| 1 | Early phase of acute pancreatitis: Assessment and management显示文摘Acute pancreatitis(AP) is a potentially life-threatening disease with a wide spectrum of severity. The overall mortality of AP is approximately 5%. According to the revised Atlanta classification system, AP can be classified as mild, moderate, or severe. Severe AP often takes a clinical course with two phases, an early and a late phase, which should both be considered separately. In this review article, we first discuss general aspects of AP, including incidence, pathophysiology, etiology, and grading of severity, then focus on the assessment of patients with suspected AP, including diagnosis and risk stratification, followed by the management of AP during the early phase, with special emphasis on fluid therapy, pain management, nutrition, and antibiotic prophylaxis. | Veit Phillip Jrg M Steiner Hana Algül | 2014 | World Journal of Gastrointestinal Pathophysiology2014,5,3: | 19 |
| 2 | 末次盛冰期和全新世大暖期中国北方沙地古气候定量重建初探显示文摘中国北方典型沙地处于东亚季风边缘区半干旱气候带,对气候变化响应敏感。本文对已有的该区域古气候记录文献资料进行整理分析和评估,对末次盛冰期与全新世大暖期两个特征时间段的古温度与古降雨量空间格局进行定量化重建,获得了其分布特征。结果表明,在末次盛冰期(约26—16ka)温度降低了5—11oc,变率为60%~200%,降温极值在黄土高原南缘;降雨量减少180~350mm,变率为50%左右,东北地区降雨量变化不大。在全新世大暖期(约9~5ka),温度升高了1.0—3.5℃,变率为20%-130%;降雨量增加了30~400mm,变率为10%-120%,其中存在一些需剔除的奇异点,从东部沿海向西北内陆降雨量增幅有加大趋势。上述结果为进一步探讨中国北方干旱-半干旱区古气候变化机理、检验东亚季风区古气候数值模拟结果等提供了定量化数据。 | 俞凯峰 鹿化煜 Frank Lehmkuhl Veit Nottebaum | 2013 | 第四纪研究2013,33,2: | 17 |

| 4 | 22-gauge core vs 22-gauge aspiration needle for endoscopic ultrasound-guided sampling of abdominal masses显示文摘AIM To compare the aspiration needle(AN) and core biopsy needle(PC) in endoscopic ultrasound-guided fine needle aspiration(EUS-FNA) of abdominal masses.METHODS Consecutive patients referred for EUS-FNA were included in this prospective single-center trial. Each patient underwent a puncture of the lesion with both standard 22-gauge(G) AN(Echo Tip Ultra; Cook Medical, Bloomington, Indiana, United States) and the novel 22 G PC(Echo Tip Pro Core; Cook Medical, Bloomington, Indiana, United States) in a randomized fashion; histology was attempted in the PC group only. The main study endpoint was the overall diagnostic accuracy, including the contribution of histology to the final diagnosis. Secondary outcome measures included material adequacy, number of needle passes, and complications.RESULTS Fifty six consecutive patients(29 men; mean age 68 years) with pancreatic lesions(n = 38), lymphadenopathy(n = 13), submucosal tumors(n = 4), or others lesions(n = 1) underwent EUS-FNA using both of the needles in a randomized order. AN and PC reached similar overall results for diagnostic accuracy(AN: 88.9 vs PC: 96.1, P = 0.25), specimen adequacy(AN: 96.4% vs PC: 91.1%, P = 0.38), mean number of passes(AN: 1.5 vs PC: 1.7, P = 0.14), mean cellularity score(AN: 1.7 vs PC: 1.1, P = 0.058), and complications(none). A diagnosis on the basis of histology was achieved in the PC group in 36(64.3%) patients, and in 2 of those as the sole modality. | 5 | Origin of CD8^+ Effector and Memory T Cell Subsets显示文摘It is well accepted that CD8 + T cells play a pivotal role in providing protection against infection with intracellular pathogens and some tumors.In many cases protective immunity is maintained for long periods of time (immunological memory).Over the past years,it has become evident that in order to fulfill these multiple tasks, distinct subsets of effector and memory T cells have to be generated.Until today,however,little is known about the underlying mechanisms of subset differentiation and the timing of lineage fate decisions.In this context,it is of special importance to determine at which level of clonal expansion functional and phenotypical heterogeneity is achieved.Different models for T cell subset diversification have been proposed;these differ mainly in the time point during priming and clonal expansion(prior,during,or beyond the first cell division)when differentiation programs are induced.Recently developed single-cell adoptive transfer technology has allowed us to demonstrate that individual precursor cell still bears the full plasticity to develop into a plethora different T cell subsets.This observation targets the shaping of T cell subset differentiation towards factors that are still operative beyond the first cell division.These findings have important implications for vaccine development,as the modulation of differentiation patterns towards distinct subsets could become a powerful strategy to enhance the efficacy and quality of vaccines. | Christian Stemberger Michael Neuenhahn Veit R.Buchholz Dirk H.Busch | 2007 | Cellular & Molecular Immunology2007,4,6: | 2 |
