| 1 | Planned second-look laparoscopy in the management of acute mesenteric ischemia显示文摘AIM: To investigate the role of second-look laparoscopy in patients with acute mesenteric ischemia (AMI).METHODS: Between January 2000 and November 2005, 71 patients were operated for the treatment of AMI. The indications for a second-look were low flow state, bowel resection and anastomosis or mesenteric thromboembolectomy performed during the first operation. Regardless of the clinical course of patients, the second-look laparoscopic examination was performed 72 h post-operatively at the bed side in the ICU or operating room.RESULTS: The average time of admission to the hospital after the initiation of symptoms was 3 d (range, 5 h-9 d). In 14 patients, laparotomy was performed. In 11 patients, small and/or large bowel necrosis was detected and initial resection and anastomosis were conducted. A low flow state was observed in two patients and superior mesenteric artery thromboembolectomy with small bowel resection was performed in one patient. In 13 patients, a second-look laparoscopic examination revealed normal bowel viability, but in one patient, intestinal necrosis was detected. In two of the patients, a third operation was necessary to correct anastomotic leakage. The overall complication rate was 42.8%, and in-hospital mortality rate was 57.1% (n = 6).CONCLUSION: Second-look laparoscopy is a minimally invasive, technically simple procedure that is performed for diagnostic as well as therapeutic purposes. The simplicity and ease of this method may encourage wider application to benefit more patients. | 3 | Differences between radiological findings of COVID-19 and non-COVID-19 infections in pediatric patients显示文摘Background This study aimed to reveal the differences between coronavirus disease 2019(COVID-19)infections and non-COVID-19 respiratory tract infections in pediatric patients.Methods Sixty pediatric patients admitted to the hospital between March 11,2020 and April 15,2020 with respiratory tract infections were evaluated retrospectively.Among them,20 patients with reverse transcription-polymerase chain reaction(RT-PCR)tests and chest computed tomography(CT)examinations were included in the study.According to the RT-PCR test results,the patients were divided into the COVID-19 and non-COVID-19 groups.The clinical observations,laboratory results,and radiological features from the two groups were then compared.Results According to the RT-PCR test results,12 patients were assigned to the COVID-19 group and 8 to the non-COVID-19 group.There were no significant differences between the two groups in terms of clinical or laboratory features.In terms of radiological features,the presence of bronchiectasis and peribronchial thickening was statistically significantly higher in the non-COVID-19 group(P=0.010 and P=0.010,respectively).Conclusions In pediatric cases,diagnosing COVID-19 using radiological imaging methods plays an important role in deter-mining the correct treatment approach by eliminating the possibility of other infections. | Beyza Nur Kuzan Bülent Aslan Taha Yusuf Kuzan Aysegül Karahasan Yagci Nuri Cagatay Cimsit | 2021 | World Journal of Pediatrics2021,17,1: | 1 |

| 11 | Geodesic Vulnerability Approach for Identification of Critical Buses in Power Systems显示文摘One of the most critical issues in the evaluation of power systems is the identification of critical buses. For this purpose, this paper proposes a new methodology that evaluates the substitution of the power flow technique by the geodesic vulnerability index to identify critical nodes in power systems.Both methods are applied comparatively to demonstrate the scope of the proposed approach. The applicability of the methodology is illustrated using the IEEE 118-bus test system as a case study. To identify the critical components, a node is initially disconnected, and the performance of the resulting topology is evaluated in the face of simulations for multiple cascading faults. Cascading events are simulated by randomly removing assets on a system that continually changes its structure with the elimination of each component. Thus, the classification of the critical nodes is determined by evaluating the resulting performance of 118 different topologies and calculating the damaged area for each of the disintegration curves of cascading failures.
