosis of HPF was calculated in all cases. RESULTS Of the 221 patients evaluated for periodic fever, twelve subjects with a clinical picture of PFAPA who were refractory to tonsillectomy and 22 subjects with a clinical suspicion of HPF underwent genetic analysis. Twenty-three patients(10.4%) resulted negative and were classified as UPF. The median age at presentation of patients with UPF was 9.5 mo(IQR 4-24). Patients with UPF had a higher frequency of aphthae(52.2% vs 0%, P = 0.0026) and musculoskeletal pain(65.2% vs 18.2%, P = 0.0255) than patients with genetic confirmed HPF. Also, patients with UPF had a higher frequency of aphthous stomatitis(52.2% vs 10.7%, P < 0.0001), musculoskeletal pain(65.2% vs 8,0%, P < 0.0001), and abdominal pain(52.2% vs 4.8%, P < 0.0001) and a lower frequency of pharyngitis(56.6% vs 81.3%, P = 0.0127) compared with typical PFAPA in the same cohort. Twenty-one of 23 patients with UPF(91.3%) received steroids, being effective in 16; 13(56.2%) were given colchicine, which was effective in 6. Symptoms resolution occurred in 2 patients with UPF at last follow-up. Classification according to the PRINTOEurofever score did not correlate with treatment response and prognosis. CONCLUSION UPF is not a rare diagnosis among patients with periodic fever. Clinical presentation place UPF half way on a clinical spectrum between PFAPA and HPF.
