| 1 | Management of hepatorenal syndrome显示文摘Hepatorenal syndrome(HRS) is defined as development of renal dysfunction in patients with chronic liver diseases due to decreased effective arterial blood volume. It is the most severe complication of cirrhosis because of its very poor prognosis. In spite of several hypotheses and research, the pathogenesis of HRS is still poorly understood. The onset of HRS is a progressive process rather than a suddenly arising phenomenon. Since there are no specific tests for HRS diagnosis, it is diagnosed by the exclusion of other causes of acute kidney injury in cirrhotic patients. There are two types of HRS with different characteristics and prognostics. Type 1 HRS is characterized by a sudden onset acute renal failure and a rapid deterioration of other organ functions. It may develop spontaneously or be due to some precipitating factors. Type 2 HRS is characterized by slow and progressive worsening of renal functions due to cirrhosis and portal hypertension and it is accompanied by refractory ascites. The only definitive treatment for both Type 1 and Type 2 HRS is liver transplantation. The most suitable bridge treatment or treatment for patients who are not eligible for transplantation is a combination of terlipressin and albumin. For the same purpose, it is possible to try hemodialysis or renal replacement therapies in the form of continuous veno-venous hemofiltration. Artificial hepatic support systems are important for patients who do not respond to medical treatment. Transjugular intrahepatic portosystemic shunt may be considered as a treatment modality for unresponsive patients to medical treatment. The main goal of clinical surveillance in a cirrhotic patient is prevention of HRS before it develops. | 2 | Gallbladder polyps: Factors affecting surgical decision显示文摘AIM: To determine the factors affecting the decision to perform surgery, and the efficiency of ultrasonography (USG) in detecting gallbladder polyps (GP). METHODS: Data for 138 patients who underwent cholecystectomy between 1996 and 2012 in our clinic with a diagnosis of GP were retrospectively analyzed. Demographic data, clinical presentation, principal symptoms, ultrasonographic and histopathological findings were evaluated. Patients were evaluated in individual groups according to the age of the patients (older or younger than 50 years old) and polyp size (bigger or smaller than 10 mm) and characteristics of the polyps (pseudopolyp or real polyps). χ 2 tests were used for the statistical evaluation of the data. RESULTS: The median age was 50 (26-85) years and 91 of patients were female. Of 138 patients who underwent cholecystectomy with GP diagnosis, only 99 had a histopathologically defined polyp; 77 of them had pseudopolyps and 22 had true polyps. Twenty-one patients had adenocarcinoma. Of these 21 patients, 11 were male, their median age was 61 (40-85) years and all malignant polyps had diameters > 10 mm (P< 0.0001). Of 138 patients in whom surgery were performed, 112 had ultrasonographic polyps with diameters < 10 mm. Of the other 26 patients who also had polyps with diameters > 10 mm, 22 had true polyps. The sensitivity of USG was 84.6% for polyps with diameters > 10 mm (P < 0.0001); however it was only 66% in polyps with diameters < 10 mm. | 3 | Comparison of visual and topographic outcomes of deep-anterior lamellar keratoplasty and penetrating keratoplasty in keratoconus显示文摘AIM:To compare visual,surgical and topographic outcomes of deep anterior lamellar keratoplasty(DALK)and penetrating keratoplasty(PK)for keratoconus(KC).METHODS:In this multicenter,prospective,randomized clinical trial 76 eyes of 71 KC patients operated between January 2011 and July 2014 in 2 tertiary referral hospitals were included. Consecutive patients were alternately selected to receive one of the two surgical methods. Thirty eight eyes underwent DALK with the big-bubble technique and 38 eyes underwent PK.RESULTS:Mean best spectacle corrected visual acuity(BSCVA)at the first postoperative week(P=0.012)and the first postoperative month(P<0.001)was statistically significantly higher in DALK group. The mean BSCVA at12 mo was not significantly different for DALK(0.30±1.99 log MAR)versus PK(0.40±0.33 log MAR)(P=0.104). The76.3% of the eyes had a BSCVA over 0.5 in DALK and 47.4%in PK group(P=0.009). The 7.9% of the eyes had a BSCVA of 1.0 in DALK and 5.3% in PK group(P=0.644). Mean spherical equivalent was-2.94 D in DALK and-3.09 D in PK group.Mean topographic astigmatism was 4.62 D and 4.18 D respectively. Regular topographic patterns were observed in 31(81.6%)of DALK and 29(76.3%)of PK(P=0.574). The most frequent topographic pattern was oblate asymmetric bow tie,seen in 39.5% in DALK and 23.7% in PK. CONCLUSION:Big bubble DALK provides an earlier visual improvement compare to PK. However,visual and topographic outcomes are similar to those in PK at 1y. Postoperative complications including rejection and intraocular pressure elevation are more frequent in PK. DALK is a safer alternative to PK for KC. | 4 | Assessments on slope instabilities triggered by engineering excavations near a small settlement(Turkey)显示文摘In this study,the mechanisms of slope instabilities triggered by engineering excavations at location of a planned tunnel portal in the northwest region of Turkey were assessed,and stability of the current slopes which has impacts on safety of a settlement(village)and agricultural fields was investigated.In the first stage of the study,in order to identify the geological units and structural properties of the sedimentary rocks in the area,to clarify the mechanisms of instabilities and to characterize discontinuity and rock mass properties field works were conducted.In this