| 4 | Fecal markers of inflammation used as surrogate markers for treatment outcome in relapsing inflammatory bowel disease显示文摘AIM: To evaluate fecal calprotectin (FC) as a surrogate marker of treatment outcome of relapse of inflammatory bowel disease (IBD) and, to compare FC with fecal myeloperoxidase (MPO) and fecal eosinophil protein X (EPX). METHODS: Thirty eight patients with IBD, comprising of 27 with ulcerative colitis (UC) and 11 with Crohn’s disease (CD) were investigated before treatment (inclusion), and after 4 and 8 wk of treatment. Treatment outcomes were evaluated by clinical features of disease activity and endoscopy in UC patients, and disease activity in CD patients. In addition, fecal samples were analyzed for FC by enzyme-linked immunosorbent assay (ELISA), and for MPO and EPX with radioimmunoassay (RIA). RESULTS: At inclusion 37 of 38 (97%) patients had elevated FC levels (> 94.7 μg/g). At the end of the study, 31 of 38 (82%) patients fulfilled predefined criteria of a complete response [UC 21/27 (78%); CD 10/11 (91%)]. Overall, a normalised FC level at the end of the study predicted a complete response in 100% patients, whereas elevated FC level predicted incomplete response in 30%. Normalised MPO or EPX levels predicted a complete response in 100% and90% of the patients, respectively. However, elevated MPO or EPX levels predicted incomplete response in 23% and 22%, respectively. CONCLUSION: A normalised FC level has the potential to be used as a surrogate marker for successful treatment outcome in IBD patients. However, patients with persistent elevation of FC levels need further evaluation. | 6 | Femtosecond-laser sharp shaping of millimeter-scale geometries with vertical sidewalls显示文摘As femtosecond(fs)laser machining advances from micro/nanoscale to macroscale,approaches capable of machining macroscale geometries that sustain micro/nanoscale precisions are in great demand.In this research,an fs laser sharp shaping approach was developed to address two key challenges in macroscale machining(i.e.defects on edges and tapered sidewalls).The evolution of edge sharpness(edge transition width)and sidewall tapers were systematically investigated through which the dilemma of simultaneously achieving sharp edges and vertical sidewalls were addressed.Through decreasing the angle of incidence(AOI)from 0◦to−5◦,the edge transition width could be reduced to below 10µm but at the cost of increased sidewall tapers.Furthermore,by analyzing lateral and vertical ablation behaviors,a parameter-compensation strategy was developed by gradually decreasing the scanning diameters along depth and using optimal laser powers to produce non-tapered sidewalls.The fs laser ablation behaviors were precisely controlled and coordinated to optimize the parameter compensations in general manufacturing applications.The AOI control together with the parameter compensation provides a versatile solution to simultaneously achieve vertical sidewalls as well as sharp edges of entrances and exits for geometries of different shapes and dimensions.Both mm-scale diameters and depths were realized with dimensional precisions below 10µm and surface roughness below 1µm.This research establishes a novel strategy to finely control the fs laser machining process,enabling the fs laser applications in macroscale machining with micro/nanoscale precisions. | Qiuchi Zhu Peixun Fan Nan Li Timothy Carlson Bai Cui Jean-François Silvain Jerry L Hudgins Yong Feng Lu | 2021 | International Journal of Extreme Manufacturing2021，3，4： | 6 |

| 8 | Clinical outcomes following salvage Gamma Knife radiosurgery for recurrent glioblastoma显示文摘Glioblastoma multiforme(GBM) is the most common malignant primary brain tumor with a survival prognosis of 14-16 mo for the highest functioning patients. Despite aggressive, multimodal upfront therapies, the majority of GBMs will recur in approximately six months. Salvage therapy options for recurrent GBM(r GBM) are an area of intense research. This study compares recent survival and quality of life outcomes following Gamma Knife radiosurgery(GKRS) salvage therapy. Following a Pub Med search for studies usingGKRS as salvage therapy for malignant gliomas, nine articles from 2005 to July 2013 were identified which evaluated rG BM treatment. In this review, we compare overall survival following diagnosis, overall survival following salvage treatment, progression-free survival, time to recurrence, local tumor control, and adverse radiation effects. This report discusses results for rG BM patient populations alone, not for mixed populations with other tumor histology grades. All nine studies reported median overall survival rates(from diagnosis, range:16.7-33.2 mo; from salvage, range:9-17.9 mo). Three studies identified median progression-free survival(range:4.6-14.9 mo). Two showed median time to recurrence of GBM. Two discussed local tumor control. Six studies reported adverse radiation effects(range:0%-46% of patients). The greatest survival advantages were seen in patients who received GKRS salvage along with other treatments, like resection or bevacizumab, suggesting that appropriately tailored multimodal therapy should be considered with each rG BM patient. | 20 | 发生于乳房假体置入女性的间变性大细胞淋巴瘤:173例临床分析显示文摘背景与目的1962年,首例硅胶乳房假体被成功置人体内。1997年,报道了首例与硅胶乳房假体相关的间变性大细胞淋巴瘤(ALCL)。笔者回顾了已发表的37篇文献报道,共79例患者,并收集了94例未报道的病例。方法复习全世界文献。向作者和主治医师寻求文献或病例中缺少的临床和实验数据。由于需要收集的病例信息很多,有时需要在多个地点获得完整的病例信息。因为不是所有被询问的作者和主治医师均提供了信息,所以造成了数据的不完整。结果ALCL病变首先在假体周围血清肿内发现,其肿块附着于包膜,肿块可侵及皮肤,可在区域淋巴结发生,也可在乳房矫形手术中被发现。临床特点呈现多样化,有的仅表现为细胞学阳性结果,肿块自发消退;有的表现为转移性、难治性肿瘤,甚至死亡。其与假体填充物(盐水或硅胶)和置入假体目的(隆乳或乳房再造)没有相关性。在假体信息完整的病例中,患者体内至少置入过1个毛面假体;18例肿块扩散至包膜外,其中9例是致命的。主要的组织化学标记物是cD-30和Alk-1;其他标记物出现频率较低。乳房假体置入女性发生ALCL的风险评估范围是1/500000—1/3000000。结论与乳房假体相关的ALCL是一种新的淋巴瘤,其起源于瘢痕,具有部位和材料特异性,且临床表现呈现多样化,由多因素引起。 | Garry S.