content,geotechnical boreholes,geophysical explorations between the boreholes,line surveys to explore discontinuity properties,preparation of slope profiles using topographical techniques and sampling of rock blocks and discontinuity planes were performed.In the second stage,laboratory tests were carried out on the samples to determine the geomechnical properties of the slope forming materials and discontinuities.Then,back analyses of the instabilities were made to assess the responsible shear strength parameters of the geological units during failures by considering laboratory shear test results too.Based on the backanalyses of the failures,the strength properties of the slope debris were estimated,and it has been clarified that the residual shear strength properties are the factors controlling slip mechanism along the beddings.Following the stability analyses,it is understood that a further instability along the mass or bedding planes in the failed and redesigned area are not expected,if the current slope geometry is not changed.However,safety factor calculated for the slope in the debris is 1.1 which is well below the recommended value in the literature.The minimum safety factor for currently stable slopes which were cut at the eastern part of the failed area is 1.22 which is also less than that suggested in literature.Some remedial measures such as mini-pile or slope flattening are suggested after analyses to increase the factor of safety for this part below the settlement. | TUNCAY Ergün | 2018 | Journal of Mountain Science2018,15,1: | 2 |

| 6 | Investigating different methods used for approximating pillar loads in longwall coal mines显示文摘Accurately estimating load distributions and ground responses around underground openings play a significant role in the safety of the operations in underground mines.Adequately designing pillars and other support measures relies highly on the accurate assessment of the loads that will be carried by them,as well as the load-bearing capacities of the supports.There are various methods that can be used to approximate mining-induced loads in stratified rock masses to be used in pillar design.The empirical methods are based on equations derived from large databases of various case studies.They are implemented in government approved design tools and are widely used.There are also analytical and numerical techniques used for more detailed analysis of the induced loads.In this study,two different longwall mines with different panel width-to-depth ratios are analyzed using different methods.The empirical method used in the analysis is the square-decay stress function that uses the abutment angle concept,implemented in pillar design software developed by the National Institute for Occupational Safety and Health(NIOSH).The first numerical method used in the analysis is a displacement-discontinuity(DD)variation of the boundary element method,LaModel,which utilizes the laminated overburden model.The second numerical method used in the analysis is Fast Lagrangian Analysis of Continua(FLAC)with the numerical modeling approach recently developed at West Virginia University which is based on the approach developed by NIOSH.The model includes the 2D slice of a cross-section along the width of the panel with the chain pillar system that also includes the different stratigraphic layers of the overburden.All three methods gave similar results for the shallow mine,both in terms of load percentages and distribution where the variation was more obvious for the deep cover mine.The FLAC3D model was observed to better capture the stress changes observed during the field measurements for both the shallow and deep cover cases.This study allowed us to see the shortcomings of each of these different methods.It was concluded that a numerical model which incorporates the site-specific geology would provide the most precise estimate for complex loading conditions. | Deniz Tuncay a Ihsan Berk Tulu a Ted Klemetti | 2021 | International Journal of Mining Science and Technology2021,31,1: | 2 |
| 7 | Anti-inflammatory efficiency of levobupivacaine in an experimental colitis model显示文摘AIM:To investigate the efficiency of levobupivacaine in treating experimentally induced colitis in rats.METHODS:Colitis was induced by trinitrobenzene sulfonic acid and ethanol in 30 rats under general anesthesia,and 10 rats were used as a sham group.Subsequent to induction of colitis,rats were divided into three groups;budesonide group received 0.1 mg/kg budesonide,levobupivacaine group received 10 mg/kg levobupivacaine and saline group received 1 mL saline solution via rectal route for 7 d.In the sham group,only routine rectal catheterization was performed without use of any material.At the end of 7 d,laparotomy and total colectomy were performed for histopathological ex-amination in all rats and blood samples were drawn for measurement of tumor necrosis factor(TNF)-αand interleukin(IL)-6 following cardiac puncture.Macroscopic and microscopic evaluations of the specimens were performed by a pathologist blinded to group assignment of the rats.RESULTS:Weight loss(P=0.016)and macroscopic examination scores(P=0.001)were significantly higher in saline group than others.Histopathological scoring was comparable between all colitis groups(P=0.350).There was no significant difference in TNF-αlevels and IL-6 levels(P=0.150).CONCLUSION:The significant improvement in macroscopic scores suggests that levobupivacaine may have topical anti-inflammatory effects in an experimental colitis model;however,this finding was not supported by microscopic findings. | Ugur Duman Aysun Yilmazlar Ersin Ozturk Sibel Aker Emre Sarandol Tuncay Yilmazlar | 2010 | World Journal of Gastroenterology2010,16,20: | 2 |
